  • Here are a few signs to determine if your metatarsal arch has collapsed. Another indication, which I forgot to mention in the video, is callusing under the ball of the foot.

Hi Ma’am thanks for guiding me. For the last three months, pain in the ball of my right foot has taken a toll on me. I had mild pain earlier but last three months got terrible and I am assuming it is due to some overuse/stepping on something pointed or there was a corn growth which has completely disappeared
1.) MRI scan all shows everything fine and indicates some recent injury
2.) seen three leading podiatrists and 2 leading Orthopedic doctors, all suggesting custom orthotics but none of the custom orthotics helped
3.) uric acid and vitamin B12 all normal
4.) foot scans show flat feet etc but from clinicals with doctors, they all seem to agree my situation is not that bad and they are not sure how to help me
5.) I use some metatarsal pads ( used by women who wear heels etc) the pada which I put in my second toe and covers the balls of the foot areas helps relieve some pain but I am now feeling the pressure moving from one area to another but it is typically between 2-3 toe
6.) did some ultrasound pain mgmt. While Pain may not be there, I still feel the pressure and so I feel insecure walking which I have stopped and completely have stopped tennis
    Can you please kindly advise
    Thanks
    Andy

Yes, typically orthotics help with pain like that. Assuming they have tried different pads and cushioning in the orthotics, I have a couple thoughts:
      1. Have you looked at your footwear? If they are either too narrow/wide, too short/long or they are compressing the foot, this can definitely cause issues.
      2. There are some stretches and exercises that may help. It relaxes the muscles around the toes and helps to lift up the arch at the ball of the foot. Like with all exercises, it does take time to see results, but if done on a regular basis, it might be worth it over time: ua-cam.com/video/WMW9bvijwe0/v-deo.html
      3. As for the metatarsal pads, I would try and move it around until you find the right placement for your pain. As a general rule, if there is pain because of pressure from the pad, move it away. If there is pain unrelated to the pressure of the pad, move it towards the pain. It's possible that you may need a larger or thicker pad.

Also, you should try HOKA shoes. I’ve had Metatarsalgia for many years. HOKA shoes with custom orthotics have helped a lot. There are numerous different types of HOKA’s so don’t give up if the first ones you try don’t work. Generally the ones with the most cushion and wide width work the best for me.

Okay so what to do next, it's collapsed. Dead straight. Had my scan and doesn't look nice, I'm over 30, can I still fix it? Is it permanent ?

In most cases, this metatarsal arch can be lifted up again through exercises. These stretches and exercises are designed to increase that arch again: ua-cam.com/video/WMW9bvijwe0/v-deo.html.
Incorporating these in your daily routine should help! Like all muscle building, it might take some time to see results.
