Stevia:
Steviol glycosides are poorly absorbed in the body and pass through the upper gastrointestinal tract fully intact. Once steviol glycosides reach the colon, gut bacteria convert steviol glycosides into steviol - steviol is then metabolized by the liver before being excreted in the urine. And the steviols have actually been shown to lower insulin and glucose due to its lack of metabolization.
Monk Fruit:
Monk fruit sweetener is made from extract derived from dried fruit - the extract is 150-250 times sweeter than table sugar, has zero calories and carbs, and does not raise blood glucose levels
Been shown in studies to reduce NF-kB and decrease the production of inflammatory molecules such as iNOS, COX-2, and IL-6
Erythritol (and other sugar alcohols):
Once sugar alcohols are absorbed they use very little to no insulin to convert to energy - not all of the sugar alcohol passes into the bloodstream, and the rest passes through the small intestine and into the large intestine. Erythritol, however, is considered the safest sugar alcohol as it the most well tolerated in terms of digestive discomfort - it has no effect on blood glucose and has zero calories (naturally found in fruits, vegetables and fermented foods)
Unlike other sugar alcohols (like Xylitol) the laxative effects are not reported to be as common because 90% of Erythritol is absorbed before it enters the large intestine and is excreted via urine
High Fructose Corn Syrup:
When fructose is metabolized, it turns into free fatty acids and triglycerides, which get stored as fat - fatty acids accumulate as fat droplets in your liver and skeletal muscle tissues, causing insulin resistance and non-alcoholic fatty liver disease.
A byproduct of liver metabolism is that your liver begins to create uric acid, causing blood pressure to increase and can result in gout - also has no effect on ghrelin and has been shown to cause overeating
Table Sugar:
Regular table sugar is broken down into fructose and glucose when it enters the bloodstream
And, as mentioned, excess fructose gets turned into glycogen, becoming a danger for those in a ketogenic state
Honey:
Honey is considered a “nutritionally dense” sweetener, but it’s packed full of fructose
Most processed honey also has added sugars and is usually pasteurized, losing most of the nutritional benefits it has
Additional:
Agave syrup is generally seen as a low GI sweetener - while Agave Syrup is about a 9.6 GI due to low glucose content, it is mostly fructose and very highly processed
It can contain up to 80% fructose which has a very high impact on our blood sugar levels and is typically seen as one of the most damaging sources of sugar
Grey Area
Artificial Sweeteners:
Artificial sweeteners are synthetic sugar substitutes that are regulated by the FDA - can be more than 100x sweeter than sugar and provide a zero calorie alternative
Studies have shown that it’s possible to successfully use artificial sweeteners in a ketogenic diet
One study, published in the journal Epilepsia, subjects successfully stayed in ketosis despite being given saccharin (Sweet’n’low) every day
Splenda typically contains maltodextrin
Downside to Artificial Sweeteners:
A study, from the Journal of Food and Chemical Toxicology, concluded that saccharin negatively impacts gut bacteria and promotes liver inflammation.
Found that saccharin elevated expression of pro-inflammatory iNOS and TNF-α in liver and indicated that it induced inflammation

Hi Thomas, it has been a few years since you posted this video. Many of the Whey Protein powder companies ON and Dymatize ect are adding Sucralose to their Protein Powder. I have a Ketogenic Smoothie made from scratch, for breakfast each morning which I add Whey Protein too. I have been reading labels and the there is not maltodextrin mentioned before or after the "Sucralose" on the labels. Would this be a "NO GO" for keto? I am not a fan of Splenda because it is so ridiculously sweet... Please help me out if you can thank you!

@@rickichase3438 Try finding pure stevia from Peru. Label shows up as Bolivian Stevia from Peru.. No additives, to bad you are alergic.
wish this list were more complete and discussed allulose and xylitol. Allulose is of high interest because it's properties are similar to sugar (sucrose), like making caramel and / or candies.

Isn’t sugar alcohol bad ? Their Monk-fruit powder has 5g sugar alcohol in it.
Asif A it’s really bad for me . They affect a very small percentage of the population like less than 3%. It makes me super sick -sweating dizzy and gastrointestinal distress
Erythritol is what helped me stick with the Keto diet, I've got a sweet tooth and it taste to me more like sugar than any of the other sweeteners and it don't make me feel sick or give me an upset stomach.
So do i count the carbs on erythritol for keto? He didn’t really confirm 😭

@@sergbarrose1474 I believe you don't count it towards your carbs because it's a sugar alcohol and it's not absorbed in the gut. It also doesn't cause a glycemic reaction. At least that's my understanding. Best of luck. 👍
Recently on news as carcinogenic
I really wanted to love erythritol but I genuinely only use sugar (pre keto) for my coffee and omg it tastes HORRIBLE in coffee to me lol I've gone with sucralose coffee syrup and it's been much better and doesn't kick me out of keto or cause any insulin spikes. The ingredients are just water, flavoring (vanilla), and sucralose, so no maltodextrin or anything.
I really like his organized approach to this. Im just starting my keto journey and this is very helpful about the sweetners because I WAS lost.
After 40-50 of no ketosis in carnivore I changed my sweetener from Stevia to Monkfruit and in 2 days was in ketosis. Thank you for the video that changed my whole situation.

Probably because you were using stevia with maltodextrin
Maltodextrin is basically sugar, and it's the main ingredient in the large packets of splenda and stevia so keep an eye out.

Erythritol tastes awful to me though. Stevia is a little better. Monk fruit is also better.

Had diabetes type 2 for 12 years .
Went on a low carb high fat journey and in 7 weeks I could stop taking my medication . Diabetes and high blood pressure.

Good for you!! While I didn't have diabetes for 12 years, I did have it for one year. Fully expected, as I had been 150 pounds overweight for like 25 years. Also had high blood pressure and high cholesterol--the triple crown of issues leading to heart disease and death! Anyway, after only ONE WEEK on keto, my blood sugars were down to normal, unmedicated. I have dropped 10 pounds a month without changing my ultra-sedentary lifestyle the least little bit. I am NEVER hungry. In fact, I sometimes forget to eat--exact opposite of my foodaholic eating habits before. But trust me, I do eat plenty! Interesting side benefits: My awful reflux/heartburn just stopped on DAY ONE--so not a benefit of weight loss, but a benefit of the diet. Also, no more gas. Seriously. Not to be too blunt, but I don't think I've passed wind even once since starting keto. Big change there. Also, no abdominal discomfort at all or irregularity--also a big change from my lifelong status. After 4 months, my blood pressure is back into normal range, so no more BP meds for me! Cholesterol panel is awesome too, with my ratio (big heart disease indicator) in the "best" category. Keto is a miracle, pure and simple.

Thank you! Those of us with hypoglycemia have trouble with honey and agave; monkfruit has been a miracle find! Just a note with Erythritol --- if corn is used to make it (for feeding the yeast), make sure it's labeled non-GMO, if not, it is what it eats :) (ref: Dr. Berg)

Yes, that's interesting. But I happened to have some Truvia powder in my pantry and was wondering about that. I checked the Truvia website FAQ and it says that Truvia uses Erithritol instead of Maltodextrin.

Yep. I've found liquid stevia is better too. But with stevia, you can't overdo it, otherwise it doesn't taste good.

@@cindymora8664 Truvia isn't really stevia, it's alcohol sugar... but that's a top 3 keto alternative. The only way to get true Stevia I believe nowadays with no filler is leaves or liquid. Any powdered one will have filler.

Seriously. I searched YT with sucralose and it comes back with TDL. So many health searches come back with TDL videos. One of the main guys that helped me lose 180lbs and maintain. Just over 2 years and I know I never have to balloon up again with the likes of TDL teaching me how "foods" affect me!

Asked Thomas for directions to the grocery store. In response he taught me the history of cartography, and, proceeded to mentor me in the art of paper making from various wood fibers. Finally he explained in detail how I might extract ink using lampblack soot and aqueus binders, which he helped me to apply to the paper in the form of a map to the grocery store.

You should check out Dr. Eric Berg, Dr. Shiva, and Dr. John Bergman. They are even better with the science details.
This is an old comment, bit it highlights an issue every M.D. or doctor in training faces. We simply do not learn this information in medical school. I hated my second year nutrition course, because it was packed with outdated information. I was screaming inside of my head that I had NO plans of teaching patients any of the information presented. Furthermore, medical school is so rigorous that nutrition courses are truncated in order to allocate more time to subjects like pathology and pathophysiology. This poses a huge issue if we want to get serious about primary prevention. Many diseases of lifestyle are linked to nutritional issues.
By the way, I am also a biomedical scientist. 🙂
I really appreciate your comparison of all these sweeteners and am pleased that I've made the right choices so far. You didn't mention Bocha Sweet which I recently discovered and so far I'm loving it. To me it tastes exactly like sugar in every way. So I'm curious what your take is on this sweetener since it might not have been around at the time this video was made 4 years ago.

Thanks Thomas, you explained why my using a 1/4 to 1/2 or even a full teaspoon of monk fruit/Erythritol in my coffee in the morning makes it actually BETTER than a full teaspoon of sugar!! I actually taste the coffee instead of sugar and not any bitterness, which is why I liked to sweeten the coffee....but now I realize that coffee flavor is hidden with the sugar!! Now I enjoy a just a nice strong mild taste. Even if I wasn't doing any dieting, it is far superior to regular old, refined sugar
For Stevia if you want to use the non-liquid form/powdery form, please purchase the Sweat Leaf brand because it doesn't have the maltodextrin as mentioned by Thomas.

My thoughts exactly! I often see Acesulfame K in my protein supplement products, really curious as to how it effects blood glucose. Seen some pretty contradictory info so not quite sure what to think.
I think Acesulfame K is the same as Sucralose…….chlorinated sugar.

Great video. Just a note: mogrosides in monk fruit stimulates insulin product in the pancreas so it does spike insulin. Compared to sugar though it is lower. It might be ok on keto but not on fasting. Erythritol can cause liver enzyme irregularities and increase fat in the belly area for some people. My friend was taking it and he had to stop because his LFTs went high. Other women experience a fat gut from it
So anything sweet is a no go 😭

Thomas, thank you for the disambiguation around sucralose! It's complicated since it is usually available in packets with maltodextrin, but pure liquid sucralose, without the binders, doesn't trigger an insulin response. Yeah, it's not great for the gut biome, but if you use it sparingly, it's totally fine on keto.
It's so complicated. And in fact sucralose still has a lot of pending research.

The only one that works with my brain, literally as a migraine with aura sufferer, is munk fruit. The rest of them give me migraines 24 hours after consuming any food items made with any sugar substitute that isn't munk fruit. Munk fruit is migraine safe. For those that suffer from migraines and are keto/low-carb followers.

Glycerol is actually a component of MCT, as is evident in the name: Medium Chain Triglyceride.
With MCT, we always focus on the medium chain fats, but neglect the glycerol component.
As the name implies, each MCT macromolecule is composed of three molecules of medium chain fats bound together by one molecule of glycerol.
So, when we take MCT, we are *always* getting glycerol with it, though it's a small amount compared to what we'd typically use in the form of pure glycerin.

@@Yoyoprojon this one is 100% monk fruit? I wonder if it can be substituted for sugar in baking (cookies, cakes, etc.).
I was using organic sucralose with no maltodextrin and no issues with gut or anything else, but the fact it could deplete my good bacteria, I just bought monkfruit. Thank you
