About this video: So what is meibomian gland dysfunction? In this video I share different strategies for mgd eye treatment that are often recommended for people struggling with evaporative dry eye. If you are looking for mgd treatment at home or looking into other medical treatments for dry eye, then this video may help you. If you are struggling with severe meibomian gland disease then it is important to follow up with a specialist in either optometry or ophthalmology that is trained in dry eye syndrome management.

Hi, I love your enthusiasm and your knowledge, and the brilliant way you impart the knowledge in a quick but understandable way. I've been through most of the videos in the eyelid hygiene list and I wondered if there is there any chance you could put your playlists in date order, with the most current videos at the top? Then we would get the most current information first, with the very useful references to other related videos as well.
From checking out your videos I suspect I have a chalazion (it's not painful when I touch it) and I am about to buy the Bruder Moist Heat Eye Compress. Does this stay hot enough for 10-20 minutes, or would it be better to buy two and use each for 10 mins? I think I got the chalzion due to my cat trying to walk across my head when I was in bed and he mis-judged it and his foot moved across my eyelid. His feet were dirty. I never thought anything of it, but I had what I thought was a stye, the next day and my eyelid was red. It's been like that for a month or two now. Is this possible or is it just coincidence?

I love your videos Dr. Eye. Thank you!. I would like to share my experience with dry eye and the tratments I have had so far. I was diagnosed with dry eye like 15 years ago. At that time it was not too bad, just some eye drops once in a while and everything was good. With the passing of time my symptoms have been worsening, concidencially after moving to Canada. It started with eye inflamation, stinging, burning, scratchy sensation. When I visited an optometrist I was diagnosed with severe dry eye and allergy reaction to the preservatives of eye drops. I have visited two doctors now, the fist one gave a 3 weeks of treatment with non-preserved prednisolone acetate 1% (equivalent to Pred Forte), the result was amazing, but after that he prescribed to me: I-drop pur gel (for the day), I-defence dry eye ointment (for the night), bruder hydrating compress (i didn't like it since it gets cold fast), systane omega3 supplements. All of these stuff didn't work on me. Then I change of doctor, and got the IPL (3 sessions) & lipflow (1 session) treatment. To be honest, I didn't feel anything special after this very expensive treatment (CAD 2400 not covered by my insurance). Nevertheless, I have continued with the maintenance of IPL 6 months later (CAD350). At the same time of IPL & lipflow, my second doctor prescribed to me restasis to be used twice a day (thanks God covered by my insurance). Only after around 6 months of starting with restasis I started feeling a relief in my eyes, however, there are days and days, sometimes I feel almost 90% good, but other days I feel terrible. Talking to my doctor during the last visit, she said that a gluten & dairy free diet could improve my sympthoms. I started right away with my new eating habits. To be honest, I am feeling way better now. Sumarizing, I am applying hot compresses at least twice a day (I got an electric one which is amazing!), restasis twice a day, omega 3 supplement, Hylo-dual drops (provided by my doctor), refresh tears, avoid cold beverages, and try the most to eat healthy. Hopefully I keep this improvement like that. It has been very helpfull to learn from others testimonials, so sharing my experience I hope it help somebody.

I've had Blepharitis, Demodex and MGD for at least 7 years and have tried almost everything. special wipes , drops, chinese herbs, restasis, compresses and nothing really helps. I'm going to try to cut down on gluten, sugar and dairy, because I believe that like with most diseases, it's not about getting something else, but eliminating what's causing it. A doctor once told me that after he eliminated sugar, his dandruff went away !

I hope to be accepted. I am so surprised by the diagnosis. I encourage everyone that has eye problems to get their eyes checked for dry eyes or at least look at the symptoms to see they might have dry eyes. I wish I would have caught this problem earlier. I’m told I have a few bumps there and in the other eye too but I’ve no discomfort yet. I have MGD for about 2 years. I do warm compresses once a day for 10 minutes. I’ve been on Doxycycline about 4 times. I see my doctor about every three months. Sometimes he squeezes my lids between 2 cotton swabs. It’s gotten better after taking doxycycline. I used to have lumps in my eyelids. Now I noticed that my lids have dry skin and gets crusty sometimes. I also use baby shampoo and a washcloth to scrub my lids occasionally. My doctor never mentioned any of the instruments you mentioned. I feel so frustrated. Thanks for talking about this subject.

You could first try warm water at the eye first instead of standard cold water, since water from the water outlet is rather cold compared to the human body temperature and fat is less soluble in cold water. Only do eyelids with warm water, not inside the eye. Gently warm your fingers with warm water, carefully rub the under eye lids from temple inwards to the nose. Very gently no pressure.

  • @TheShredfest89
    @TheShredfest89 2 роки тому +8

Interesting thought. MGD is caused from 2 reasons. 1) De-intervation of parasympathetic stimulus from near phone/computer work/meds/toxins/glands becoming insulin resistant 2) tear lake reduction (from causes above) that dries out the opening of the meibomian glands. Once the opening is dried - the oil atrophies and the glands do as well. It doesn't happen from mysterious reasons.
The inflammation is secondary to the reduction of tear lake. SPKs form and the tissue reacts with inflammation. All the tear lake reduction occurs secondary to meds, age, near work parasympathetic overload, lack of blinking, ect ect.
Here is the final redpill. Literally use a properly sized punctual plug. Collagen lasts 3 days. 6 month can fall out- not all lacrimal canals are made equal- usually lasts 3-5 months- if the pt will tell you they felt way better then they will tell you its dry again- and spks didn't resolve- it fell out. Silicone has its own issues but are awesome if they are well managed. And watch what the patient tells you. DES vanishes. Watch acuities improve.
Also systane hydration pf is the best oct drop on earth for removing spks.
You dont have to believe me on plugs. Just trial it. God bless. Thanks for making dry eye management more mainstream.

Hi doctor - Perhaps you can comment on 2 important things I have learned as a new dry eye/mgd sufferer (1 year):
    (1) We sufferers quickly seek help and self-education on the internet. We are looking to address the CAUSE as well as the SYMPTOMS. I learned that fluctuating androgen hormones (both in male and female) greatly affect our sebaceous glands - which includes the meibomian glands. Perio/post menopausal women experience MGD earlier and at twice the rate of men., with onset usually at the perio/post menopausal time. In men, dry eye/MGD onset is tyoically later as testosterone drops further. No eye doctors have ever mentioned possible androgen/hormone testing/treatment, although I see lots of opthal research saying research is ongoing. Ha ha! I guess kindof like all the male birth control still being "studied". Which leads me to realization #2.
    (2) Some eye products or using too many and doing too much can make things worse. The highly recommended OTC preservative-free eye products could be the cause of your eye margin irritation or worsening it. I have no known allergies, but discovered that my eyes do not like most Refresh products - drops or ointment. So far I am doing well with Systane. My eye lid areas also did not like castor oil, aloe vera gel, or even a little vaseline around my eye area after warm compress. I probably irritated the delicate eye area skin by doing too much -- too hot a compress, too long, too much massage, washed with diluted baby shampoo in the morning and Occulids wipes at night was too much. I was exhausted and frustrated and kept adding more, trying to get on top of things! While waiting for my next eye appt, I stopped using everything, except Systane night gel and drops during the day (started clean and back to the beginning). My eyes started to improve after 2 weeks of eye product detox. Eyelids were still inflamed but improving when I finally saw a dry eye specialist, who still prescribed Cyclosp and a steriod cream. I put off the Rxs and continued detox. My eyes are much better. I am still figuring out my individual routine, which may change as needed with symptoms. But I am going to talk to my OBGYN/endocrinologist about DHEA/androgen hormone and MGD. (I also see studies correlating MGD with high cholesterol, low vit D, obesity, so I know MGD can be multifactorial.)

Very well said. I used to take finasterde and my eyes were killed bcs of severe dry eye . Then I stopped finasterde ( which is a testosterone suppressor and prevent its conversion to DHT) and my eyes got better and eventually normal . But again I have started Spiro and dry eyes problem has returned. So androgen has a direct role in creating dry eyes.

I have been suffering from increasingly painful meibomian gland dysfunction. My doctor has me on a regimen of Omega 3, Refresh Omega 3 drops, warm compresses, eyelid cleansers, etc. I took Cequa & then Xiidra (very expensive). Xiidra worked well at first, but then it didn't. When the price increased 100%, I just stopped using it. I'm seeing my doctor tomorrow. I am so miserable. I want to explore the use of Lipiflow. It's apparently expensive & not covered by insurance. But I'm willing to try anything! Thanks for this very informative presentation.

Thank you for the helpful videos. Its refreshing to see an eye doctor that actually has dry eye and knows what they're talking about. I am so frustrated by how expensive it is to keep your eyes in good shape. Its bad enough that my glasses cost me around $350 every 3 years (I get the Crizal Sapphire 360 stuff because I hate reflections so they're worth it but its still a lot of money), now I have to add all of the dry eye stuff. I'm only 36 and I've got MGD to the point that after watching your video this morning I went and pulled back my bottom right eyelid and could actually see the glands were backed up and white so I did some massaging and gentle squeezing and that cleared up. Previously my eye doctor had only diagnosed me with blepharitis and ocular rosacea which aren't a walk in the park either. I'm disabled and certainly can't afford to spend $500+ on LipiFlow or whatever. I went and bought the EyeEco Tranquilize XL (with the sodium acetate packs because I wasn't fond of the Bruder packs that seem similar to the green microwave ones) at your suggestion but I'm really struggling with whether I should buy the NuLids or not. $280 is a lot of money for someone on a fixed income. Is it possible to get the same results with daily eyelid massage after using the Tranquilize XL and then using hypochlorous acid? I am so tired of my eyes being red, itchy, watery, and at the same time feeling like they're dry (especially the right one around the outer right edge, even the eyelid itself feels very dry) but ugh, $280 is like two months of groceries for me...

I was apparently born with Sjogren's Syndrome and I always had problems with my immune system attacking my tear glands 100% of the time. My upper tear glands were cauterized in 2006. Now, I use Restasis drops. My health insurance helps pay for it. Otherwise, it would cost around $530/month. The only artificial tears that work for me is TheraTears. I get them in vials because it's too risky for me to use the bottle. If I accidentally touch the tip of the bottle, I'll get an infection in my eye. It has happened before. I can't use regular eye drops because my eyes are too chronically dry. I did try punctual plugs in 2006, but the upper ones kept popping out. That's why my ophthalmologist cauterized them. I don't know anyone personally who has eyes that are as chronically dry as mine are practically all the time. When I put my artificial tears in my eyes, my eyes start to get really dry in 10 minutes. It's extremely frustrating.

IPL combined with manual expression straight after seem to be the way. The key is to target the root cause rather than mere symptoms, the root cause being inflammation. I’m hoping to get IPL with expression done as soon as I find a dry eye specialist that is able to perform this.

One technique that has helped me, not mentioned here. is the use of puncta plugs. These are small silicone plugs gently pressed into the teal ducts to keep tears in the eye and on the surface of the cornea from draining away. If you have good tear flow you probably wouldn't want these as your eye would be continually overflowing with tears. I have Sjogren's with dry eye as well as MBD. When the plugs kept squeezing out of my tear ducts, my Dr. finally recommended cauterizing my tear ducts shut and that too helps keep tears as well as oil in my eyes and on my corneas. Good luck to everyone struggling with these conditions. Keep trying to find a combination of treatments that work for you. They're out there but require the patient to find them and then follow them with diligence.

I’m an optometrist and the contacts are not causing MGD. If you treat dry eye or MGD, your contacts will feel more comfortable.

Great question! This is very common. This can be due to dry eye and the oils drying out on the lids, or it could also be caused by blepharitis or infection. Cleaning with eyelid cleaning products is a great thing to do on a regular basis, I also suggest to avoid any air flow to the eye while sleeping (such as a fan).

It could be a demodex infection as your symptoms sound similar to what I had. Demodex is a form of mite that is especially active at night, they can cause secondary bacterial infections. Tea tree oil is toxic to demodex. If you think you might have this, you can treat yourself.
I treated myself by mixing tea tree oil with olive oil (50:50 ratio), then shook the bottle and applied to eyelids and eyelid margins with cotton pads. Left it for 10 mins, it stung a little bit, before washing off and drying off with cotton pads. This helped me a lot.

How do I find a dry eye specialist? I’m not happy with the eye clinic I go to. They only give me antibiotics drops and just cut open glands to drain them.

Hey Doc, what are your thoughts on ICL? I see a lot of your videos have to do with glasses, contacts and Lasik. Have you ever talked about ICL? There doesn't seem to be enough ICL content on UA-cam. Seems like the Lasik industry is way bigger (marketing..).

I have a question on one of your previous topics. In the past, I've had two separate retinal tears, due to vitreous detachment. Both were repaired with laser surgery. I need some help with self diagnosing flashers.
Obviously, I've made the right call twice already. In fact, when I had my first tear, the ophthalmologist did not see it and she told me that I was fine. The flashers persisted, so I got a second opinion and a Retinal Specialist performed laser surgery on my eye. A few years later, it happened again. I skipped the routine eye exam and I went straight to the Specialist. He repaired a second tear.
Over the past ten years since my last surgery I have experienced temporary flashers many times, probably at least 3 times per year. I watched them carefully and each time, they went away, so I did not see a doctor. I guess that I correctly assumed the flashers were caused by "normal" vitreous detachment. And for the record, I get a complete eye exam at least once per year.
So, here are my questions: What is the longest period of time that a patient will experience "normal" temporary flashers? How can I be certain that I am in the clear each time? On the one hand, it's pretty scary, knowing that if I get it wrong, I could suffer permanent vision loss. But, on the other hand, if I ran to the doctor every time I saw some flashers, I might have 4 or 5 false alarm doctor visits in a single year. When should I transition from carefully watching and waiting, to ringing the fire alarm and insisting on getting an immediate eye exam?
