    I had very stubborn and melasma. My dermatologist finally prescribed Tranexamic acid. My results just using tranexamic acid were amazing. I saw results within the first three weeks and after two months my melasma was 90% gone. Of course I also am very religious about wearing a mineral sunscreen everyday, even indoors. I also resumed Retin-A after the first month. I know that I will have to continue some sort of maintenance regimen for the foreseeable future, at least I don’t feel self-conscious when I am not wearing make up.

    Spot on Sam... T.acid for VEGF 1 modulation. Works wonders especially for the ETR variant of melasma. Unless contraindications, its my go to oral medication. Safe, effective, cost effective. ....Friday, is my melasma day, 6 patents in an afternoon! zzzzzzzzzz

    I just checked and it looks like it isn't approved in the USA yet. I wonder if the doctor will approve it for off label use? I've had melasma since I was pregnant 17 years ago! It has faded, but I still have noticeable spots. I've tried everything and nothing has made much of a difference. The only "good" thing about melasma is that I've used sunscreen religiously since I was diagnosed with it and learned that sun exposure makes it worse. I would like to be rid of this for good. Especially now that I'm in my 40s and am battling other skin woes .

    I haven’t found anything that really works for me. It is impossible to hide from the sun in summer. You can’t wear a big hat everyday specially going for lunch on a day to work. I find that every treatment is just a temporary effect if there is any positive improvement at all.

    What if your melasma is in the dermis rather than on the epidermis, will topicals work on those?

      No, topicals don't work for dermal melasma as creams cannot reach the inner layers of skin

      afaik it's a UVB problem - so heat shouldn't be an issue

    I use glycolic acid a couple nights a week at night. Can I use hydroquinone on top of my AHA?

      Pretty sure you can, but not simultaneously. Like, my doc told me to use Retin-A in morning and Hydroquinone at night(even though it's supposed to be twice a day).

    When I was preg w my 7 yr old I got melasma on my forehead n lil on my check 6 weeks after giving birth it went away than I got preg w my 5 yr old n it came back same place but after birth it didn’t go away Instead it spread all over my face n for a yr now I hav used Hydroquinone n other topical melasma products n see no difference whatsoever why isn’t anything working? What else can I do?

    Ho how long can you use products with arbutin or liquorice in them , do you need rest breaks with arbutin as it’s a derivative of hydroquinone or can you safely apply twice a day indefinitely, should you take rest breaks with tyrosinase inhibitors?

    Hydroquinone worked for me the first time but melasma came back...and hydroquinone made it darker. I am now using kojic acid with other components I don't remember in a cream named lasderma, no change still. And of course using tons of sunscreen. Does someone has some advise that works? Dr. Sam never responds =(

      Try lightning cream with kojic acid , arbutin , vitamin c , niacinamide and use a retinoid at night , if your a beginner to retinoid then try differin adapalene gel 0.1% every night to increase cell turnover

    Mine came on after stopping progestin only pill....2 years and it's still here :( don't understand why it goes away for aome and not others! Makes no sense. Done all active ingredients and hasn't budged at all.

      It's hormonal and yes it has a cure.. A permanent cure.. No peels, no lasers.. Melasma tends to return after peeling and lasers.. It also return when u stop tranexamic acid

    If I’m still on the pill and having issues with malasma, is it worth treating it while remaining on the pill? I was told by my doctor that as long as I’m on the pill it’s not going to go away. So treating it with using retinoids for example is pointless until I’m off the pill. Can you please give me your opinion! Many thanks in advance.

    I had my thyroid removed in July. My PCP put me on thyroid replacement hormone therapy. Can I still take tranexamic acid while on HRT?

    Hi Dr Sam! Why is physical sunblock better than chemical? The ppd is usually higher with chemical sunscreens with newer filters (tinosorbs, etc), so im a bit confused as to what to choose (im prone to pigmentation). Thank you!

      Hey Lavi Dillon 🤗 I am not a certified esthecian or dermatologist but I have read and researched on the topic well for my mom and from all that research , I have found that 1) melasma in your case is most likely due to all that hormonal imbalance after or during pregnancy or due to lack of b complex or sometimes even thyroid imbalance.
      2) it mostly likely gets cured after pregnancy but one has to prevent their skin from sun damage as it's known to further more aggravate the pigmented skin issue.
      3)Also, it's a known fact that retinoids like Retin-A can cause more good than harm to the baby and even the mother itself since it's known to have damaging effects on the fertility of the mother and the baby itself can have the added risk of developing disorders like depression and anxiety.
      On a concluding note, try to just avoid using retinoids especially in oral forms since you can risk putting your baby and your health in danger ..

    Hi Sam. I'm not quite your perfect target audience being a male...sorry! I am 51 and in the last 5 years or so, I've developed quite pronounced melasma...nose bridge, cheeks and very symmetrical pattern on the forehead. I am termed olive skinned I believe. I think as I am of tenuous Indian decent and with holidays of the past with out protection I'm quite prone now. I have used hydroquinone but stopped due to fear and now trying Meladerm and P20 protection spray./cream, but it seems the results are minimal if nothing. My question is do you have experience of melasma in males and if so, any advice? Can a male take the oral you mention? Also is there help from the NHS in this respect? (melasma) I feel distressed with it daily, particularly as I can't cover it with makeup to any degree. Thanks for listening. Neil.

      I was wondering the same thing , some people report hair lose and that their hair getting grey after they stop using the pills . My doctor. Prescribe me the med and I am still debating whether I take it Or not .

    Oral tranexamic acid is the only thing that helped my mostly dermal melasma. Unfortunately, it started to come back almost immediately after stopping the pills, so topicals are important to continue. Melasma is such a pain, but luckily (for me at least) it has faded considerably as I've gotten older.

      I had the same experience. It faded substantially while I was taking the tranexemic acid and then starts to darken almost immediately once I stopped taking.

      What is the side effect when you use the pill ? I am still debating whether to take the pill or not . But if it come back when you stop taking the pill , I rather find other alternative remedies

    I have melasma To have a good coverage does my mineral sunscreen need to contain both zinc and titanium ? I’ve noticed that some mineral sunscreen contain zinc with other ingredients but no titanium. Would I still get good protection with a sunscreen with that no titanium on it.

    I just want to say thank you as I saw a previous video and introduced azaelic acid to my routine. I also started using the foreo ufo using green light at the same time so not sure if that had an impact (would love to hear your views on the efficacy of that). My melasma is finally calming down after years of chucking expensive creams with promises at it with no effect. Thank you so much. Just one question in a previous video you mentioned a glo therapeutics product which doesn’t appear to be available any more. Any good alternatives?

      yes, hairfall and bloating and as soon as you stop tranexamic acid your melasma will return
      it's better to treat the underlying cause.. Melasma is an internal issue!

    Thank you so much! I’ve tried hydroquinone, and the melasma lightens while using it, but comes back after the 3 months. I have olive skin, so I’m worried about lasers. I’m off the hydroquinone for 3-4 months and want to go back. Is it safe? My dr didn’t even want to make a follow up appt!

      Bounce-back for melasma seems to be a real problem. Mine's def a LOT worse. Dr Sam, have you any advice on this for us, please?

    It's so annoying to hear it be about pregnance, too much sun, the pill etc. These same things are mentioned in every video. None of this has anything to do with me. Never been pregnant, hasn't used pill for long time, haven't had any sunbathing. Have had too much sun 1-2 times but that was 20 years ago. 😎 😎 😎

    Hydroquinone @ 4% made my melasma worse and ruined my skin when I stopped using it!!! Never again

      Laura Meadows, where do you live?, ¿do you apply and reapply sunscreen?, ¿use hat, sunglasses and umbrella?, because these skincare habits are crucial to lighten the melasma, I hope your reply🙂

      @@mariac.delgado6780 yes I wear sunscreen every day, never go in the sun and always wear a hat when sunny. I live in the north of England too. I have had melasma for 5 years and used hq 2 years ago. After that I have managed to lighten it myself with vit c, retinol, niacinamide and latic acid and of course sun cream daily but it has taken alot of time and money to get my skin texture back to normal as it ruined my skin. I truly believe hq is not safe to use whatever the circumstances

      Laura Meadows, I live in the tropic, ideal ha?, I've asked you those questions, because although I've followed all my dermatologist recommendations, I've noticed that everytime I use hydroquinone 4%, my spots have taken a darken brown color, but ¿what do you mean by it changed the texture of your skin?, it hasn't changed the texture of mine, has worsen the color of my spots, ¡enough for me,!, really I don't understand why our skin behaves in a different way with it, I congratulate you because the other treatments you used have worked for you, did your dermatologist apply laser to you? Thank you for your reply and loves😘

      Used hydroquinone and it removed my pigmentation but with summer it came back. Now they don't sell it in my country. So I suffer repeatedly. Was very satisfied with Cosmolan clinic procedure, skin got clear. But pigmentation keep coming back with spring

    Does the concentration of uv filters affects the efficacy of sunscreen and how do we determine or estimate that from their position in the ingredient list because I have seen many uv filters come after solvents such as butylene glycol in the ingredients section
    Please help

      It is more important to look at the overall formulation of the finished product rather than individual filters. Solvents are important to solubilize the sunscreen filters.

      @@alicedsouza2528 look up the Instagram of kindofstephen. He is a cosmetic chemist who goes over this particular question. Apart from concentration of filters, the form (size, shape, encapsulation) also affects how evenly the filter particles are distributed and this in turn affects the UVA protection.

    I need help. I cant go out because my melasma is right on my face and i hate it. I dont want to be alive looking liike this. Tried creams and nothing works. Just staying inside like a freak and cant even go to the shops. I have to get shopping delivered. If theres no cure for this then I would honestly rather be dead. Scarred by the sun at a time my long term girlfriend left me. This has ruined any hope of meeting anyone new. Just an ugly freak with weird shit stains on my face.....not my arms or anywhere on my body where it wouldn't really bother me....no it's right under both eyes where it can't be covered up. I'm 38 and my life is basically over.

    Hi Sam. I'm not quite your perfect target audience being a male...sorry! I am 51 and in the last 5 years or so, I've developed quite pronounced melasma...nose bridge, cheeks and very symmetrical pattern on the forehead. I am termed olive skinned I believe. I think as I am of tenuous Indian decent and with holidays of the past with out protection I'm quite prone now. I have used hydroquinone but stopped due to fear and now trying Meladerm and P20 protection spray./cream, but it seems the results are minimal if nothing. My question is do you have experience of melasma in males and if so, any advice? Can a male take the oral you mention? Also is there help from the NHS in this respect? (melasma) I feel distressed with it daily, particularly as I can't cover it with makeup to any degree. Thanks for listening. Neil.

      eweepurburger try covering with a colour corrector first like peach or orange then apply foundation
