  • Levels Chief Medical Officer, Dr. Casey Means, chatted with our advisor, Dr. David Perlmutter on the discovery of how the elevation of uric acid in the body is a causative mechanism in the development of metabolic diseases, and one of the key links between obesity, diabetes, heart disease, neurologic diseases like dementia, ADHD, fatty liver disease, and more.
Sign Up to Get Your Ultimate Guide to Glucose: levels.link/youtube?...
    They also covered:
    - The several mechanisms of how uric acid causes cellular dysfunction and insulin resistance, including oxidative stress, inflammation, and even preventing insulin from getting out of the blood stream effectively.
    - The evolutionary reasons why the metabolic dysfunction and weight gain caused by uric acid would have benefitted us historically in terms of low access to food and water, by letting us more easily pack on weight and raise blood pressure. And how we now live in an evolutionary-environmental mismatch where this survival mechanism is now hurting us in the face of caloric abundance.
    - The different dietary and environmental factors that generate uric acid, including fructose, purine rich foods and umami foods, dehydration, alcohol, and more.
    - The optimal levels for uric acid.
    - How uric acid impairs our hunger signals, drives us to eat, and even promotes risk taking behavior. We talked about how to reverse high uric acid levels as a way to unlock optimal health.
    00:00 - Intro
03:12 - More than just gout
    05:06 - A causative role
07:44 - The number one cause of death

I am 66 and use no pharmaceutical products - prescribed or over the counter for any reason whatsoever. I used to be morbidly obese according to two medical doctors. I used to have a fatty liver condition. I used to have pre-diabetic symptoms. I used to have edema - swelling in the feet and lower legs from excess fluids. That edema sometimes caused serious infections called cellulitis that would require hospital stays and intravenous antibiotics. I used to have COPD symptoms from decades of smoking. I used to have occasional high blood pressure and other chronic health problems for decades. All of these are gone. And they are gone because of intermittent fasting and eating high quality locally produced real foods.
For just over two years, i have been eating just one good meal every day that i prepare myself. I avoid sugars, all highly processed junk 'foods' and excessive carbs - no breads, cereals, pastas, etc. I am healthier and stronger than i was 10 years ago. It is almost like aging in reverse! Barring any serious injuries, there is no need to see any medical doctors. I have no plans to see any in the foreseeable future.

@@doddsalfaThanks very much for taking the time to reply with advice! While I was a vegetarian I bought the lower carb Ezekial bread that was very expensive but I love bread and it was rated low carb. I also decreased peanut butter with just enough so I could taste it. If I went for a Subway sandwich I'd get the thin wrap but it was rare I ate in a restaurant or fast food place so that doesn't really count. I did make the big mistake of having a lot of Atkins candy like the peanut butter cups that were almost no carbs but high in "protein" which was the whey protein that for me triggered gout. So I tapered that but even then I'd still get gout attacks so I stopped Atkins candy all together and the gout went away. I at the start of keto or Carnivore Diet got gout two times. I learned I'd get it once, but after that I didn't get any gout attacks, but I do take Allopurinol and maybe some day I can quit that too. My uric acid was 5.4 last time about 2 months ago. I was very dehydrated and exercise riding my bike to exhaustion creating a good workout but not drinking enough water and learned the hard way getting two gout attacks even though I'm on CD, so on CD I don't get them as long as I stay adequately hydrated. So the longest time spans ever of no gout on CD, but I'll look into your veggie diet. what kind of veggies don't lead to gout? What is your diet like? Thanks again!

My grandmother walked everywhere she went, ate mostly out of her garden which took up the better part of her back yard and worked in that garden often. I remember her taking the peels and veggie scraps out with a shovel and burying it in the garden. She drank milk, ate butter, made homemade bread every week, baked cookies and cakes and would eat the fat off of the roast along with mashed potatoes and gravy. Breakfast was eggs and bacon when or oatmeal. She had "juice glasses" that would maybe hold a half a cup of juice that she would squeeze from oranges. She had sauce dishes that held homemade apple sauce and other fruit sauces. Meat and milk and dairy were from local sources. She died one month before turning 104.

I told him that I had been to the endocrinologist a couple of years back and he told me to watch my TSH levels. They came back very hi. I begged the doctor for some of the tests that Dr. Berry and his colleague recommend as part of a health screening and he said no. No Uric acid test recommended. So none done. I just can't wrap my head around how these doctors practice medicine. My experience has totally been that they don't want to find out what's wrong they just want to cram a pill down your throat and damn you if you challenge or question their approach!

He was savagely attacked as a charlatan by Ansel Keyes who was promoting the heart diet hypothesis which proposed saturated fat as the enemy.
Ansel Keyes won hence the change in western diets from our traditional animal based foods to margarines, trans fats and seed oils. The demonisation of traditional fatty meats, eggs and butter continues.
The results can be seen in the health, weight and fitness of the American population, closely followed by the UK, Australian and NZ populations.

Sorry what's left to eat ?
Doesn't it depend on what's wrong with you and what you may or may not be sensitive to ?
Our grandparents and great grandparents did not have gluten sensitivities and they ate some sugar and salt but generally were healthier.
So possibly gluten etc are problems now because of how wheat etc is processed or even grown ie GM.
They consumed less sugar because they cooked and ate unprocessed food ie no hidden sugars. They couldn't afford to over eat either.
Our soil and seeds are not natural anymore and poor quality. This will be highly significant but is not discussed.

So stick to avocado oil, Olive Oil, coconut oil, ghee, butter. There's also a long list of really low carb veggies that are excellent. AVOID ALL fruit juices because they are high in fructose = sugar. Don't consume any vegetable oils. That should get you started. There's hundreds more.

      Thanks, I understood that.
However, there's something not right that we have to avoid naturally occurring carbs incl. fruit.
So I'm not sure that food science has got it 100% right.
When I look around graveyards I can see many long lives were lived eating a little ( or maybe a lot!) of every food type incl. sugar but little would have been refined and even the sugar would have been unrefined cane sugar ie more like molasses.
So the soil would have been better, the flour less processed and refined, no GM seeds.
But also I suspect there are toxins in our water, air & food that may not be known that are bringing about metabolic illness associated with food.
      Why?
Because we can't eat what our grandparents and great grandparents ate even if we don't eat processed foods.
So maybe there is an elephant in the room that we don't see !

All the elders in my family have lived well into their 90's and very healthy minus a few exceptions and some aches and pains. In Colombia food was and in many instances is still grown without a lot of chemical enhancements. I have spent the majority of my life now, in the USA, looking to avoid foods infused with all kinds of garbage and I'm sure you know what a challenge that can be. There is a lot of information confirming that the grains used before the industrial revolution were indeed whole and not refined, now you can't even find those kinds of flours and people who have tried have great difficulty finding the seed material to replicate that kind of wheat etc. I have been off of sugar for a year and a half and on a very low carb diet. I am by no means on the perfect diet but just this little bit has helped tremendously. I do eat real food without additives but it is a real chore in today's world to eat healthy. There are so many temptations, lack of time and maybe also laziness to cook. Easier to pick the easy way out. I do love my sweets so when I hear people say that once you give up sugar for a few weeks you don't crave it....... all I have to say to that is I don't believe it. Do you try stevia as a sweetener ?
I'm also wondering about coconut sugar because it may not be refined so perhaps it's not the sugar itself but the actual cane that is grown and especially the chemicals used in the processing.
As I said ( I'm in UK) & the gravestones speak ! They didn't warn us off sugar and carbs ! but many couldn't afford to eat excessively and there were no mass produced snacks - I avoid snacks when I'm out & about 'cos they contain nothing but chemicals.
Anyway, good on you to manage a low carb diet and no sugar!.
I can't quite do that ie my carbs are around 100 g a day & I use honey as an immune booster every day.
So I aim for portion control and not snacking and doing a minimum 12 to 15 hr fast.
This appears to stop weight gain and I feel well on it & don't need meds.

There needs to be studies, replicated, positively confirming Dr. Perlmutter's hypothesis. Frankly, after all the hype over grain consumption being the source of all ills, it is now hard to immediately accept his latest idea. I have heard nothing presented by him that clinically establishes the veracity of his sweeping claims.

Anecdotally, I suffered from chronic gout (Uric Acid levels) as a function of how much fructose (HFCS) i was consuming and creating metabolic dysfunction, for nearly 8 years. I would have about 2 flares each month. I started a carnivore diet in January this year and had the Uric Acid levels measured each month (decreasing from 9.2 to 5.9) by June (this year). No HFCS, Sugar or substitutes or alcohol. In March (going back in time a bit) I stopped taking Allopurinol. I had also lost 42 lbs by May.
Two weeks ago I wanted to test my response to HFCS and consumed 13 grams (intentionally). I had (and still recovering from) a gout flare within 10 days of consuming the 13g of HFCS. After 7 months I was shocked and thankful that I had Colchicine and Allopurinol. So it appears that I have more work to to on my metabolic health.
Thank you for publishing this video and the contribution of Dr. Perlmutter.

Now as they are old and stuffed up with wealth or simply retired they’re trying to whiten themselves and suddenly become a hero. Their ‘coming out’ means little to me the older they are! I respect YOUNG doctors who are able to resist the wealth the pharma offers for upselling their drugs and keeping the patient with chronic disease for long long time 😢

This was a superb video (for me). I’ve been obese for years. I was slender all my life & when I began to feel the shift , told my family physician I thought my metabolism had changed. She said the metabolism doesn’t chance. I’ve taken ALL the shame & blame as I blew up. Now I’m 73 yrs old, a former dancer shame makes me unwilling (and unable) to walk or even stand without pain. I swim to get things moving . But I absolutely crave carbs. I drink smoothies (organic fruit). I’ve been told I just let myself go. I’m a shame. I have to “face this “, etc. I can assure yo shame or being yelled at daily has not worked. I DID, however, buy a glucose monitor when I became interested in intermittent fasting. Recently I found the two medicines I WAS ON caused weight gain (1) and the inability to lose (2) weight. So again, what I assumed was my weak character flaws & failures while water fasting were perhaps exacerbated by simply not understanding. Namely, the role of sugars as presented here. Get that monitor and starting over. Wish me luck. Need both books (gave his grain brain book to a neighbor who was diagnosed with Alzheimer’s ). Thank you for your guest today. Thank God for recommending I watch it.

Hi Ziggy at the beginning of the pandemic I started 2 regimes..firstly intermittent fasting and secondly the wim hof method.
Intermittent fasting of 16:8 works but I then started a monthly 48 hour fast to supplement the daily fast. Tough at first but actually something I found I started to look forward to and extremely effective to loose weight.

I have no medical knowledge but following a crash and reading a book by a woman who was in a wheelchair young from arthritis and decided to stop all the meds and use researched old remedies on diet. The key ingredient was Cider vinegar and honey in warm water first thing every morning. Within a month of being off the meds and a diet she worked out she walked for the first time in ages.
So to avoid getting arthritis in damaged joints etc I started taking the cider vinegar honey in warm water every morning. What happened was that my acidic stomach and bowel disappeared and never came back. Before that just a cup of coffee was enough to cause me a problem. An older neighbor who had a few arthritic joints decided to try it and told me that a surprising side effect was that cramps in his legs at night had stopped happening. He only ate fresh organic food and didn't drink or smoke so he had a healthy duet already. So I suggested to another elderly neighbor who suffered badly sometimes from gout to also try it, and wow, it also reduced his gout to almost zero. It came back if he stopped taking it. I don't know how it works or if relevant to this discussion, but I'm just sharing my experience with cider vinegar.

I was taking maybe a dessert spoon at most in a small glass of water.. the honey, apparently a relevant ingredient in the mix, I added about a teaspoon ful. It wasn't exact.. in the book the woman varied what she took. She suggested anyone going to drink alcohol to take the same glass before going out to drink..
You could start with just a teaspoon of cider vinegar and I don't think it matters how much water or honey.
One thing to know she said if you have quite bad arthritis is that it can take 3 weeks to a month to see a difference and in those weeks you can have more pain in some joints because - as I visualized her words - the cider vinegar is breaking down some hard stuff jammed in the joints and that's the cause of the pain..but once that's gone the joints feel easier.

I have gout. I have had it since 17 at my most fit time in my life playing on 3 soccer teams.Diagnosed finnally at 24. 7 years of hell and doctors like this who know just enough to be completly wrong and misdiagnose me. Gout caused by high uric acid serum levels is a genetically inherited disease of DNA metabolism due to the impaired metabolism of the purines in DNA. All primates have this metabolism somewhat compromised. Sugar has ZERO DNA. Fructose is not directly metabolized to uric acid as this quack aserts. Most food does have DNA in it except refined foods like refined sugar. He is conflating obesity and sugar metabolism with DNA metabolism. Some skinny people have gout. Obese patients with type 2 metabolic sydrome x likely have gout from excess food hence excess dna as well as comnpromised kidney function the inability to excreet enough uric acid given the amount ingested over time. Fat people eat more food and therby ingest more DNA and thats why uric acid is high. There is no conection to sugar metabolism of fructose which does not require insulin. The only connection is a renal one(kidney) and how having a constant high blood sugar and insulin level impairs the kidneys function to excrete uric acid. High uric acid is a symptom of another serious disease like cancer or over eating not a cause of weight gain diabetees or heart disease. symptom of problems NOT cause. Fructose does not become uric acid EVER in scientific reality.

Bottom line: reduce fructose, increase exercise, take quercetin, stay hydrated, reduce carbs and sodium, do not drink alcohol or drink juice or processed foods, eat meat in moderation, as they will slow down your metabolism, increase inflammation and increase fat storage. It’s important to understand that no natural food-including fruit-is inherently bad. The overall composition of your meals matters more than any individual component when it comes to your health. The extent to which a particular fruit impacts your metabolic health depends primarily on:
      - How much sugar is in it
      - How much fiber is in it
      - And the glycemic index of the fruit

I developed an addiction to sugar in my thirties and now have been obese for twenty of my 76 years. Looking in the mirror has been a source of shame and frustration that I blamed on my lack of willpower and discipline. The medical community only reinforced that opinion with comments to "not eat so much". Now, after being diagnosed with pre-diabetes (my doctor suggested eating more beans) it seems logical that I need to reduce my sugar intake to almost zero and increase my protein intake as well (more beans). This video confirms that my decision is correct but also points out that reducing my sodium level is just as important.

I tried ‘The Atkins diet’ which was fashionable at the time and it worked amazingly well. 15 years later my weight is still under control and I am very fit and healthy. I remember at the time Atkins getting almost universal condemnation from just about everyone, everywhere in the medical profession or media. Researching my symptoms I then stayed away from the typical foods and drink that exacerbate the condition but still had another flare-up. I then researched my medications and discovered that my blood pressure medication, a diuretic, can cause uric acid levels to go up, plus delete potassium and magnesium, which no doubt contributed to my episodic severe depression. Neither the literature attached to my prescription nor did the doctor warn me about uric acid or mineral depletion nor were those things checked in my blood work. Not so much that doctors are “owned” by Big Pharm as much as healcare schools are highly funded by Pharmaceutical Companies which affect the curriculum, leaving no time for relational metabolic courses. It’s not just medical schools as much as all healthcare schools. In the 1970s, a Nutrition class was a required course. This no long the case and is not even an elective. Much scientific research has been done since then, but is still not included in the healthcare curriculum. A relatively new specialty of Metabolic MDs is now available but not covered by Medicare and most insurance companies which puts it out of range for most people. Cardiologists are becoming advocates of this but one must already have the destructive process evident. Sort of too late.

    • @MargueriteFairProductions
      @MargueriteFairProductions Рік тому +3

@@keithcitizen4855 No, I disagree completely. What we eat determines our level of health. Yes, he ate it when he was younger, but that is where the answer lies. Had he known more about food reactions with the body, he would not have been eating that way and extended his life. Even a younger person who's immune system was compromised from something else could die from eating that food later at night and then drinking orange juice. It was not old age that killed him, it was not understanding food combinations, not eating a high salty meal later in the day and then adding all that sugar into his body. He died from an explosive food combination. He had no medical conditions, took no meds. It was his last meal. So the point is, if we understand as we age, that our bodies change, we start losing digestive enzymes after age 40 and we do not get proper nutritionists for our new older bodies and do daily exercise, we can extend our lifespans and then simply die in our sleep when our hearts electrical current finishes. I have been studying this for a long time. He died in 1984. I have made many improvements to my diet and reversed a lot of damage from my studies. I appreciate your opinion, which you are entitled to, but in this case my conclusion is based on facts and understanding the human body. Stay healthy. The very first time I came across information that identified uric acid as a "problem" was when I read the book "Diet for a Small Planet" back in 1972 when I was 15 and considering becoming a vegetarian.
The uric acid argument was used to make the case about how eating meat generates way too much uric acid for the body's ability to eliminate it. High protein meant high uric acid.
Ironically, we are now learning that FRUCTOSE is a major player in uric acid production.
I became a vegetarian for a full 15 years, but when I finally returned to being an omnivore, my health, vitality, and cognitive skills took off dramatically. I went from being a low-level technician to becoming a high-level computer programmer and software designer.
Now I am getting old so I am needing to "up my game" and learn some new tricks.

In 2016 I was diagnosed with osteoarthritis. Lab results showed elevated uric acid so my doctor gave me dietary instructions intended for people who suffer from gout. Basically, it was aimed at reducing intake of purines. Now, I am a meat eater and can hardly survive without meat. Otoh, I have already been on a reduced carbohydrates diet and close to zero refined sugars. No fructose syrup is used where I live. So I was directed to reduce meat intake. The food that was deemed safe was carbohydrates that I despised. In a rather short period of time I managed to reduce my uric acid by dietary measures to very low, but my rheumatic pains persisted. Now, it is very possible that I had high levels of uric acid for many years but didn't know it as it has never been measured. It is of note that I started suffering from digestion problems three years prior to developing osteoarthritis. These digestion problems were not taken seriously by the doctors who simply said it's irritable bowel syndrome. I was very puzzled and still am to this day as to what caused those digestion troubles as I didn't smoke, barely drank alcohol, avoided refined sugars and limited other carbs, ate sufficient fruits and veggies and was reasonably active. Possibly ate more protein and drank less water than necessary. So, I while I think that corn syrup is certainly bad and should be banned, it cannot be the only cause of elevated uric acid. There ought to be other factors at play.

They were not trained on it and don’t care to do their own research to be a truly effective doctor. They think for some reason it is not important for them to do their job because the first question they ask, when you walk in, is “what brings you in today?” That way all they need to do is figure out what drug to give you that they were told is for that ailment. So many people in my life "men" that drink alcohol, eat grains out the ears, are way overweight and have been for years, that should have had gout by their 60th year have had no incident remotely resembling gout. WOW! Finally someone closes the loop. This is indisputably the best explanation of the human (and animal) nutritional and digestive mechanism I have ever learned. Clear and complete. I am just shy of 60 yrs old and never have I ever completely agreed with anyone else's conclusions over this matter. There was always something wrong particularly the frustrating debates over meat and plant based diets. I have always believed that all living things evolved by adapting to their natural environment in order to survive. This is a law of nature be it for omnivores, herbivores, panda bears, black flies and whatever else, but you have included the important factors of when and where mother nature offers the food. As you say, berries come out in early fall. Bears will prepare for hibernation by stuffing themselves with berries, fructose, in order to create as much fat as possible. This fat keeps the body warm and the brain alive. Fat is by design a good thing. I could never get my head around our own body producing something bad. You nailed it.
Humans are the supranatural beings that have adapted their environment to themselves. We have modified our food through processing and distribution to the point that time and place is insignificant towards eating anything we desire. Our survival doesn't absolutely require health, until we become monstrous at which point, by definition, reproduction stops.
Our only natural predators are organ failure, microbes, poisons, and considerable cellular damage or mutation, each of which we continue to create unnatural methods to fight against. Natures only remaining influence is the attraction to reproduce. The next beings evolving from us will be just a torso with a huge head, eyes, ears, nose, a mouth and a butt hole. Of course internally they will have respiratory, circulatory and digestive systems. Humans will birth them, heal them, feed them, transport them, protect them and praise them as beings of superior intelligence.
Again WOW. This insight is truly the huge breakthrough modern day man (woman) needs to recover our health. It is a foundation upon which our knowledge will grow. It will influence all sciences, and perhaps become the holy grail to reverse the aging process. A giant leap for humanity.
Thank you so much Levels and Dr Means and Dr Perlmutter for making this subject so easily understandable despite the scientific terminology. I have subscribed and look for forward to many more of these fascinating lectures.

I have seen over 7 doctors, and two foot specialists, and even one gout specialist about it. They gave me a diet to stay away from meats and alcohol. Finally, about three years ago I began to suspect high fructose corn syrup caused my severe gout. I had gout in both legs and could only painfully crawl around my house for 3 weeks. I finally went to the emergengy room and 45 min after sitting there in pain. The doctor came in and said, so what the f##k do you want from us today ! I told her pain relief from gout. She told me the clinic had never seen me for goutlike symptoms before ! I told her I had been coming to this clinic for 50 years and had seen at least 10 doctors for gout ! I finally changed to a new clinic and got on alopurinol. Now I know it was sweets and sugars causing my gout !!! All those years of suffering could have been prevented had I only seen a knowledgeable doctor.

My uric acid level is 6.9 mg/dl. I lift heavy weights every day and run 5-10k almost on a daily basis. But I do drink a lot of water, with lemon and lime sometimes, I don't add any sugar in it. I eat healthy, I don't supplement my diet with any protein powder, I just eat chicken thigh or steamed fish (flounder or bronzini) as my protein source, sometimes scrambled eggs. My daily diet always comes with a big bowl of steamed vegetables such as broccoli, cauliflower, and carrots without adding any salt, and raw celery stocks. I don't eat refined carbohydrates such as noodles/rice/bread. I NEVER drink any alcohol. I take vitamin D3/K2, I take cod liver oil, and potassium citrate just to be safe. So I am wondering whether my high uric acid level (6.9 mg/dl) is caused by intense exercise (excessive breaking down of muscle fibers)? Is decreasing my training volumn the ONLY solution? I honestly would like to ADD training sessions as opposed to cutting it down. I am doing everything I can to keep my uric acid level low while maintaining my training volumn. Please let me know if there is any way I can try to lower my uric acid level without sacrificing my exercise routine. Thank you!!!!!
Also, I do 20-4 intermittent fasting, which I know would lead to higher uric acid level, but I think the overall benefit of fasting outweighs the cost. I never had gout, but I want to keep a lookout for it.

Hi, Im 62, Pretty good athlete with a similar training regime and diet regime to you (low carbs no sugar, probably too much fructose through pure fruit..apples, berries etc) . My body weight is above normal BMI BUT bodyfat about 20%. I can drop weight to normal BMI but I ;look like a skeleton when I do this and I lose strength as judged by major lifts. The upside of any lost weight is that my cardio improves a little. My VO2 max is 40-45 depending on exercise (elite at my age) . I prefer rowing. I got a uric acid meter (blood test) and have been using it to monitor to uric acid levels with a view to managing gout attacks that occur every 9 months or so. Here's what I have learnt by testing things and seeing what impact it has on measured uric acid level.
My base uric acid is the same as yours. This is top end of normal.
1] It is tough to shift uric acid level. I was obese 40 years ago but got all the weight off and kept low carb zero sugar pretty much all the time.
2] With more focus over the last two years it seems the attacks are getting better. Less pain etc. I am aiming to extend the time between attacks. I take no medication other than ibroprufen during an attack. Maybe four does over a couple of days.
3] Water intake has NO EFFECT on uric acid level. I have gone periods of time with dry, (no water at all) heavy exercise (half marathons) and at other times drank four liters of water a day for a few days. There is no measurable difference in UA levels. The available meters are error prone so I often take multiple tests at anyone time to ensure improved precision. I do a half marathon every week on the rowing machine. I have tested this multiple times.WATER has no effect on uric acid levels in the blood.
4] My glucose levels are normal judged by the blood tests (again measured 6 times a week)
I have tried multiple protocols to bring UA down. The only way that brings the level down reliably is:
      a) A 45-48 water only fast. Take note that I have always intermittently fasted - i.e 40 years. It is my normal way of eating
      b) After 12 hours of the fast I row a half marathon (21km). This is at base rate (level 2 or what ever you want to call it. Heart rate at 135 BPM . Although old my resting HR is 49 and Max HR is 180 (I measure max HR every two weeks.. interval up to failure over 20 minutes)
Before the end of the fast I measure UA levels. These drop somewhere between 20% to 50% at the 40 hour mark. For the record I do a heavy weights session (compounds lifts etc) before I break the fast.
I break the fast relative gently and go back to intermittent fasting BUT two days later the UA level are back to your 6.8mg/dl again.
It is tough to shift UA level.

Bottom line - Doing more exercise is likely to help.

@@DrBobCherry Thanks for the reply. My UA level now is actually around 8-9mg/dl, super high! But I never had a gout stone attack, and I have always been in shape throughout my 33 years of life. Two best ways to lower down UA for me personally: 1. drinking at least 3 liters of water daily; 2. stop intermittent fasting (I do 20-4 fast daily, and I also run at least 6k daily, AND I also lift weights daily). Some studies find that UA elevation that is caused by fasting does not lead to gout stone attacks UNLESS the subject had gout stone attacks prior to adopting the habit of fasting. , ; and.

@@sx99cornell I agree w/your conclusion, the TRE is too limiting, and likely breaking down tissue, thus raising UA. I’d consider spreading out to two meals 4/5 hours apart. Eating window of 6 hours total, or even up it to 7.
Congrats on your self-activism. But I’d avoid obsession w/one’s physique.

Dr. Perlmutter-- I bought your book "Drop Acid" and also a uric acid home test kit. I was unable to reduce my uric acid with diet alone, but after monitoring my uric acid and my blood pressure I was able to demonstrate a direct correlation between my uric acid level and my blood pressure. Now on Allopurinol, my blood pressure has come down from 149/82 to 128/72 or even on occasion, 109/68. (I should include I am type 1 diabetic for 56 years.) The proof is in the pudding as they say. You are 100% correct! Thank you!

I'm 13 years years younger than you, but was also diagnosed with gout at 14. Never medicated for it, but doctor's advice was basically to lose some weight, avoid offal (in the UK we eat things like steak and kidney pies), and always keep hydrated. Probably had five or six minor attacks over the years - most relating to food. More recently, I have gone low-sugar in my diet and lost some weight, and supplement with Quercetin, Rose Hips, vit C, vit D, and so on. Glad to have confirmation that what works for me is the right thing for my body, and has a scientific basis.
