    I am RA patient since i was 19. I became a high protein carnivorous about 10 years ago, got off of all meds and no pains. Work out 5 days a week and decided to become a personal trainer last year so here I am, now I am a personal trainer at the age of 51.

    I follow and have followed numerous Keto/Carnivore video "Doctors" over the years.
    Many are just show ponies and lack any real experience as practitioners and are soon relegated to the Unsubscribe bin!
    Dr Cywes is by far one of the most articulate, accurate and admirable people on the subject. I have gained so much more insight because of his ability to convey information.

    Thank-you for covering this!! This is very relevant to me as I started to develop uric acid kidney stones on a strict carnivore diet. I had no issues with stones as a ketovore. My urine pH is very acidic. I thought it was purines, but now I understand this must be protein wasting. Light bulb moments here. Looking forward to the next video as I'll need to make some adjustments to the diet.

    Just over 4 months carnivore, finally symptom free! Still on 2 of my 4 meds - but that's good going! I have very aggressive and severe RA, and I've had it for almost 20 years. Amazing how removing crap from your diet actually allows you to heal. So simple.

    That was really interesting! I was wondering, if I need to worry about uric acid, for a while. Gout sounds like an absolute nightmare.
    I'm team 'high fat', though. Lean protein just isn't satiating to me. Almost like carbs. There's just nothing like fatty meat, to trigger that 'switch' in my brain, that truly means I don't want food anymore for the rest of the day.

      i used to suffer from gout a lot, i now eat more protein than ive ever done and i dont get gout except when i lapse and eat sugar/carbs.

      Before I quit drinking years ago I had several bouts of gout. I had it In my foot and it was some of the worst pain I've ever endured. I quit drinking eight years ago and never had it since. Even now since I am a heavy carnivore, I've had no problems.

      Interesting. I noticed, that I feel very achy and stiff in the morning, if I even had a little bit of alcohol. Like 1 glass of wine. It's not a feeling of being 'hungover'. My head is clear and everything. But my muscles and joints feel like I did something really strenuous the day before. It goes away pretty quickly, though. Alcohol does weird things to the body!

    In my own n=1 experience tracking Uric Acid daily, meat definitely directly contributed to increased uric acid. This stems from genetic hyperuricemia that I inherited from my father.
    After eating a heavy meat diet for a year (and eating no fructose), I proceeded to lower my uric acid from 10 mg/dL down to below 5.5 mg/dL by consuming dairy and whey protein (a purine free protein source) for a period of time while abstaining from meat.
    Context is very important when thinking about uric acid, fructose is not the only culprit.

      @@Mr-hn2bp Exactly. Although this is highly context dependent. For example, a person that is obese and consumes high fructose corn syrup-filled sodas most likely has their hyperuricemia from those factors. But someone with genetic hyperuricemia may be perfectly healthy, athletic, eat zero fructose, and still experience hyperuricemia from purine intake.

      @@notaras1985 Currently eating around a 33% protein, 33% carbs, 33% fat diet as far as protein, fat, and carbs go, but staying away from processed foods. My bloodwork was getting bad on carnivore/keto. Probably due to my northern european, grain farming genetics, carbs seem to be important for me personally.
      Dr. Cywes had a similar patient that was seeming perfectly healthy on a pure carnivore diet but needed to add in carbs in the form of honey to bring bloodwork back to healthy levels. This need for carbs probably varies person to person.

    We shared this video to our Facebook group so our members would watch this important information! Thank you for another informative video and debunking the PSMF stuff. The PSMF recipes have derailed some of our members increasing their BG and killing ketone numbers. We are high fat, moderate protein and low carb to reverse metabolic health issues with therapeutic levels of ketosis.

    Thank you as a lean mass hyper responder and this push to get my protein up between 110-140 grams a day has caused my bun to rise to 23. I just recently started having gout symptoms. My creatine is climbing too at .89. I am a 51 year old lean female. My bun/creatine ratio is high at 26. Cholesterol total 373, triglycerides 66, hdl 101. Uric acid 4.8.

    Thank you for your explanation. I've been carnivore for over 1 year and have had SOME success, buy my uric acid levels are the highest they've ever been. Everyone talked about fructose, but never about not enough fat. Going to up my fat intake and see how my levels are. Thank you again for the relevant info!

      Just an FYI, purine intake definitely does have an affect on uric acid.
      Protein turnover from muscle breakdown does as well, but its important to recognized that BOTH do this.
      If one has genetic gout (as opposed to obesity-induced gout), dietary changes alone, depending on the person, may not be adequate to lower uric acid.

      During the first 18 months to 2 years of being low carb I was eating a lot of "permitted" berries; it helped me to stick to low carb and was much better than what I had been eating before changing my WOE. Since then I've learned and understand about fructose and am now keeping them to a minimum. Reminders are good that's why I keep watching the videos and reading comments.

    Thanks for this video, it confirms my own experience with fructose and uric acid gout. I can eat meat and drink red wine and beer without having gout attacks. But if I drink rose or white wine with a large amount of residual sugar, or eat an apple or orange every day for 2-3 weeks, I get a gout attack.

      Any other suggestions? I find you comment interesting, will try other now too.
      I've always avoided red wine, beer, bread, sardines etc. sometimes a drank beer for a day (spiked with unallowed) and keept consistent further and not getting problems

    Hi Dr thank you for the informative video I had gout for many years my uric acid levels were high I was told it was because my body was dehydrated from not drinking enough water so I bought the clearly filtered water pitcher filtration system I drink a lot of water all day long and haven't had gout since who knew

    I've tried to protein sparring modified fast in the past. I feel better not eating. It just sucks the energy out of me. I asked one of the "top experts" in PSMF's and they had no clue why I reacted that way. They were also telling me about how I need to protect my muscle. I have over 200 lbs of lean mass. They have less than 100 lbs of total mass. I'm pretty sure they aren't my biggest fan and I will run into them again in a few months.

    Two things trigger gout for me. Fructose and allulose. Gout is extremely painful and crippling, so I avoid those two things like they were cyanide. I have been 99% carnivore for 3 years but tried to still drink a glass of iced tea during the day and both those sweeteners were tried and abandoned. I eventually gave up on the tea as well.

      Interesting, I've been reading/hearing more that allulose reduces uric acid significantly (Bikman). Mostly anecdotal so far, I believe.

    My bun is in the 40s. And I'm on a keto diet. I was taking a lot of amino acids and collagen. But now I switched over to whey protein. And it's still high. So I'm about to lower my protein intake and increase fat.

    Am a runner and we use a lot of carb powders with cyclic dextrin and sports gels. Could uric acid be high because of them ?

    Love this man, as a LMHR on a ketovore diet I suffer from gout, did not know why my uric acid levels are constantly high averaging between 6 to 7.5, I do try eating fatty meat but probably eating to much also am loosing my muscle. Hoping to learn what I should be eating in the next episode.

      Interesting... Could you elaborate a little more on your diet? What and how much are you eating per day? I am ketovore as well and uric acid level is 7.1 but I have not had gout since being on this diet (for 9 months now). Not sure why uric acid level is too high other than, like Dr. Cywes said, I may be eating too much lean meat or fructose. But I eat very little fructose that I know of.

      If you are dehydrated, you're uric acid will stay high due to lack of water for the kidneys to flush it out. That was my issue. I had gout from fructose. Got rid of the fructose and the gout along with it. However, uric acid remained elevated. Keep your kidneys doing their job and stay hydrated.

    Could I use BG and BK measurements to find out whether I consume more protein than I need?
    As according to my CGM and CKM readings, when I eat more than a certain amount of protein (average 330g), my BG is higher during the following day and night, and BK level is lower respectively.

    Very Insightful, sounds like I am in the too much Protien, and not using it up...My gout flares up and I have been in the Keto Carnivore lifestyle for 13 months now. Off 90% of my hypertension meds, and I lost 61 pounds so far, I need to burn up what I am intaking looks like...

    As a counter position, I suffered from severe gout for forty years. Attacks happened during times of stress in particular during the first academic term in the university where I taught. In the last four/five years, since retirement. I brought my weight down to slightly above normal BMI. Once this has been accomplished {this took about a year) I then sought to defeat gout. In the first instance I took medical advice (usual generic solutions - i,e don't eat purine heavy things quit drinking (i drink very little anyway). This advice was ineffective. I am a scientist so I bought myself an electronic uric acid meter and glucose monitor for about $20. I calibrated it against an official medical blood test. I then set about looking how to control uric acid in my blood. Both increasing and decreasing it. This is what I found FOR ME
    1] Low carbohydrate diet had little impact on uric acid levels - the gout recurred. I would add that when I control weight usually I go low carb.
    2} I had a slight knee issue outside gout. I did keto for seven months It took me about a month to get into the keto cycle. Over six months my uric acid levels increased particularly when I did cardio (I do about five hours rowing a week). The keto diet exacerbated the gout attacks and during them I had to actively manage the pain. The initial knee problem - inflammation- was not resolved using keto.
    3} I tried intermittent fasting i.e one day a week fast going up to two days a week after a months acclimatization. . I actually enjoyed the fasting and felt better BUT after exercise my uric acid level went through the roof. This was tested for about six months. I did not fast to lose weight. I maintained the calorie level to maintain weight.
    4] All the above meant reducing fruit intake to virtually zero, As the above had adverse effects I then moved to a diet containing fruits (up to 15 portions a day of different fruits including bad ones like melon and banana). Protein I got mainly from high fat Greek yogurt and a bit of fish and red meat every two weeks. Mixed unsalted nuts were chucked in too. The result of this was a slightly lower uric acid level which I maintained for about two years -The gout attacks still occurred every 9 months but were much less severe and more manageable.
    5} The final part was I noticed that if I had a sourdough sandwich late at night (a very big no no by many health standards) my glucose level dropped by one point AND uric acid levels dropped by 100 units the following morning. This really shouldn't happen!! This put my UA acid levels slightly above healthy average for the population
    After four/five years work, six blood tests week and being consistent and firm with my diet, my gout has not recurred in over 15 months. In the lead up to the "totally clear" time I had one minor attack of gout in the previous 9 months.
    If my UA levels increase I now I have to either
    a) calibrate the electric meter or
    b} I have two sandwiches before bed.
    Note that my diet is pretty much counter to your recommendations.. i.e high, really high, diverse fruit intake, little red meat and what amounts to 30% by calories in carbs. Ice been pretty much clear of gout for two years. and UA is under control.
    Conclusion
    The above to a lot of time and testing - I was heavily driven to sort out the problem and was fortunate enough to be scientific. The above process is not for everyone. Frankly I think compliance would be the downfall of anyone attempting the above.
    I have realized that my body behaves very differently to some people. Having said that I do cardio of five hours a week, strength train five times a week and I have ended up with a largely plant based diet (us tp 35 different planes a week) with fat coming from yogurt and nuts and a limited amount of meat. I keep my BMI on the high side of normal. I drink in moderation. I treat food largely a fuel or nutrition.
    If you go to the CDC web site in the States or NHS website in the UK - or even the World health organisation site) the guidance is to do what I ended up with. There are no easy or general solutions to health but the above guidance from national health bodies actually seems pretty good.

      As I know when the body is adapting to keto the ketones could be find in urine and they stand first in the queue to UA. So the levels of UA are rising at this period. And should become low after keto adaptation. In theory )))

      Are you saying that soda bread lowers uric acid levels? It's just that you say that your levels were above normal the next morning. I'd like to know as my son suffers from painful episodes of gout.

      @@versewriter8123 Hi, sorry to hear that your son suffers from gout - it is really horrible. Sour dough bread is my go to for a little bit of carbohydrate before I go to bed. Equally if my UA has been high a couple of days I can eat a small ice cream the body will just break it down in glucose what ever the source.. FOR MY body I need a balanced diet i.e equal split with fat, carbs and protein. I tend to eat a lot of fibre because of a family history of colon cancer. Other bodies will almost certainly behave differently. A universal help seems to be to keep yourr BMI around normal, drink in moderation don't smoke. Beyond that, if the attacks are bad then get medical help and testing.
      As the CDC , NHS and World Health Organisation indicate/. A large number of ailments are prevented or delayed with lifestyle choices. If everyone chucked in a few hours of cardio ans strength training a week -over together with diet described above then the pharmaceutical companies would go out of business!!!

    21:20 my gout is out of control on carnivore.....reason being is I listen to the protein protein protein push that is abundant on the internet nowadays.

    My uric acid averages about 4.4. I eat about 3600 cal/day. 40-45% protein which is 300-400g protein. This rest is animal fat. I have been dairy free carni for 5 years. I am 47 yo, menopausal, with pan ulcerative colitis, hypermobility type ehlers danlos syndrome, chronic pain, and pan hypo pituitarism. Does this mean my protein intake is not too high?

    Do you think high Uric acid in the blood, paired with high pressure in the coronary arteries could be causing the insult to the arterial walls? I asked this question on Ben’s video and never got a response. I don’t think I have heard a great explanation as to the root cause of arterial damage besides “high cholesterol”.

      High sugar levels in the blood causes glycation of vascular proteins through a nonenzymatic browning reaction called Millard Reaction. This is a browning process turning cooked food brown. Fructose is seven times as potent as glucose in this browning process due to its more strained 5-member ring causing it easier to exist in a linear conformation. Galactose is 3 to 4 times more potent as glucose in glycation of proteins.
      High blood pressure physically damages the blood vessels. Pathogens can cause inflammation.

      @@Mpantiques, when you lose weight you inevitably also lose muscle which consists mainly of protein. As your body "eats up" its own muscles, it creates uric acid as a byproduct.

    Started as a strict carnivore, but after six months I've migrated to ketovore. All just under a year. I no longer have gout attacks. At 62 yo, beautiful BP. My markers are all excellent, except for LDL which I couldn't care less about. If you have metabolic issues and haven't tried the carnivore lifestyle, what the hell are you waiting for???

    I've heard that most of the allulose in the shops is synthetic because it's too expensive to extract only that from natural sources.

      What is the difference between natural and synthetic versions of allulose? A molecule is a molecule. If it was different... It wouldn't be that molecule. I know when it comes to vitamins there are a whole family of molecules that are given the shorthand names but their molecular names are distinct.

      @@Trahloc Looking at seed oils vs animal fats there is a very small difference and that allows seed oils to fool our body into thinking it has additional cholesterol that it doesn't, thereby lowering cholesterol levels and earning the health symbol.
      Synthetic molecules could be the same, I don't know enough about the actual chemical structures of allulose.

      @@Meathead-10810 I haven't heard someone state the mechanism for why seed oils lower cholesterol before. Folks have just always asserted it and pointed to observational research. Thanks for giving me something to look into more specifically.
      As for molecules, allulose is a specific molecule. The actual chemical name apparently is D-Psicose. It's a stereoisomer of fructose. You can think of it as the mirror universe version of fructose, except it's the good one. "Vitamin E" is a family of four different molecules. Vitamin C is one specific molecule, Vitamin C from an orange or from a lab are *exactly* the same. A purified sample made from orange is 100% identical to a lab sample of the same purity. No one would ever be able to tell which is which (except perhaps by the level of purity a lab could create).

    How come, then, that gout was traditionally the disease of the kings? They didn't eat huge amounts of sugars. What they mainly ate was huge amounts of meat. So protein intake must at least be an important driver of high uric acid levels.

    DR. Cywes, thanks for another great lesson and giving me AHA moment... been on keto, then high fat carnivore. My BP started creeping up, to 170/100 and stated gaining fat! I found Maria's and Todd's Emmerich videos about Protein Sparing diet( i dont think so thwy have any medical or any health care field related education) and I thought well , I need to lower my high fat intake and start eating more lean protein . Maybe thats why my joints giving me some pain now?!
    But my question is
    Doesn't our body make saturated fat if needed?

      Yes because of the loss of water and electrolytes. Carbs retain water. When you go keto, that's the first thing that happens. Your system gets flushed. You have to up your water and electrolytes. Minimum 2 litres per day.

    This is all 100% correct. Been thoroughly covered by Robert Lusrig years ago.
    I have been a vegetarian for a decade and I developed gout (before I learned all this stuff). I have been a keto vegetarian for 11 months now and ny gout has gone, bur more than being keto I am also *very* strict about fructose consumption (no more than 4g per day).
    I now eat 4 eggs a day and my gout is gone. Previously i would eat perhaps 4 eggs per week when I had gout.

    As a gout sufferer I am puzzled. There is an idea that high uric acid in the blood is what leads to crystalisation. But it is known that gout can strike with normal levels, and that some people with high uric acid do not always get gout. Also Allorurinol can trigger gout. So what is going on - what other conditions other than concentration cause the crystalising phase of gout?

      I've heard predictions of 3-9 months and even 5 years. However, I don't think the metabolic disorder goes away just by avoiding food to which you are intolerant. I think metabolic syndrome is caused by an underactive parasympathetic nervous system causing elevated levels of cortisol and adrenalin driving high blood glucose.

I do suffer from cptsd and a bunch of autoimmune disorders along with newly diagnosed metabolic syndrome (im 38). I guess best approach to health is a holistic one.       @@ordinaryvalley Yes, that's what I was thinking, "attachment disorders" and stuff like that which causes abnormal brain development in the first 3 years of life due to chronic stress. This causes the brain to adopt all these coping mechanisms that become pathological later in life. This doctor called Gabor Maté talks about this stuff.

      Excessive carbs are poison that destroy the endocrine system and mitochondria over time. If you heal that by removing the poison, but then go back to the poison, it's only a matter of time before the endocrine and mitochondria are destroyed the second time.

    Been keto/carnivore-ish for 9 months. 2 months ago uric acid was 7.1 and BUN was 14. I eat NO fructose/sugar that I know of. I put a little stevia in my coffee. I eat mostly meat with a little veg, and some peanuts. That's it, really.
    I have an appt with Dr. Cywes in Aug. I really want to get my metabolic heath in order as I have heart disease (CAC of 600). A little scared of consuming more sat fat as my cardiologist warns me not to. Anyway, can wait for my appt with Dr Cywes. Gotta get a plan...

      Good question. I went heavy cream only, no sweetener. If I use sweeteners I find I am constantly craving sweet which is just torturing myself. When I avoid sweet taste completely my cravings go away.

Before I went ketovore 9 months ago I used to put gobbs of Coffee Mate creamer and 2 tsp's of sugar in my coffee. I was able to easily adapt heavy cream and stevia. So getting rid of stevia should be a breeze! Here goes nuttin'!

      Drink plenty of water to flush out uric acid.
      Saturated fat is neutral but PUFAs are toxic. Omega-3 and -6 are toxic to the mitochondria and in the blood.

    Thank you for posting this video! Look forward to the next video, and the consult in a few months. I still don't understand the deal with my numbers, because I don't even LIKE lean meat, but am still high in uric acid and BUN/Creatinine Ratio. Is it the chicken thighs/wings? I try to include fat bites and beef with all meals, even when having fish, chicken, or duck. Often OMAD, but usually 2 meals a day. I occasionally have some sucralose in a drink, or allulose, but haven't had fructose for 10 months. Concerning...I hope it's not from my heavy cream latte! 😰

      My last gout flare was when I was not eating high protein but I was doing intermittent fasting and the loss of water, minerals(potassium) and dehydration was enough to set off an attack. Recently I got up to 140 grams of protein daily without a flare but I use a high potassium electrolyte. I've reduced my protein since then due to becoming more and more hungry. My first gout flare was when I was in my 20's and vegetarian.

      Amount of food at meals: between 8-16 oz of protein. If it's not fatty enough, I use eggs, butter, duck fat mayo, cheese, or beef fat trimmings in addition (maybe 10-15% of total meal).

    Carb addict with no control and still eat too much protein. It’s hard to eat till “comfortably stuffed” so I’m not grazing all day. Carnivore OMAD with elevated BUN and creatinine. Work out daily…it’s hard to get this right.

    This is good info. Thank you. I am anti mainstream doctor since 2020. But i have a blood test center on the street corner here in phuket thailand.
    I am thinking i should do a consultation.
    I had my first gout attacks in 2022. Age 58 It started 10 days into my doing intermittent fasting 18/6. Then i had 3 more in 2023 and i started carnivor in july 2023. Lost 40 lbs. Had my last gout attack 3 weeks ago. My 3rd in 2024. I take short term prednesone for the attacks. I have a auto immune . But not sure what one it is. The prednesone i discovered makes me feel like 25 again. I love it. Wish i could take it every day. I have stage 2 kidney disease borderline 3 as of 4 years ago. Not sure where it is now. Heavy proteinurea

      Stay hydrated buddy! Thailand's humidity will zap you but you gotta get the water in everyday. Minimum 2 litres so your kidneys can carry on and flush that uric acid for you. Fat and water are your friends. All the best to you!

    Anyone here have good results on Carnivore diet for RLS Restless Leg Syndrome. I have a friend that suffers greatly with this and says it is hereditary but I think diet has to a contributor also. Just don’t know many people with this syndrome.

      Research Gabapentin. It has helped me attain deep sleep and diminished sensation of RLS. 300mg, at breakfast and lunch. 600mg at bedtime.

    15:25 seems to be one of the most critical claims in the whole video but he provides no supporting evidence.
    His claim is that eating meat doesn't increase purine levels significantly because the purines that are in the meat are not absorbed adequately.
    This is the main debated point, and he spends time explaining how the textbooks have it all wrong but then he just makes a claim that the "purines are not absorbed adequately" and moves on. No studies. No citations of texts. Nothing.
    He says he doesn't see purine levels increase in his patients, but shows no records or proof of that.
    We literally just have to treat him like a priest and blindly believe his words.
    This is *_SOOOOO_* frustrating.
    It's a 25 minute video...
