Testosterone and the heart - the missing link?

  • Testosterone is the principal male sex hormone and we know that in general, as men get above the age of 40 testosterone levels start dropping. We also know that as men get above the age of 40, their overall cardiac risk and overall risk of mortality starts increasing. So it appears that overall cardiac risk starts going up around about the same time as testosterone levels start falling. What we still don't know is whether they are both simply a consequence of aging or whether in some way testosterone is protective for cardiac risk.
    In this blog, i will review exactly what we know about testosterone and the heart so far.
    Testosterone is produced in the male testes and about 98% binds to a carrier called SHBG (sex hormone binding globulin) and about 1-2% circulates unbound or 'free' The free testosterone is perhaps the most potent biological form.
    Testosterone is responsible for development of primary and secondary sex characteristics and also increases both muscle mass and bone density.
    When men become testosterone deficient, they often develop reduced libido, erectile dysfunction, reduced energy levels, bad mood and increased irritability. Overall sexuality, health status and quality of life all deteriorate.
    The prevalence of testosterone in middle aged/older men is quoted in the literature as being anywhere between 2.1-12.8%.
    Patients who are particularly vulnerable to being testosterone deficient include:
    CHF
    Type 2 diabetes
    Obesity
    COPD
    HIV
    Opioid users
    Currently the way it is diagnosed is through an early morning blood test which measures total testosterone levels and if the level is less than 200 ng/mL then that is classed as testosterone deficiency. There is some controversy however over how good the assays are and especially if the value is at the lower levels of normal, what that means. Some scientists have suggested that maybe measuring the levels of free testosterone may be better but it is still unclear.

    I have been on testosterone replacement therapy since about 2005. Recently my newest caregiver got concerned about the amount I was taking as my levels were at or over the specified limits. Not much, but over. So she started to cut back on thee dosage. Her stated goal for me was to be in the lowest half of the range. I lost energy and the ability to concentrate as well as before the adjustment. She kept cutting it back and soon I was having short-term memory loss along with the other issues. I complained to her about these things, and to her credit she referred me to an MD. She is a PA. He, an oncologist, doubled the dose I was taking and it all cleared up like magic. I am now at the top of the range again and feel wonderful. My next birthday I will be 91. I do about an hour a day of moderate exercise then take a day off when the body tells me to. Bodies talk.

    I have been on TRT for three years. I use a liquid testosterone on my wrists from a compounding pharmacy at cost of only $100 for three months supply. Have been getting continuous aerobic exercise since 1977 and since using TRT, my athletic performance has improved dramatically. I've done several triathlons finishing in the top half of the field and feel exhilarated after each.. Have set new personal best average speeds and top speeds on my road bike. I feel better and look better than the vast majority of my age group and my libido is shockingly high. My most recent testosterone level is 823 and I'm 75. I just don't see why more docs don't recommend TRT - it works. Thanks for a terrific UA-cam channel.

      Kudos for the Video clip! Excuse me for chiming in, I would love your opinion. Have you heard about - Qanucas Meyyatt Smasher (should be on google have a look)? It is a good one of a kind product for skyrocketing your libido minus the normal expense. Ive heard some awesome things about it and my friend Sam at very last got astronomical results with it.

      Similar here. Half marathon runner since on TRT. I guess my only concern is having a ruptured plaque unexpectedly. But I have chol, LDL/HDL, other annual exams as well I'm on CHOL-lowering agent and a Ca2+ channel blker for essn hytn. I've been happy w/ my increase in fitness level over the last 4-5 yrs on the therapy. I fell that I can monitor my exertion and do keep a heart rate monitor on while running ----and feel like I'm not pressing the limits.

      I am 78 and I have had so many good results I can't even list them all here. The best thing was the I was PRE-Diabetic at 141 and now my blood sugar is 90 to 110 and I lost 56 pounds without dieting. I also take ANASTROZOLE to keep my Estrogen levels low and THIS shank my prostate 65%

    I'm so glad I found this. I'm 55 years old male who has four cardiac stents after having angina, no MI. Significant family history present. I weight lift and do cardio regularly and was still having issues controlling my BP, even on medication. I decided to check my Test level and it was found to be 234. Many doctors told me going on test would be dangerous, however I decided I wanted to try. After 6 weeks, my BP stabilized back down to around 120s systolic and 70s diastolic. I have so much more energy and feel so much better. I'm kind of shocked a larger study hasn't been done on this, as most studies seem to be small, and this seems to be such a game changer. To anyone reading this, I am not promoting this at all as each person is so different and would highly suggest they have the discussion with the Primary Care Doc. But I do believe it's helping me and making me healthier.

      As he said, they’re still debating the effect on lipids and cholesterol which is probably why a lot of doctors are still very careful. Glad it’s working for you, I’ve been on testosterone for 8 years now and feel great too. I’ve only had 1 working nut my whole life and I had a very low test level yet above the minimum, feeling depressed and without energy. Hopped on @ 30.

      Thank you for sharing. I am 50 and have been having borderline hypertension for almost 10 years but controlling it very strictly with diet and exercise however it has been very very challenging. I wanted to try TRT but from what I have been reading there is a risk of increasing blood pressure and was a bit worried. I will definitely look into it

    Maybe a test for low testosterone should be included in the regular blood tests performed during an annual physical.

      Absolutely! And for women too, along with estrogen and progesterone. A doctor wouldn’t think of denying a thyroid hormone or insulin hormone deficient patient hormone replacement therapy so why should estrogen, progesterone and testosterone be any different?

    I started TRT when I was 33. My total T over multiple blood tests was between 316-342 but my free testosterone was O.4%. I was experiencing fatigue, and low sex drive. I’ve been on TRT for 5 years now. My Dr has me checked every 3-4 months. Levels are stable between 750-850 and free of 24%. I was worried about heart effects listening to old mainstream media information. Looks like the latest research is proving the exact opposite. Cardiac events and Low T is a thing. Thank you for all you do!

      @@kylewish3949 My Doctor has me on 200 mg cypianate .5 CC every 3/4 days. Shooting into stomach. It keeps me at a consistent level instead of 1 shot of 200 once a week.

    I am a woman on testosterone for my heart. After many episodes of heart problems added to other problems I chose to get my hormones tested (both blood and saliva), the result; very, very low T. I have been on T treatment for 5 months and I feel wonderful!
    This is NOT only for males, women need testosterone too.
    Even if blood or saliva levels show normal, who are Dr.'s comparing those levels to? An average person of the same age that does not have diabetes and eat at McDonald's everyday? An average person non diabetic with no heart problems who exercises everyday? Everyone's DNA is different and it is not a one size fits all!!
    Because a persons levels show "normal" or "within range" should not be the only way nor an accurate way to treat them without taking their symptoms into factor too. You said it yourself "should we be testing testosterone?" yes you should! And if a Dr. can't test them, refer them to a Dr who will. It is a simple test that can help a lot. Should a Dr. be going on symptoms as well? YES! It is a mix of both. No studies are going to be in alignment by simply going on level readings only or symptom readings only. It needs to be both simultaneously.
    Take me for example, a female in my early 50's, 5'4" 135 lbs. and I was diagnosed/told I have coronary artery disease, cardiomyopathy and who knows what else they want to label it as. I had all the classic symptoms of low T beyond the cardio part. Had the Dr asked and tested or at least referred me to have testing in the other areas my low T would have been found 3 yrs earlier and treated just by the symptoms I stated to them in very beginning. But, when those symptoms became bigger and stronger in those and other areas is when I took charge and had my T tested. Last week I just had all my 6 month testing done on my heart and every one of those tests came back with passing flying colors. Why and how? From the T treatment. I don't take any heart medicine, never have or over the counter meds at all. Hell, I don't even take aspirin, and that's against my Dr.'s recommendations. It's all natural or nothing. Why? again because there is not a one size fits all and those meds are nothing but poison and cause further complications. My Dr knows my feelings and may not fully respect them, however the support is there and now the two Dr.'s work together.
    So men, and women, have your testosterone levels checked. You can buy a test kit online. Find a bio-identical hormone doctor who can prescribe it and a compound pharmacy will fill it. It costs less than my monthly cellphone bill for a 3 months supply and majorly improved the heart and overall health.
    More people need to start putting their own health in their own hands because a lot of doctors are dropping the ball and doing a huge disservice to patients by relying on written literature, scales, charts and excluding ALL symptoms and tests when only certain ones read "normal."
    I really hope you can pave a way to more integrated treatment.

    Thank you for addressing his issue. Have been on TRT for over 10 years after connecting with a specialist ac UCSF Medical Center. My testosterone level at the beginning was below 225 and I lived with is level for most of my adult life. Had prostatectomy 9 years ago but stayed on the TRT by my choice. The quality of my life has improved greatly since beginning the TRT.

      All of my blood work over the last 9 years shows no abnormalities. No heart issues detected in stress test 3 years ago. My life energy level is great thanks to the TRT injections.

    I am 69:and have been on Reandron undecanoate injections every 10 weeks under an endocrinologist - I have had 5 glute muscle injections so far and I can say it has been great so far - a big spike of testosterone one week after the injection which sends me sex urge overdrive but it has a long tail of decline before the next jab - overall I very happy and at my age and very grateful for this lifesaver !

    I found your videos a couple days ago I just wanted to thank you I was diagnosed with wolff-parkinson-white syndrome when I was 19 I had an ablation in 2001. but I've been struggling with heart palpitations more so now as I get older I'm 38 I will be 39 in July. anyway, your videos have helped me to better understand my palpitations and when to worry. see I also struggle with high anxiety which comes basically from a lack of understanding but you have help ease that anxiety by giving me understanding ;-) so thank you!     I started TRT 11 weeks ago & it has changed my life!!
    I was very active, but I had to force myself out of habit because ive worked out my entire life. I was depressed & had high anxiety.
    My libido was non existent.
    I'm a healthy 48 year old male.
    Now I still work out 4x a week with weights as well as 12 miles of cardio. 3 miles of vigorous cardio (HR to 130bpm- 150bpm) after each 1 hr weight workout & I look forward to my training sessions.
    I had occasional palpitations prior to starting TRT.
    Now 11 weeks later, my sex drive is excellent, my palpations are gone,
    My depression & anxiety are non existent. I honestly feel great.
    I lived for years feeling terrible.
    I cannot sing the praises of how much this has given my life back.
    200mg of testosterone cypionate every 2 weeks.

    Great topic doc...my post graduate thesis was " level of serum free testosterone in male patients presenting with Acute Coronary Sundrome" and in my study, i found that patients who presented with Acute Coronary Syndrome had lower lever of serum free Testosterone . 👍

    York Cardiology The replacement therapy I got in February (1 shot, lasts up to 3 months) noticeably increased my muscle strength, reduced joint pain, nearly eliminated arthritic and back pain, reversed the creeping weight gain, increased overall energy levels and stamina throughout the day, and completely stopped my occasional atrial fibrillation symptoms. I didn't get a chance to get a blood chemistry test before the lockdown, but I'm sure a number of factors improved because my quality of life was vastly improved. The effects are gone now (also beginning to notice occasional faint atrial fibrillation) that the followup treatment scheduled for 2 months ago is not going to be possible due to facility pandemic medical priorities. Thank you, Dr. Gupta, for verifying my results weren't placebo/psychosomatic effects.

    I lost a testicle in military action and damaged the other when I was 24. My T levels stayed steady until I reached 55, and then we waited. I’m 65 now and give myself a shot a week and I feel so much better.

    Wow. Interesting. I was hospitalized for HF and a-fib, in May. I'm 49 y.o. have gained weight(60 lbs. in last 5 years.)
    Had 2nd cryo-ablation, last month.

    My testosterone level was 150. One Dr was worried about red blood cell count, so she wanted me to stop the TRT. I felt like I was gonna die . By the way i also have heart failure. After four days the Dr gave me testosterone injection. Within minutes i felt much better. The TRT really helps. You are really spot on with everything you talked about. My heart Dr says loose weight you'll be better. It's just not that easy. But the TRT does make it better. Now I have to find a Dr like you. Also you had a video about a year and a half ago that introduced me to TRT. Thanks

      Placebo effect I thupud have told you your test was 1000 ngdl then you would have instantly felt better. Unless they gave you testosterone suspension then you didn't feel better in a few mins you just tight you did bc you got a shot they probably injected you with saline

    44 y.o. I've been diagnosed with AFib and low T almost the same time. Had 2 ablations, the second helped but still have few light AFibs a day. For low T I was prescribed Nebido. It looks like it works but not much, mood and everything swings too much during 3 months. I decided to change to Testosterone Enanthate, 1 shot/week. Works much better. It looks like it even reduced number of AFib episodes.
    Unlike many of you here I was doing too much extreme endurance sports. And according to some researches it increases the risk of AFib by 10x.
    Anyway, life with TRT is much better.

    I have been on TRT for almost 2 years now and cannot imagine life without it.
    I stopped gaining weight,
    my exercise recovery is much better
    my aFib symptoms have greatly decreased
    my mood is much better
    sexual function and recovery has improved.
    Everything is better on TRT except my insomnia which has not changed,
    BUT, when I do sleep, I sleep much much better with TRT than without.
    Unfortunately, doctors in the U.S. are not trained on hormones and so rarely test hormone levels in men.

      i have been on TRT since 20years and i feel young,healthy and powerful.sometimes it reduce the libido but cialis always help me with that.....i can not imagine my life without TRT

      @@jordixboy he uses doses of short half-life testosterone too far from another, this way body stops producing testosterone and between doses he has nothing. That's why some doctors, in theory, try to match long chain testosterone with more readily available testosterone. That's it or make the patient take low dose shots every two or three days. Also, not only the body reduces or stalls production of natural testosterone due the presence of exogenous hormones, but according to how much fat is in the body, it enhances estrogen levels (which could cause ginecomastia and affect libido).
      Unfortunately, due to European and American duress, testosterone is thighly controlled worldwide. So, in pratice, no way someone can get access to it without a prescription. Finding a doctor who prescribes it is enough of a miracle. They err a lot on the side of caution. Few doctors are more liberal in prescribing it.
      However, due control, it's take it or leave it. If it were readily available again, maybe I could go on this therapy. But that's a no second thoughts choice, since there's no going back, I find it too much power on the hands of a doctor for the rest of my life.

    Just got diagnosed with testosterone deficiency, vitamin d deficiency, insulin resistance and thyroid antibodies.
    Started my vitamin d regiment 3 weeks ago and also something for my blood sugar, also just started magnesium last night.
    All with a healthy diet and exercise I can somewhat return to normal hopefully

      Hi James saw this and thought I’d comment. Out of all the hymn list the one thing that stands out is TPO you had or positive for thyroid antibodies, not sure if you’ve had it but also get you TSH checked and your free T4 levels. NHS rarely check T3 but worthwhile. I take thyroxine for a diag under active thyroid. If you do have a struggling thyroid due hashimotos (the antibodies check for that) that will cause insulin sensitivity issues and also problems with testosterone and these things improve once you’re treated. Treatment is a small thyroxine tablet a day. It’s no problem. It could be a lot of those issue might be caused by something quite treatable. Get your thyroid checked again by GP. Endocrine stuff is all linked so one bit goes wrong it’s not uncommon for other issues to crop up.

    I am 66 Type 2. 174lb 6' tall. 2 yrs ago had a Stent put in a heart artery. 1 yr ago T levels were 9.0 now 16.4 after eating better and taking ShilaJit/Tongkat Ali/Ashwaghanda. I feel allot better. Just wrote a 3 hr exam at UBC and now studying for my PMP.

    I was diagnosed with low-T (91 ng/dL) alongside type 2 diabetes, no diabetes medicine has any effect on my diabetes. It wasn't until I started treating the low-T with Jatenzo (oral T) did my A1C drop drastically (4%) in a 3 month time span. My hematocrit runs between 56-58% and hemoglobin 19 and feel amazing!

    I have had mitral valve surgery and suffer with familial hypercholesterolaemia. I was diagnosed with testosterone deficiency and low LH 10 years ago for which I have been using TRT.
    I can report that I live the life of a man younger than my 65 years and that I do not have ischaemic heart disease. Whether this good health is due to the TRT I cannot be certain - but I believe it is.

      I recently suffered at heart attack and had a stent put in 2 weeks ago. I have another artery blocked at 75% I’ve been told. I am 46 years old.
      My dilemma is, do I continue with my TRT whilst I am waiting for the other artery to clear or be treated or should I continue with my 180mg Test that I started about two months ago. I was feeling amazing whilst on TRT and was recommended this whilst I was living in Dubai (my test levels were 220). I watched your videos and shared them with all my friends.     I am 78 and I have had so many good results I can't even list them all here. The best thing was the I was PRE-Diabetic at 141 and now my blood sugar is 90 to 110 and I lost 56 pounds without dieting. I also take ANASTROZOLE to keep my Estrogen levels low and THIS shank my prostate 65%. I have a full head of hair and pee like an 18 year old again. I far as I am concerned TRT is a bit like the "Fountain of youth" at least in the way I feel. I still look aged but that the least of my worries.

    Do you address dark circles in any videos? Under my right eye is particularly worse, my diet would be the best in the family but I do have extreme allergies and heard this could have something to do with it, I also get more than 7 or 8 hours of sleep per night so dont think its lack of sleep and never feel too fatigued throughout the day so quality of sleep is good I'd say

And a question more related specifically to Dilated Cardiomyopathy: Are there any studies on how to actually train and regain heart muscle?       As any woman can tell you, it doesn’t matter how hard you train, muscle will not be built unless their is adequate testosterone. Anyone with chronic fatigue and fibromyalgia can say the same. There is a wall at which more exercise does not increase fitness and become destructive to the body.

    Ive watched alot of your videos, can you try to find a link to a bad gull bladder and PVC's after eating? I haven't had my GB tested but I have a suspicion. Most of the time after I eat I get PVC's. Ill be fine all day until I eat.

    I have researched this myself. There appears to be an estrogen link. As men grow older testosterone drops and estrogen increases. When estrogen gets out of balance by being too high this is apparently a very high risk factor.

    Hi. I hope I will find time for the consultation with you in York. Currently I live in Leeds. Please make a video about other POTS/dysautonomia symptoms. My heart rate and blood pressure is now absolutely normal coz I take bisoprolol, but I still have occasionally shortness of breath (dyspnoea) , dizziness, sleeping disturbances, post exertional malaise, strenuous exercice intolerance ( squatting, push ups). Intolerance not because of the heart symptoms, but because of my nervous system hypersensitivity, even though I am not stressful and not worried. So dizziness, fatique, tinnitus, head blood pressure/circulation, reddish eyes, blurry vision, eye floaters may appear after strenuous exercices (if 15 push ups or 15 squatting I may call strenuous?) . I am also become easily overwhelmed and feeling over stimulated in busy environments, where a lot of hustle and bustle. Also my other symptoms, but now they are rare, may include: brain fog, light-headedness, blurry vision, eyes floaters, red eyes, pinching feelings in my eyes, unability to walk in straight line, coordination problem, forgetfulness, irritable bowel syndrome, diarrhea, appetite loss, very acute feelings (odor, noise, light), tinnitus, pulsations in my head, vibration in my head, clicking noise in my head, insomnia, tremor (hands), derealization, depersonification, feeling of the lump or stone in the throat, excessive sweating, coldness, numbness, burning feelings on my skin (nettle-like) and many others. I even had very strange symptoms like pain in my hairs. Also some smells on the streets may make me dizzy or cause dyspnoea ( gasoline,paint,nail polish removers, bleach, maricuana). So I have some kind of hypersensitivity symptoms, especially in case of stress/bad sleep/sun heat/exercices/watching some movies (horror for example). I have complete alcohol and caffeine intolerance. Even few sips make me symptomatic. I also have very bad head pressure and blood circulation in the head when I am flying in the plane. Also tinnitus and pain in the ears when I am flying. So my heart pressure is now sorted by bisoprolol, but is there any drugs for the head pressure and head related symptoms? My MRI is good. Maybe I should make head CT as well? And maybe neck CT/ultrasound?

      @@UmarAli-el8et Hi. I am much better now, but not because of the medicines. I found them completely useless from this kind of sickness. I believe that doctors have not yet discovered drugs from this kind of dysautonomia, unfortunately. So the only thing that helps me is the correct dayly regime (moderate exercise, right diet, healthy food, and stress avoidance). But most importantly - running/jogging. Initially I couldn't run even two minutes. But after 6 months of training, I went from 2 minutes to 30 minutes run every day. Also I noticed if I stop running, then my symptoms slowly coming back. So I decided to never stop running. I am trying to run 30 minutes every day and I look quite healthy if I do that. But it's very important to note that overexertion can be dangerous and harmful, so if I feel like I cannot run 30 minutes some days, than I run only 20 or 25.

      @@UmarAli-el8et I dont know what were my T levels, but just like I said when I started I was not able to run even 2 minutes (I was exhausted, had palpitations and what not). So I had to learn how to quickly walk first and only after few months of walking I started to run a bit, and then more and more when my heart and nervous system got used to it. But it was tough journey, because if I overexerted a bit, then I had to lie down and recover for a few days. So its very important not to overexert and listen to your body if you wanna try this one day.
