Great video Dr.Biernacki. My question for you is, if the taping holds down the 2nd toe, doesn't that increase the impact/loading force during "toe-off'? From an engineering standpoint, my perception would be to tape up and over the foot vs. under. I scanned back and forth over that section of the clip but there was only background music so I didn't really get how taping downwards changes the force distribution or has an arch-itectural (pun intended) effect. Thanks in advance! I'm dealing with a momentary sharp shooting pain right into that toe on getting out of bed and it's beyond frustrating.
The duct tape thing worked like a charm. I've been in constant pain for well over a month, and shortly after applying the duct tape it was gone...full body weight...nothing...no pain. Awesome video...thank you.

The ligament holding your second metarsal phalangeal joint together at the bottom is called your plantar plate. There is some overlap between a plantar plate year and capsulitis. This tape job holds your toe from bending too far upward and hyperextending that joint! Good luck!
Thank you for explaining the WHY of why the toe is taped. That’s the only thing that cld have been covered better in the video. I’m glad I found it in the comments though!! I’m always more compliant as a patient when I know the reason behind what I’m doing!
I just got diagnosed with it yesterday by my Podiatrist, she taped 2 toes together, I got it through gardening and bending that second toe. Thanks for all the info.
I'm currently working through this after seeing my podiatrist and it's exactly the same as you talk about. I have the strong sturdy shoes, the orthotics that raise my arches and also put a small lump under my affected foot base and also the tape. It does make a difference and I'm also stretching my calves etc plus doing self foot massage. Great video!

THANK YOU SO MUCH for this video! I just saw an orthopedic surgeon last week for metarsalgia in the EXACT toe you are speaking about. YOU gave a much better explanation--especially with the visual aids, and a better explanation as to why calf stretching is so crucial. Thank you!!!!!!
I love this doc! As a competitive tennis player, for the first time am experiencing 2nd toe capsulitis, a surprise to my “invincible” 65 yo body, and am putting together a Tx prgm with all kinds of goodies - healing, prevention, best practices. Thanks for you guidance; I prefer to avoid visiting a DPM just yet.
Just saw your video. Woke up with an excruciating stiff second toe yesterday. I work in a foundry on concrete all day and then usually 4 to 6 hours in the garden, pasture and around my place getting things done... so im on my feet approximately 17-18 hours a day. After i watched your video, I immediately grabbed the duct tape and followed your instructions... my foot and toe is still sore, but not even close to the pain i was experiencing! I can walk without limp! Thank you so much!
Thanks Doc I’ve searched everywhere and finally got the answers I needed. My pain has been going on for more than 8 months and finally got some answers. Thank you!!

Ive tried the tap method and I gotta tell you IT HELPS!!! No it is not the solution but man, I can tell the difference. I do not cut my duct tape in half down the middle - though this is a lot tougher to put on correctly, its worth the pain (to eliminate some pain).I also just got "custom" orthotics and these two together at least allow me to walk around!! Note - my feet are so bad Ive gotten to the point where I cant even walk around the house with out my orhotics on. I know there are a few underlying/chronic problems going on that have diminished the amount of muscle/fascia that I have under my feet but this was a no brainer to try - I really didnt have to buy anything (just a $1 roll of duct tape at the dollar store). Good job doc!
thanks, I have had this problem for a couple years, I have been to the foot dr twice, given a shot, and told to tape my toe..but never like that, great explanation, and my calf muscles are really tight..which could be some of the issue..I didn't realize that these were all connected... I have been wearing really good work boots, and orthotics with them, but, still having this issue..im hoping your info helps me fix this for ever...thank you!!!!

Michigan Foot Doctors I’m watching more! My Morton’s toe is turning into a claw in part because of osteoarthritis not helped by my years of dancing and running). I’m trying to avoid surgery. This in particular is a big help. There’s a study that found that consistently taping can make a permanent change of a millimeter or two. I’d rather try that than have surgery! Looking for more videos. Keep up the good work!

thank you for this information. You are awesome. I am an avid tennis player trying to avoid surgery for a torn ligament that was caused by my bunion. My doctor has me using a small device that slips on my 2nd and 3rd toe to help relieve the pressure on the 2nd toe, a sports orthotic and NB sneakers. You reference a small gel device to stop the big toe from going under the 2nd toe. The link to that product now references a product to protect the bunion instead. Can you please update the link so I can order the product you are recommending. Thank you again.

That is a lot of pressure on the feet! Sometimes cross training can help until bruising can calm down. We have also seen success in alternating shoes to allow them to be less compressed.
@@DrTomBiernacki i changed work shoes with a thick sole, did the tape thing and ran another 10 miles pain was short lived and toes feel good next day. I run on rural side roads and because of years of layering they have a steep down edge slant little flat gravel and then a dirt hump and my middle toe is launching off of the hump on right side and my big toe isn't doing as much work as it should and left is just at a slant Now that I'm aware I'm going to be more cautious of where my feet are launching from. Also shoes are worn bad in the areas with most impact.
I'm glad someone knows what their talking about, but my toe doesn't hurt where you've indicated and it's more on the first ligament to the tip on my second toe and it happened all of a sudden one morning when I woke up, still hasn't gone away and the doctor tells me if I didn't break it I shouldn't worry, 2 years later, same pain, more tolerable now but still prevalent.

Thank you so much for this information. I fractured my talus and my cuboid a year ago and they didn’t find out until 15 days later that I had 2 fractures no surgery required. My second toe was stiffed like I had a stick in it. Finally an specialist after a year discovered that in my last MRI that I had few months ago I had fluid and that is why I have stiffness and numbness transferring to the rest of my toes specially to the 3rd, 4th and 5th toe. He is requesting another MRI and acupuncture. Waiting still and they want me to start vending my toe down but when they even try to massage my foot the numbness gets worse. I would like the right treatment instead of getting worse. The feeling of stiffness and numbness is constantly there, worse in hard surface like concrete. Thank you for your knowledge.

Heh-oh…🙋♀️bunion here with capsulitis. I have seen a podiatrist twice and both times was given medication- once a shot and once pills (which I have not taken.) I was given absolutely no information on natural care. This was very informative, as I am looking for natural alternative care. I really am trying to not find myself dependent on medication. Thank you very much for posting this video. I was wrecked by "surprise" capsulitis this weekend, although after studying the problem over the last few days I now realized I could feel this building. Bad shoes (cowboy boots and pointy dress shoes), not stretching well enough, lots of barefoot, no inserts - I could feel something coming. I think my symptoms are minor but I tell you I could hardly walk for a couple days. Thanks to lots of rest, ice, salt soaks, stretching and the new inserts and tape job I learned from you, I have seriously seen a 95% reduction in pain. I am diligently resting and caring for the injury. I know I will have it past me in just a few weeks and will not repeat those bad habits again. Thank you so much for your help in my recovery Doctor.
Had a very bad Podiatrist remove joint in Great Right Toe (GRT) due to arthritis (I did not have @50) and use a BioPro 10062 implant which destroyed my sesamoids and ligament. Had reconstruction surgery in SF to wrestle the nasty implant out of the toe bone and replace with silicone one. 21 years ago. Over time, second toe was put to work. So it has pulled away from the "gang of toes" and is nestled next to GRT. Lately lots of pain and swelling on top of foot below toes. DPM taped my toe , but by the time I got home "the toe was mightier than the paper tape" and moved back to its GRT buddy. Can this really be remedied after more than 20 years? Wihout surgery. Your video is great. Hubby would love duck tape solution!
Thank you so much!! I woke up with a stiff second toe and it is making me limp, which I know is not good for the rest of my bones. Your explanation makes so much sense.
Thanks. That was a good explanation. I still don’t quite get the how the normal Achilles unloads that 2nd toe. I am also wondering how the taping helps with that.
THANK YOU!! I went to a foot store that sells over $1000 for a 3 pc set of insole for foot pain! I did not buy them as I thought something has to be researched about my ball of foot pain and its the second toe area..
Hi there. Sounds like we have the same issue! I work 9 hour shifts on concrete. The ball of my left foot was always kinda sore after work but Recently the ball of my foot was SO sore I couldn't stand on it. (On ball like between 2nd and 3rd toe.) And it seemed swollen on top and same place underneath. Also weird numbness/tingling on top of foot. So long story short I've been to the podiatrist once for it so far, they did an x-ray and said it was metatarsalgia and I got $40 orthotics with a metatarsal cushion. I really suspect it's a stress fracture, since having this issue I've tried to do a lot of self-education. Apparently a stress fracture doesn't show up on an x-ray, only on an mri, stress fractures won't reflect on regular x-rays. So if pain persists you can push for an MRI which might find the problem for you! That is my hope for my next three week follow up. Hope this info was helpful! Good luck to you!
@@loving_lioness3518 Thank you for the reply. I hope your pain goes away with the supports. I picked up a $30 hard insole on ebay which helped out alot on a trip to were we walked over 10 miles a day. My feet were fine. The last 2 days of 10 I took them out and my feet were still good. Now back from holiday I'b back to walking less and the balls of the feet hurt. I guess it is time to wear the insoles and yoga is a every morning routine now. Especially things that are stretching on the toes. I also bought the 1/4 pads that stick on your feet. They really helped on the long daily walks! I'm self insured so I have learned researching everything before heading into a doctors office. LOL

You are the first doctor to describe to a T what’s going on with my foot! Thank you very much! Mine seem to have started when I wore a new pair of (cute) shoes for the first time. And that was 15 months ago. Since I have to walk a long way just to get to my desk, I’ve resorted to wearing practical Ecco shoes with a metatarsal pad. The base of my second toe is curved downward whereas all the others are in normal position, hence the pain. It seems to me if I had something to support the base of that toe to push it UP that could remedy the situation. But I’m not a doctor! I’ve been to two ineffective podiatrist’s so I’ve resorted to finding yoga routines however there seems to be no effective ones that I have been able to locate. I have even resorted to my tens unit but that doesn’t work either. ¯\_(ツ)_/¯ I am 64 years young and don’t have time for this! I have too much I want to do in retirement to include tap dancing again and I don’t want this to sideline me! Your suggestions are greatly appreciated!!
Usually the metatarsal pad isn't enough, you need a solid orthotic with a metatarsal pad built in plus a good shoe like a Hoka may be what you need. www.michiganfootdoctors.com/recommended-products/
Check out the orthotics on this page with the metatarsal pad and shoes, this helps many of our patients with metarsalgia, but if some basic stuff doesn't help make sure it gets checked out by a professional.
I went on a quest to grow my calves. I destroyed them every week (reps to failure). After about 4-5 months I started having some pain in ball of foot. I think I have capsulitis. I hope this works for me.
I had capsulitis for over a year and it improved by using a Metatarsal pad. Unfortunately, I am now developing a hammertoe on the second toe caused by the metatarsal pad. My Podiatrist gave me a crest pad and recommended I continue to tape my second toe, but I don't see any improvement and she says there's really nothing else that can be done. The joint in my toe is changing color, at times it's purple. I'm really desperate for help here in California.

Loved your video and your patient kind manner! I think this is what I have - not sure - I had bunion surgery a few years ago and it left my toes very stiff (especially big toe) with a very limited range of motion. Also, would the toe separator big toe / 2nd toe spacer thingy be a good substitute? Or maybe one of the others you have listed in the links? Final question: how long of a period of days or weeks should this be used? Thanks for the video!

Unfortunately it is impossible to be specific unless the condition is examined and known, the severity and the surrounding life factors are known. Always get evaluated in person if having a problem.

Told I have a morton's neuroma and metarsalgia. Still in pain after getting expensive orthotics. The pain moves from ball of foot to toes when I use met pads. Gonna try this.
I was just told I have metatarsalgia. Through my research I suspect it's actually a stress fracture. (That's hugely misdiagnosed) the only way for them to see stress fracture is on MRI, it won't show on an x-ray. So maybe push for an MRI? Tell them pain is still persisting. That's my plan....if I still have pain at my 3 week follow up, I'll push for MRI.

This legitimately made a huge difference. Night and day. Thank you.
Thanks for the kind words!

Thank you for the video. I love how you show bad and good examples. I wear and like the same shoes your wife likes. I appreciate your plain English explanation. I give you a 10 !
Good job doc!
thanks, I have had this problem for a couple years, I have been to the foot dr twice, given a shot, and told to tape my toe..but never like that, great explanation, and my calf muscles are really tight..which could be some of the issue..I didn't realize that these were all connected...

thank you for this information. Best explanation so far. I run 5 to 10 miles days off and work on a concrete floor 12hr shifts days on and I weigh 190
That is a lot of pressure on the feet! Sometimes cross training can help until bruising can calm down. We have also seen success in alternating shoes to allow them to be less compressed.
Hope your toe shows some relief!
But always get checked out by a doctor if worried!
Love you. Like a dose of humor and pain relief combined
I'm also only 26 yrs old so I can't attribute it to age injuries or over compensation on my ability to move heavy or awkward objects.
Very good!! informative!!
The transcription is smack in front of the main area being shown, very frustrating.

The hyperextension is what causes the buckling at the knuckle, the tape can hold it straighter so that the ligaments on the bottom of the knuckle have time to recover and feel better.

Thanks for watching!
Cowboy boots are tough, just because they have a heel and you can't fit an orthotic in their usually. One of the toughest shoes we see for sure!
You are AWESOME! Where have you been all my life!!!?????

You are the first doctor to describe to a T what’s going on with my foot! Thank you very much! Mine seem to have started when I wore a new pair of (cute) shoes for the first time. And that was 15 months ago. Since I have to walk a long way just to get to
my desk, I’ve resorted to wearing practical Ecco shoes with a metatarsal pad.
Haha well you've got that going for you, good luck!
This is exactly what happened to me. I'm 64 too and wore a cute pair of heels with pointy toes ONCE and my right foot has been a nightmare since!
I LOVE YOU!!! YOU HELPED ME A LOT. ❤❤❤🦶❤

I think this is what I have - not sure - I had bunion surgery a few years ago and it left my toes very stiff (especially big toe) with a very limited range of motion. I will try the orthotics I had made a couple of years ago (but rarely use) and the taping..I was already doing the stretching. Will let you know if this helps but really appreciate your videos especially since I cannot got out to see a podiatrist now. Thank you !
And we appreciate you watching!, Thank you so much!
I would love an update: hopefully you are feeling better! 😊
@@zenbrooklyn thank you yes 👍 I am
Simple and well explained!!!! I thought I knew a lot about feet being an athlete well experienced on injury and care, along with my experience in lots of specialties. Last 10 years in orthopedic and neurosurgery along with conditions.
I learn so much in these few minutes watching your videos. Thank you!!!!!!

Wow! I just added tall arches to my boots. It took a lot of pressure off the toes! Excellent. Thank you. Wondering, though, why my THIRD toe drifts away from the second toe. Thanks much for the arch trick.
It's hard to give specific advice without evaluation and x-rays. One thing to consider is watching our Morton neuroma video. This could be an explanation, especially if you get soreness and numbess in that region after a long day of standing. Good luck and hope you feel better!
Love your fun communication! Very helpful. Can you tell me the name of the toe spacer you recommended? Can’t find it in prior comments. Thank u

This actually made a difference for me. Do you just keep it on when walking or sitting, but take it off at night when you sleep? Also, would the toe separator big toe / 2nd toe spacer thingy be a good substitute? Or maybe one of the others you have listed in the links? Final question: how long of a period of days or weeks should this be used? Thanks for the video!
Looks like Im doing everything wrong, no wonder Im suffering from metatarsalagia right now, Thanks for the video
Nice delivery Doc. Thank you.
Thank you that means a lot!
Straight to the point and informative thank you. I'm not sure if this what I got going on , entire foot hurts but when hiking ( trying to lose those pounds) the ball of feet are on FIRE. Is this second toe?
Well explained really helpful
We appreciate you watching! 😊❤🙏
If possible, we would appreciate a quick rating (hopefully good), as it makes a HUGE difference!

We are working hard every day to make better and more meaningful content! This is what keeps us going, for sure.
What are thoughts on surgery if the pain persists?
How long should the tape remain on the toe during the day? For how long should the taping method be practiced?
Unfortunately it is impossible to be specific unless the condition is examined and known, the severity and the surrounding life factors are known. Always get evaluated in person if having a problem.
yeah man, you earned that like. Been struggling with a very slow recovery of capsilitis for a few months (getting old sucks). That taping method is helping me out as we speak.
How long did it take for you to heal?
People never come back to report an update that would help somebody else out
Shoes with thick soles. ‘Gravity’ shoes.
@@crispycruiser4654 youtube no longer sends out notifications on me replies, which would help me out too. But the answer is it took me 6 weeks, but the taping was off after about 3
Super clear and concise. Thank you so much!

Thanks for watching and being a fan, we really appreciate you!
I appreciate your video. Thank you very much
