Science question: some people gain weight until obese, morbidly obese, whatever, and then the weight stabilizes for a long time. Why does it stabilize?
I read somewhere that uric acid rises initially when starting a ketogenic diet but settles down after a while. Assuming uric acid causes gout (although I really don’t believe any of the accepted medical advice on anything any more) then you might get a gout incident in the early days of a keto diet.

for a friend, is keto or low carb good for gout?
Some say that it is. I’m not sure if there’s any studies which explore this, however, so it’s just conjecture.
