This is too funny to me, because I've been calling Sudafed PE "Sudafed Placebo Efficacy" since the Combat Methamphetamine Act put pseudoephedrine behind the pharmacy counter.

I had used phenylephrine HCl for over 20 years with very good results. However, I never followed the recommended dosage. For some reason, it didn't ever work for me. BUT - when I took a 1/4 dose at night before bedtime, I had excellent results that lasted over 16 hours. Sometimes doctors prescribe TOO LARGE a dose when an original dose doesn't work. ( I forgot to mention: Matt's a lucky guy! :)
I was confused thinking that the nasal form of the medicine was also declared inactive and it always worked for me.

I usually use PE at half dose and it works really good for me. It works better for me than regular Sudafed did back in the days.
I've been a pharmacist for 25 years, and I've been telling patients that long that phenylephrine isn't effective.
The downside to pseudoephedrine isn't that "You have to talk to the pharmacist", it's that we're required by law to have a valid ID to log the sale (even though every bit of meth today is imported by Mexican drug cartels).
Also, everyone please be aware that continuous use of Afrin/oxymetazoline nose sprays for more than three days can cause worse "rebound congestion" when it's stopped.

The thing is that this was known for many years, back to when they started changing their marketing to promote phenylephrine products to avoid the hassle of the pseudoephedrine regulations (due to the rise in meth abuse). This should have been done a long time ago.
You also should have mentioned Propylhexedrine (Benzedrex), which was a life saver before I had septoplasty and inferior terbinate cautery. My life is much better after that surgery.
But what would you take if you want to decongest and fall asleep? What will knock you out best?
Antihistamines make you drowsy, much more so than decongestants.

For a med student, would you say it's better to do IM or FM if the goal is to become an outpatient PCP (no fellowships desired)? Or is it more or less the same either way? Thanks!
Oh this is a great question! I would say it depends on if you want to treat kids. If you do, then choose FP. But if you are someone that likes deeper knowledge about a smaller group then IM is a good fit. IM tends to see much older patients, but not always. I work in a clinic with FPs. I’m so glad you’re going into medicine. It’s the best profession!

This is true. For men, it can cause urinary hesitation. And overall, for older people sometimes it can cause confusion and increases risks for falls since it is a sedating medication.

What’s the best medicine for seasonal allergies? I do suffer from nasal decongestation year round though. Sometimes my nose is clear sometimes not.
This depends on if you want to take a pill or nasal spray. If a pill, then Zyrtec (generic is fine too). If you want a nasal spray then any of the nasal steroids are good but they take time to work. And if you’re entering a particularly bad season then use both. You may see an ENT to see if nasal polyps are a problem for you. Thanks for watching!
I'd like to recommend being sure to drink plenty of liquids. Not all at once, but spread out over each day, and not too much. Good ol' water is the best and it helps your body dissolve things that can cause congestion. Also, if you're not restricted from it, try starting to sleep on your back with your chin elevated and expanding the muscles in your nose, while trying to breathe through your nose, if it's not completely obstructed from the congestion. That imitates the action of those nasal strips that pulls the sides of your nose open.
