It wasn't obvious for me that he was insane in the book ending. There were some signs telling Andrew still could choose. But yeah, in the film it occurs much more obvious and optimistic. Anyway, both are really good
He came in on a boat from off of the island and maybe the story is actually that he was really onto some MK Ultra stuff happening here in the US and made the mistake of going there to gather evidence when they already knew that he was onto them. They then do exactly what the Rachael in the cave said they would and make him out to be crazy and they do it so well that they even get the audience to question whether he is crazy or not. Every piece for either possibility is there but personally I think that just enough is there to suggest that he is not crazy to make that the reality but it is just explained away as hallucinations. There are reasons that we do not have asylums like that anymore and they are not just because of the barbaric procedures. It is also way too easy for someone to be deemed insane when they are not and then held against their will. It is a great movie on a lot of levels and I choose to believe that it is a story where the bad guys win.
