Your out breath is where you need resistance. You strengthen your air lung capacity and recover faster during sports activity after you return to rest after say a tackle. You should be able to hold your breath under water for 2 to 3 minutes. Result vary. It does work if you have the discipline to do it nearly everyday.

I think it would be best to show the device to your doctor, who will be more familiar with your history and tell you if this would help specifically with you. COPD is not something to mess around with (I don't need to tell you, of all people) and it's best to have your doctor sign off on any and all treatments / alternative management tools. Hope you manage to get something to help!
I'm a trumpet player. Apart from the benefits described here, I think it's going to help with my playing, hopefully increasing my range. To play a brass instrument well, you need to have a strong exhalation capacity, blowing out hard and fast through the horn.

I use the Power Lung, it uses threshold resistance unlike most devices that use restrictive resistance. But it's fine to start with the $20 kind.

Don't use this use sprimometry twice a day for a month 5 min deep inhalation and 5 minute deep exaltation it's will help to improve your lung capacity

Turning nob toward + increases resistance.
