  • Inhaled insulin Afrezza has been around for years, but a lot of people with diabetes (and even some providers) still have questions about how to use it. In a nutshell, rapid-acting insulins like Humalog or Novalog can take 30 minutes to start working, but Afrezza kicks in almost immediately and is out of your system in an hour and a half. Dr. Edelman and Dr. Pettus answer the most common questions about Afrezza, and offer important tips on how and when to use it.

    Great video! I love seeing education on Afrezza. I’m a T1D and I use Afrezza for everything, mealtime and corrections, along with Tresiba for basal. It’s the best! Last A1c was 5.6.

    Going in for my FEV test next week. Can't wait to give this a try! And thanks for turning me on to GLP-1s and SGLT-2s, too. These have been real game-changers for me!

    I am a Type 1 diabetic and was in the first clinical trials of inhaled insulin (circa 2004), and used it for 7 months. I had a miserable experience. It was detrimental to my lung function after just a few weeks. I do have history of asthma. Over time it became slower and slower to act, to the point that I had to dose 1-2 hours before eating! Eventually, I developed antibodies to it, and was taken off the study, which was meant to last a year. My question: Is this insulin the same, or how might it be different? If it is bad for people with asthma, why would you prescribe it for anyone, given the possibilities of lung damage?

      Afrezza is different than Exubera. It’s not indicated for anyone with asthma, anyone who is a current smoker or who has lung disease. It has been proven to be extremely safe on the lungs. Your situation was certainly unfortunate and I hope you’re doing well now.

    Sometimes I have an issue with going low before my meal digests (like within 1 hour of eating and taking my insulin) because the insulin is working faster than my digestion (or at least that’s what I assume is happening). Then I eat something to bring my sugar up, which stabilizes my blood sugar for a couple hours. Then when my original meal finally digests, I go high sometimes. I take Humalog. I would be afraid of going low with Afrezza because it works even faster than Humalog. Is this a concern for most type 1s?

      It is not a concern for most type 1s. The best way to try Afrezza the first time is to treat a high blood sugar and see how you respond. Remember that 4 units of Afrezza is like 2 units of regular injected insulin, however it’s hard to compare since the time course of action of both insulins is quite different.

    Are there any prescribing issues w/r/t - coverage when you're on a pump, Medicare, etc?

    Is this insulin available in Bangladesh? Can 12-year-old children of type-1 diabetes eligible to use inhaled insulin?
