How to Eat 3 Donuts and Stay In Range

    Steve and Jeremy eat three donuts in a 15-minute time span to see if they can keep their blood sugars in range (between 70-180mg/dL). They each try a different technique…Steve uses ultra-rapid-acting insulin with a pre-bolus, and Jeremy uses inhaled insulin Afrezza. Please keep in mind we are not advocating that anyone eat three donuts at a time, but this experiment demonstrates that, with the right tools and education, you can eat your favorite foods in moderation while living with diabetes.

    So you're in a 9am office meeting, there are donuts. "Excuse me folks, I just need to go run some stairs, I'll be right back" is not exactly an option. So yeah, it can be done, but it takes some doing. I'm especially sympathetic to Steve's experience at the end where he had to stomp on the brakes with a big proactive-correction, then had to slurp some lemonade to stop the downslide on the other side. "You can have whatever you want, just bolus for it" has a hidden caveat: "But the less insulin you have to take, the easier it is to manage!"

      Start doing body squats. If anyone asks what you’re doing, just tell them you’re making sure your blood sugar stays in range.

    What a fun video, loved watching you guys eat yummy donuts, great tips! Usually when I decide to eat donuts I wear a rain coat, hat, and sunglasses so I’m not recognized because I feel like I’m sinning... UHG! Type1 44 years, still on planet 🌎 earth...

    Fascinating exercise. I've watched a lot of good videos on YT and have received instruction from my Diabetes (T2) clinic staff, but this video really taught me how and when to add more inhaled insulin if needed. Many thanks, guys!

    I'm low carb so even seeing people eat this makes me feel ill. I'm all about experimenting and learning but 3 donuts is too much and that is way too much work to even bother.

      Incredibly irresponsible video and confusing to diabetics who hear conflicting info. Organ damage starts at glucoses above 140-150. Why do this to themselves?

The international bodies of diabetes organizations deemed 70-180 the ideal range…that’s why it’s called “time in range”, so wherever you got your information is wrong. We’ve received an overwhelmingly positive response on how to live life normally with diabetes.

    I appreciate this video so much. I've been taking insulin for 23 years and I still have mental hurdles to jump over every time I inject. Even if I know I have enough carbs to cover - it's irrational. But now I can picture Dr. P inhaling 12/24 units and being A-ok ^_^

    I wish I had the bravery to stay in range. I hate lows, 120 with an arrow slanted down gives me so much anxiety, high anxiety!

    Wow, as a type-1 diabetic who eats a low carb diet, this video stresses me out lol. Also, according to the instructional video, after inhaling afrezza, you're supposed to hold your breath for as long as is comfortable... I take afrezza.

    Great idea for a video! Questions:
    1) Was Steve using a rapid insulin e.g. Humalog/Novolog, or an ultra-rapid insulin such as Fiasp/Lyumjev in his pump?
    2) Did your pumps adjust their basal rates over the 3 hour period (e.g. Basal-IQ/Control-IQ), or were the basal rates fixed?

      I was using Humalog or Novalog, not the ultra-rapid insulin, but that wold have been better - I just didn't have any with me. Both of us are on hybrid closed loop systems. Jeremy is on Tandem CIQ and I'm looping. Both systems were doing their thing.

      @@TCOYDtv Can you help me understand - does the LOOP only deal with the natural up and downs of BS - for example in over night hours? The LOOP does not sense a rise and treat for the donuts - Obviously not - but I wasn't sure when the Loop knows to dose? Guess I will have to watch that video ;-)

      @@jannastern4620 The pump will give insulin if it predicts your blood sugar is going up, but when you eat something with that many carbs (like the donuts) you need to give an extra bolus. That is the hybrid closed loop system. The next generation will be a totally closed loop system where you don’t have to give an extra bolus or do anything about it, you just eat.     Hey! Thanks for the video! I’m a mom to a 10 year old T1, diagnosed 10 months ago. The thing that I wonder about is with continuing to give so much insulin, such as those 8 extra units upon seeing it go towards 160s, aren’t you sure to see it go low within a few hours? I know you can try to catch it before it goes low but is there some way to prevent the lows? It seems like I often need to give more insulin than should be required for some things but then always have to catch lows. Thanks 😊.

      Well for starters, it’s much easier when you’re only eating one donut, and not three. Pre-bolusing is key. You can also try to plan some exercise after the donut so the insulin is more effective and works faster. Set the CGM low alert at 80 in order to try to catch the lows, and if your child isn’t on a CGM, that is a must!! Also, in a few years when your child is 18, you can try using Afrezza.

    Thank you for making this! I wish I could show this to every "Food Police" idiot who ever told me, "You can't eat that.....".
    I'd just like to add, thank goodness for modern insulins and CGM's and pumps. This was far more difficult (probably impossible to stay in range) 35 years ago, when I was first diagnosed.

    With Loop, you enter the carbs plus 50% of the fat as carbs. Carbs @ 4hrs at the time you eat. Then a fat carbs entry 90 minutes in the future @ 3 or 4 hours, depending on the shop. Use an auto bolus branch. Done.
    That initial drop is very common. Minimal prebolus.

    Hi. This was my first TCOYD video. Loved it.
    How long before eating the donuts did Steve take the 8U & 3U boluses please?
    (Did Steve's grandparents have a house on Long Island, NY (near mine)?)

      I gave the 8 units 20 minutes before I ate, and then the 3 units when I saw the trend arrow going diagonal up. And YES my grandparents had a house on Long Island!!! My mom’s side of the family is from there - her maiden is Fleischer. My dad is not from there but Edelman is a s common name.

      Although the hard core software engineers would say it messes up the algorithm, I use Afrezza on top of the insulin in my CIQ all the time (as well as my patients) and do very well. Remember that these hybrid closed loop systems regulate the insulin delivery based on the rate of change and predicted glucose, so if it sees glucose changing because of Afrezza, it does its own adjustment.

      Well we don’t recommend eating three donuts every day, but if you do indulge, make sure you enjoy it, and if you’re on mealtime insulin you can always take more and just monitor your blood sugar. If you’re not on insulin, get out and get some exercise! Or, in exchange for the donuts, you can also cut back the calories in your previous meal or the meal you eat after.

And how, pray tell, is eating THREE donuts "moderation"?

      I did have to have a small box of juice near the end of the video..so technically Jeremy won. BUT he used Afrezza which has a rapid on and rapid off time course of action...a great way to avoid stacking.

      @@TCOYDtv I had heard of it before. I’m MDI, this was a fun experiment. But only one that can be done early in the day because I’d be afraid of crashing if it was late afternoon or later in the day. I still manage to stay 70-120 75% of the time, and eat pretty much what I want. But this video gave me anxiety with all the early preventative corrections lol

    The worst video you all have ever made.controlling diabetes is about doing what you can to minimize meds.The unspoken truth about meds/insulin is that people use it to less compliant.What’s needed is a mindset of appreciating healthy habits,not finding ways to avoid them
