A novel treatment for Keratosis pilaris

  • Keratosis pilaris is a common condition that can affect up to 80% of adolescents. A variety of topical agents are usually used in its management. This includes urea and salicylic acid containing emollients. However, treatment is not always successful with these creams. In this video we will look at the use of a commonly used oral medication for acne, isotretinoin, and how this may be helpful in severe and resistant Keratosis pilaris. It is based on the article published recently in Clinical and Experimental Dermatology.

    I’ve used adapalene to great success. So this makes sense. The only problem is that it causes severe dryness. After trying other treatments (ammonium lactate, urea, salicylic acid) and finding them to be too irritating, I’m trying adapalene again, but slathering a moisturizer on top. I’m two weeks in and starting to see results.

      You are right. Topicals can sometimes irritate the skin. Another natural oral option is Vitamin A as it could sometimes be a substitute for isotretinoin. Here is a video on it

      @@abhaysingh-mi2sp Yes, adapalene worked for me. It took a few weeks for me to start to see results, but after two months, my arms were clear. So be patient!
      Also, moisturize! Adapalene can cause some nasty dryness so you need to also moisturize. CeraVe has been fantastic. It’s gentle, non-irritating, and not sticky.

      Adapalene cream is a retinoid cram and is one of the treatments used for KP. So it will help to reduce the appearance of KP but doubt if it will get rid of it completely. @@abhaysingh-mi2sp

      As mentioned in the video, oral isotretinoin can be used in low doses for isotretinoin. A dose of about 20mg three times a week may be helpful as mentioned in the video. Vitamin A can be used as an alternative if isotretinoin is not available. Here is a video on it

    I have severe KP, was prescribed isotretinon for a month and a half, there was a significant improvement on the bumps, however no improvement on the pigmentation. Can chemical peels, lasers, micro blading help with that?

    I've had this my whole life. the entire length of my arms to my wrists, my legs, and a tiny bit on my chest. I also will get tiiiny random hard spots that I can pick off. (not sure what those are but I wonder if they are more keratin) Thank you for this video. How would I go about asking my doctor to try this medication? I feel like they wont listen to me if I ask.

      Some dermatologists will be happy to use Isotretinoin for keratosis pillars. If they are unable to provide it, another option may be to use Vitamin A, which could be a natural supplement that mimics Isotretinoin to a certain extent. Here is a video on it.

    Isotretinoin doesn't help rather increases the severity of kp..at least that's what i experienced

    Hello sir! Except my face(only a little bit of kp on it) and neck, all my body is full of sever kp and dark pigmented from the kp. My KP is not here and there, it is full of my entire body! I've seen so many doctors, they all say it is too much. If it is not itchy leave it as it is. But how I live with it? It's so dark and pumped all over my body. It has been 12 years. I've never wear sleevless clothes or any skin shown clothes in this entire 12 years! Even if i'm living in hot wether conditioned area. It is like hell! What shall I do sir, please help???

      The oral isotretinoin mentioned in the vide is an option worth considering.

    Hello doc , I have a kp all over my body and going under body laser just started I have kp on my face as well and also some bone issues can I take a cod liver oil to balance the keratin and u suggested isotretinoin how to use them and what to use for skin ? Could you pls suggest me what to use and how to use

      As mentioned in the video, ow dose Isotretinoin has been sued for extensive keratosis pillars. You could use the doses mentioned in the video, maybe starting at a low dose of about 20mg a day. There is no good literature on cod liver oil treatment for keratosis pillars.

      Most studies suggest that isotretinoin may need to be taken for about 4 to 6 months before a beneficial effect is noted.

      Yes, it can help the pigmentation too. It takes a lot of time for pigmentation to resolve in dark skin though.

    I have had Keratosis Pilaris all over my body since my childhood. I have consumed Isotretinoin 20 mg for 1 week but It caused several side effects like dry mouth, muscle pain, back pain, headache, etc. So, what should I do now, please suggest to me doctor

      The side effects of isotretinoin is dose related. So lower doses may have lower side effects. Another option is to use Vitamin A as it mimics isotretinoin. Here is a video on it.

      As mentioned in the video. low dose isotretinoin is a good option for KP if other treatments have not worked. If you are unable to get hold of isotretinoin, oral Vitamin A can be considered too. Here is a video on it:

    Sir, I had severe cystic acne after I stopped ocps which were given to me as I have PCOD. My doctor prescribed me isotretinoin which I took for almost a year. All the acne from my face were cleared but it had NO effect on the kp on my arms and other areas. If anything, it has spread even more. What do I do now? I also had a side effect of discoloration of my lips during the 10th month of isotretinoin which I was super scared of as it looked like vitiligo. After stopping isotretinoin the natural colour of my lips came back. Do I start applying topical gel over my arms or what?

      If there are side effects with Isotretinoin, topical retinoids are preferable for keratosis pilaris. 10% urea containing creams of lactic acid creams may also help. For the acne, metformin is an option of there is PCOS with acne. Here is a video on it

      Using topical retinoids may be helpful if KP is causing dark marks. It may be worthwhile confirming the diagnosis first as that is key to using the right treatment.

      Blood Vitamin A are usually normal in keratosis pillaris. Isotretinoin is worthwhile if keratosis pilaris has not responded to any other treatment.

      Lasers can help the redness but I think the hyperkeratosis (roughened skin) still requires a retinoid agent or topical agents that smoothen the skin. If isotretinoin is difficult to get, then oral Vitamin A is an option too.

    Hi doctor thank you for your video. Unfortunately i have developed keratosis pilaris on my arms and legs severely 2 months after i stopped accuatne for ny acne. It has been 3 years now and it has not gone away. Do you have any ideas as to why i developed it after isotretinoin when i didnt have it before? Thank you.

      Sometimes, medications can unmask previous dry and sensitive skin especially as keratosis pillars is associated with dryness of the skin. It is paradoxical that a medication can be used to treat a condition but can also cause it at times. It may be useful to moisturise the skin regularly. Following the principles of using the moisturisers mentioned in this video may be helpful

      Isotretinoin does not cure the condition, but may keep it settled for a few weeks if not a few months. Some use maintenance doses of once or twice a week too.

      Good question. Vitamin A in large doses can have a similar effect to Isotretinoin (Accutane). However it is likely to cause considerable dryness of the skin and lips and can also accumulate in the liver. Its a certainly an option if you do not have access to Isotretinoin. I will try to do a video on high dose Vitamin A in the future.

      Low doses are usually sufficient as high doses can cause side effects like very dry skin. Another natural substitute for isotretinoin is the use of oral vitamin A.       Sorry to hear of your skin problem. There are quite a few creams that could be used in keratosis pillars which your doctor could prescribe. Isotretinoin is more difficult to get as only dermatologists generally prescribe it. One option may be to use Vitamin A as it could be a substitute for isotretinoin. Here is a video on it
