I am dealing with this issue currently. I have learned more from your videos than I have from my primary care provider, orthopedic specialist and physical therapist combined over the last couple months. You explain things in a way that makes it easy to comprehend. Thank you thank you thank you!!
I really can't thank you enough for your educational videos. They are literally life changing. I'm working on strengthening my glutes now after years of exercise on my quads. This information is so important thank you !
How are you feeling 3 weeks later? I’ve had this problem for over 6 months, and my doctor and PT say my quads are stronger, yet my pain is worse. I’ve also been doing some glute work, and my balance is better, but I’m thinking I need to do more and of course, stop working the quads as much.
Your advice 100% agrees with my personal experience with my knee history. Thank god I found your vids. I got the worst advice from my therapists and my pain was out of control. ❤
I am trully grateful finding your videos coz it has informed me so much about my knee problem due to osteoporosis and osteoarthritis.. your exercises has helped me to move without medication.. god bless you in helping a lot of people❤
I’m a cyclist and a runner .Most of the time I’m ok and I’m so glad to have found your channel, because you’ve confirmed everything I believe about my patellar pain. I nearly got a knee replacement but so pleased I didn’t. I would like to be able to walk downstairs without looking like a 90 year old which I find very depressing, seeing as I’m a competitive athlete 😂
What is the best position to be in bed for sleeping? My knee hurts when I’m laying down. I don’t want to make it worse. My physical therapist is giving different info. Your videos have helped so much more. Thank you.
Dear doctor, your channel is priceless! I thank Universe for this gift! I found it in my despair, when my chondromal-a patella and meniscus got dramatically worse after long awaited improvement. Wrong walking, wrong exercises...nobody told me how to do it right in my clinic(
Thank you so much for explaining. I have watched almost all videos about this topic. I can't express how thankful I am. I got diagnosed yesterday and I have pain for 3 years and I was exercising quads a lot and it made it worse😢
Thank you. I’m a typical quad dominant runner and your advice to not train quads is refreshing eventhough training quads is what most therapists advise. I wonder what muscles, next to the glutes can be trained to strengthen knee and running posture.

Hello Dr.David, first of all, thank you for all your content and ideas that are more progressive than the classic medicine approach (holds true for Germany as well). I (33y old, hobby sports guy, did a lot of running, marathon and volleyball) injured my already not super healthy knee during a turnament, where I hit my foot and knee. According to the MRIs there is more liquid in the knee, but only a minor damage to the cartilage due to overusage and maybe the accident, however I lived in significant pain the last two years, even if not moving the knee at all. After adopting your "glute vs quad muscle imbalance ide" as well as feet/ankle training into my worout and finally feel like Im on the right path. Now, after two years of pain following an sport accident (knee and foot) I get to a level, where, when I use kineso tape, I do not have significant pain anymore during my daily life. Now my question: would you suggest that I could start playing a little volleyball again with kineso tape as long as the pain is not significantly higher afterwards (same day or days after), or would you suggest that I first need to reach a pain free level without the tape/sleeves before slowly returing to jumping/runnning sports? Thank you for your time and any help :)
Hi Pascal, We're sorry to hear you're dealing with knee pain. It's great to hear that you've found some relief with the strategies you've been implementing, especially considering the challenges you've faced over the past two years. Regarding your question about returning to volleyball, it sounds like you've been making significant progress, especially with the kinesio tape helping to manage the pain during your daily activities. While we understand the desire to get back into sports, it's crucial to prioritize your recovery and avoid exacerbating any discomfort. We would recommend proceeding cautiously and gradually reintroducing volleyball into your routine. Starting with kinesio tape as support is a good idea, but it's essential to pay attention to how your knee feels during and after playing. If you find that the pain remains manageable and doesn't worsen significantly, it could be a positive sign that you're on the right track. However, we would also advise striving for a pain-free level without the tape or sleeves before fully engaging in jumping and running sports. This may require some patience and continued dedication to your rehabilitation exercises, but it's crucial for ensuring long-term joint health and preventing further injury. Ultimately, listen to your body and trust your instincts. If you experience any significant discomfort or pain, it may be a sign to take a step back and focus on strengthening and stabilizing your knee further before returning to more strenuous activities. We wish you all the best with your continued recovery and return to sports!
Your exercises and advice have really helped and reduced my knee pain but I always have pain when going from sitting to standing. The worst is if I’m sitting at the table eating and always have difficulty when initially standing. Please can you help with this Dr David Thank you for all your videos.
My wife has chondromalacia stage IV, she’s had injections. I will definitely lead her to do the glutes and foot exercises and avoid the quads and see if things go better. Thank you ❤

What is the best position for sleeping. My knee hurts more when I lay down. I use pillows to relieve reassure but I don’t want make my knee issue worse. Any suggestions?
Another great video. I spent about a week doing your glute exercises.. It took a bit to become used to isolating that muscle. WOW, did those wipe me out once I figured them out. SO, I feeling pretty good, got on my spinning,,,QUADS....after 4 days of spinning,,,Yep, my knee gave out while just walking up stairs and I feel like I'm back to ice and heat before I can get going again. It's been 48 hours and hopefully by tomorrow I can get back to listening to your recommendation. Do you have any ideas on how to turn back time?

Hi Dr! You had mentioned that it is due to muscle imbalance that chondromalacia occurs. But what about if I had injured my knee and as a result I have chondromalacia?
great video! I was curious.....when I went to PT, the guy recommended these inserts that that actually forced my knee the opposite direction because I was having pain on the inside of my knee. Your video mentions to get the arch supports to get your knees inline more. Looking at my x-rays..I am or have lost a lot of cartilage..more so inside my knee. should I get arch supports instead of the other inserts?

Is there any research out there on peptide injections for this problem ? I’ve been suffering with front of knee pain from this for around 6 years. 3 months ago I started extensive physical therapy to help the muscle balance / tracking issues. Made great progress but I feel like exploring adding in injection based therapies.
I have patella degeneration and I don’t feel pain, but I swelled up once. I’m trying to learn how to keep it from swelling up. I walk around 13 miles a day for my job. I’m trying to change jobs, but that could take a month. In the meantime, I’m trying to learn how to keep my knee from swelling during that time.
Hey Jennifer, so sorry to hear you have been dealing with patella degeneration and constant swelling :-(. We are hoping you are succesful in finding a job that adjusts to your needs ASAP! In the meantime, some of the strategies you can implement are: improving the way you walk, strengthening your glutes, finding ways to allow your thigh muscles to relax and not takeover your motions. Here are some playlists and videos that could be of help! Thanks for the video. I did ask this in another related video, but you again mention 'healing' the chondromalacia patella here, and I wonder exactly what you mean by healing, since that rough/bumpy uneven cartilage (especially if you've had it for a long time) supposedly doesn't actually go away without surgical intervention.
I have grade 3 kneecap arthritis. My IT band is painful and lumpy and so are my quads and adductors all the way to my hips. It hurts to walk. This all became symptomatic after falling on my kneecap in September 2023. I’m not sure where to start my rehab. I have a torn hip labrum on the same side. Can you tell me which video of yours to start with. I don’t want to have exploratory surgeries to figure this out. Thank you and blessings.

For chondromalacia patella you have asked us to do glute strengthening,, is it sufficient ,, as our physio and Dr always insist on quads .. please advise
Thank u , i am 13 years old girl,i have knee pain ,Doctor told me that your cartilage is soften, there is no swelling, take a break for your dance and shuttle. X-ray looks normal.can it cures completely and go back to my activities?please advise me.
Hi I hope you are well. I appreciate you are busy. I have Patellar Chondromalacia and there is also an intra-articular loose body, a small structure around four millimeters in size, which may be a detached fragment of the articular cartilage. The articular cartilage injury is graded as four, The ligaments and menisci appear normal, and I have a Patella Alta, which is a slight elevation of the kneecap that may or may not contribute to the altered biomechanics and erosion of the articular cartilage. The report also mentions minimal fluid within the joint space, which is not of much concern. I see online Dr, You are to be the best around when it comes to Chondromalacia so any advice how to best treat it will be much appreciated as I’m fit 44yrs old and I’ve just got a HA injection but don’t think it’s helped. Again, I appreciate you are busy Colin Carrick
Hey Colin, sorry to hear that you have chondromalacia patella :-( especially at such a young age! This is likely due to overusing your quads for a long period of time. There is a muscle at your knee that not many people know about called the articularis genu, which pulls the patella up causing Patella Alta! This muscle is part of the quads. So, the best advice is to massage your quads very well and get them to relax a bit more while simultaneously working on improving the use of your glutes. Your glutes will commonly be relatively weak when you are overusing your quads and it will help restore muscular balance to your knee. This can help chondromalacia patella by allowing any damage that has been done to slowly heal over time. This needs to become a daily practice for you! Hi doctor...after starting glute strengthening exercises that you have recommended after how much period one can start strengthening quads or hamstrings?
I did softwave therapy for both knees and it made one of my knees hurt even more for about a week after. I only did 1 session because the provider used a very high level and it was so painful . I would have gone back if the intensity was lower to start.
Hey doc, my patella is slightly laterally offset due to a fall years ago , is there anyway I can fix it so the alignment is normal again. As I get pain when running etc
This info is helpful.. however for me it's too late already. I am only 36 and already have a small narrowing of my knee joint space and a lot of pain to come with it. Not seeing a bright future for myself 😢
Hey Stefaan, sorry to hear you are experiencing decreased joint space :( The good news is you are young enough that you can turn it around! The Knee Meniscus Recovery Program is designed to help people have the best chance of avoiding unnecessary surgery, injections, or having to rely on pain medications. This course guides people through exercises and stretches that target key body parts that have the greatest potential to prevent or help with meniscus tears or trouble after meniscus surgery. Hey Elif, we are sorry that you are having to deal with a grade 2 meniscus tear and grade 1 chondromalasia patella 😔 Our recommendation is to not do any quad exercises until you have strengthen your glute and ab muscles first. If the hamstrings and quads are overworked, it’s like having too much weight on one side of the seesaw, which puts extra strain on your knee. Our advice is to focus on strengthening your glutes and core, which balances the seesaw and eases the pressure on your knee. Once your glutes are stronger and the pain in your knee decreases, you can gradually reintroduce hamstring and quad exercises. The goal is to maintain a balance between the strength of your glutes, hamstrings, and quads. “Knee Meniscus Help” ua-cam.com/play/PLJVIPiFvv1mZ9Vh4eRH3UlmMeqOUtB-6a.html “Chondromalacia Patella Help” ua-cam.com/play/PLJVIPiFvv1mah5PV-1Y-fv1UHrmbqnXQE.html Whatever exercises you do decide to do, we recommend to start with one or two exercises, beginning with the smallest range of motion and the lowest intensity. As you feel stronger, you can gradually increase both the range of motion and intensity. It’s best to do these exercises every day throughout the day. Aim to tire out the muscle without losing form or control. Consistency is key. Always avoid working through pain; soreness and mild discomfort are normal, but pain is not. We hope this helps! Please keep us posted on your progress 🙌🏽
Correcting the imbalance can get more complicated depending on the muscle imbalance a person has. If it's more caused by movememts in the transverse plane you will have too much femoral external rotation on one side and internal on the other. So the focus will be on different exercises and stretches to balance things out. An example of sports that could cause this are surfing, golfing, tennis, snowboarding etc. All acruvities that require rotation predominately in one direction. Often times in rotational sports the primary imalance causing issues is going to be imbalances of the anterior and exterior obliques muscles. Research the anterior and posterior obliques slings to get a bettet understanding.
I would like to ask if there is a chance that the second stage of patellar chondromalacia can be cured according to the rehabilitation method you mentioned?
Hey Wes, all chondromalacia can be healed over time. The worse the damage is, the longer it will take. However, the pain will likely dissipate long before the damage is completely healed. So, yes, chondromalacia can be healed even in stage 2!
@@epmanualphysicaltherapy I would like to ask about the Patella Pain Recovery Program course. If my English level is very poor, will it be difficult to learn it? Because there are texts on UA-cam that can be displayed and translated into Chinese
Thank you so much, so glad I found your videos. Just got diagnosed with patella alta and chondromalacia (grade 2) caused by it. Every doctor that I went to suggested strengthening my quads is a way to solve the issue...which, I see now, will only worsen the problem by dragging patella up even more.. Will definitely start doing glute exercises and relaxing my quads right away since I don't really feel any pain right now just some tightness in the knee zone. Racket sports is a huge part of my life, can't really imagine not being able to play them. Hoping I will find some relief and eventually solve the issue. Thank you for sharing your knowledge!

My orthopedic doctor stated I had only 10% of hyaluronic acid remaining in my right knee which swells up on the outside of the knee when I do a lot of walking. What is the best therapy for this situation? Thanks, Aaron
Hello. I have a 1cm x 0.5 cm cartilage defect in the central part of the patella. Chondromolation stage 3 to 4. I play football, I am 28 years old. I haven't played football for 6 months. In 2 months I should have knee surgery (microfracture). My condition is improving now, I can slowly run. I'm interested when you say that quads should not be trained. When can you start training your quads and hamstrings, because I can't play soccer without it.. I want to be a fully active athlete, I also play tennis. That's why I'm wondering when it's time to start training other muscles as well?? Do you think surgery is necessary or not?? I also currently have hyaluronic acid in my knee.. thanks for the reply :)
Hey Ervin, sorry to hear about your cartilage defect behind your patella :-( You can start to train your quads after your muscles are in better balance. You know your muscles are in balance when you're confident that you use your glutes very well in nearly all physical activity. If you're afraid you're going to hurt your knee more because it's already starting to hurt a bit or swelling, then your quads are too strong and your muscles are out of balance. More glute muscle strength is needed. Most cases like yours can get by without surgery, but its always a personal decision that you have to weigh based on your needs and belief of how well you can heal. It's great that you got hyaluronic acid, hopefully it's helping you. It often takes 3 to 12+ months of training the glutes to get the imbalance properly fixed to take pressure off the knee cap and allow you to confidently play sports. Without seeing you in the clinic to accurately diagnose and treat you, the best advice would be to hold on surgery and go through fixing the muscle imbalance to see how much you can improve. Microfracture surgery has mixed results and you'll need to fix the imbalance anyway or you'll end up injuring the cartilage again even after you've healed from the surgery and return to sports. We highly recommend learning more about our 28 Day Knee Health & Wellness Boost Program. It's focused on fixing the root muscle imbalance that leads to knee cap cartilage problems. Hey Paula! This method of exercise has shown benefits for some people. However, what we have seen a lot of is overuse of the quads. If this is the case, then transferring the muscular dominance to the glutes may be a more effective method of dealing with chondromalacia patella. Hope this helps!

Hello Dr.David,
first of all, thank you for all your content and ideas that are more progressive than the classic medicine approach (holds true for Germany as well).
Hi Pascal,
We're sorry to hear you're dealing with knee pain.
Please can you help with this Dr David

Hey Shaoqinglihu696
You're welcome! We are so sorry to hear that your wife is suffering from chondromalacia patella issues. We know how painful it can be to deal with and we hope that she can feel some relief soon. 🙌
We have a chondromalacia patella playlist she might also find beneficial. You can access it here:
“Chondromalacia Patella Help”
ua-cam.com/play/PLJVIPiFvv1mah5PV-1Y-fv1UHrmbqnXQE.html
Start with one or two exercises, beginning with the smallest range of motion and the lowest intensity. As you feel stronger, you can gradually increase the range of motion and intensity. It’s best to do these exercises every day throughout the day. Aim to tire out the muscle without losing form or control. Consistency is key. Avoid working through pain; soreness and mild discomfort are normal, but pain is not.
If your wife has experienced positive results we would encourage you to look into the Patella Pain Recovery Program at some point. The program is similar to our in-person treatments minus the manual manipulation. Unlike traditional treatments like medications, injections, or surgery, we target the root cause of your knee cap pain: muscle imbalances. By addressing these imbalances, we provide lasting relief for conditions such as Chondromalacia Patella, Patellofemoral Pain Syndrome, Knee Tendonitis, Knee Bursitis, Runner’s Knee, Jumper’s Knee, and Osgood-Schlatters Disease.
Our program is designed to pinpoint the phase of patella pain you’re experiencing and guide you through a progressive series of exercises tailored to maximize your recovery. By strengthening and balancing the muscles around your knee, you’ll not only reduce pain but also prevent chronic issues and avoid unnecessary surgery.
Take the first step towards a pain-free life and reclaim your mobility. Join our Patella Pain Recovery Program today and start your journey to long-term knee health and freedom.

Same thing happened to me. What did you do
Thank you!!

great video!

How about the hamstring muscle do we have to shut it down too?
What are your thoughts on backwards walking for this condition?
So walking backwards is probably a bad idea... as the "knees over toes guy" recommends because that only strengthens the quad muscles ?
Is there any research out there on peptide injections for this problem ? I’ve been suffering with front of knee pain from this for around 6 years. 3 months ago I started extensive physical therapy to help the muscle balance / tracking issues. Made great progress but I feel like exploring adding in injection based therapies.
I have patella degeneration and I don’t feel pain, but I swelled up once. I’m trying to learn how to keep it from swelling up. I walk around 13 miles a day for my job. Hey Jennifer, so sorry to hear you have been dealing with patella degeneration and constant swelling :-(. We are hoping you are succesful in finding a job that adjusts to your needs ASAP!
Cartilage can regenerate ...just takes a long time..also fibrocartilage can form .

Kinesiology tape.. does it work for Chondromalacia Pattela?
Hi
I hope you are well.
Again, I appreciate you are busy
Colin Carrick
Here are some videos for activating your glutes properly in case you are interested in learning more:

Hello! What is your opinion about shock wave therapy for treating chondromalacia patella?

I have grade 2 meniscus tear and also grade 1 chondromalasia patella at the same time in the same knee. Am i should do quad exercise or not ?
Hey Elif, we are sorry that you are having to deal with a grade 2 meniscus tear and grade 1 chondromalasia patella 😔
Our recommendation is to not do any quad exercises until you have strengthen your glute and ab muscles first.

Thanks a lot for your help 😊
E3 rehab would disagree on arch supports, injection, PRP, and surgery
I would like to ask if there is a chance that the second stage of patellar chondromalacia can be cured according to the rehabilitation method you mentioned?
Hey Wes, all chondromalacia can be healed over time. The worse the damage is, the longer it will take. However, the pain will likely dissipate long before the damage is completely healed. So, yes, chondromalacia can be healed even in stage 2!
@@epmanualphysicaltherapy OK, thank you for your reply!

Racket sports is a huge part of my life, can't really imagine not being able to play them. Hoping I will find some relief and eventually solve the issue. Thank you for sharing your knowledge!

Hey Ervin, sorry to hear about your cartilage defect behind your patella :-(
You can start to train your quads after your muscles are in better balance.
You can learn more here: www.epmanualphysicaltherapy.com/knee-health
