You are spot on! I love your videos!! I started out with meniscus tears in both knees almost two years ago. After months of PT and tons of front leg raises, I ended up getting PFS- grade 2 Chondromalacia Patella. The front leg exercises just make everything worse. My leg muscles got so tight from all the exercises, and I could barely walk last year. So, then I was dealing with meniscus tears, PFS and I tore my hamstring muscle in PT. I switched PT's and now been through three PT's and they just gave me the same exercises that make it worse. I'm so glad I found you here. You are a Blessing!!! Thank you so much for all your information. I started a lot of the exercises you recommend last week and I'm already feeling a difference. I'm walking so much better too. Using glute muscles instead of quads. The exercise I really love is the standing glute raise. Genius!!! That's a new one for me and so helpful!! Much love and blessings!

@@tr6151 I am 95% healed now. I feel great and so thankful! I ended up seeing a chiropractor and that's why my knee wasn't healing. My back, neck and hip were out of alignment due to an auto accident I had a while ago. The chiropractor gave me stretches to do and I take Turmeric daily. I'm now back at the gym getting stronger and I still do a lot the glute exercises that are recommend here by the doctor and now using some leg weights. I also took a lot of the advice on here and I think with using a good muscle massager (Bob & Brad) . was massaging daily - sometime twice a day. My hamstring and quads were very tight and was causing a lot of my pain. It's been a long roller coater ride, but I'm so grateful I finally got the right help that I needed. I had to see the chiropractor for over two months- three times a week to get my hip and back in alignment. If anyone on here is having long term knee pain, please see a good chiropractor also. I think this may be a problem for a lot of people. And yes I am walking pain free now! :)) I'm walking faster than I have in the last two years. I hope this helps and I hope you feel better soon!

I have no words for describing how helpful all these videos are! Indeed, every doctor says to "rest", "not move"... I am currently post-injection and looking for ways to improve and get back to some activity!
Too much rest was really bad for me during my knee situation, muscles get weak fast but take longer to build. Then walking can be even more unbalanced and difficult, especially when in pain. Yet using the knees much when they're in pain is hard and problematic too. Seems finding a balance is best, whatever you can without hurting them and maybe as many sitting or laying exercises as you can that don't only work one muscle. Good luck getting back activity! I hope to get back to normal too.
I have condromalacia patella in my left knee since 2 months, I am doing all the exercises that you recommended along with low resistance cycling , walking . I already see so much improvement. And I am hoping in next 3 months my knee would feel normal. I am doing all the exercises for 1 hr every day and 40 mins cycling and walking . I did quads exercises before and they make me feel worse even at the begging I was thinking I feel better but that was false perception and I was end up in devasteted years of knee pain and tears thinking that nothing is going to help me, but now I been doing those gluts exercises for few month and I think I see some progress even it is going slow but at list I have hope that in time I will obtain some good results , as I really want to avoid surgery and I do agree with you that medication and injection are very temporary fix just to mask the pain without addressing the root of the problems ! Thank you for the video. You're the first person to recommend against quad exercises that I've seen for condromalaysia. I did quad exercises for 6 weeks with no improvement. Will be going strictly glutes and hamstrings and hopefully beat this thing!!
@@epmanualphysicaltherapy HEY, An important question here is which muscle need to be strenghthen based on tilting of your knee cap. Ist case , if your knee cap is tilted towards lateral side , 2nd case if your knee cap tilted towards medial side.

Hello and thank you very much for your videos. I have chondromalacia patella since over 10 years. I'm from Germany and It's quite shocking that orthopedics specialist don't tell us anything helpful at all. But it seems to be the same in the US. Ten years ago a found a very good physical therapist in Munich who has been working for Bayern Munich and the German national soccer team. It was a totally different world and treatment. Nothing to compare with normal ordinary physical therapists. It's not magic but a proper understanding of biomechanics of the human body. And what they were telling me is exactly what your saying here 👍 normally a physical therapist in Germany is not looking for muscle disbalance, none of doctors, when your are a public health patient. My last visit at an orthopedics specialist was totally useless. It's so frustrating because nobody cares. I will stop training my quads and keep on focusing on the glutes like the PT told me ten years ago
@@joshhazel742Hello, the cartilage behind my knee cap isn't getting any better anymore. since the beginning of 2023 it started on the right knee as well. Currently I am doing something similar as I did ten year ago. I found a good training program in the university hospital in Hamburg. They have a coloration with the local soccer team and the police. It is similar to the training I did back then in Munich. They even know each other :) Its not rocket science. It is mostly about balance, leg axis 🤔 and strengthening the gluts and core muscles. All of the exercises are without machines. You can call it functional training. Unfortunately, it is only about 10 h training and 10 h physiotherapy. I mean this is good in Germany, normally you only get 6 * 20 min from the public health service.
Hope your issue has been improving. I understand as well how frustrating it is when a doctor can't seem to explain things to you or doesn't seem to really care to help. They often just want to throw bandaids on the issue instead of helping to find how to really fix it, and sometimes in the USA that is because insurance companies don't want to pay for further testing or more expensive treatment. But sometimes the expensive stuff isn't always necessary. It can just be hard to find what is really best. We are lucky to have the internet to share stories and information and at least be able to try or ask about different things. A lot of misinformation there too, and conflicting information, but at least it's better than just what a doctor or two say.

Selbst zahlen ist aber möglich wenn man ein Rezept von einem Arzt hat. Derjenige, der mich damals behandelt hat ist allerdings nicht mehr da. Aber so einen Physiotherapie/Bewegungstherapie mit der Qualität habe ich nie wieder gehabt.
I got pfps in PE class in Feb 2022 and have had it ALMOST everyday since. I noticed that I had these 2 week periods every few months with zero pain or grinding and then it all returns. Idk but I want those 2 week periods to be permanent!
I recently diagnosed chondromalacia pattela ,I'm I'm shocked because I'm young active man , there is no injury...I can't digest...then Dr said tha same strengthen quad muscle,then I search you tube randomly,I saw videos of him ,then I realised I too over work quad muscle,now I'm doing hamstring work out feels better,thank you.
My physio taped up both my kneecaps to help my ligament tears but this treatment gave me chondromalacia patella grade 1 :( Just starting on your glute exercises with confidence 👍
I wonder if the taping prevented normal movement of the kneecap. I feel like it makes sense that having it too tight or wearing that or braces too much could do that. When I wore a brace for my injury it often seemed to produce pain at times and still didn't enable me to do things I couldn't during anyway, so I don't like to wear one. Good luck with the recovery! @@Sis.Sweets thank you. I'm thinking that the tape lifted the patella up too much and stretched the cartilage causing the damage. The physio acknowledges what happened as a result but still my body is an unusual case with sensitive tissue and lack of strong healing response that he did not anticipate. It's already getting better as long as I commit to my recovery and don't fall into situations which trigger a flare up. Good luck with your recovery 👍
Thank you Dr. David. Initially I had patella tendonitis, which got better after about a year's of squatting, but now I have this chondromalacia patella issue. Just like what you said from 3:40, my squatting exercises strengthened the front of my quad muscle, but my side quad muscle remains weak, resulting in misalignment of the knee cap. I will check out the glute exercises you recommended and start working on my side quad muscles. Your video give me hope. Thank you!

Best way to diagnose? I'm suggested an MRI. My problem in knee occured 2 years ago. Months of rest but getting up from kneeling, doing knee pressed exercise has flared up the knee again. 1. My ortho did rotation, stretch of the knee and leg but I did not scream in pain. 2. I only have a nagging pain in the horizontal part across the knee. 3. Walking sometimes excarbetate pain. 4. Swelling on right knee is so much more prominent. 5. I thought I had bursitis but he hinted at chrondromalacia. Less chances of ligament or meniscus tear. 6. Asked me not to walk or jump or jog on hard surfaces. But how long not to do it?

@@epmanualphysicaltherapy hi thank you! I didn’t have a knee replacement but underwent an arthroscopic surgery for a chondromalacia patellae grade 2/3 and minor meniscus tear. This was recommended by the surgeon… Three years after the surgery I still have more pain than before the surgery… I used to do a lot of sport (triathlon) and can hardly run after the surgery. It is clear that in my case it would have been sufficient to do exercises to rebalance the muscle strength, work on the glutes and hamstrings and do some stretching.
As a bodybuilder I think I suffer from this. When I regularly did squats I had no issues with my knees but for the last year I was almost exclusively doing leg presses for my pressing movement. I guess it created some sort of an imbalance
Omg I'm dealing with this right now. It's been 9 weeks. I was on crutches for 7 weeks. My knee was so swollen and painful I could barely bend or straighten. It seems to be improving finally but it's still pretty bad. My surgeon can't seem to find the cause of my issues even after a MRI. I had no choice but to rest. Hi doctor . The explanation u have given in this video 100 true . I had a minor issue and when I consulted the consultant they start QSE and HSE which made my condition worse . Plus it pushed patella more Towards the joint . Can u guide me in one thing . One question - you say the key is to address muscle imbalance and that the typical imbalance is that the quad muscles are too strong vs. other muscles; therefore you say quads do not need to be exercised and the focus should be on the glutes. But, what if the imbalance is WEAKNESS in the quad vs. the other muscles: does that change the treatment approach? In this case, would it not make sense to strengthen the quads (vs. the glutes)? 4:27 This is surprising to hear since every other PT seems to say that strengthening your VMO is the #1 priority to helping fix chondromalacia patella.
Hey Mike! Unfortunately, your VMO is not a functional muscle. Throughout your normal daily life, you are not going to use it very often. So even if you strengthen it, you will not be using it. The idea behind strengthening the VMO is that if it pulls your patella to the inside more, it will be able to counteract the stronger muscles on the outside of the knee. Even if the VMO is strong, you will not be using it enough throughout the day to make a difference. Instead, we take an alternative approach that focuses on decreasing the overuse of all of the thigh muscles. This will put less force through the patella and knee joint, therefore decreasing pain and inflammation in the area. Hope this helps!

So far this is the best video explaining this problem. Mine is lateral on both knees, and have lax ligaments. My questions: 1. At the gym should I also stop curl, abduction and adduction weight exercises? Earlier videos I understood to stop only the ones strengthening the front quad. 2. Both knees have equal damage. Why does only the knee that had the surgeries hurt? 3. The better, no pain knee has the weaker side glute. I'm confused. I expected it to be the other way.
@@binny9820 I still have the knee pain despite the glute exercises. Maybe I'm not doing enough? Three weeks ago my right patella dislocated. It took 2-1/2 weeks to heal. This happens bc of my congenital knee condition. Maybe these exercises help those with developed knee problems instead of born with them. I still do them. It can only help. And I've shared these videos with others.
I was diagnosed with chondromalacia patella but because of lateral tracking pressure. In that case, wouldn't it be recommended to strengthen the quads as well?
After hiyaluronic shots and then stem cells injected (which I paid good money for) and a physical therapist telling me I need to exercise my quads... I finally found you. I'm really trying but somewhat loosing faith in these 5 years of struggle. I'm scared to get on a bike, scared to cycle and basically hooked on diazepam because of my knees. Are you doing better now? I am in acute pain and waiting for my appointment to get an injection for the first time. The physiotherapist I saw last year also insisted on the quads exercises.
@@MissSarahGM I feel you... Injections will help, if you hadn't done an MRI, make an appointment because knees are complex and in my case only that scan showed the cartilage issue. Also, supplements help but in the long run, not immediately. Better yes, but it takes time for it to heal, so patience is key...

Though its the forst time i am hearing not to strengthen quads and hams for knee but only gluts...i see the point. I too have knee pain for 3 years now, and been doing the leg raises since that long, but its not healing my knee. Recently i was diagnosed with chondromalacia patallea. Yes I am feeling better after almost 2 months now...strengthening my gluteus medius (abductors) as well as gluten maximus (extensors). Just extensors did not help much.
Hi brother, i have been diagnosed for chondromalacia patella 2 years ago will this ever heal completely so i can run and play football again. I cant put weight on my right knee without having pain on it. I cant climb and go down the stairs in a normal way.
Hey @linagacha1164, sorry to hear about your chondromalacia of 2 years :-( Whether or not you can get back to running and playing football again depends on how much you're able to heal. It sounds like your knee is very flared up right now since you cannot put weight on it, climb or go down stairs normally. You might need to rest it a bit until it feels well enough for you to do glute exercises without making your knee hurt more. The potential for your recovery depends on how much you can strengthen your glutes and reverse your muscle imbalance to take pressure off your knee cap cartilage.

I was diagnosed with this in 2015. First it was my right knee then my 2nd knee as well. Now the pain is gone but my knees still crack a lot and are sometimes quite loud (crepitus). I do have some little pain sometimes but it's not a big deal for me.
What if you have patella maltracking laterally plus superior lateral edema on the fat pad? Will get pads also heal once you fix the patella maltracking as well?

About a year ago I experienced my first pain with chromolaciam. Im a runner and mechanic so lots of use. Its back a year later and seems to be worse. One thing i read was about flat feet and muscle imbalnce. I had a bulged L5 but it hasn't been any issue in over 6 months. In terms of taking with a doctor would you start with an orthopedic and then mention my feet issue or see a podiatrist too. Im trying to develop a plan to prevent injuries and stay mobile. Im a runner almost 40 but im starting to have injuries add up more frequently and afraid ill lose my ability to run prematurely
i just started running 10 days ago. and my knee hurts a bit when walking and running. but it doesnt hurt when i put pressure on it. is it only natural? because i havent run for around 10 years.
So true, I was thinking the same , myself , today. That these doctors themselves never had this tormenting, chronic, debilitating pain, and therefore it's easy to say, do strengthen VMO! Isolate it!! U r not exercising correctly, so no relief!! I have been suffering from this pain for more than a year now and despite many injections and exercises . Nothing helped much in my pain. It feels like, it will never go. When I do intense quads especially try to strengthen VMO, my whole quads get sore for 2 to 3 days and it s all the more painful. I m so frustrated 😢

Recently dislocated my kneecap after I did it when I was 14 and also after a full knee dislocation at 16... Pain is much better and I can walk and rest without pain, although I have been diagnosed with severe patellar chondromalacia. I'm 20 years old, athletic and am hoping my young age might help a bit. My surgeon did note that building the quad might help which is counter to what I heard in this video! I was wondering if there was a reason for this? perhaps chondromalacia caused as a result of muscle imbalance requires different treatment than my specific case? I'm meeting with a PT tomorrow so hoping all goes well... I'm grateful for your video as there isn't much info on this topic.
From your videos I worked out myself how I had caused my problem and hopefully what would help. Age 79. The doctor just said after x rays, bad osteoarthritis!!!! Referred me to NHS physio would suggest all the Wrong exercises. No thanks. It is a guessing game if I have chondromalacia but looking like it is. Health care in the uk now is abysmal...
Would your advice be the opposite for someone with patella alta and/or patella dislocation concerns? In other words, for those people, they want the patella to sit as deeply in the trochlear groove as possible. Their problem isn't lack of cartilage behind the kneecap. It's the kneecap being so minimally in the groove and it's too free floating (leading to easy dislocations). So would quad exercises and VMO exercises, that create more downward pressure on the kneecap into the trochlear groove, as well as adding stabilization on the sides in case of minor patella movement, be advised? More so than glutes?
I am dealing with same problem from last 3 years. Now I am playing basketball and it still hurts most of the time it’s off and on. Now I started focusing more on posterity chain strength training. I am just wondering if I should keep playing basketball throughout the rehab.
Walking around the block is what always triggers my knee pain?! I was a competitive cheerleader in highschool which is when my knee pain began. I went to an orthopedic surgeon, was told I had tendinitis and to wear knee hands. Fast forward 15 years and still have knee pain. If I rest then, the pain will usually subside. But I love working out and can only do “knee friendly” leg workouts which keep the pain down. But then if I do too much walking or standing, the knee aching is back and lasts days and days. I have 4 small children so I am usually walking. Any advice?

I had an X-ray and doc said looks good with no signs of arthritis. Would this show up on an MRI because based on symptoms I’m thinking I have this or bursitis. I've suffered with PFPS for decades and I've had crepitus since childhood. My knee pain just got severely worse last Sunday to the point that it hurts when I lift my leg with my knee at a 90⁰ angle. It hurts just placing a little pressure when cycling on the back leg/pedal. I've never had it hurt like this before. I'm used to the pain then squatting or crouching but not this. Putting on pants is painful. I've tried VMO stuff before and that never made sense to me because it's difficult to isolate. I've also tried glue medius exercises. Nothing worked

Help, I have a bakers cyst and this last august pseudogout in my whole left leg, I still have trouble bending it. I’m having PT once a week. 1991 I did have knee surgery after a ski accident, trimmed meniscus, shaved femur condyle, cruishett ligament repair. Medial knee hurt just before pseudogout, then my posterior thigh hurt. Now it’s stuck, won’t bend.?surgeon wants to do TKR. I just want to start with a partial…. If that….scared. Your thoughts… plus, I had 2 PRP treatments, while I’m taking low dose aspirin ( massive MI 2008)
I don't understand why Bakers cyst aren't really treated. I have one too and I can feel the small soft lump where it is. They said ",it's a small one" I don't know why it hurts so much. They also say that I have. "little" knee arthritis. Omgosh sometimes I can't even walk on it! I hope my new orthopedics Dr will have more options for my "little" bit of knee arthritis!
Hey there Doc, I have an issue where my patella tracks or is being dragged more Medially and can't find much information about this issue online, most time it's the other way around that it tracks laterally? I do have bad quad strength and overall strength in my legs due to unactivity. I'm just curious about how you would adress this issue. Do I need to release the Vmo that's pulling or just get bigger quads in general? When I sit on the ground and flex I notice my Vmo pre-firing over the lateral side. I got a shooting knee pain from squatting exercise after not squatting in a while..But that same knee is also on the same leg where I have heel pain, which may have had some cause..I had trouble going up stairs for a while..I got better after 2 months, and then got the pain again after playing bowling. However while climbing stairs or jumping or running I have feeling pain on upper side of knee cap. Do you think that can glute exercise help me? By the way, as my doctor said, I am tried to strength my quadriceps muscles since after fraction I lost a lot of muscles.Even I did a lot of exercise, still my muscles are so weak compare to my healthy leg. Hey sir, I have very athrophied VMO in my right leg - the worse leg of my two kneecaps and I also have very good mind muscle connection for my glutes, also I have a good tonality in them and they are really a dominant muscle for me. I just had HA injection yesterday and is it normal to have lots of pain. Also Dr. Said I should see a back doctor to rule out nerves… then he also mentioned TKR potentially for this. Just so scared…thank for all you do Dr. David and team.
I have chondromalacia grade 4 injections made it worse ! took 6 weeks had a flare up then they advised quad physio made it so bad I was on a crutch for 3 weeks . Watching Dr David I can walk without a crutch and slowly doing tailgating and clams what a difference
What about if you want bigger quads? I’m into bodybuilding and a getting post traumatic arthritis; CM has literally every health professional telling me to avoid quads. What are some exercises to do, specifically? Maybe a video? I know about avoiding deep squats, but what about sumo squats which target glutes, or partial ROM leg extensions (I know these are the worst for the patella but they are the best for quad growth) Loved the video, it’s calmed me a lot since getting the diagnosis. Most things I’ve read have just scared me into thinking I’ll be a cripple for the rest of my life. (I’m in my early 20s)
I have the same question. For what its worth the physiotherapist told me shallow squats are ok and also leg extensions, avoid lunges and deep squats. That was recently so I've only done a couple of workout sessions, too early to report back. I have Grade 4 in one knee but only a small spot. The info out there on exercises is really confusing. A lot of the body building channels when they mention knee problems are not necessarily being specific to CM.

You are spot on! I love your videos!! I started out with meniscus tears in both knees almost two years ago. After months of PT and tons of front leg raises, I ended up getting PFS- grade 2 Chondromalacia Patella. The front leg exercises just make everything worse. My leg muscles got so tight from all the exercises, and I could barely walk last year. So, then I was dealing with meniscus tears, PFS and I tore my hamstring muscle in PT. I switched PT's and now been through three PT's and they just gave me the same exercises that make it worse. I'm so glad I found you here. You are a Blessing!!! Thank you so much for all your information. I started a lot of the exercises you recommend last week and I'm already feeling a difference. I'm walking so much better too. @@tr6151 I am 95% healed now. I feel great and so thankful! I ended up seeing a chiropractor and that's why my knee wasn't healing. My back, neck and hip were out of alignment due to an auto accident I had a while ago. The chiropractor gave me stretches to do and I take Turmeric daily. I'm now back at the gym getting stronger and I still do a lot the glute exercises that are recommend here by the doctor and now using some leg weights.
I also took a lot of the advice on here and I think with using a good muscle massager (Bob & Brad) . was massaging daily - sometime twice a day. My hamstring and quads were very tight and was causing a lot of my pain. It's been a long roller coater ride, but I'm so grateful I finally got the right help that I needed.
I had to see the chiropractor for over two months- three times a week to get my hip and back in alignment. If anyone on here is having long term knee pain, please see a good chiropractor also. I think this may be a problem for a lot of people. And yes I am walking pain free now! :)) I'm walking faster than I have in the last two years. I hope this helps and I hope you feel better soon!

I have no words for describing how helpful all these videos are! Indeed, every doctor says to "rest", "not move"... I am currently post-injection and looking for ways to improve and get back to some activity!
Too much rest was really bad for me during my knee situation, muscles get weak fast but take longer to build. Then walking can be even more unbalanced and difficult, especially when in pain. Yet using the knees much when they're in pain is hard and problematic too. Seems finding a balance is best, whatever you can without hurting them and maybe as many sitting or laying exercises as you can that don't only work one muscle. Good luck getting back activity! I hope to get back to normal too.
I have condromalacia patella in my left knee since 2 months, I am doing all the exercises that you recommended along with low resistance cycling , walking . I already see so much improvement. And I am hoping in next 3 months my knee would feel normal. I am doing all the exercises for 1 hr every day and 40 mins cycling and walking . Hello and thank you very much for your videos. I have chondromalacia patella since over 10 years. I'm from Germany and It's quite shocking that orthopedics specialist don't tell us anything helpful at all. But it seems to be the same in the US. Ten years ago a found a very good physical therapist in Munich who has been working for Bayern Munich and the German national soccer team. It was a totally different world and treatment. Nothing to compare with normal ordinary physical therapists. It's not magic but a proper understanding of biomechanics of the human body. And what they were telling me is exactly what your saying here 👍 normally a physical therapist in Germany is not looking for muscle disbalance, none of doctors, when your are a public health patient. My last visit at an orthopedics specialist was totally useless. It's so frustrating because nobody cares. I will stop training my quads and keep on focusing on the glutes like the PT told me ten years ago
@@joshhazel742Hello, the cartilage behind my knee cap isn't getting any better anymore. since the beginning of 2023 it started on the right knee as well. Currently I am doing something similar as I did ten year ago. I found a good training program in the university hospital in Hamburg. They have a coloration with the local soccer team and the police. It is similar to the training I did back then in Munich. They even know each other :) Its not rocket science. It is mostly about balance, leg axis 🤔 and strengthening the gluts and core muscles. All of the exercises are without machines. You can call it functional training. Unfortunately, it is only about 10 h training and 10 h physiotherapy. I mean this is good in Germany, normally you only get 6 * 20 min from the public health service.
Hope your issue has been improving. I understand as well how frustrating it is when a doctor can't seem to explain things to you or doesn't seem to really care to help. They often just want to throw bandaids on the issue instead of helping to find how to really fix it, and sometimes in the USA that is because insurance companies don't want to pay for further testing or more expensive treatment. But sometimes the expensive stuff isn't always necessary. It can just be hard to find what is really best. We are lucky to have the internet to share stories and information and at least be able to try or ask about different things. A lot of misinformation there too, and conflicting information, but at least it's better than just what a doctor or two say.

@@isinski674 Hallo, das war kein Orthopäde sondern ein Physiotherapeut. Sein Name ist Oliver Schmidtlein. (OSPHYSIO training & therapie) Allerdings behandelt er nur auf Privatrezept. Selbst zahlen ist aber möglich wenn man ein Rezept von einem Arzt hat. Derjenige, der mich damals behandelt hat ist allerdings nicht mehr da. Aber so einen Physiotherapie/Bewegungstherapie mit der Qualität habe ich nie wieder gehabt.
I got pfps in PE class in Feb 2022 and have had it ALMOST everyday since. I noticed that I had these 2 week periods every few months with zero pain or grinding and then it all returns. Idk but I want those 2 week periods to be permanent!
Yeah it's bad but some people would like to have these 2 weeks of pain and then zero pain for every few months

My physio taped up both my kneecaps to help my ligament tears but this treatment gave me chondromalacia patella grade 1 :(

I wonder if the taping prevented normal movement of the kneecap. I feel like it makes sense that having it too tight or wearing that or braces too much could do that. When I wore a brace for my injury it often seemed to produce pain at times and still didn't enable me to do things I couldn't during anyway, so I don't like to wear one. Good luck with the recovery! Hope to hear it works. Hoping to get back to normal myself.
@@Sis.Sweets thank you. I'm thinking that the tape lifted the patella up too much and stretched the cartilage causing the damage. The physio acknowledges what happened as a result but still my body is an unusual case with sensitive tissue and lack of strong healing response that he did not anticipate. It's already getting better as long as I commit to my recovery and don't fall into situations which trigger a flare up.

Just like what you said from 3:40, my squatting exercises strengthened the front of my quad muscle, but my side quad muscle remains weak, resulting in misalignment of the knee cap. Best way to diagnose? I'm suggested an MRI. My problem in knee occured 2 years ago. Months of rest but getting up from kneeling, doing knee pressed exercise has flared up the knee again.
1. My ortho did rotation, stretch of the knee and leg but I did not scream in pain.
2. I only have a nagging pain in the horizontal part across the knee.
3. Walking sometimes excarbetate pain.
4. Swelling on right knee is so much more prominent.
5. I thought I had bursitis but he hinted at chrondromalacia. Less chances of ligament or meniscus tear.
6. Asked me not to walk or jump or jog on hard surfaces. But how long not to do it?

I'll go in to see him in about two weeks. Afraid of what he is going to say. If he wants me to do quad exercises, I don't know what I will say if he wants me to exercise my quads. "Well this physical therapist on UA-cam says......" I'm sure that's not going to fly with a Dr......I couldn't exercise my quads anyway

I had knee crepitus for all of my conscious life but the pain is a new addition at age 31.

@@epmanualphysicaltherapy hi thank you! I didn’t have a knee replacement but underwent an arthroscopic surgery for a chondromalacia patellae grade 2/3 and minor meniscus tear. This was recommended by the surgeon… Three years after the surgery I still have more pain than before the surgery… I used to do a lot of sport (triathlon) and can hardly run after the surgery. It is clear that in my case it would have been sufficient to do exercises to rebalance the muscle strength, work on the glutes and hamstrings and do some stretching.

As a bodybuilder I think I suffer from this. When I regularly did squats I had no issues with my knees but for the last year I was almost exclusively doing leg presses for my pressing movement. I guess it created some sort of an imbalance

It seems to be improving finally but it's still pretty bad. My surgeon can't seem to find the cause of my issues even after a MRI. I had no choice but to rest. One question - you say the key is to address muscle imbalance and that the typical imbalance is that the quad muscles are too strong vs. other muscles; therefore you say quads do not need to be exercised and the focus should be on the glutes. But, what if the imbalance is WEAKNESS in the quad vs. the other muscles: does that change the treatment approach? In this case, would it not make sense to strengthen the quads (vs. the glutes)? 4:27 This is surprising to hear since every other PT seems to say that strengthening your VMO is the #1 priority to helping fix chondromalacia patella.
Hey Mike! Unfortunately, your VMO is not a functional muscle. Throughout your normal daily life, you are not going to use it very often. So even if you strengthen it, you will not be using it. The idea behind strengthening the VMO is that if it pulls your patella to the inside more, it will be able to counteract the stronger muscles on the outside of the knee. Even if the VMO is strong, you will not be using it enough throughout the day to make a difference. Instead, we take an alternative approach that focuses on decreasing the overuse of all of the thigh muscles. This will put less force through the patella and knee joint, therefore decreasing pain and inflammation in the area. Hope this helps!

So far this is the best video explaining this problem. Mine is lateral on both knees, and have lax ligaments. My questions:
1. At the gym should I also stop curl, abduction and adduction weight exercises? Earlier videos I understood to stop only the ones strengthening the front quad.
2. Both knees have equal damage. Why does only the knee that had the surgeries hurt?
3. The better, no pain knee has the weaker side glute. I'm confused. I expected it to be the other way.

Three weeks ago my right patella dislocated. It took 2-1/2 weeks to heal. This happens bc of my congenital knee condition.
Maybe these exercises help those with developed knee problems instead of born with them.

Are you doing better now? I am in acute pain and waiting for my appointment to get an injection for the first time. The physiotherapist I saw last year also insisted on the quads exercises.

Injections will help, if you hadn't done an MRI, make an appointment because knees are complex and in my case only that scan showed the cartilage issue. Also, supplements help but in the long run, not immediately. Better yes, but it takes time for it to heal, so patience is key...

Though its the forst time i am hearing not to strengthen quads and hams for knee but only gluts...i see the point. Yes I am feeling better after almost 2 months now...strengthening my gluteus medius (abductors) as well as gluten maximus (extensors). Just extensors did not help much.

Hi brother, i have been diagnosed for chondromalacia patella 2 years ago will this ever heal completely so i can run and play football again. I cant put weight on my right knee without having pain on it. I cant climb and go down the stairs in a normal way.

Whether or not you can get back to running and playing football again depends on how much you're able to heal. It sounds like your knee is very flared up right now since you cannot put weight on it, climb or go down stairs normally. You might need to rest it a bit until it feels well enough for you to do glute exercises without making your knee hurt more.
The potential for your recovery depends on how much you can strengthen your glutes and reverse your muscle imbalance to take pressure off your knee cap cartilage.

I was diagnosed with this in 2015. First it was my right knee then my 2nd knee as well. Now the pain is gone but my knees still crack a lot and are sometimes quite loud (crepitus). I do have some little pain sometimes but it's not a big deal for me.

About a year ago I experienced my first pain with chromolaciam. Im a runner and mechanic so lots of use. Its back a year later and seems to be worse. One thing i read was about flat feet and muscle imbalnce. I had a bulged L5 but it hasn't been any issue in over 6 months. In terms of taking with a doctor would you start with an orthopedic and then mention my feet issue or see a podiatrist too. Im trying to develop a plan to prevent injuries and stay mobile. Im a runner almost 40 but im starting to have injuries add up more frequently and afraid ill lose my ability to run prematurely

I have been suffering from this pain for more than a year now and despite many injections and exercises . Nothing helped much in my pain. It feels like, it will never go. When I do intense quads especially try to strengthen VMO, my whole quads get sore for 2 to 3 days and it s all the more painful. I m so frustrated 😢

Recently dislocated my kneecap after I did it when I was 14 and also after a full knee dislocation at 16... Pain is much better and I can walk and rest without pain, although I have been diagnosed with severe patellar chondromalacia. I'm 20 years old, athletic and am hoping my young age might help a bit. My surgeon did note that building the quad might help which is counter to what I heard in this video! I was wondering if there was a reason for this? perhaps chondromalacia caused as a result of muscle imbalance requires different treatment than my specific case? I'm meeting with a PT tomorrow so hoping all goes well... I'm grateful for your video as there isn't much info on this topic.
From your videos I worked out myself how I had caused my problem and hopefully what would help. Age 79. The doctor just said after x rays, bad osteoarthritis!!!! Referred me to NHS physio would suggest all the Wrong exercises. No thanks. It is a guessing game if I have chondromalacia but looking like it is. Health care in the uk now is abysmal...
Would your advice be the opposite for someone with patella alta and/or patella dislocation concerns? In other words, for those people, they want the patella to sit as deeply in the trochlear groove as possible. Their problem isn't lack of cartilage behind the kneecap. It's the kneecap being so minimally in the groove and it's too free floating (leading to easy dislocations). So would quad exercises and VMO exercises, that create more downward pressure on the kneecap into the trochlear groove, as well as adding stabilization on the sides in case of minor patella movement, be advised? More so than glutes?
I am dealing with same problem from last 3 years. Now I am playing basketball and it still hurts most of the time it’s off and on. Now I started focusing more on posterity chain strength training. I am just wondering if I should keep playing basketball throughout the rehab.
Walking around the block is what always triggers my knee pain?! I was a competitive cheerleader in highschool which is when my knee pain began. I went to an orthopedic surgeon, was told I had tendinitis and to wear knee hands. Fast forward 15 years and still have knee pain. If I rest then, the pain will usually subside. But I love working out and can only do “knee friendly” leg workouts which keep the pain down. But then if I do too much walking or standing, the knee aching is back and lasts days and days. I have 4 small children so I am usually walking. Any advice?

I had an X-ray and doc said looks good with no signs of arthritis. Would this show up on an MRI because based on symptoms I’m thinking I have this or bursitis. Yes, I'm exactly the same, I had a knee x-ray, which was fine, but I still have what I call a toothache in my knee!
I've suffered with PFPS for decades and I've had crepitus since childhood. My knee pain just got severely worse last Sunday to the point that it hurts when I lift my leg with my knee at a 90⁰ angle. It hurts just placing a little pressure when cycling on the back leg/pedal. I've never had it hurt like this before. I'm used to the pain then squatting or crouching but not this. Putting on pants is painful. I've tried VMO stuff before and that never made sense to me because it's difficult to isolate. I've also tried glue medius exercises. Nothing worked

Help, I have a bakers cyst and this last august pseudogout in my whole left leg, I still have trouble bending it. I’m having PT once a week. 1991 I did have knee surgery after a ski accident, trimmed meniscus, shaved femur condyle, cruishett ligament repair. Medial knee hurt just before pseudogout, then my posterior thigh hurt. Now it’s stuck, won’t bend.?surgeon wants to do TKR. I just want to start with a partial…. If that….scared. Your thoughts… plus, I had 2 PRP treatments, while I’m taking low dose aspirin ( massive MI 2008)
I don't understand why Bakers cyst aren't really treated. I have one too and I can feel the small soft lump where it is. They said ",it's a small one" I don't know why it hurts so much. They also say that I have. "little" knee arthritis. Omgosh sometimes I can't even walk on it! I hope my new orthopedics Dr will have more options for my "little" bit of knee arthritis!
Hey there Doc, I have an issue where my patella tracks or is being dragged more Medially and can't find much information about this issue online, most time it's the other way around that it tracks laterally? I do have bad quad strength and overall strength in my legs due to unactivity. I'm just curious about how you would adress this issue. Do I need to release the Vmo that's pulling or just get bigger quads in general? When I sit on the ground and flex I notice my Vmo pre-firing over the lateral side.

@@Shining1712 no i was not taking anything for 10 years, the pain just comes and goes when I played football or doing something with my knee for longer periods. Just now few weeks back I started taking some Glucosamine and chondroitin, collagen tablets which is same as diacerin or whatever your doctor gave

@@TriHard0thank you for replying😊..... My orthopaedic said I'm having it by birth only now it's showing symptoms so we can't do anything for this....he demotivated me😂... And prescribed diacerin for lifetime..... And he said to do strengthening of quadriceps but I'm not following him ..... I'm focusing glutes strengthening.
Is Chondromalacia the same as Chondropathy? I have patella chondropathy and not sure what to do to help myself :(

I have had kneecap disloactions. But I am pain free now. Doc diagnosed with CMP with knee cap moved to one end like u explaine din th video... I can do pain-free walking and stairs.

I have grade 4 chondromalacia. Is it possible to heal this? Im a personal trainer and i do a lot of glute training but its not improving. I should mention i have bunions. Thank you.

How definitive is an mri for diagnosing this problem? Especially compared to xrays?

Hello sir, I hit my knee to the desk very hard and had fracture on femur medial epicondyle 4 months ago. I recovered from fracture and I can walk properly without any pain. However while climbing stairs or jumping or running I have feeling pain on upper side of knee cap. Do you think that can glute exercise help me? By the way, as my doctor said, I am tried to strength my quadriceps muscles since after fraction I lost a lot of muscles.Even I did a lot of exercise, still my muscles are so weak compare to my healthy leg. Sometimes I am being too much pessimistic about being able to run and jump again or being comfortable at stairs.

I have very athrophied VMO in my right leg - the worse leg of my two kneecaps and I also have very good mind muscle connection for my glutes, also I have a good tonality in them and they are really a dominant muscle for me. My quads however, are very thin and weak. So what now? Can my tight/weak hip flexors be the issue? Thanks!
I just had HA injection yesterday and is it normal to have lots of pain. Also Dr. Said I should see a back doctor to rule out nerves… then he also mentioned TKR potentially for this. Just so scared…thank for all you do Dr. David and team.

I have chondromalacia grade 4 injections made it worse ! took 6 weeks had a flare up then they advised quad physio made it so bad I was on a crutch for 3 weeks . Watching Dr David I can walk without a crutch and slowly doing tailgating and clams what a difference
What about if you want bigger quads? I’m into bodybuilding and a getting post traumatic arthritis; CM has literally every health professional telling me to avoid quads. What are some exercises to do, specifically? Maybe a video? I know about avoiding deep squats, but what about sumo squats which target glutes, or partial ROM leg extensions (I know these are the worst for the patella but they are the best for quad growth)
Loved the video, it’s calmed me a lot since getting the diagnosis. Most things I’ve read have just scared me into thinking I’ll be a cripple for the rest of my life. (I’m in my early 20s)

I have the same question. For what its worth the physiotherapist told me shallow squats are ok and also leg extensions, avoid lunges and deep squats. That was recently so I've only done a couple of workout sessions, too early to report back. I have Grade 4 in one knee but only a small spot. The info out there on exercises is really confusing. A lot of the body building channels when they mention knee problems are not necessarily being specific to CM.
