      If it's the sebaceous gland reduced or missing only to determine a scarring alopecia then a biopsy is not necessary. Reflectance confocal microscopy can detect sebaceous gland size, proliferation and best of all doesn't create a new scar that a biopsy makes. Biopsy should be discouraged due to pain and scarring that they cause.

    Does telogen effluvium unmask androgenetic alopecia even if a person is on minoxidil and finasteride. Also, would someone experience telogen effluvium if hes already on minoxidil, finasteride, and dermarolling? Or do these treatments fail to compensate the actual root causes of telogen effluvium ?

    I am experienceing hairloss all around my scalp, density decreasing gradually from 5 years (front to back side same density). After covid my hairfall increased lot. And I am low in b12 and vitD. I don't have any habitats like smoke drink and my age is just 24 i taking care my hair in all possible ways other than min and fin. There anybody facing same type of hair loss.

    Is it possible for androgenic alopecia to suddenly accelerate? I've known I have AA for years but it was kind of mild. However in the past year, it really accelerated. I've recently started topical finasteride after watching your videos, so I hope I can reverse at least some of the damage. I'm in my mid 30s.

    Hi, doctor Rob.
    I am very similar to your case in hairloss
    I have a slight recession in my right temple and thinning in my left temple and I have been diagnosed with AGA, thinning in the crown and in the temples and all the treatments they gave me from procapil to minoxidil didn't work,
    even when I stopped Minoxidil my hair didn't fall out.
    Also, I don't notice any miniaturized hair when I am in the shower.
    Also the recession I have in my temples can grow hair, I don't know how but it did grow some hair, even after years of not having any hair.
    I don't understand what is happening in my scalp because I never respond to any treatment.
    I hope you see this because I want to look for information on what's happening and what's causing my hairloss even If I have to become a dermatologist myself and do researches.

    In my case i knew my hair was thining but it was normal thining , but this year i don't even comb my hair because whenever i do there is a bunch of hairs on the comb , even without it if i just run my hand over my hair , it falls so easily , i don't have iron defienecy , neither a vit d thats why im clueless what would be the reason for it

    yo i have been following your advices for over a year now but i have a weird problem with my hair, its thick and healthy thx to god but its suppppper greasy and also it has tons of Dandruffs! like very crazy! i usually shampoo twice a week and hair is awesome the day i shampoo but the day after it gets super oily again, i have been trying the no poo thin for 10 days now and i see no improvement, my hair is super oily that i can style it in impossible ways and it would stay like that lol, any advice? also i always shower with very cold water but the last 2 times i washed it with hot water and no shampoo, i never use hair blower but i did it tonight and it didn't to a lot really cuz my hair is long and oily. help me out!

    I saw the at home androgenic alopecia diagnosis technique in one of your previous videos, and it was not clear to me whether you meant that androgenic alopecia is characterised by the shedding of hairs of various thicknesses, or whether you meant that androgenic alopecia is characterised by having hairs of various thicknesses. It could be that we all have hairs of various thicknesses, but people with androgenic alopecia shed hairs of all thicknesses whereas people without androgenic alopecia shed only thick or thin hairs. Could you please clarify whether or not the test described proves whether or not you have hairs of various thicknesses, and that people with androgenic alopecia have hairs of various thicknesses, whereas people without androgenic alopecia don't, and so if you have hairs of various thicknesses as proved by this test you likely have androgenic alopecia? Also thanks so much for your work, you seem to be a very good natured and motivated by helping others

      Thank you. It depends on your stage of AGA. In the very first stage, all hairs will shed of the same diameter. In later stages, the shedding hairs will vary in size. At the end stages, you might not notice much shedding at all due to the relatively lower number of hairs left on the head. Hair follicle miniaturization is just one of many hallmark characteristics of AGA, and can be present in other hair loss disorders. But it’s usually a strong indicator of AGA, so should be taken into consideration alongside other factors like age of onset, rapidity, family history, hair loss presentation, etc.

      Science is a more of a current best guess, not really “fact” or “truth”. Most scientific ideas we’ve held in the past have been altered or refuted entirely as time goes on. It would be silly to think that the ideas we have now are anywhere close to infallible

      Dht doesn’t decrease, testosterone does, dht maintains leveled threw out even tho testosterone keeps falling is this AR sensitivity which keeps minituarizating the follicles.

    What is your take on taking saw palmetto to reduce the hairloss process (DHT) while combining it with minoxidil for regrowth? Ive never hear people trying this protocol before, its always finasteride and minoxidil

    Thanks for the information, Rob. Question - have you heard anything about dermarolling causing premature aging and collagen breakdown due to telomere shortening? I want to give it a try but not if that is the ultimate result.

      Of the studies I've read, scalp biopsies following repeating microneedling seem to show the opposite: no scar tissue formation. With that said, some transplant surgeons say they've anecdotally noticed scarring in their patients who microneedle. This doesn't make a ton of sense to me, since these surgeons are transplanting hair from donor areas which wouldn't generally be microneedled in the first place. In that regard, it's hard to discern if scarring is even due to microneedling, or if scarring is due to the natural progression of AGA (which also resolves in scarring known as perifollicular fibrosis). The longest-term data on microneedling exists in acne patients, and seems to show improvements to scar tissue over 8+ months of microneedling.
      Long-story short: we don't have enough data to know. My personal opinion is that scarring from microneedling is 100% possible, but that if you're adhering to reasonable needle lengths (0.6-0.6mm) and wounding frequencies (once every 1-4 weeks), the likelihood of this happening is probably on the lower side. Hopefully more studies will come out to help answer this question.

As far as scarring goes, I recently read that people that are susceptible to keloid scarring should avoid it, which is something I never thought of but makes total sense. Keep up the amazing work!

    Every doctor tells me I have androgenic alopecia. My hair started that way I’d say. Temples slightly receded, but followed by diffuse thinning across the top. As a male, this is a kinda weird pattern

    Can you make a video about diffuse unpatterned alopecia? I.e. DUPA? There is a lot of debate in the hairloss community regarding what causes this type of hairloss.

      We have content on this coming. I personally think DUPA better aligns with female pattern hair loss than androgenic alopecia in terms of its pathology, and that there are a whole host of other hormones aside from DHT at play. I also think other hair loss disorders are misdiagnosed as DUPA when they are probably more so related to alopecia areata incognita, telogen effluvium, and others that can overlap in presentation. There’s video content about this already inside our membership, but some of it we will post here in the coming weeks.

    Great stuff have dived into the bioenergetic aspect of hair loss??? Dr. Raymond Peat believed that stress hormones like cortisol estrogen and prolactin were the main culprits.

      It’s not relevant for most people. Androgenic alopecia is the primary cause of hair loss in men and caused by DHT. If you stress is so high that you’re losing your hair, you have bigger issues than the hair loss.

      You have to first control dandruff. You should start using a shampoo which contains Ketakenazole along with few days medicine which will eliminate dandruff issue as after medicine completion for 10-15 days you just need to continue using shampoo every alternate day. But the only issue you will face is dryness of hair. For that you every time need to use any conditioner.

Because my dad has a full set of hair at age 65 his hairline from the corners is receded a bit, but my hairline corners is worst than his by a little bit and I noticed it was receding when I first noticed that I had seb dermatitis. I’ve been using 2 percent keto for the past week twice a week along with t sal on a different day that I’m not using keto shampoo and it does go away after I’m done using it but it returns once my hair dries

      We just released an email about this. The short answer: two clinical studies suggest it's no more effective than microneedling. My opinion is to save your money and try microneedling over PRP.

    My hair has thinned out a lot over the years (late 40's), but i always still had enough hair to style etc. Or so i thought. Took a photo of the top of my head a few days ago... there is some pretty diffuse thinning going on across most of it, just not as much right at the front. No clean bald patches thankfully, but the forest has been cleared enough to have a picnic in the woods. I understand now that I just have the illusion of hair haha. Thankfully i am taller than most people, so they would only see it when i am sitting at my desk haha. Still, it was an eye opener. I had just started finasteride and minoxidil, because i knew i was thinning out, just didn't realize how much was already gone. Luckily it was a camera/tv in a bottle shop/liquor store that showed the back of my head to me as i walked in one day, otherwise i may have let it go much longer.

      I don't as a woman and I still don't let it bother me. I used to have crazy thick hair. However, after several rounds of antbiotics, I lost A LOT of my hair. It might take a long while to get it back. I just got another buzz cut.

      Well it absolutely did and still does affect my confidence, even if from an objective point of view my hair isn't bad looking. Going bald is no option for me at the moment so I'll get a transplant soon. I know that self acceptance is the only real way to get comfortable in our own skin. Having your self worth depend on external validation is never a good thing. Yet I somehow convinced myself that it is the right path to take.

      scalp massage daily, possibly more times a day, dermaroller once or twice a week, castor oil. Hairs will start popping out, but needs lot of work and at least 8 months. Also fix your diet, no more junk food, no more sugar, start doing some exercises@@vazhashengelia6562

      Congrats on your success! I have a centrist viewpoint on natural vs. conventional. I care about education, not pushing people down any treatment path. Yes, there are people who see success on both sides.
