I have a 1995 4runner 4 x 4 with manual transmission I used to have the same problem, but the bracket where the clutch is attach is was broken so what I did I got a new bracket new clutch new flywheel new master cylinder and lower sleeve , problem solved now the clutch is so smooth it feels like it just came out of the dealer.
Is it seized? If so I like to use penetrating oil or heat to get something like that loose. Cleaning up the threads with a die if you can get at them might be worth it too.

Thanks! Yeah we found the problem. The clutch pedal bracket was cracked and not allowing for full travel.
