      I looked Dr. Idriss up on the internet, and I am so impressed with her credentials for such a young woman. She’s a clinical instructor and a former assistant professor at top teaching universities in NYC. Yes, the smart, straight A, pretty, nerd all of us wanted to be in high school.

    I am in perimenopause and live in Germany. My gynaecologist said testing hormone levels isn’t accurate because it even fluctuates during the day. She listened to my symptoms and prescribed HRT. I have been on it for 3 years. It’s bio identical and completely different than birth control pills that I could not deal with (side effects). I sleep better, my depression is gone, no more cold flashes, eye infections etc etc.
    I read a lot of scientific studies and listened to gynaecologists. The risk of breast cancer is not as high as was previously assumed (with ONE faulty study). The risk of breast cancer is higher when you smoke or are obese. HRT also reduces the chance of cardiovascular disease, dementia, osteoporosis etc. I did try a bit on my face and used sunscreen. It must run off with sweat. Now I have an big brown spot on one cheek. So be careful!

      That's my understanding as well. Your estrogen level fluctuates during Peri, so testing your level to determine dosage is not worthwhile and costly

      The baseline risk of BC is still pretty high, even for those without a family history. It's the one of the most common cancers of women, and 1 out of every 8 women will get it during their lifetime, which is massive (for a cancer). Watching my mother-in-law (who took HRT) slowly die after BC spread to her brain was not pretty. I personally wouldn't risk it. She did all the "right things" (mammograms, ate right, never smoked, annual doctor's appointments, etc) and it still got her.

    I entered peri menopause at age 33, I’m now 54. No one ever informed me how freeing menopause would be. Yes, there are many positives of menopause that I wish would be spoken about alongside the more negative. Everyone will experience their own menopausal journey differently and it is not all negative. 🙂

    Im 41 and had my levels checked at 37 after having issues trying to conceive. All levels point to me being in perimenopause. Pretty sure I've been there since 36. My skin changed and ive had insomnia for 3 years now. No one has ever talked about being able to start that young. Thank you so much for bringing this information to light.

    50 year old me, wishes the me from my 20’s, paid more attention to sunscreen, weight-baring exercise and diet. Because perimenopause not only affects skin, it affects bone density and reduces muscle which is why we put on weight and start having aching joints. Starting a gym membership at 49 was a game changer in reducing aching joints, helping sleep and reducing perimenopause symptoms; that and learning to eat to a Macro to fuel the body. Getting started with a personal trainer is how I began, and I recommend, because a lot of us are clueless at the start and struggle to help ourselves when we’re already struggling physically with the affects of perimenopause.

      Do read my long comment on this page. Weight bearing exercise is brilliant but HRT is the best thing you can do for your bones and your health and quality of life as you age.

    I started perimenopause at 38, went through menopause at 43 and was lucky to start HRT right away; Dotti patch, Prometrium for sleep, and Testosterone for libido and brain fog. HRT saved my life and my quality of life.

      What do you mean lucky enough to start HRT right away? Did you start immediately at 43 once you stopped periods for a year, or did you start before that?

      @@VKatI meant lucky because only 8% of women that can take HRT are on it due to either lack of access to a doctor that a)knows about menopause and or b) will actually write the scripts.
      (Dr. Mary Claire Haver 's page is full of stories about docs who aren't up to speed denying treatment.)
      And I had a hysterectomy at 42 and started HRT at 43 because that is when the hot flashes started and we finally figured out it was perimenopause all along.

      @@VKat Just in case you are wondering, lots of women start hrt way before their periods stop for good. Hrt is to help with symptoms caused by declining hormone levels whether you still menstruate or not.

    When I turned 46, I noticed I needed some extra care for my skin, I started to build my routine, at 47 I found your videos and watched all of them, as well as other dermatologists, 600 plus videos and now at 48 I Love my skin, love , I don't need any make up. ❤️ thank you. I look better than 20 year old gals. ❤🎉

      Omg me!! I hate it here . I had and the key word is had beautiful skin like where ppl always thought I was using filter pictures wasn’t but as soon as this perimenopause hit my skin is so dry, I think I have a melasma and my skin looks looks dehydrated. I started verisol collagen and I’m gonna start using Castrol oil to see if it helps. Ppl call me ma’am now 😩😩 I

      What’s your routine? I want my youthful look back I don’t expect to look 20 but at least 30s I do have a chunky face which that does help me a lot, but I feel like ever since I turned 46 my skin needs help.

      Please watch recommendations of the main ingredients dermatologists suggest for different skin conditions. Write a list of all ingredients you need, then start trying products that have those ingredients. It takes months to experiment though. Everyone is different. Main ingredients are vitamin c, niacinamide, retinoids, spf. Double cleanse. Glycolic acid. ​@@Taty14002

    While I really appreciate your speaking out about this and your knowledge is much better than most, 17 beta-estradiol is in fact the body-identical form of oestrogen and is the safest form of HRT when used transdermally. It is still systemic when applied as a gel, spray or patch as it is absorbed into the blood stream that way, as opposed to orally, which is when there becomes a risk of blood clots etc as it has to be processed by the liver. Regulated, body-identical HRT (rBHRT) NOT compounded, bio-identical HRT (cBHRT), is very safe and actually health protective against all stage causes of early death, actually offers protection against certain types of cancer, dementia, osteoporosis, heart disease etc etc.
    rBHRT itself does not *cause* breast cancer, however it can feed some rare forms of cancer cells that are *already present* causing them to grow faster, however there is some evidence to show that this actually improves outcomes as they are diagnosed and treated faster. The evidence shows that we are at a much higher risk of developing breast cancer by being overweight or by drinking a glass of wine several nights per week.
    There are also numerous women, including doctors, who are BC survivors who still use rBHRT afterwards due to the massive benefits to their quality of life and also the protetcive effects against other health issues. Most oncologists are only interested in reducing even the smallest of risks or recurrence however a lot of women are now starting to ask what is the point of surviving if their quality of life is so diminished. All women who have concerns should have their own individual risk score calculated by their oncologist and so make a truly informed decision. Also, check out the latest edition of the book "Estrogen Matters: Why Taking Hormones in Menopause Can Improve Women's Well-Being and Lengthen Their Lives -- Without Raising the Risk of Breast Cancer" by Dr Avrum Bluming, a medical oncologist and cancer expert.
    Unfortunately, most doctors, even gynaes, have been given almost no training on Peri/Meno and rBHRT, so it is really important to find a doctor who is a Menopause specialist, and to also educate yourself, so that you can be your own best advocate. Check out the resource library on the charity Balance Menopause website that has massive amounts of evidence based articles, podcasts and videos in multiple languages, that address the myths and misinformation around everything Peri/Meno and HRT.
    I am not sure about the safety of using oestrogen topically for aesthetic reasons, however using rBHRT systemically, transdermally has huge positive impacts on the appearance of your skin on your whole body along with your hair, nails, bones, muscles, ligaments, cartilage, internal organ health etc etc. Every single cell in our bodies uses Oestrogen, you can imagine it as the lubricator of the body, and so when we start to have a deficiency of it, our cells, skin, bones, membranes, arteries, organs etc become drier, less elastic and more brittle and prone to disease. If you are starting to notice skin issues, you will notice quite quickly an improvement once you start rBHRT, along with the alleviation of other symptoms. Although the long decline of hormones in Peri means that you will need to constantly tweak and increase your dosage for optimum results, plus it can take up to 18 months to find the right dosage combinations and delivery systems for you so it is not a quick fix.
    rBHRT form of Progesterone is really important also, but this should be taken orally for systemic effects and it is safe to do so. Be sure to make sure that it is micronised, true Progesterone and NOT a synthetic Progestin, as many women really don't tolerate the latter well and the benefits are much greater with the former. It is important to note that you may have some less than pleasant *settling in* symptoms for the first few weeks or months as Progesterone is the first hormone to drop off and it drops quite drastically so it can be an adjustment for some women's bodies to have it return to good levels after a number of years.
    If you are still getting your periods, your doctor will probably tell you to take it cyclically so that it helps keep your cycle regular, but this can be problematic for some women as you take a high dose for just two weeks and then two weeks off, so this causes our Oestrogen levels to shift wildly every two weeks as Progesterone has a mopping up effect on Oestrogen, so one suddenly can have symptoms of a sudden decrease in Oestrogen and then a sudden increase when you finish the two weeks on Progesterone.
    Most women who experience symptom issues with this twice monthly shift, find it can be made less of a problem if one adds an extra pump (or two) of Oestrogen gel on the days you are taking Progesterone. However, it is not necessary to stay on the cyclical dosing after a few months if it is still causing problematic symptoms, you can switch to continuous Progesterone which means taking the same, *lower* dose every night so there is not that swing in levels twice a month. You may need to advocate for yourself with your doctor though, as this is not a usual way of prescribing. If you look for the "doctor's guide to prescribing HRT" on the Balance Menopause website, it talks about this and you can show this to your doctor.
    Progesterone has its own protective effects against future health concerns although less well researched until recently. Most doctors think you only need to take it if you have a uterus still, in order to protect against uterine cancer (via the mopping up of excess Oestrogen), however there is no real reason one should stop taking it after a hysterectomy, not least due to the positive benefits it has on mood and sleep. Again though, you may need to persuade your doctor to allow you to continue on it as it is an unusual choice. If however you have had Endometriosis it is imperative you continue to take it as even if you have had the Endo surgically excised by an expert Endo excision surgeon, there still may be some cells left and so they could be fed by unopposed Oestrogen and return to cause problems by starting to make their own Oestrogen again.
    Lastly, Testosterone is in fact a female hormone too. We have it in higher quantities in our bodies than we have Oestrogen, it is just that we have it in lower levels compared to men, which is why it is considered a male hormone. You may need to also add Testosterone gel into the mix, it is currently only prescribed for low libido but it can also have a positive effect on mood, cognitive symptoms and energy levels but you may need to shop around to find a Meno doc who is comfortable prescribing it off label.

    Perimenopause and menopause is something that isn't talked about much i feel like. Thank you for the video. I was deep into perimenopause before knowing it. Just was diagnosed last year at 46, but symptoms had been happening since 39 or 40. At 40 i felt like i woke up one day and i didn't recognize myself. I have been fortunate to look younger than i am most of my life. But i felt like age caught up at 40. I started YouTubing videos of influencers and found your channel. Thank you for all your advise! I feel like i have turned my skin around these last 3 years.

      Same! I’m Canadian and was a ballet dancer so truly didn’t spend a lot of hours out in the sun, so I always looked much younger than my age, but then at about 41 it hit me hard and I didn’t recognize myself at all 😢

      ​@@deedee7780 I went from just using moisturizer for oily skin after cleasing and makeup with sunscreen in it to a whole new skincare routine. I now use all moisturizing products for dry skin. Cleanser, double cleanse if wearing full make-up, toner, vitamin C serum, firming serums from No7, firming moisturizer, and actual sunscreen, not just what was in my makeup. And I have lived on a farm my whole life. Rode horses since I was 2. So lots of sun exposure. But this new skin care routine has helped so much.

    I was diagnosed at 31 with perimenopause. I thought I was going crazy. By then my fsh was 141. I'm turning 37 tomorrow and my symptoms are not as severe anymore. Thankfully, I was blessed with three babies before that

      Not for me, I started at 13 and was very regular. But it does run in my family, it started for my grandma at 35 and my mom at 40. Apparently my started in my 20s but my Dr wouldn't test my fsh because she said it's just too rare. You have to be your own advocate

    This was great! Thank you! I am 42 and have just realized in the last month or 2 that I have been in perimenopause for several years now & didn't realize it. My biggest issue is sleeping and waking up like every hour at night. I thought it was just my lot in life for my age. Anyway I'm glad you mentioned melasma with these hormone creams. You're one of the few doctors that will touch on that so I appreciate that bc Melasma is a big deal to a lot of us who have it.

    13:07 My peri-menopause lasted 10 years……and I realised that only in the last 2 years of peri ! My peri started at 40 and ended 2 years ago at 50. I suddenly gained weight at 40 and got severe acne, started getting hyperpigmentation and, later, melasma. For the first time in my life, I got severe insomnia. Leaving aside the systemic changes, my skin changed drastically in the last 2 years of peri. My face, from being an oil refinery you could cook a whole day’s meals for a family from, became bone dry. Skin became dull, sallow, my melasma became horrible. 3 and half years of all this and then I discovered Dr. Idriss. So am fine now. Also, my gynaec just told me to pop a paracetamol when the pain got bad and gave me something for heavy bleeding. My tests were fine otherwise. And all this happened during the first lockdown, when going to a hospital was very scary.

      HRT saved me. I was already using it prior to a hysterectomy six years. I experienced everything you did, but in the two years after my hysterectomy, but we finally got my HRT right after two years, and I’ve been fine for the last four years.

      Even though you are in menopause and not fertile anymore, you still need estrogen to age healthy. Talk to a doctor that is open to HRT. Good luck.

      @@jinde75 genuinely thanks for the suggestion - I’m open to it but I would like to know how HRT will make my life better ? I’ve been menopausal for 2 years now and get regular check ups - nothing has shown up so far. My skin is doing great……my bones are no longer achy…..my heart is fine…..I’m fine emotionally. I have a doctor in my family and I cross check every test/report with her. Is there something else HRT can help with ?

    Feel free to post as many videos on this topic as you’d like!! I just started perimenopause this year (46) and it is wild! So much uncertainty of what is going on and when with my body. It’s definitely made me take a look at the bigger picture and getting older. My symptoms haven’t been too bad, but the face falling and weight gain are ROUGH. One thing that has helped my moods, sleep and stress is Cortisol Relief by Vitanica. They also have a bunch of menopause specific support herbal remedies.

    I hit perimenopause at 39. My cycle disappeared for months and my gyno confirmed perimenopause. My skin went downhill quickly within a few months (marionette lines, deep frown lines, melasma, papupustular rosacea, weight gain, hot flashes, aching joints). Wish I would’ve seen your videos years ago so I would’ve known to be prepared for that to happen SO quickly 😅. I thought it would be a gradual change over a decade.

      @@jinde75 My gyno put me on birth control pills for now because she says she wants to wait a few years to start HRT. The BC pill did stop a lot of the symptoms.

    I used bio identical estradiol, birth control and collagen in every form. It worked! Until I was diagnosed with inflammatory breast cancer, Stage 3b. There's no lump with IBC, but the mass went down significantly after I stopped all of this after needing a biopsy. It turned out that my breast cancer was estrogen and progesterone positive, meaning that I had to be forced into menopause immediately and began taking hormone blockers. Who knows if the birth control and estradiol were even a tiny bit to blame. But, none of my doctors at Memorial Sloan Kettering told me that it couldn't have effected the growth or pace of growth. I'm a die hard skincare junkie, my skin is recovering post treatment (I'm cancer free and in remission.) My skin will be fine, and I look at aging as a gift of time now....as long as I keep looking younger than my age. I just hate thinking that I did something that ultimately was so traumatizing. But, I also ate a ton of tofu and soy, so there's that too. Cancer is random. I'm just grateful that my PCP suggested a mammogram.

Live in the present and have a ball. You've lived through Cancer, there's nothing else you can't squarely beat with grace.

    Thank you for covering this topic! I spent 10 years from 40 to 50 with a female doctor, who was a few years older, and she never once mentioned perimenopause. In hindsight my symptoms were obvious. My last couple of appointments I told her I had learned about perimenopause and she still refused to help or do anything. I stopped seeing her and have an appointment with an obgyn soon. So many years of awful symptoms and I didn't even know what was happening. All that to say I learned more about perimenopause from the first part of your video than I did from my female doctor. I do hope thw younger viewers are watching so they're prepared better than I was.

      One thing to keep in mind regarding info; the WHI study from the early 2000s has been dissected, debunked and reanalysed. Any physician who claims hrt, especially oestrogen, causes cancer is not up to date, anyone who claims you can only be on hrt for a short period of time, ditto. I'm not saying hrt is 100% risk free for everyone, we're all different. There are some really good GYNs out there who really advocate for us, like Mary Claire Haver and Kelly Casperson. If you feel like you should look more into peri menopause, pop their names into the UA-cam search bar. Also Dr Lisa Mosconi is worth checking out. She's a neuroscientist who focuses on the effect oestrogen has on the brain.

      I’m 41 (turn 42 next month) and I’m in the same boat. I’ve had hot flashes for a while, but they are the worth they’ve ever been now. Bioidentical HRT, here I come. Good luck to you!!

    At 51, my skin has never looked better. The products available in these times surpasses what was available when i was younger. I used to be allergic to tret when i had acne. Now i wear it over k-beauty milky exfoliants and strong Paula's Choice exfoliants. Im often assumed to be in my late thirties. Im convinced people with oily acne skin when young dont have aged skin when older. Staying out of the sun was the best thing I ever did. I still have a regular period but at my age i have to be in perimenopause. Nothing has changed except im more emotional. I even have a 26 year old lover. Tret, high quality retinal, milky toners and skin vitamins really keep the skin glowing and hydrated.

    This is awesome info but so very generalized regarding HRT. From my understanding working with several functional drs, reading a ton for years, and using Bioidentical hormones, one can not be on any form of estrogen without PROGESTERONE to counter any negative effects from estrogen. Progesterone also has beautifying properties for the skin and amazing for sleep! Most of us in perimonopuase are Estrogen Dominant and need progesterone to balance the Estrogen. It truly is about the balance. It is also surprising to know that many of us in peri are also lacking testosterone. Again, the balance is key with hormones. Ultimate Human has an amazing podcast that covers all this on how to balance women's hormones naturally using BHRT w/ Dr. Carrie Carda.

    I had a partial hysterectomy at 39 and my skin went apeshit! I got on Adapalene and then Tretinoin, added a vitamin C product, took my moisturization to the 1000th level and prioritized my diet and exercise regimen for aging bones. At 48 I started having hot flashes and I started drinking even more, having my home AC on a lot more and a hand held fan when travelling. I will be 49 next year and I do not know if I am peri or full menopausal but I will meet that like I do with everything else: great doctors, prioritize diet and movement and try to minimize my stress!

      What about HRT? In addition to what you are already doing. It is bio identical now.
      Lots of people are scared of hormones but nobody bats an eye when thyroid hormones are prescribed.

    I would disagree with a blood test of hormones to test for PERImenopause.
    There is a gynecologist here on UA-cam that specializes in perimenopause and menopause.
    She says there is NOT an accurate blood test for perimenopause because hormone levels fluctuate too greatly to determine with any accuracy.
    She agrees with ruling out thyroid or other reasons before diagnosing perimenopause.

    As a fellow doctor though in Family Medicine, i must say your takes and way of presenting is so patient oriented that it puts many doctors to shame!

    Also for myself, i am curious if any have a good advice for me. I am a red head very light skin girl so since I was little kid I have always been the one with the tomato red face. And I am finding it hard to distinguish between just physiologically redness and the fact being like light skinned always will show it off more (and the need to accept that is just my face) or if it is pathologically (like the beginning of rosacea) or sensitivity (though I do have not have any itchiness, pain, etc)
    Also best tip in the world with taking a step back and see what pops up and not worrying about the fine lines, such a good advice!

    So pleased you posted this today! I’m on HRT oral pill for both estrogen & progesterone. It is working but was constantly the cream. I think I’ll pass now considering my oral pill is starting to work now after 8 days. Thank you beautiful girl ❤

    Late 40s and perimenopause! I feel like I’m pmsing & menopause symptoms at the same time! Brutal! And let’s not go near of having to change my entire skincare routine and type of exercising! Everything that worked in my 20/30s went out the window! We definitely need more awareness because I had no idea until recently so thank you for the video!! 😅
    PS I can’t wait until I’m fully menopause! Hopefully it will be better!

      Look after your bones - will feel very achy, can have weird digestive issues. I got palpitations too. Gynaecs don’t tell you all this.

      @@user-yx6so1dv9d Yep, palpitations. Check! Hot flashes, night sweats. Check. And the weird digestive issues....no one told me about that either. It wakes me up at night and/or keeps me awake. It's like adding insult to injury because your sleep is already disrupted in peri/meno/post. But I've been on HRT for almost six weeks and it IS helping, somewhat. They say it can take up to three months to start really being effective.

      Full menopause was not great for me at all. I suggest you get on HRT and check out videos from Dr. Mary Claire Haver for lots of fantastic information.

      We don't go through Menopause, we go into it until we die. The negative impacts of this hormone deficiency on our bodies can cause early deaths, dementia, and drastically reduce our quality of life as we age. We can and should be taking HRT until we die. Some women are now starting it in their 90s as they are still suffering symptoms and had been scared off it until recent public awareness changes (at least over here in Europe), but the best protective health effects are when we start HRT within a 10 year window around Menopause so do not wait until it is too late. See my long comment elsewhere on this page for more info. And don't wait, research trained Meno docs in your area dna make an appoitnment asap as the waiting lists can be long and it can also take up to 18 months to find the right dosages and combo for you. You can always come off it if you decide it isn't for you.

    Blood tests are not to be used for diagnosing Peri due to the fact that your oestrogen levels can fluctuate wildly every day so the levels at the time of the blood test can make it seem like you have perfectly normal, pre-Peri levels. Symptoms only are what need to be used to diagnose Peri.

    As so many others have said, your hair is a knock-out! So beautiful and angelic! I also agree with another comment that the more peachy eye shadow and (what looks like) a light sherbet green eye liner are much more complementary to your eyes than the pink colors you normally wear. I found this video very interesting, although at 70 and way, way past the life stage discussed, it was good information to pass along to younger friends/family members.

    If anyone is not getting the support and care needed during perimenopause, menopause, or post menopause the doctors at My Alloy are great! They specialize in caring for and treating women in this phase of life and are pretty affordable too. I’ve heard that some HSA’s will reimburse for their services but I don’t have personal experience with that. I do know that My Alloy and the doctor I was assigned to helped me tremendously when my regular GPs and multiple OBGYNs through my regular insurance did not and would not discuss or acknowledge perimenopause much less HRT options.

    The perimenopausal transition years are hard and even after you are in full menopause dealing with your skin issues can be really challenging. Your skin type can change so much- on top of your collagen production dropping off, products that were your tried and true for years suddenly don't work for your skin any more. I always had oily skin so all my skincare was chosen to help deal with that issue, but I can't use most of it anymore because it is too drying now. I can not count the number of products I have purchased over the years that would have suited me wonderfully once and now are too stripping or not hydrating enough. I'm interested in learning more about the estrogen face creams - if you do more videos discussing them I will definitely watch them.

    I am 42 years old and went to see my dermatologist. I like how she didn't discuss AT ALL the idea that I could be in perimenopause with the acne I started to experience. Most doctors suck and just give you a drug and move on. Thanks, Dr. Idriss.

    I honestly miss being on the pill... My skin looked flawless and my periods were extremely light, I barely even knew they were happening. Even my neck looked thinner and my features softer! But I don't think the anxiety was worth it lol. Thanks for all the info, I will definitely look into HRT when the time comes.

      Les reines du shopping, it was a French game show about 5 woman (1 each day) whom go shopping for 3 hours with an theme (summer with a dress, be hot for a first date). The woman whom was presenting the show is Christine Cordula.
      She used to be a model from Brazil, before she won the heart of many french people.
      Anyway, she gave a lot of adviced on look, how to dress according to youe figure, and else.
      All those words to say, before Botox, your could try a different hairstyles, included color. As wrinkles settle in, your hair color needs to be lighter it will smooth your face sort of.
      For instance if you have olive skin with jet Black hair, go for a brown color.
      My wild guess is the same goes for clothes and make up.
      The idea isn't to look like a teenager but find a style that might suits you more.
      I followed her advice, did a side color (white on top, black on the bottom) : people started to call me "Miss". 🤫

    Thank you so much for the clear explanation. I started the estriol cream after the published study showing improvement when paired with glycolic acid. I am eagerly awaiting more studies with results. I have not noticed significant change but want to stay on it until we know more. Love your content!
