Maybe someone on the outside let's say Obito has been trying to mess with the genjutsu slightly altering the effects of a "perfect world" making it more dark. It would explain how Boruto starts out all sunshine and rainbows then gets dark, all leading too Kawki sending Naruto out of the genjutsu or something similar.
