Vault Loyalty Inspector, thats an interesting one as it insinuates that in the beginning Vault tec was a lot more paranoid aboit peoppe becoming wise to their schemes therefore used loyalty inspectors alongside the overseer to make sure people werent straying from testing.
Four months late, but I'm surprised you didn't add the extremely rare encounter of Amata being held hostage and questioned by the Enclave to this, but I believe in order to achieve that, you have to make the vault uninhabitable and even then it's an extremely small chance for it to happen, and when it does, you find her next to the vertibird to the right of the vault

Neutral Karma runs are the true runs where you actually think about the actions you commit. You don't just do good or evil. Like I didn't let the ghouls take over tenpenny tower as I think those ghouls are just as bad as Tenpenny for killing all the alright people in the settlement but I still kill Tenpenny to even out the karma loss. Its that kinda run where you end up doing things because you believe they are right not because the game tells you they are right which is far more fun than I evil kill all or I good I help everyone even when the outcome is morally questionable.
Yeah he has to stay alive during your escape from vault 101, then he needs to stay alive and exit the vault during the quest "trouble on the home front". Then you need Nutral karma, and you can then recruit him in Rivet city in the muddy rudder at the bottom of the stair well.

The companion's karma dialogue really highlights why fallout new vegas is a better game. In 3, even if youve nuked megaton and killed dozens of people, good followers will return if youve increased your imaginary karma score. And there's no nuance between them, they all basically have the same criteria and might as well be the same character but with a different character model In fallout new vegas, every induvidual follower has different criteria for when they will leave, and when that criteria is broken, they dont come back. For boone and arcade gannon its the legion, but even then theyre different -- boone shoots legion on sight, but arcade is willing to work with them if he thinks is for a betrayal later on. Veronica wont like you if you destroy the brotherhood, and even though the brotherhood is opposed to the NCR, veronica herself is alright with them. Meanwhile fawkes still wont go into the reactor at the end of the game. No reason, just bad writing. New vegas just has way better writing.

Companions won’t leave if you change karma. If you dismiss them then try to get them back, but have the wrong karma, you will be unable to.
