Hair Loss and Hormones: Dr. Omer Ibrahim Breaks It Down
Hair loss is one of those things that can feel so isolating, especially for women. But trust me, you’re not alone, and it’s not your fault. I had an amazing conversation with Dr. Omer Ibrahim, a dermatologist who’s just as passionate as I am about breaking down the stigma and getting real answers.
We talked about how deeply tied hair is to our identity and how hard it is when that starts to change. Dr. Ibrahim explained the common causes, like androgenetic alopecia, and how hormones like testosterone and DHT factor into it all. We also covered treatments you’ve probably heard of, like minoxidil, spironolactone, and finasteride, and newer, exciting ones like PRP and exosomes.

Breast Cancer Prevention & Menopause: Expert Advice with Dr. Lisa Larkin
I am soooo grateful you brought this conversation to the show!! I landed in the hospital for 12 days w heart issues (low pulse, bp and syncope) which I’ve had my whole life but is sooo much worse and more frequent now (I’m 53 and getting closer to that year mark for menopause every week) and ultimately the doctors sent me home (with a loop recorder) as they didn’t know what to do w me. On one hand, I deeply appreciate them not putting a defib in me without knowing for sure but boy do I wish hormones came up as a topic (I’m on HRT but do I need testosterone too? Different dosing of my estrogen/progesterone cocktail)? Your convo armed me with some good talking points for my next apt. Thank you 🙏🏽 🙏🏽🙏🏽 Question: if want to donate $$ to more female heart research, which orgs do you recommend?
Just to be clear, Dr. Kelly : the women in the study who were followed afterwards, actually stayed on the oral estrogen and synthetic progestin even after the negative ( at the time) initial WHI findings??
Looks like you can NOT get DHEA in 🇦🇺
What about the terrible side effects of bisphosphonates??? What is the alternative? I’m on estrogen patch 0.25
In most of the research and scientific debate around HRT women's quality of life is seldom mentioned as an outcome. A lot of clinicians have an opinion about risks vs benefits, forgetting that whilst the risks are fairly well defined, only the user can quantify the benefits to her as an individual.
Omg!! I’m an American living in Australia! I’m coming 🎉🎉🎉 I share your info w all my clients- yahoooo!!
❤
Thanks!
Thanks!
Great to know, thank you for ALL you awesome information - but what percent please
I learned A LOT!!!!!! Fascinating. Warm regards Jennie
Thanks!
Thank you SO MUCH for having this conversation 🙏🏽 I’m sending it to so many clients (I’m a life coach for midlife moms- they neeeeeed this! (As do their moms!)
amazing!
I was diagnosed with IC when I was about 25. Even had a cystoscopy. I had heparin bladder infusions and only got worse. My urologist wouldn't listen to me until one day I lost it at his office. Got a new urologist and he told me I didn't have IC. Thankfully I've been fine ever since. It was literally just some infection that took a good long while to get better. It was such a difficult period of my life. All the ER visits, catheter, self catheter ect .. I'm so glad I'm not dealing with that anymore.
So glad you are feeling better!
pumpkin seed oil daily… i had read it blocks dht and gave it a shot made a huge difference for me
During perimenopause, I sought out a NAMS and A4M fellowship trained MD. I had had to use prednisone multiple times in the past. She said my bloodwork seemed to indicate PCOS. My Fasting Insulin blood test was also sky high. She put me on Rx metformin daily. She said it can be used to _prevent people from developing diabetes_. Yet when I spoke with my OB/GYN and family doctor, they really had no idea about the use of Metformin. So frustrating. Imagine the massive cost savings and suffering prevention if we prevented high-risk female patients from developing Diabetes. 😮 How do so few HC practitioners know this information?
Thank you.
You're welcome!
I pay 24$ a tube, CVS in Michigan. No insurance ❤
How/who/ what kind of doctors do i search for that will do all this testing and follow through like you ladies?? Whi does dexa scans and vo2 max, etc
Just came home from my first appointment with my menopause specialist. She recommended Imvexxy for me... and then recommended your podcast. :) I plan to be around here checking things out once in a while! I love that I'm not broken. I am getting older, but I am not ready to just accept every change to my body and my well-being. Thankful for the new science in this field! Thank you for sharing!
Wonderful!
I appreciate how direct and thorough you are. You aren’t hesitant to probe into all the major concerns. Thank you for this interview!
Thank you!
Question: So start Menoxidyl and Spironolactone together. Can you stop Menoxidyl after you recover hair and stay on Spironolactone to preserve, or do you stay on both forever? Does Dr. Ibraham, do online consultations? Thanks!
You're awesome, thank you for your life saving work...
Thank you!
Dr. Casperson, I've been taking 200mg of progesterone for about 1.5 yrs. Yesterday I saw a new menopause provider and she asked me why I was taking 200mg and that I should be taking 100mg. What I was told is that progesterone works well for sleep and that it doesn't hurt to take 200mg. What do you recommend?
I want you as my friend ha-ha. You are super smart, funny, and talk about taboos, makes us in ease about issues…Thank you for all your educations! Question: I am taking all, Estrogen gel, 0.75 Progesterone 200mg. and Testosterone. Can I also take DHEA 10mg or the prescription stuff is enough alone? I am 58 and have been on HRT for a year now.
What helps with teeth and gum issues? I noticed my vision getting bad too. I'm on hrt. I'm 42
Carnivore diet has helped many people with this issue.
Carnivore diet has helped many people with this issue.
Sadly, many of the misogynistic physicians are female😢
You women ARE THE ANSWERS to SO many PRAYERS!! Thank you for your bravery and guts!!
Excellent as always Thank you Dr. Casperson 😊 ✌❤ I would love to hear the truth & facts about the WHI of 2002 PLEASE!!!
Thank you for these amazing answers...I am THISCLOSE to shopping for a new Doc...I'm going in prepared to discuss hormones, again. This is her last chance LOL
Excellent video🙂
Thank you!
I live in Cleveland and have been to the Cleveland Clinic. They have the worst bedside manner. No one in the area likes them. The doctors think patients are liars and doctors and nurses hate working there. The only good thing about them is cardiology, MS and epilepsy. If you don’t have any of those, you get terrible care.
Testosterone stopped my hair loss. In combo with estrogen and progesterone it actually grows now. It’s the only thing that has worked.
thanks for sharing!
Am I the only one who HATES topical minoxidil😮? It makes your hair oily, the hair sticks to my head, I feel dirty when using it.
You are not the only one!
Excellent! Thank you!
You are welcome!
I am a 54 yo woman who has RA that happens to be seronegative. I also am on hormones. Sometimes it’s still RA even if the tests don’t show it.
Caroline Petersen is the most knowledgeable person I’ve ever heard discuss the topic of progesterone. She talks about progesterone in a way that I believe the importance of it but I’m worried that there’s some disagreement among the menopause expert community on what HRT is needed during perimenopause. Carol pretty much says that progesterone is the only hormone needed during perimenopause. That estrogen and testosterone aren’t needed until post-menopause. She teaches that progesterone is in fact the beneficial hormone for bones and NOT estrogen. I am currently experiencing some pretty painful and debilitating symptoms of what I imagine are part of the musculoskeletal syndrome of menopause. I’m already on some progesterone. The question is should I also be on estrogen and/or testosterone? Or is Carol telling the truth that it’s progesterone which will make my muscles and bones feel better?
Woman, please let ur guest speak😂
Thanks for this podcast, it answered a lot of my questions but also raised more.....Can hair loss also be caused by dermatographia, or can it play a part among other things?
I found my hair thickened (ponytail thikness) after I increased my protein.
I eat protein everyday it’s done nothing for my hair
Thanks for sharing!
Two great docs - thank you for covering this topic!!
Thank you!
How can you do a podcast on hair loss in women and not touch on peptides???
Because they are poorly studied.
Thank you so much 💓
You are welcome!
One more thing: I started multiplying everything I learned also from other doctors like Dr Louise Newson, Dr Rachel Rubin, Dr Mary Claire Haver and others. ♀️💕 Many women who then because of my advice started using vaginal estrogen reported having trouble: from a burning sensation to developing ecxema... there must be some really unlucky ingredient in the carrier substance that causes irritation (at least over here in Germany) and when you look at the list of ingredients it's all different (horrible sounding) chemical substances... Any tips on what to do if you truly react allergically? Why can't the producers just use good old plane coconut oil or something anti-allergic like this???😢
So so funny and educational at the same time! One valuable piece of information: About 50% of women on systemic HRT will need vaginal estriol as the vaginal stuff does not get into women's bloodstream! Vaginal estriol has about 1/100th of the potency of one systemic HRT hormone dose!!! So good to know! Best wishes from Germany, Dr Casperson (you have a true supporter, fan and friend here in Bavaria♀️💕)!!!
Thank You! Hello to all of Bavaria!
Yes, would like a video on the problems with the WHI study.
After watching various videos including yours, I asked my gyn for HRT about 6 months ago. I was already using the estradiol cream. Recently, after 6 months of using the HRT I decided to stop because my lupus seemed to be flaring and I was experiencing bright red facial rashes all the time. A few days after discontinuing the HRT the rash has subsided. I did leave a message with my gyn asking about this, but no one called me back, so I made the decision on my own.
I didn't know that I can have a progestogin IUD and take oral progesterone?? I'm interested to know about that
I am so glad that my sexual health provider suggested I listen to your podcast a couple years back. I have learned so much about how a female body works. I'm trying to warn other younger ladies about this, but sadly no one really wants to listen. This information needs to get to the younger generation so that they have the knowledge to take care of themselves in their older years. That is my wish.
Right! I agree, they need the knowledge!
I use estradiol gel and I’m on progesterone pessaries 200mg because I was concerned about my liver having to process it orally … what I thought I heard you say in this video is that it’s the estradiol that could even though it’s rare that it would cause blood clots if taken orally … would it be safe for me to take the progesterone micronised body identical orally …if I have had a fatty liver? Thanks in advance I’m due for my 3 monthly review with my GP in a week s time It would be helpful to have this clarified
I'm 62 years old, and I was on a compounded estrogen/testosterone cream and a progesterone pill. After several years on it, I started having post menopausal bleeding. The gynecologist I saw told me all my symptoms pointed to endometrial cancer, which is caused from too much estrogen and not enough progesterone. I had a pelvic ultrasound that was abnormal and an MRI that was abnormal, but the biopsy didn't show cancer. The gynecologist told me to stop taking hormones. I have an appointment with a surgeon because I need a hysterectomy. The doctor who was prescribing my hormones told me to go back on them because the cancer risk is so low. I'm not sure who to listen to now.
Always get second opinions
So, I guess it's up to Gen X ladies to lead in this? _With pleasure!_ 😉 I'm 57 and have been on HRT for almost seven months. Feeling better, but I'm still not sleeping well. I'm so grateful to the North American Menopause Society for listing local approved providers. My _female_ GP wouldn't even discuss putting me on HRT 😞
