    "When the pandemic hit, we had no choice but to pivot to telehealth medicine," said Dr. David Guirao. "This helped us take care of a majority of our patients, and we quickly realized that many of our fears related to this new virtual approach were really unfounded."
    Remote patient monitoring and chronic care management using tools like HealthSnap or virtual visits allow Prisma's dedicated health care providers to maintain a close connection with patients, working around a patient's busy schedule to ensure they're still receiving the quality of care they deserve. Thanks to HealthSnap, Prisma's doctors can monitor their patient's health and catch problems like missed medication or changes in blood pressure or other important signs quickly when they happen outside of the clinical setting.
    Doctors have been able to provide a timely recommendation to visit the ER after HealthSnap provided information suggestion a developing emergent problem, saving time and potentially saving lives.
    "I can truly say that this program has enhanced patient trust," said Dr. Guirao. "We truly care for them. Just having those periodic check-ins helps our patients feel like they're not alone in this process."
