    Thank you so much for this explanation. I was diagnosed with fatty liver several years ago. I asked the gastroenterologist what I could do to fix it. He told me not to worry about it, that it was HIS job to worry, not mine. Foolishly, I believed him and now have cirrhosis. I've never been a drinker, so I KNOW it was the carbs. Learning from you, I can be sad that I had no idea but grateful that I can stop the progression.

      @@peacewithHim but you DON'T know it was the carbs. High carb processed foods are ubiquitous with seed oils. Also processed foods contain so many other artificial ingredients that could also be problematic. Seed oils are most likely the cause. You didn't get NAFLD because you ate too many oranges.

    Really great video. NOW I UNDERSTAND. We eat a mountain of sugar and carbs, and that prevents the liver from doing it's job. Dr. Cywes, your knowledge of the liver is totally amazing, and there must have been a point in your medical training when you spent a great deal of time studying the liver, and look how that benefits you now that the newest trend in medicine is going low carb.

    Thank you so much for your awesome videos, Dr. Cywes. I would love to hear you comment more on allulose. According to some sources it promotes GLP-1 production, while occupying fructose and glucose receptors. What I am most curious about is what benefit an obese person on a near-zero carb diet that eats once a day would see from supplementing allulose.

    Thank you for this video. I did not know you actually read and answer comments. I have also wondered with that question. I also think about the human evolutionary history and metabolic flexibility for survival. If we were meant to be strictly carnivores, would we not have the sharp teeth like cats? Since our body has adapted to its current state and survived the process, Our bodies are highly adaptive, at least until now, where for the first time in history, we have more of an overabundance of food choices, to include substances which are actually non-foods, or highly toxic processed substances like seed oils, preservatives, etc.. It is an interesting subject that I imagine both of us could go on for many hours. I learn it mostly by necessity because I have been diagnosed type II for about 10 years. I would say that I know more about my bloodwork results than my primary doctor. He is astonished that I refuse to take stations and meds like GLP1 meds. I am low carb for sure but struggle with carb addiction. I am now 65 and have just started Medicare and I believe they allow video consultations that can be paid by Medicare. I will find out more but would love to consult with you. I am low carb, use intermittent fasting, A CGM, but am having a hard time losing stubborn resistant fat. I have about 80 lbs I still need to lose. My uncle was 450 lbs when he died of type 2, by taking insulin shots. I was just learning this stuff then. His doctor mocked me when I suggested he go off insulin injections. They had never done a fasting insulin test on him. I suggested that type 1 was a cause of not enough insulin and type 2 was because of too much insulin. He dismissed it, unfortunately. Even my dad stated " Do you think you know more than the doctors know?". I stated that it seems that in this case, I do. It is tough that I have to fight a system that is totally ass-backward. Anyhow, sorry about the long comment. Just saying that if you are willing to do video consultations, and take on new patients, I think Medicare will cover it under kits special needs plan. I will try to contact you on the number you left in another video, if that is ok. Thank you for your videos and knowledge

    I fast 18-20 hrs per day. i lost 45 lbs 6 years ago over 5 months. I'm 64 now, male, 233 to188 lbs. 6 feet 1 inch tall. I lift weights 2-3x/wk, do HIIT 2-3x/wk on a bike- 8-10/ 40 second all out sprints. Total mileage riding including sprints varies from 7.1 to 9.3 miles at a park, no traffic on paved surface.

      ​@@m007mm i guessed he meant 8 to 10 times of 40 secs each, sprinting on a bike, so the sprint is bike riding. While the HIIT is the regulars such as lunges, pushups etc.

    Thank you for this important lesson. Going to have to listen again to absorb. My question, there is a dr. Advocating for menopausal or post menopausal women to do a day of high carb eating to help regulate hormones. I am afraid to go there after a year and half of very low carb eating. However, i have adrenal exauhstion that is not being relieved even with high fat. Weight is stuck too. Would love to hear your take on what she advises. Thank you, i thoroughly enjoy your short but direct lessons. 😊

    Yes! This helps clarify. And, now how does the daylight we eat help steer away from glucose toward restoring ketone burning? And, then, how does the very narrow band of wavelengths of light, that we extracted out of daylight to drag indoors with us, harm this process of evolutionary adaptation for ketone dominant use as fueling brain?

      Anytime I absolutely must have an antibiotic, I eat full-fat yoghurt or Bleu cheese for a week after taking my last antibiotic. Carnivore "probiotics". Hope this helps.

    I'm wondering if you have any ideas on this (not from a diagnosis standpoint, but from physiology.)
    When I went on LCHF diet, my total cholesterol, LDL, and HDL went up, Trigs went down. Not quite enough to meet the Feldman trial criteria but my Dr. thinks I'm an LMHR, or at least on the continuum somewhere close to it. Trigs/HDL ratio went from high of 7.45 and now is consistently in the 1 to 2 range (as computed using mg/dL.)
    When I go on backpacking trips (like 5 weeks, 500 miles order of magnitude) and I eat Carbohydrate-Rich All-Processed (CRAP) diet (because unfortunately it's logistically very hard to keep up the LCHF on such trips) my total C drops dramatically over that time, from just under 300 down to 150 total. Then it returns to "normal" fairly quickly (like within a couple of weeks) when I resume real life.
    What from a physiological standpoint is my body doing?

      I am a 60 year old long distance thru-hiker currently hiking the Appalachian Trail from Georgia to Maine (2,200 Trail miles). I hike carnivore and one meal a day (OMAD) with no snacks. I hike in a fasted state and eat after I am done hiking for the day. For purposes of personal interest and n=1 science, I wear a CGM and also monitor my glucose and ketones with my KetoMojo meter on trail. I also wear a FitBit Charge 6 fitness and sleep monitor.
      I have a history of insulin resistance and associated metabolic diseases that were all reversed when I started a ketogenic diet.
      I am now 5 years keto and 2 years carnivore.
      On trail, I eat homemade pemmican (50% beef tallow and 50% powdered beef jerky). Pemmican does not spoil and can be safely kept unrefrigerated for years to decades. Native Americans have been making pemmican (from bison and elk) for 1,000s of years.
      I also eat Usinger's salami, cheese, and powdered eggs. I receive mail drop food packages for resupply every 100 or so miles. The hike is going very well with no food or nutrition problems to report after 1,700 miles under foot. Trail weight is stable. Only 500 miles to go to reach the terminus at Mt Katahdin in Maine.
      HDL is 90
      Triglycerides are 37
      LDL is 126
      NMR lipoprofile parameters are mostly all pegged to the green. LP-IR Insulin Resistance score is

      @@petertownsend252 I commend you on your resolve to stay on LCHF/OMAD. I Ran into another hiker in Maine a couple of years ago who also managed it. It's not impossible, to be sure.
      I'll be hiking the last section I still need to complete in another week - 100 mile wilderness (It's actually only 99.7 miles) and Mt. Katahdin. I'll miss you by a few weeks or else I'd love to talk with you about it. Your screen name looks familiar, like perhaps we have communicated before? Anyway, I've been section hiking the AT, doing 566, 270, 480 miles in the past three years (the 270 was supposed to be longer but was interrupted by a fall and dislocated shoulder), the rest of the miles over the years before I retired. 66 years old and still going.

    Your body has functions to make all the glucose it needs. Why? It is a survival function and cannot be stopped, because some parts of your body cannot function well without glucose. Two points are Making all and a Survival function. Therefore you don't need to eat any carbohydras or interferer with this gluconeogenesis.

      Excess protein is deaminated to glucose. You may be over doing the amount of protein at each meal.

      nope.
      we are humans, nd absolutely everything starts in the brain and the associated mind.
      First step: be honest to yourself. Observe yourself. Use a tool (also a human trait) to track your diet (cronomter) and your feeding behavior, just for 1 week. It will be revealing.
      TG are produced by the liver from carbs, or if here is too much energy taken, or if you are chronically hyperinsulinemic, or obese, then spilling fat out of the fat cells. look for these things first
      Albeit, there is an inheriited genetic disorder for hypercholinesteremia. But even that can be controlled by diet.

    This still doesn't explain why we have such strong taste sensors for sweet. We have a craving for sugar because our mouth says it's delicious. So why would we evolve to crave sugar if it was poisonous to us???? Unless it's not, and too much sugar is NOT the cause of metabolic dysfunction.

      being 64, I grew up watching the Lawrence Welk show and the male dancer was Bobby Burgess. Years later he was asked in an interview
      what he ate before performing. He replied that all he ate was a steak and an orange. Curious. Years later I heard someone say that if you
      have protein and some sugar that the sugar would help with most body functions and the protein would be left for muscle growth. It worked
      for Mr Burgess and I imagine for many body builders. Most people need some carbs.

    Just so you know, Dr Cywes, one of the plant-based influencers on YT (no names mentioned) is recieving carni-criticism for his big hand gestures. It’s a tic I can tolerate, but I too might decide someday to watch less triggering lectures for info-tainment. 😮

      That'd just mean the addiction is overpowering the resolve if in fact he was trying to avoid all carbs.
      I have some carbs from time to time, keeps the metabolic flexibility up.

    Wonderful I just watched Dr Ben Bikman vid of Randle cycle that came out a bit before this. For me these two vids together elucidate much about metabolism for someone with 9th-grade biology from 1974.
