Only first print copies had it and the people who bought those probably threw away the insert because they didn't win. So it's one of those oddball inserts that you never see.

Where do you get your plastic game covers to protect against scratches? Do you use those plastic PET cases at all? Not sure if those keep game cases 100% scratch free but they seem legit.
RetroProtection is a great place to buy that stuff. I use box protectors for cardboard and the plastic baggies for jewel cases/DVD style cases. As far as I can tell they don't scratch the case. I'm picky about condition as you can tell and I would notice something like that.

It does, in fact the 2nd print version of this game removes the contest info on the back because the contest had expired. Fun fact, this was the first game I opened on camera but I've never posted it. It's sooo bad LOL.

Same here. It was Insomniac's breakout game and making it nonlinear was a nice spin on the 3D platformer.
