The "Epley Maneuver" Went Wrong! Now what? (BPPV)

  • Every once in a while, when I'm helping treat someone for posterior canal BPPV using the Epley Maneuver, not only does it not work, but sometimes it actually makes the vertigo worse...
    Let me tell you, it's not fun!
    Here's what happens: instead of the Epley Maneuver getting the little inner ear crystals out of the canals, the crystals go into the adjacent canal instead... this results in "horizontal canal BPPV (vertigo)".
    This type of vertigo can be much more intense, and you have to treat it differently. Unfortunately, many PTs don't know how to treat horizontal canal vertigo, and this can create a difficult situation.
    In this video, I'll explain how the Epley Maneuver can cause horizontal canal BPPV, and I'll explain how to treat it!

    Is there a particular maneuver that someone could do once a day as a precautionary, just to catch a bad bout of bppv before it happens

    When I did my self lest yesterday I was dizzy on my left ear. Decided to try the right and it was worse. So I did the Ellet for the right ear and it was horrible. At the end when I sat up I almost threw up and had to bend back over in a cold sweat. I waited five or six hours and tried again and did throw up. Later in the evening I tried doing it for the left ear. I was dizzy on each part and the sitting up was bad but not as much as doing the other side. This morning I did the left ear and less dizziness at the end and feeling a bit better. Am going to try the left again now. I think it was my left ear all along and could I have made it worse doing the right ear twice.

      Did you have someone check your eyes while doing the testing? You want to make sure that you have upbeat/torsional eye movements before you treat with the Epley. If you're eyes are moving horizontally (side to side), the Epley won't help and will only make you feel sick.

      @@GordonPhysicalTherapy No, there were no openings with anyone I contacted. I have had vertigo at least a dozen times in the past. I am usually fine after one or two Maneuvers or waiting a few hours. Today I went to a GP because of intense pressure around my eyes and nose and was told I had fluid in both ears making me thing it might be Meniers that caused the episode. I had a cold right before this all happened. The spinning is gone and just the pressure around the eyes still there. I am on prednisone for 5 days and from what I have read that is the first thing to do with ear fluid. My thought is now to get into an ENT and try and figure out what is causing so many episodes. I am 68 and have had them for 20 years and really just accepted that it would happen on occasion. I live outside of Phoenix and it seems 6-8 weeks is the normal for a new patient. I kept doing the Epley and then the one for horizontal. I was having no spinning or dizzyness doing the maneuvers which made me start thinking something else was causing my issue. I stopped doing them yesterday and feeling a whole lot better. So much for self diagnosis.

    I've done epley from time to time for decades. Worked well. I just did it for rt ear and got nauseous and sweaty. Still nauseated 30 minutes later and can't stand up. Obviously rt side otoliths loose. I have noticed certain foods provoke it, especially Tabasco and other peppery stuff.

    I had canal switch after BPPV manaever. They did Eply manaever. But still i can 't sleep because of slight diziness & nausea.I feel jolt in my head.

    I went to a pt. They tested me for bbpv which I had mildly on my left side. They didn’t have time to clear it so I went back 2 days later. They did the epley maneuver on both sides and it was negative. This was on Wed. I have felt worse ver since and don’t know what to do. Is this normal? They tested for both canals.

      It's possible you have cervicogenic dizziness at not inner ear BPPV. Is your PT fairly experienced at helping people with BPPV?

    Does it disappear forever?
    I went to the doctor after experiencing vertigo for seven days, and they prescribed me some medication. However, I continued to experience mild dizziness in my head, which made me feel heavy at the back of my head and neck, alone, anxious, and frightened.
    can anybody help me out ?

      You might not be dealing with BPPV, but it's possible you have some issues related to your neck. Has your doctor/provider considered your neck as a possible cause of your symptoms?

      Try vitamins take vitamin b12 vitamin D and magnesium all one a day first thing when you start your morning, I suffered with vertigo for the longest an thought I would never get a normal life back, thanks to God I feel like a renewed person 🙏 hope this truly helps and if it helps you or someone give me the feed back God Bless 🙏

    Wouldn't the maneuver for both sides work in this situation?

      If the vertigo switched from posterior canal to horizontal canal, you'll want to use the Gufoni Maneuver or BBQ Roll instead of the Epley. It's a bit confusing...

    As I'm laying on my back during the epley maneuver my dizziness gets worse and worse until I'm forced to sit up. I can't finish the maneuver because my dizziness gets worse not better

      Have you seen the direction of your eye movements? Are you sure you have posterior canal vertigo and not horizontal?

    What do you do if you don't get nystagmus anymore? Nobody can tell which canal and which side and sometimes the maneuvers make it worse sometimes they help a lot but i never get nystagmus anymore like i used to. Dealing with this since 2011

    on like my fourth day of epley and for whatever reason it switched sides, was always my left side, i did my left, nothing happened but then surprisngly it happened when i did the right side
