I had periodic bouts of vertigo over the past year and did some attempts at self-treatment at home based on youtube videos. Eventually, I decided this had gone on long enough and went to the ENT. He sent me to a vestibular therapist. The vestibular therapist treated me for a horizontal canal BPPV, but when more episodes of vertigo happened, we decided to get a VNG done. The VNG showed a left posterior canal BPPV and they did the Epley there for that. About a month later, I had vertigo again when rolling over in bed, went back to the vestibular therapist who did Epley for right posterior BPPV. She also prescribed eye exercises to strengthen my vestibular system. Fast forward another month, I had been feeling pretty good, no vertigo, just an ever so slight still off balance feeling when washing my hair or certain movements like that, but very mild. The vestibular therapist had me scheduled to see her to check on how I was progressing with the eye exercises. When she did the tests to check my canals I had a very mild vertigo and spent the next 48 hours feeling really bad (off balance, somewhat nauseous). I went back to see her again 2 days later and I was feeling good when I walked in, but again the tests to check my canals made me feel really bad for the next 3 days (actually this brings us to today and I still feel bad, so who knows when it will end). I feel off balance as if I were on a boat or looking through new progressive lenses or drunk. It's better when I am still. No vertigo, just not right in the head. Any ideas what could be going on or where I should go from here?
Thank you for sharing your experience, Sally! It's sounds frustrating, but it also sounds like you're making progress. If the tests for vertigo are still aggravating, my guess is that you still have some level of BPPV (still some debris in your canals). It sounds like you're working with a skilled vestibular therapist, so I would keep following their advice. Please let me know how it turns out. Good luck!
I am only a follow vertigo sufferer but cinnarizine antihistamine helped me after the epley manoeuvre made me feel much worse and very nauseous. I was back to mild vertigo after about 5 days
Guys, if the eply is not working. Do the half somersault! Highly highly it will fix it!!! Do the half somersault 3-6 times with 15 mins breaks after each one. And it will work!! If the eply is not working don’t keep doing it! For me it wasn’t working after 4 times and it only made it worse and more dizzy. I tried the half somersault by dr Foster and it fixed it after the 3rd time! Sometimes you have to go forward and down instead of back and down… I’ve had vertigo for 15 years and Dr Foster’s half somersault has always saved me! Good luck!
I've never done much with that technique, as it's more of a self-treatment approach and always seemed awkward. But it sounds like it's a good option! I'll have to look more into it ;-)
@@andiedealessio1693 When you get the crystals back in do you still have the after-effects of light-headedness? Or are you completed healed? The epley's have put my crystals back in but it takes days if not weeks for the after-effects of "air-headed" feeling to leave.
I swear my bppv is so spontaneous. I did the eply once about 2 months ago and it worked. I felt about 90 percent normal in regards to dizziness and vertigo. Yesterday out of nowhere while at work I suddenly started to feel like I was off balance. I know my body and been dealing with this enough to know what’s coming. I came home did eply to my left ear and had a 40 second latency while dong the dix hallpike. Once I felt it I just continued the eply. Felt ok but woke up and felt dizzy still. Did dix hallpike to left ear again and sure enough the vertigo came on at about 40 seconds so I decided to just stay in each position for 2 minutes. Did it once again for 2 minutes each position again. Feel ok but we’ll see tomorrow. I hate the sleeping upright and the staying upright for 24 hrs post manuever procedures. My back and neck always end up hurting because of these it’s super uncomfortable. This is the most stubborn condition I have ever dealt with. Ughhh
Thank you so much for these videos - you're helping so many people. I learnt from this that I have the horizontal eye movement. Explains why the Epley manoever doesn't work and just makes me nauseous.
I had my first episode vertigo 10 days ago, cured it doing the Epleys and now an hour ago it returned. This time it's worse, I will be doing the maneuver soon, the different this time is it's making me nauseous, which rarely happens. Hope the maneuver works again, not sure what's causing the crystals to get out of position.
I hope doing the Epley Maneuver again worked! If not, have someone check the direction of the movement of your eyes when you first lay back and tip your head back. If your eyes are moving side-side, you now have horizontal canal vertigo, and you need to do the Gufoni Maneuver and NOT the Epley. I hope that makes sense :-)
I had the Epley manoeuvre two days ago and my eyes were doing the left to right and I am now way worse - I was only getting symptoms at night when rolling on one side - now I can’t walk straight and feel like I’m falling over all of the time 🙈
If your eyes are moving side-to-side you have horizontal canal BPPV, and the Epley will NOT help. You want to do the Gufoni instead. Check out my videos on how to diagnose and treat horizontal canal BPPV!
What canals are involved with the horizontal eye movement? I recorded myself and I saw horizontal nystagmus and I also got nauseous and threw up. I’d like to know which canals are involved with that because you said the epley won’t work for that.
Hi, Mohammad. No BPPV isn't life threatening... but it can be very aggravating and scary at times, especially if you get a big wave of vertigo unexpectedly. I did it one time on the right side affected....damn once I lay down on the right side the room is spinning hell...and after 3 4 second it stopped immediately I hold for 30 second and turn my head to normal side and hold it for 30 second and I raised my self up slowly.. and sleep with 2 pillows for three days....gonna check it again after 3 days
It sounds like you're not doing the entire Epley Maneuver, which would also have you rolling to your opposite head (head down towards the floor) before you sit up. You definitely don't want to skip that final step. Does that make sense? I have a video on how to do the Epley if you need it...
@@GordonPhysicalTherapy yeah I did the rolling to the opposite head ( head down toward the floor ) and sit up with a lil fuzziness I kept doing it and this is the fourth days...from 100% dizziness now I feel 40% of dizziness lol....gonna doing it and still doing it till it done.... actually this is the fourth time I experienced vertigo...the 3rd times was 2 years ago.... and I only did 2x manuever and it gone away...this time is hard for me...I went to the doctor last 2 days...she checked at my ears...there is full of ear wax inside hahaha...and the left ear is got infection...so she gave me antibiotic...she told me dont do manuever....she afraid I'm going to passes out hahaha...but it's ok im still doing manuever...

Thanks, I woke up in the middle of the night and bent over to put my slippers on to go to the bathroom and had a sudden and very intense feeling of dizziness and the whole room started spinning really bad. I bumped into the walls trying to get back to bed and when I rolled over to my right side everything was spinning really badly still so I went onto my back again. Its been 5 hours since I have been out of bed now. I just found your videos tried doing the laying down test and turned my head to the right then waited 30 seconds and turned it to the left, then sat up and did it on the left and did a round of 3. I feel a little bit better but still feel weak and light headed. How can I test the rest of this if am here by myself with no one to check my eye patterns, I dont feel safe to drive to a therapist today. Is there anything else I can do to improve the dizziness? If the eyes are moving side-to-side, you're dealing with horizontal canal BPPV. I have a couple videos explaining what to do next, including the "Supine Roll Test" and the "Gufoni Maneuver".
How long after effects of maneuver stay? I had done maneuver twice positional diziness has resolved upto a great extent but lingering effects are not gone . Its been 10 days. I feel pressure in my ears jolts in my head and slight spinning while sleeping
Severe vertigo chronic, not sure it's bppv and hospital doc was incompetent. Think I may have fistula from altitude blow out, as I suddenly became deaf crossing mountain. pITA, docs who can do surgery of this type hard to find in local area.
Hi there! Thanks for this video. I was treated for my horizontal and posterior canals in two separate sessions. After yesterday’s appointment, everything felt completely normal and I had no off balance feelings. Last night before bed I noticed some pressure in my head which I attributed to the maneuvers. I laid down in bed normally for the first time in a few days without spinning but unfortunately, 2 minutes into laying down the room started to spin very intensely and my off balance symptoms/ dizziness / nausea returned as I got myself up from bed. Is this the Crystal returning to the canal? Or is this part of residual dizziness that my PT explained I need to work through? Thanks so much for sharing your expertise!
Most likely, this is the crystals returning into the canals. If possible, I would have your treating PT check your canals again as soon as you can. Recurrence of vertigo is very common, so it might take more sessions to keep the crystals where they belong. Also, after I treat clients, I typically recommend that they sleep with head elevated for 2-3 nights, just to reduce the likelihood of the crystals floating back into the canals. That might help you in the future :-)
I’ve seen a physical therapist who said my history sounds like BPPV but the Dix-Hallpike was negative. I had an episode in the appointment but didn’t have nystagmus but the therapist said my history is consistent with BPPV and didn’t know what else to suggest. I’ve tried Brandt-Daroff for a week and Epley once a night for 3 nights. Also tried half somersault and had an episode after each try. What do I do?
See if you have cervical vertigo. Nucca chiropractor or Blair chiropractic in your area. Your c1 and atlas could be off. I've been dizzy for 5 weeks and am avoiding doing the epley like the plague...I'm so scared of it.
Did the therapist perform the Epley for you, or did you only do it for yourself? It doesn't sound like your therapist was super familiar with BPPV. Like "pinkah boo" commented, you also want to rule out dizziness/vertigo coming from your neck. I hope you've gotten some help!
Please can you advise? What if I feel the dizziness only when turning my head down? It’s not right or left side movements that cause my vertigo. Shall I do the the manoeuvre for both sides then? Or is there a different exercise for my type of vertigo?
If you feel dizzy looking down, start with the "Dix Halpike Test" to determine which side to treat. More than likely, you'll only need to treat one side.
My vertigo was limited to after I laid-down and I experienced no nausea. This has been going-on for about 6 or 7 weeks. I have right-ear pressure, so I assumed it was that side. I perfomed the right-side EM and was significantly worse immediately after - I now have a constant mild vertigo while upright combined with moderate nausea. Any suggestions? Thanks.
Hi, Jill. If you're having some constant vertigo, as well as pressure in your ear, I'd suspect an inner ear infection or possibly Meniere's. If you haven't already, I'd consider seeing an ENT (Ear, Nose & Throat) specialist to figure out the diagnosis. Or, you can look for a PT well versed in vestibular issues and see what they think. Is seeing an ENT or PT an option for you?

I've had this problem before, and the ENT tried Eply a few times without success then refered me to a "specialist" - who came-off like a used-car salesman: did another Epley & announced, "You're better - that'll be $250".
@@jillbluerei4806 that's too bad to hear... $250 might be a good deal if it fixed your problem, but it sounds a bit shady. If the Epley isn't working, and you have the sensation of pressure, I'd be surprised if you have BPPV. Getting another opinion from an ENT is a good place to start. I had my first vertigo episode about two months ago and was very intense the first day. I started PT and was diagnosed with BPPV and unilateral vestibular hypofunction. I went to weekly visits for 4 weeks and then I stopped going to PT because I felt better, no dizziness! Then, less than 2 weeks after I felt better, I was dizzy again, that lasted about a week and I was not dizzy for a couple days, but it's back again. Feeling kind of hopeless that this is my new norm... is there any hope for getting rid of my dizziness?
Hi, Jessica! Recurrence of BPPV is very common, so don't despair! Get back in to see your PT again, and they should be able to clear it up for you. Let me know how it goes :-)
@@GordonPhysicalTherapy Thank you, they also told me I had underlying hypofunction, so I have been doing exercises to help that. I am feeling a little hopeless, as I've been dealing with this for a couple months and it seemed to go away completely but only for 3 weeks and now it's back - feeling down that this is going to be my new normal.
If you decide not to do any of the Maneuvers and decide to just wait it out will the crystals go back into position or will you permanently have disorientation and dizziness and vertigo until you fix it
Get it fixed as quickly as possible. There's no guarantee those crystals will dissolve or relocate on their own. And I've seen people go years with vertigo before finally coming in for treatment. Sooner is better :-)
I have fullness in my ear with swaying feeling in my head.. day and night.. seen ent had mri and ct scan. I'm fine. I think ots meniere's. Do you know how to work with meniere's?
Hi, Terrie. It sure sounds like you have Meniere's Disease. As a PT, there's not much I can do to treat it. But there are some natural remedies if you search for them online. I don't really know how well they work, as I haven't seen people try the remedies first hand. Sorry I can't be more helpful than that... if you find something that works, please let me know :-)
Exactly same 24 7 dizziness in bed sitting standing all positions. Also frequent ear infections tinnitus and pain. Epley is not as easy as some make it sound its a nightmare and it can worsen thngs. Some ppl cant tolerate it. I personaly got a seizures from phobia and vomit. I thnk i have meniers

I’m experiencing the rapid eye side to side movement. I’ve tried the Epley maneuver and now I can’t lye down (face 90 degrees looking up nor to the right) without getting vertigo. It seemed to have gotten worse. I am planning to call an ent … or do I need a referral first? Should I/ Can I just go to a vestibular physical therapist first?
I just had manouvers for right side superior canal (third ones actually). It was in a clinic with special "astronaut training" chair (if you know what I mean) and specials goggles to detect nystagmus. The vertigo came after 10 seconds. It was unbelivable strong (I couldn't stand it) but did fade away after 10 seconds. Now it is my third day after that, sleeping in the chair (3 nights in a raw). I am so scared, but have another visit booked within 14 days. Working with psychotherapist in parallel. Is BPPV something that I will get rid of one day? Does vit D helps. What else to do to treat it? What diet? Any answer would be helpful. 😢
Glad to hear you're working with a specialist! Yes, based on what you said, they should be able to get rid of your BPPV. When you follow up with them, they'll likely test for vertigo again, and with any luck you'll be clear. And if you're not 100% yet, they should be able to continue treating you until you are. I was diagnosed with BPPV 2months ago and did the Epley with a PT and after one session it was completely gone. Now 2 months later its back! Why does it come back? And is this something tht is going to be happening forever every other month now? What can i do to lessen it coming back?
Excellent question! Unfortunately, BPPV does come back at a later date for a percentage of people (I believe it's around 10%, but it depends on age and a few other factors). The good thing is that you now know how to treat the vertigo, or you know someone who can help. With any luck, it won't continue to come back every few months... the goal is to get the little ear crystals back where they belong and then for them to stay put. It just doesn't always work out the first time for people. I hope that helps!
I’m dealing with that right now, every time I get in the first layback position of epley, I spin, hen I finish……..I’m on 4th time tonight and I still spin in that position. Dammit.
Your videos are very informative thank you so much!! So a few days ago I get terrible vertigo any side im on or whenever I lay down on left and right. I go to the audiologist and he tests dixhallpike and it shows positive on both ears but left more jntense so we do epley for that. Today I test the left side with dixhallpike and it’s negative no spinning, but it doesn’t explain why i am still spinning badly when i tilt my head to the left so I find one of your videos And i do supine roll test and BAM i have bad vertigo that is more like beating to the left. I test the right ear and it happens again. So i started the bbq roll for left ear first and did it 3 times. Im gonna repeat again before i go to bed and see if it has improved. If im fine on the left side im gonna do the same for the right ear starting with epley first if needed then bbq roll. Does this sound logic to you? Really wanna know what you think. I’ve decided to do it on my own from now on since I keep getting bppv repeatedly.
Overall, your logic is pretty sound. I prefer the Gufoni over the BBQ roll for treatment of horizontal canal, but that's just me. How are you doing now? If you're not fully recovered, I'd highly recommend finding a PT who specializes in helping people with BPPV. It can be really tricky to self-treat when the horizontal canals are involved. Let me know how you're doing :-)
For whatever reason, sometimes it can be really difficult to put crystals back where they belong, while other times it's quite simple. I'm not sure if the crystals get "jammed" or if it's just a larger crystal that's harder to move back to where it belongs. Either way, here's what I recommend: First, make sure you're treating the correct canal. This is done by understanding the "Dix-Halpike" test as well as the "Supine Roll Test", or having someone experienced with BPPV help you understand these tests. Second, if you're clearly treating the correct canal, just keep trying for a few more days, doing 3-4 reps of the "Epley Maneuver" at a time. Third, if this still doesn't work, try to get some help from someone in your area. I know this can be hard to do, especially in certain parts of the world. But if there's someone in your area who can help you, that might do the trick. I hope this helps! Please let me know how things go and if you have more questions.

Is this wrong?
I've had moderate disequilibrium for the past seven weeks. My audiologist performed the Epley on me eight times in one session and it had me spinning for three days. I saw a vestibular physio and they performed it only once because my reaction was so severe. That said, I did have some relief for one day before some of my symptoms came back. I decided to have the Epley again done by a different vestibular physio today (as the original was totally booked) and I feel a thousand times worse than I did. I now have another appointment for an Epley for next week and I am terrified to go. Am I just giving up too early or is it possible this is not BPPV? Sorry to ask for advice over the internet, but this condition is absolutely brutal. I do present with nystagmus (not horizontal) and it is definitely my left ear. Thank you in advance (if you see this comment).
Stop doing the eply if it is not working! Do the half somersault instead! For me I had the same problem and kept trying the eply only to not work and make it worse and more dizzy with floaty feeling. I had enough and did the half somersault myself at home with 15 mins breaks between each one. I pushed through the dizziness and by the third time it was gone. Spinning and dizziness stoped. Sometimes the eply will not work and you have to go forwards and down instead of back and down. Look up doctor Foster’s interview video and watch the whole thing:… good luck!!

@@GordonPhysicalTherapy Not a problem and thanks for checking in. It was BPPV but it was not fully resolved until the final appointment. As a result, I was dealing with vertigo off and on for around three months. When the calcium carbonate was finally repositioned, I was left with dizziness, not vertigo. Basically, the sensation I was moving, not the world moving around me. I was told this was because I had BPPV for so long that my inner ear had to be retrained as it had been compensating. For this, I sought the help of a vesitbular physiotherapist and with many eye and balance exercises I am slowly getting better. I still feel like I am on a boat some days, but I also see the vast improvment. Thanks again!
@@luluwatermelon7226 Hi Lori, I wish more people talked about the after-effects of BPPV Epley maneuvers. I had BPPV for the first time last year with intense vertigo. Thank God for Google, I found it was BPPV and did the Epley maneuvers since the ENT dr. just confirmed I had BPPV, gave me some meclizine, and sent me on my way. Once you get the crystals back in the canal you will feel LIGHT HEADED, slightly DIZZY for a while. It sucks so bad. But I guess I'd rather have that than vertigo. And it's due to what you said, the crystals were out for so long that now your head needs to readjust itself with them being back in place. I had a tiny bit of BPPV last week (no intense vertigo, just the world slowly turning) and after a few epley's the crystals are now back in place but I'm having that AIR HEADED feeling again. Just know, IT WILL GO AWAY. Mine did. I was getting pretty depressed last year cuz I thought my new normal was slight dizziness. But slowly it went away. Sadly it came back a bit - the dr. said it could come back. What's even worse, there really isn't anything we are doing wrong, it just happens. The good news is now we know what IT IS and can work to get it better.

My sister is 32 years old and she has suffered from meniere disease for years and she always complains to me that I had to bought her Dr Madida herbs I saw on UA-cam and she is telling me last month that she is cured completely and don’t long have the symptoms like vertigo or dizziness, feeling of fullness in the ear or ringing, hearing loss, imbalance, motion sickness, nausea, or nystagmus
Hello Gordon thank you for the informative video, i have been suffering from Bppv for 2 years now, i was working as a flight attendant for more than 6 years and i believe this is what triggered it. I have seen many ENT doctors that said that i don't have bppv, however only the TRV chair could detect it. I made many sessions with the TRV chair with no results whatsoever. The ENT doctor after so many sessions kinda gave up. Please advice on what to do next as this vertigo is kind of ruining my life.
Have you checked for miniers disease, occipital vertigo, cervical vertigo…………..occipital nerves can get pinched up and cause dizziness too, I have that and BPPV sometimes……research those
This is a very frustrating video to watch. I have been going to therapy for my dizziness for almost 2 months now and I am probably going to cancel my next session and stop going. I have been through the epley maneuver several times and it didn't work even though my therapist thought otherwise. I had no spinning or anything. I have also done vor exercises and that has done nothing either. My dizziness consists of feeding very heavy headed and feeling off balance constantly and it happens after sitting for a long period of time and pretty much every time I move my head. A lot of times looking down or bending over will cause the dizziness to happen and even when I'm in the shower it happens. I always feel worse after having the modified epley done and I have been through it more often 10 times. I do believe that my therapists are having difficulty trying to figure out what is going on but I honestly can't keep suffering while they are attempting to decifer the issue. So I will probably cancel my next appointment and get with my doctor and see if I can hopefully get an mri or something done. But I don't think I can handle another epley treatment. I have already told my therapist what I just said here but they don't seem to be listening very well so I will just have to stop going. But that's why I mention that this video is frustrating to watch because it appears that no one really knows what to do about dizziness.
Hey, Vernon. I'm sorry to hear how frustrated you are... in my opinion, you're absolutely right: your therapists don't actually know what's causing your vertigo. If you have constant sensations of unsteadiness, as well as the sensation of fullness in your head, there's a very good chance you DON'T have BPPV. In which case, you're correct again: doing the Epley over and over again won't help, it will just make you feel worse. Here's my advice: talk to your doctor and see if you can get a referral to an ENT (Ear, Nose & Throat) specialist. With any luck, they can help you figure out the root cause of your vertigo/unsteadiness. Good luck, sir!

I already canceled my next appointment with the therapist and I told them why and they apologized and told me that they hoped I might come back but they understood if I didn’t. I will be talking to my family doctor by phone Friday afternoon and see what he says and go from there. I will mention ent to him unless he has another idea but I will still mention this. 20 years ago I was in the same boat.....chronically dizzy and nothing worked. I had a client who was a ENT and he suggested it may be Meniere's.....and indeed it was. Long story short, I've lived thru your agony for the past 20 years. Its life changing for sure. Hopefully you can get Meniere's ruled out, that would be HUGE.

What is the Epley if we don't see what it is how can we fix it
I am only a follow vertigo sufferer but cinnarizine antihistamine helped me after the epley manoeuvre made me feel much worse and very nauseous. I was back to mild vertigo after about 5 days
Guys, if the eply is not working. Do the half somersault! Highly highly it will fix it!!! Do the half somersault 3-6 times with 15 mins breaks after each one. And it will work!! If the eply is not working don’t keep doing it! For me it wasn’t working after 4 times and it only made it worse and more dizzy. I tried the half somersault by dr Foster and it fixed it after the 3rd time! Sometimes you have to go forward and down instead of back and down… I’ve had vertigo for 15 years and Dr Foster’s half somersault has always saved me! Good luck!
I've never done much with that technique, as it's more of a self-treatment approach and always seemed awkward. But it sounds like it's a good option! I'll have to look more into it ;-)

Please keep in mind it depends what canal the “crystals” are in. Always a chance of making things worse, but I have also had that maneuver work.
@@andiedealessio1693 When you get the crystals back in do you still have the after-effects of light-headedness? Or are you completed healed? The epley's have put my crystals back in but it takes days if not weeks for the after-effects of "air-headed" feeling to leave.
What do you mean by “ forward and down “ not “ back and down” ?
I swear my bppv is so spontaneous. I did the eply once about 2 months ago and it worked. I felt about 90 percent normal in regards to dizziness and vertigo. Yesterday out of nowhere while at work I suddenly started to feel like I was off balance. I know my body and been dealing with this enough to know what’s coming. I came home did eply to my left ear and had a 40 second latency while dong the dix hallpike. Once I felt it I just continued the eply. Felt ok but woke up and felt dizzy still. Did dix hallpike to left ear again and sure enough the vertigo came on at about 40 seconds so I decided to just stay in each position for 2 minutes. Did it once again for 2 minutes each position again. Feel ok but we’ll see tomorrow. I hate the sleeping upright and the staying upright for 24 hrs post manuever procedures. My back and neck always end up hurting because of these it’s super uncomfortable. This is the most stubborn condition I have ever dealt with. Ughhh

I had my first episode vertigo 10 days ago, cured it doing the Epleys and now an hour ago it returned. This time it's worse, I will be doing the maneuver soon, the different this time is it's making me nauseous, which rarely happens. Hope the maneuver works again, not sure what's causing the crystals to get out of position.
I hope doing the Epley Maneuver again worked! If not, have someone check the direction of the movement of your eyes when you first lay back and tip your head back. If your eyes are moving side-side, you now have horizontal canal vertigo, and you need to do the Gufoni Maneuver and NOT the Epley. I hope that makes sense :-)
I had to do multiple sessions a day for 4 days before it cleared on one occasion. It will eventually resolve.

I had the Epley manoeuvre two days ago and my eyes were doing the left to right and I am now way worse - I was only getting symptoms at night when rolling on one side - now I can’t walk straight and feel like I’m falling over all of the time 🙈
If your eyes are moving side-to-side you have horizontal canal BPPV, and the Epley will NOT help. You want to do the Gufoni instead. Check out my videos on how to diagnose and treat horizontal canal BPPV!

I did it one time on the right side affected....damn once I lay down on the right side the room is spinning hell...and after 3 4 second it stopped immediately I hold for 30 second and turn my head to normal side and hold it for 30 second and I raised my self up slowly.. and sleep with 2 pillows for three days....gonna check it again after 3 days
It sounds like you're not doing the entire Epley Maneuver, which would also have you rolling to your opposite head (head down towards the floor) before you sit up. You definitely don't want to skip that final step. Does that make sense? I have a video on how to do the Epley if you need it...

Thanks, I woke up in the middle of the night and bent over to put my slippers on to go to the bathroom and had a sudden and very intense feeling of dizziness and the whole room started spinning really bad. I bumped into the walls trying to get back to bed and when I rolled over to my right side everything was spinning really badly still so I went onto my back again. Its been 5 hours since I have been out of bed now. I just found your videos tried doing the laying down test and turned my head to the right then waited 30 seconds and turned it to the left, then sat up and did it on the left and did a round of 3. I feel a little bit better but still feel weak and light headed. How can I test the rest of this if am here by myself with no one to check my eye patterns, I dont feel safe to drive to a therapist today. Is there anything else I can do to improve the dizziness? I used a mirror when I did the Epley by myself- worked well!
What if the eyes goes from side to side and not up and down, I read that the epley do not help.
If the eyes are moving side-to-side, you're dealing with horizontal canal BPPV. I have a couple videos explaining what to do next, including the "Supine Roll Test" and the "Gufoni Maneuver".
How long after effects of maneuver stay? I had done maneuver twice positional diziness has resolved upto a great extent but lingering effects are not gone . Its been 10 days.

Severe vertigo chronic, not sure it's bppv and hospital doc was incompetent. Think I may have fistula from altitude blow out, as I suddenly became deaf crossing mountain. pITA, docs who can do surgery of this type hard to find in local area.
I'm sorry to hear that. It doesn't sound like you have BPPV, but it's easy to check for.
Hi there! Thanks for this video. I was treated for my horizontal and posterior canals in two separate sessions. After yesterday’s appointment, everything felt completely normal and I had no off balance feelings. Last night before bed I noticed some pressure in my head which I attributed to the maneuvers. I laid down in bed normally for the first time in a few days without spinning but unfortunately, 2 minutes into laying down the room started to spin very intensely and my off balance symptoms/ dizziness / nausea returned as I got myself up from bed. Is this the Crystal returning to the canal? Or is this part of residual dizziness that my PT explained I need to work through?

Most likely, this is the crystals returning into the canals. If possible, I would have your treating PT check your canals again as soon as you can. Recurrence of vertigo is very common, so it might take more sessions to keep the crystals where they belong.
Also, after I treat clients, I typically recommend that they sleep with head elevated for 2-3 nights, just to reduce the likelihood of the crystals floating back into the canals. That might help you in the future :-)

I’ve seen a physical therapist who said my history sounds like BPPV but the Dix-Hallpike was negative. I had an episode in the appointment but didn’t have nystagmus but the therapist said my history is consistent with BPPV and didn’t know what else to suggest. I’ve tried Brandt-Daroff for a week and Epley once a night for 3 nights. Also tried half somersault and had an episode after each try. What do I do?
See if you have cervical vertigo. Nucca chiropractor or Blair chiropractic in your area. Your c1 and atlas could be off. I've been dizzy for 5 weeks and am avoiding doing the epley like the plague...I'm so scared of it.
Did the therapist perform the Epley for you, or did you only do it for yourself? It doesn't sound like your therapist was super familiar with BPPV. Like "pinkah boo" commented, you also want to rule out dizziness/vertigo coming from your neck. I hope you've gotten some help!
Please can you advise? What if I feel the dizziness only when turning my head down? It’s not right or left side movements that cause my vertigo. Shall I do the the manoeuvre for both sides then? Or is there a different exercise for my type of vertigo?
If you feel dizzy looking down, start with the "Dix Halpike Test" to determine which side to treat. More than likely, you'll only need to treat one side.
My vertigo was limited to after I laid-down and I experienced no nausea. This has been going-on for about 6 or 7 weeks. I have right-ear pressure, so I assumed it was that side. I perfomed the right-side EM and was significantly worse immediately after - I now have a constant mild vertigo while upright combined with moderate nausea. Any suggestions? Thanks.
Hi, Jill. If you're having some constant vertigo, as well as pressure in your ear, I'd suspect an inner ear infection or possibly Meniere's. If you haven't already, I'd consider seeing an ENT (Ear, Nose & Throat) specialist to figure out the diagnosis. Or, you can look for a PT well versed in vestibular issues and see what they think.
Is seeing an ENT or PT an option for you?

I've had this problem before, and the ENT tried Eply a few times without success then refered me to a "specialist" - who came-off like a used-car salesman: did another Epley & announced, "You're better - that'll be $250".
@@jillbluerei4806 that's too bad to hear... $250 might be a good deal if it fixed your problem, but it sounds a bit shady. If the Epley isn't working, and you have the sensation of pressure, I'd be surprised if you have BPPV. Getting another opinion from an ENT is a good place to start. I had my first vertigo episode about two months ago and was very intense the first day. I started PT and was diagnosed with BPPV and unilateral vestibular hypofunction. I went to weekly visits for 4 weeks and then I stopped going to PT because I felt better, no dizziness! Then, less than 2 weeks after I felt better, I was dizzy again, that lasted about a week and I was not dizzy for a couple days, but it's back again. Feeling kind of hopeless that this is my new norm... is there any hope for getting rid of my dizziness?
Hi, Jessica! Recurrence of BPPV is very common, so don't despair! Get back in to see your PT again, and they should be able to clear it up for you. Let me know how it goes :-)
@@GordonPhysicalTherapy Thank you, they also told me I had underlying hypofunction, so I have been doing exercises to help that. I am feeling a little hopeless, as I've been dealing with this for a couple months and it seemed to go away completely but only for 3 weeks and now it's back - feeling down that this is going to be my new normal.
If you decide not to do any of the Maneuvers and decide to just wait it out will the crystals go back into position or will you permanently have disorientation and dizziness and vertigo until you fix it
Get it fixed as quickly as possible. There's no guarantee those crystals will dissolve or relocate on their own. And I've seen people go years with vertigo before finally coming in for treatment. Sooner is better :-)
I have fullness in my ear with swaying feeling in my head.. day and night.. seen ent had mri and ct scan. I'm fine. I think ots meniere's. Do you know how to work with meniere's?
Hi, Terrie. It sure sounds like you have Meniere's Disease. As a PT, there's not much I can do to treat it. But there are some natural remedies if you search for them online. I don't really know how well they work, as I haven't seen people try the remedies first hand. Sorry I can't be more helpful than that... if you find something that works, please let me know :-)
Exactly same 24 7 dizziness in bed sitting standing all positions. Also frequent ear infections tinnitus and pain. Epley is not as easy as some make it sound its a nightmare and it can worsen thngs. Some ppl cant tolerate it. I personaly got a seizures from phobia and vomit. I thnk i have meniers

I’m experiencing the rapid eye side to side movement. I’ve tried the Epley maneuver and now I can’t lye down (face 90 degrees looking up nor to the right) without getting vertigo. It seemed to have gotten worse. I am planning to call an ent … or do I need a referral first? Should I/ Can I just go to a vestibular physical therapist first?
I just had manouvers for right side superior canal (third ones actually). It was in a clinic with special "astronaut training" chair (if you know what I mean) and specials goggles to detect nystagmus. The vertigo came after 10 seconds. It was unbelivable strong (I couldn't stand it) but did fade away after 10 seconds. Now it is my third day after that, sleeping in the chair (3 nights in a raw). I am so scared, but have another visit booked within 14 days. Working with psychotherapist in parallel. Is BPPV something that I will get rid of one day? Does vit D helps. What else to do to treat it? What diet? Any answer would be helpful. 😢
Glad to hear you're working with a specialist! Yes, based on what you said, they should be able to get rid of your BPPV. When you follow up with them, they'll likely test for vertigo again, and with any luck you'll be clear. And if you're not 100% yet, they should be able to continue treating you until you are. I was diagnosed with BPPV 2months ago and did the Epley with a PT and after one session it was completely gone. Now 2 months later its back! Why does it come back? And is this something tht is going to be happening forever every other month now? What can i do to lessen it coming back?
Excellent question! Unfortunately, BPPV does come back at a later date for a percentage of people (I believe it's around 10%, but it depends on age and a few other factors). The good thing is that you now know how to treat the vertigo, or you know someone who can help. With any luck, it won't continue to come back every few months... the goal is to get the little ear crystals back where they belong and then for them to stay put. It just doesn't always work out the first time for people. I hope that helps!

I’m dealing with that right now, every time I get in the first layback position of epley, I spin, hen I finish……..I’m on 4th time tonight and I still spin in that position. Dammit.
Your videos are very informative thank you so much!! So a few days ago I get terrible vertigo any side im on or whenever I lay down on left and right. I go to the audiologist and he tests dixhallpike and it shows positive on both ears but left more jntense so we do epley for that. Today I test the left side with dixhallpike and it’s negative no spinning, but it doesn’t explain why i am still spinning badly when i tilt my head to the left so I find one of your videos
And i do supine roll test and BAM i have bad vertigo that is more like beating to the left. I test the right ear and it happens again. So i started the bbq roll for left ear first and did it 3 times. Im gonna repeat again before i go to bed and see if it has improved. If im fine on the left side im gonna do the same for the right ear starting with epley first if needed then bbq roll. Does this sound logic to you? Really wanna know what you think. I’ve decided to do it on my own from now on since I keep getting bppv repeatedly.
Overall, your logic is pretty sound. I prefer the Gufoni over the BBQ roll for treatment of horizontal canal, but that's just me. How are you doing now? If you're not fully recovered, I'd highly recommend finding a PT who specializes in helping people with BPPV. It can be really tricky to self-treat when the horizontal canals are involved. Doing bppv 3x daily for 3 wks. Still feel imbalance. Spin gone on off. Vertigo june2019 again nov 2020. can my crystal get jammed ? And wont go back
For whatever reason, sometimes it can be really difficult to put crystals back where they belong, while other times it's quite simple. I'm not sure if the crystals get "jammed" or if it's just a larger crystal that's harder to move back to where it belongs. Either way, here's what I recommend:
First, make sure you're treating the correct canal. This is done by understanding the "Dix-Halpike" test as well as the "Supine Roll Test", or having someone experienced with BPPV help you understand these tests.
Second, if you're clearly treating the correct canal, just keep trying for a few more days, doing 3-4 reps of the "Epley Maneuver" at a time.
Third, if this still doesn't work, try to get some help from someone in your area. I know this can be hard to do, especially in certain parts of the world. But if there's someone in your area who can help you, that might do the trick.

I've had moderate disequilibrium for the past seven weeks. My audiologist performed the Epley on me eight times in one session and it had me spinning for three days. I saw a vestibular physio and they performed it only once because my reaction was so severe. That said, I did have some relief for one day before some of my symptoms came back. I decided to have the Epley again done by a different vestibular physio today (as the original was totally booked) and I feel a thousand times worse than I did. I now have another appointment for an Epley for next week and I am terrified to go. Am I just giving up too early or is it possible this is not BPPV? Sorry to ask for advice over the internet, but this condition is absolutely brutal. I do present with nystagmus (not horizontal) and it is definitely my left ear. Thank you in advance (if you see this comment).
Stop doing the eply if it is not working! Do the half somersault instead! For me I had the same problem and kept trying the eply only to not work and make it worse and more dizzy with floaty feeling. I had enough and did the half somersault myself at home with 15 mins breaks between each one. I pushed through the dizziness and by the third time it was gone. Spinning and dizziness stoped. Sometimes the eply will not work and you have to go forwards and down instead of back and down. Look up doctor Foster’s interview video and watch the whole thing:… good luck!!

@@GordonPhysicalTherapy Not a problem and thanks for checking in. It was BPPV but it was not fully resolved until the final appointment. As a result, I was dealing with vertigo off and on for around three months. When the calcium carbonate was finally repositioned, I was left with dizziness, not vertigo. Basically, the sensation I was moving, not the world moving around me. I was told this was because I had BPPV for so long that my inner ear had to be retrained as it had been compensating. For this, I sought the help of a vesitbular physiotherapist and with many eye and balance exercises I am slowly getting better. I still feel like I am on a boat some days, but I also see the vast improvment. Thanks again!

Thank God for Google, I found it was BPPV and did the Epley maneuvers since the ENT dr. just confirmed I had BPPV, gave me some meclizine, and sent me on my way. Once you get the crystals back in the canal you will feel LIGHT HEADED, slightly DIZZY for a while. It sucks so bad. But I guess I'd rather have that than vertigo. And it's due to what you said, the crystals were out for so long that now your head needs to readjust itself with them being back in place. I had a tiny bit of BPPV last week (no intense vertigo, just the world slowly turning) and after a few epley's the crystals are now back in place but I'm having that AIR HEADED feeling again. Just know, IT WILL GO AWAY. Mine did. I was getting pretty depressed last year cuz I thought my new normal was slight dizziness. But slowly it went away. Sadly it came back a bit - the dr. said it could come back. What's even worse, there really isn't anything we are doing wrong, it just happens. The good news is now we know what IT IS and can work to get it better.

My sister is 32 years old and she has suffered from meniere disease for years and she always complains to me that I had to bought her Dr Madida herbs I saw on UA-cam and she is telling me last month that she is cured completely and don’t long have the symptoms like vertigo or dizziness, feeling of fullness in the ear or ringing, hearing loss, imbalance, motion sickness, nausea, or nystagmus

Hello Gordon thank you for the informative video, i have been suffering from Bppv for 2 years now, i was working as a flight attendant for more than 6 years and i believe this is what triggered it. I have seen many ENT doctors that said that i don't have bppv, however only the TRV chair could detect it. I made many sessions with the TRV chair with no results whatsoever. The ENT doctor after so many sessions kinda gave up. Please advice on what to do next as this vertigo is kind of ruining my life.
It's possible you have Mal de Barquement Syndrome. Have you heard of that yet?
Have you checked for miniers disease, occipital vertigo, cervical vertigo…………..occipital nerves can get pinched up and cause dizziness too, I have that and BPPV sometimes……research those
This is a very frustrating video to watch. I have been going to therapy for my dizziness for almost 2 months now and I am probably going to cancel my next session and stop going. I have been through the epley maneuver several times and it didn't work even though my therapist thought otherwise. I had no spinning or anything. I have also done vor exercises and that has done nothing either. My dizziness consists of feeding very heavy headed and feeling off balance constantly and it happens after sitting for a long period of time and pretty much every time I move my head. A lot of times looking down or bending over will cause the dizziness to happen and even when I'm in the shower it happens. I always feel worse after having the modified epley done and I have been through it more often 10 times. I do believe that my therapists are having difficulty trying to figure out what is going on but I honestly can't keep suffering while they are attempting to decifer the issue. So I will probably cancel my next appointment and get with my doctor and see if I can hopefully get an mri or something done. But I don't think I can handle another epley treatment. I have already told my therapist what I just said here but they don't seem to be listening very well so I will just have to stop going. But that's why I mention that this video is frustrating to watch because it appears that no one really knows what to do about dizziness.
Hey, Vernon. I'm sorry to hear how frustrated you are... in my opinion, you're absolutely right: your therapists don't actually know what's causing your vertigo. If you have constant sensations of unsteadiness, as well as the sensation of fullness in your head, there's a very good chance you DON'T have BPPV. In which case, you're correct again: doing the Epley over and over again won't help, it will just make you feel worse. Here's my advice: talk to your doctor and see if you can get a referral to an ENT (Ear, Nose & Throat) specialist. With any luck, they can help you figure out the root cause of your vertigo/unsteadiness. Good luck, sir!

I already canceled my next appointment with the therapist and I told them why and they apologized and told me that they hoped I might come back but they understood if I didn’t. I will be talking to my family doctor by phone Friday afternoon and see what he says and go from there. I will mention ent to him unless he has another idea but I will still mention this. 20 years ago I was in the same boat.....chronically dizzy and nothing worked. I had a client who was a ENT and he suggested it may be Meniere's.....and indeed it was. Long story short, I've lived thru your agony for the past 20 years. Its life changing for sure. Hopefully you can get Meniere's ruled out, that would be HUGE.
