Violent Patient! Paramedics Need Urgent Assistance! [Vancouver, Canada]

- Captured here is the scene of a medical emergency call that started off with a man lying on the sidewalk and screaming erratically, before passing pedestrians called 911 and asked Vancouver firefighters from the nearby Fire Hall No. 2 for assistance. When fire medics approached the man was seen climbing the side of a construction waste container and diving in head first! Due to the sensitive nature of the scene these events were not filmed. Paramedics from the BC Ambulance Service soon showed up to assist firefighters and it appeared that the patient then became violent towards the emergency services. Several units of the Vancouver Police Department showed up within minutes to quickly assist firefighters and restrain the allegedly violent patient inside an ambulance. Firefighters had to disinfect their uniforms and equipment thoroughly after clearing from the encounter. Engine 2 of Vancouver Fire & Rescue is also seen responding and to an unrelated emergency while police and paramedics were still working on the patient. VPD later escorted the ambulance and patient to St. Paul's hospital for treatment.

I worked in Vancouver on the downtown east side for 2 years and was always really impressed by VPD. I remember someone very high on meth trying to smash out our side window on the ambulance with a metal rod and we called for police urgently and I was really blown away at how quickly we had 4 cruisers pulling up surrounding us from every direction.... like seconds later. Vancouver Fire dept was always really great to work with too.

Sorry that the medics were in a bad situation inside the ambulance. I was a emt for 15 years. I had a similar experiance with a patient. God bless all first responders and pray they all come home to their families after their shifts.

@@MadCuzUBroke I've went on plenty of ride alongs with BCAS and and my local fire departments medics, and I can say that being an EMT is a tough, but also brilliant career, definitely someone that I really want to do.

My wife is a Paramedic in Canada. In July 2018, she was attacked by a crack addict who became violent. She was kicked in the head, and suffered her 3rd concussion in 19 years on the job. She is still off dealing with headaches and memory loss.
I’m sorry to hear about that... Emergency services should never have to face violence. Sadly the reality is that paramedics are having to endure a lot more than they used to. This is becoming a problem around the world and I hope people will become more aware of it.

Sorry to hear. My career as a paramedic came to an abrupt end after a 2 storey fall. No violence on that occasion but had been at the sharp end of more used syringes, knives and other weapons than I care to remember. I sincerely hope two years on that she's back to work. It's a real shock when your dream career suddenly ends unplanned.

So in the States, our officers typically park and leave their lights on; especially when parking in the street. I noticed that these VC officers turned them off on arrival. Is that just SOP?
John Hall Turning the lights off is definitely a typical move for VPD. They are also often seen using lights and sirens in short ‘bursts’ when responding, rather than keeping them on all the way. I’m not sure why exactly they do this, but since I’ve seen it so often I do think it is SOP here. That being said they usually do leave their lights on whenever they are obstructing traffic in any way.

@Streamer LX4 This is based on the state, actually. For instance, New York uses red for police, fire, and EMS. In some other states, they use red and blue for all vehicles. There's a unique history behind the requirements for emergency vehicle marking per-state. California, for instance, requires a "steady red" to be forward-facing on all emergency vehicles.
Streamer LX4 It’s not all the same all over North America. Traditionally the standard for police lights is red/blue/white and red/white/amber for fire and EMS. However it’s a choice for departments and States/ Provinces, wherever laws allow. Some police departments use all blue lights, some use all red, other places use blue and red for the fire department...
It really depends. Overall I think blue white and red is the most visible (in combination) for all emergency vehicles, but all blue would also be good.
Some agencies here in California do the same thing yeah the ambulance already had its lights on so really no need for all the emergency lights to be on fire department here does the same if not needed.

Nice catches! Great to see VPD's fast response to their fellow first response colleagues! As a paramedic student studying in Toronto, we were shown plenty of these possible incidents where you call police, and let them handle the guy while we medics sit back and have a beer (nah). I noticed that VPD cars often leave their lights off. In my school, that's actually typical practice, unless you're in heavy and fast traffic. The reason for this (at least in Toronto) is by having your lights on, you could attract large amounts of unwanted attention, like snoopy bystanders with their phones out, the media wanting to get a story, or anyone that wants to bother you.
SnipballTheBestBall Thank you! It’s always nice when different agencies work well together. Unfortunately that’s the only thing paramedics can do in some of these situations. Nobody deserves to be in danger while working, but sadly levels of violence towards first responders are on the rise.
Interesting information on why they leave the lights off. That’s actually the most plausible explanation I have heard so far! I’m thinking that’s the reason, because in the age of smartphones it can be quite disruptive to draw too much attention to a scene...

Thanks mate! :D Haha sometimes you just have a feeling and are in the right place at the right time... (but I’m not always this lucky lol). Oh BCAS responds in the opposite lane all the time, especially on Broadway in Vancouver to avoid traffic jams.

Yes I think everyone was ok, the police showed up very quickly and the situation seemed mostly under control after a few minutes. 👍🏼
I think all services did an excellent job considering the situation!! It does not take long for any situation to go sideways and I know from personal experience that the quick response times to situations like these are crucial, and if you're in the back of an ambulance and your patient is attacking you, it's crucial to try to restrain your patient because there's so many thing that the patient can use to harm you and it's in situations like these where panic buttons come in handy.
Most of these responses in this part of East Vancouver are drug overdoses. Where I work, the issues with homeless people & drug addiction & overdoses is out of control!

@@TGGGlobalEmergencyResponses Yea it is. My EMS instructor told me a similar story that happened to him. A drugged up man in their ambulance fought for 20 minutes straight, and the fire fighters came to help and still couldnt get the dude under control. Eventually he snapped out of the trance but its still scary.
just a regular day at downtown's east side... unfortunately the index of public disturbing due to drug users in this area is increasing each day.

Good to see BCAS in action. Amongst the best EMS services in North America. Still have a little way to go to catch up with some other countries, but they're getting there.

Thanks for the comment! I’m not an expert on how most North American emergency services compare (except for a few in Washington State perhaps), but I’ve always seen BCAS as being quite professional and friendly. Another very professional service I can think of is King County Medic One.
lol our ambulances are becoming privately owned. Plus ours are not free - cost about 2k-5k if you need one

Firstly, BC Ambulance Paramedics were in charge of the call. As they are for all medical calls in BC. Van Fire was assisting them. Not the other way around. Second, there is no such thing as "Fire Medics". They are simply first responders. Just as with any other fire department in BC. Vancouver fire department has chosen to try and promote themselves by using misleading terminology on their vehicles. It's actually written as "Fire/Medic".

Filming VFRS would be so much more awesome if they had Q sirens. However I do understand why they choose not to use them. When you have such a big fleet of trucks (and already have a more than effective electronic siren) you can save on costs for the maintenance of Q sirens and invest the money in more crucial things.
I understand the cost savings. But Montreal Fire makes it a point to have Qs on all their trucks and it's sexy as hell! It's just too bad, I know Toronto and Ottawa don't have Qs either.

So what they were doing to the patient when the two police officers went inside the ambulance with the two back doors open? Wow was the patient arrest at all? Great work I couldn't stand there for that long. But wow it took awhile to settle him/her down.

The police would mainly be there to help restrain the patient and make sure the paramedics are safe. If a person becomes violent it would be the job of the police to control them, before the medics can continue treatment. The paramedic specialist most likely arrived to sedate the patient in oder to make the transport safer. It’s hard to tell if anyone was arrested, but I would say that at least one police officer rode along inside the ambulance, while another officer followed in the patrol car on the way to the hospital.

Because I was there and saw it happen in front of me. And I caught the police escort to the hospital by chance.

Great catches, although, to be fair, attacks on emergency services are NEVER ok. We have a similar sort of problem here in Australia, which is why I’ve always been a proponent of arming paramedics with at LEAST tasers...
Thank you very much! I couldn’t agree more, emergency services deserve to be safe at work like any of us. That is an interesting point of discussion, I can definitely relate to your sentiment there, however I do see how arming medical personell with any sort of ‘weapons’ would be ethically challenging.
000 Responses Yes, attacks on paramedics are on the increase. I've had 2 guns, 3 knives, and a few needles pulled on me (Brisbane) over the years. Lost count of the number of bites and spits.
We administer droperidol to violent patients which has been a great help. There will come a time when tasers will become standard.

It’s a Code 3 siren as far as I know, but does sound very similar to Hong Kong sirens. The VPD yelp tone sounds almost exactly the same as HKFSD ambulances... 😄

Could have been yes, did appear to be a mental health emergency of some sort. However I don’t want to speculate too much about these things, the video is mostly about the vehicles of course.

"Paramedics arrived to assist Firefighters"????????????????? Uh not. Fire departments assist Paramedics. Paramedics are the only service that has the legislated statutory responsibility for responding to medical emergencies in BC. Fire Depts. choose to assist. They are not required to by law.
Um...did you READ the description??? The patient was located meters away from the fire station, so pedestrians alerted firefighters to the patient and they chose to approach him. They were later assisted by paramedics, who still had to respond to the scene. So yes, the paramedics arrived to assist firefighters. I am well aware of all the regular procedures, thanks a lot.
You realize you just sound like some insecure paramedic who is either jealous of Vancouver Fire, or just needs to criticize everything to make yourself feel important. Why the multiple negative comments? Vancouver Fire and BCAS have a great working relationship. Each knows their roles in the different types of calls we do together and we have each other's backs. I am a paramedic in Van and I have never had an issue with them stepping on my toes or interfering with the patient care side of things. They are nothing but helpful when I work with them.
@@foskco87 Not bitter at all. Retired in fact. The last 20 or so years I have never had any issue working with any dept. You are obvious a newbie years ago things were not so friendly. Even the occasional fights.
