- Frozen shoulder, which is technically known as adhesive capsulitis, is a condition that causes stiffness and pain in your shoulder joint from adhesion build up. See Doctor Jo’s blog post about this at: www.askdoctorjo.com/frozen-sho...
For a frozen shoulder, signs and symptoms typically begin gradually, worsen over time and then resolve, usually within one to three years. Stretches and exercises can help speed up the recovery time.
The first stretches are called pendulums. For a frozen shoulder, using a small weight in your hand helps open up the shoulder joint. Use a chair or counter top for balance, and lean over so your arm hangs down towards the ground. Move your body, not your arm in circles so your arm swings around like a pendulum. You can also rock front to back and side to side. Start off with 10 of each and work your way up to a minute of each.
The next set of exercises will be passive range of motion (PROM), but it is sometimes active assisted rang of motion (AAROM) as well. Take a stick of some sort. Broom sticks work well. You can do some of the movement on your own, but really try to let the cane, broom, or stick do most of the movement. You are going to do all the movement with your good arm. The "bad" or injured arm is just going along for the ride!
First, hold the broom stick straight out in front of you with your good side. Place the other hand on the stick. Lift the stick with the good side slowly, and try to relax the other arm so it does not actively move. Take it as high as you comfortably can, and slowly come back down. Then, put your elbow by your side with your hand straight out at a 90 degree angle. Put the hand of the bad side on the end of the stick again. Keeping your elbow at your side, push outward slowly until you feel a stretch, and then bring it back in. Finally, put the hand of the bad side on the end of the broomstick. Slowly push the arm out to the side and up, and slowly come back down. Do all these 10 times.
Now you will do shoulder shrugs. Lift your shoulders up towards the ceiling, but try to keep your head and neck in one spot. Don’t bring your ears down towards your shoulders, lift your shoulders up. Do 10 of these. Now for shoulder circles. You can roll them clockwise, and then counter clockwise. This will stretch out the shoulder and neck muscles. Start with10 each way, and work your way up from there.
The last stretch is a shoulder flexion stretch using the ground for assistance. Get on the ground and sit on your feet in a child’s pose position. If you can’t get on the ground or your knees hurt too much to bend them, you can slide your arm on a table or countertop. You will slide your arm forward with your thumb facing upward towards the ceiling and lean your body forward until you feel a stretch. Hold it for 3-5 seconds, and start off with 10-15.

If you're working through a frozen shoulder right now, I just wanted to tell you: don't despair, it does take a great deal of time but it'll go away eventually. Mine took about two years, but it may have healed quicker if I had done any of these exercises. But I was just in too much pain to do any exercise. But it did heal on its own eventually.

11/24/2023 My Orthopedic sent these exercises to me after diagnosing me with frozen shoulder. I have been doing them for 5 days, and there is a remarkable difference. Pain is so much better and mobility is much improved. Thank you so much for providing this. It is greatly appreciated. ❤

I came across your video 5 years ago when I developed a frozen shoulder. These exercises were way too painful so I didn't do them. Two months later my shoulder froze completely and then it took 6 months of intensive physical therapy to get better. Now I'm developing frozen shoulder for the other shoulder, so I'm back here. These exercises are much needed, sooner or later, when we can tolerate the pain, we got to do them to get better.

I developed frozen shoulder after my arm was immobilized for 5 months following surgery. The physical therapist was directed by the surgeon to work on my elbow, wrist and hand. So I did Doctor Jo's shoulder exercises on my own. After performing these exercises every day for 3 months, I have complete range of motion again and no more pain. They really work!

Just recovering from a humerus fracture after a fall (2 months since the accident). I am using these exercises together with my physio's prescribed ones and I have found them extremely useful. Best frozen shoulder video on UA-cam! I sustained 2 fractures in my arm and the limb was immobilized. After 10 weeks, one of the fractures was angulated and was not healing, so it required internal fixation surgery, followed by more splinting. After this prolonged immobilization I developed adhesive capsulitis. These exercises - performed only to the point of tolerance - are gradually developing increased range of motion of my shoulder. (However I don't use a weight during the pendulum exercise, as I am restricted from load bearing.) I like the lightweight PVC pipe with a bend at the end rather than a heavier wooden dowel. I especially appreciate the instruction to only go as far as the stretch but not pain. Other videos show young healthy fit people taking the stretch to its limit. We frozen shoulder people can only go so far! I am performing these exercises daily until I start seeing an OT. Thanks again!

Adhesions are basically scar tissue. Tissue that is formed from irritation or injury, and binds things together.
Thank you for this!!! I stopped using one arm because of (idiopathic, to me :-) pain, and after a few weeks I had almost no range of motion left. Your exercises have saved me! I'm getting better range every day.

I did a modified version of your first exercise & it released the nerves. I could feel blood flowing again. I have frozen shoulder and a little winging of the scapula.

Thanks for explaining what the "pops" coming from my shoulder are. I'm recovering from GBS and my doctor thinks I might be developing frozen shoulder. I can't raise my hands past my head. I don't think it is frozen shoulder. I think it's just going to take time for me to heal from the GBS I had. But I'm using the stretches and exercises that you have here and they seem to be helping. I'm able to raise my hands up a little higher but it's a slow progress. Just have that popping from my shoulder sometimes that concerned me. Thanks for the helpful video!

Dr Jo, can I ask a question? I have developed shoulder pain on both shoulders at the same place. It's not under the arm, but on top side of the shoulder, at the joint. It is causing pain on movement forwards and back, but not so much to the side. My movement is only restricted by pain, not adhesions, so I push through it as I don't want to get frozen shoulder.
I also have pain across the back of my neck, and the front of my chest (right at the top - not just collarbone but right across). I'm assuming this isn't Frozen Shoulder? Do you know what it could be?
I do have Long Covid and a lot of pain with that and also with fibromyalgia and cervical spondylosis, but this pain doesn't feel like any of those conditions. I thought I'd injured my left shoulder somehow, but now I have the identical pain in the right one! Any advice would be appreciated!!
Great video! The pedulum swing is so hard for some patients to get correctly. They seem to have trouble relaxing the muscles to get that free swinging motion going.

1 - Supraspinatus and subscapualris tendinosis
2- Mild thickening of inferior geno-humeral ligament
3- Minimal fluid in subacrominal - subdeltoid bursae
4- Fluid along the biceps tendon with sutle hyperintensity of tendon, suggestive of tendonitis
5- Mild changes of osteoarthritis in the gleno-bumeral joint
6- Minimal synovial effusion
7- Mild degenerative changes in the acromio-clavieular joint

Thank you for this detailed video, I broke my shoulder (humerus) last year and did physio for 7 months and developed a frozen shoulder and I can't lift my arm higher than shoulder height, I've never actually been shown how to exactly do these exercises. How many days to I need to do this until I can start to move my arm more freely?
You're welcome. You can do these 2-3 times a day. It really depends on how severe your case is. Sometimes people get immediate results, but sometimes it takes awhile longer. Good luck.

Thank you so much for making this video. As the term says "adhesive capsulitis", does it mean my joint is actually stuck (glued on) inside or is there something wrong with my serratus anterior and trapezius? I learned that these two muscles have to work together to move the scapular and it enables the humerus to move up (when we try to raise our arm).
You're welcome. Technically adhesive capsulitis is when the capsule itself is inflamed and irritated causing scarring, or adhesions, that prevent the movement. However, that could be caused from muscles imbalances.

I like those shoulder shrug exercises the best. Anything that is easy! I don't believe in the no pain, no gain axiom anymore.
I'm glad you like them. Yes, no pain, no gain is really only when you've had a surgery, and you have to push through surgery pain, but otherwise you have to listen to your body. Good luck.

It just depends on how severe it is, and if it's really frozen shoulder or something else. I mis-wrote and meant to say 6-12 months. If you aren't in PT, it will take much longer. If you are, sometimes it is much shorter.

I WAS ON PT FOR THE R SHLDR FOR 1 & 1/2 YR, AND I ALSO EXERCISED THE L AT THE SAME TIME. AND I'VE ALSO HAD PT ON THE R NOW FOR A YR. PLUS 12 SESSIONS OF THE MOST POWERFUL LASER. NOTHING SEEMS TO HELP THIS ONE. {DIAGNOSIS WAS BURSITIS, SUPRASPINATUS TENDON IMPINGEMENT & CALCIFICATIONS, HOWEVER, THAT WAS THE SAME DIAGNOSIS FOR THE R SHLDR)
I KNOW EVERYONE IS DIFFERENT AND EVEN OUR SIDES ARE DIFFERENT. IT'S QUITE FRUSTRATING - A TOUGH LESSON ON PATIENCE & PERSEVERANCE.

Thanks Dr. Jo. I’m feeling some encouragement watching your videos about frozen shoulder. Do you know if it usually shows up on an MRI? And how long can it take to break up those adhesions in your opinion/experience (if that’s the issue)? I was doing something in daily life on my other (less injured side) and the next day or two my motion is limited more already). I’m trying to keep moving it vs not moving it even though some of the movements aren’t super comfortable. I’m going to ask my PT today about it more. Most tell me that the shoulder likes movement and to keep moving it even if it’s uncomfortable (but not painful, which can be difficult to differentiate at times with this issue for me). Thanks again for your help and if you have any capacity to answer any of my questions (left 3 comments total in two of your similar videos)
You're welcome. Yes, a true frozen shoulder will show up on an MRI, but sometimes with rotator cuff tears, the symptoms can feel similar. Frozen shoulder/adhesive capsulitis is when the capsule itself gets inflamed and scars up. So it really depends on what all is going on in your shoulder as far as healing time. Hopefully your PT will be able to give you more answers. Good luck!

@@AskDoctorJoafter MRI they said capsule is sprained and started treating for rotator cuff tear. They decided I have secondary frozen shoulder after I am unable to do the lateral movements for a month in therapy. I'm overwhelmed and depressed. Thanks for giving me a little bit of structure & hope.

(Oh I’m a golf professional) PT guy said bicep tendinitis and possibly the beginning of adhesive capsulits (frozen shoulder) tomorrow I meet with an orthopedic doctor (Team doctor for Florida State 🏈who I know really well & trust) see what he recommends.
