  • 👉 The piriformis muscle is a small muscle in the buttocks that helps with hip rotation and pelvic stabilization during movement.
    👉 Tightness and spasms in the piriformis muscle can compress or irritate the sciatic nerve, which runs through or under the muscle in some people.
    👉 This compression or irritation can cause sciatica pain, which can radiate from the lower back down the back of one or both legs.
    The piriformis muscle is a small but crucial muscle located in the buttocks, connecting the sacrum (lower back) to the top of the femur (thigh bone). Its primary function is to help rotate the hip and stabilize the pelvis during movement. Due to its location and function, the piriformis muscle is susceptible to tightness and spasm, which can lead to nerve entrapment and pain.
    When the piriformis muscle becomes tight or spasms, it can compress or irritate the sciatic nerve, which runs through or under the muscle in some people. This compression or irritation can cause sciatica pain, which typically radiates from the lower back down the back of one or both legs. Sciatica pain may also be accompanied by numbness, tingling, or weakness in the affected leg.
    To alleviate piriformis muscle tightness and prevent nerve entrapment, stretching and strengthening exercises can be helpful. Massage therapy and chiropractic adjustments may also be beneficial in reducing muscle tension and relieving pain. It is important to address the root cause of piriformis muscle tightness and nerve entrapment, such as poor posture, overuse, or injury, to prevent future issues.

    I asked my orthopedic doctor about my piriformis muscle being the cause of my sciatica and he said "it's extremely rare," and then proceeded to not do any checks at all that might confirm it or rule it out. They put me on gabapentin, which hasn't done shit for my pain. I told him if this goes on too much longer I'm gonna be in a wheelchair. He said no, you're nowhere near that bad. Oh yeah doc? Have you seen me trying to walk through a grocery store? Do you have to sit down and rest after literally two minutes of activity?

      Get an EMG and lumbar MRI! Tell your doctor your leg is going numb and you need these two tests. If your lower extremity results present as abnormal with radiculopathy and no herniated discs or bulges compressing the nerve then you can confidently go back to your DR and be like dude. Then what else is causing the sciatica? The EMG caught it!
      Don't give any one person too much power over your body... Even if they are the supposed professionals.

      Thts exactly what it is my chiro told me tht i do samatic streches yoga streches and i have a massage gun tht seems to work well along with epsom salt baths .. sometimes if it goes down my leg its bad then ill take a muscle relaxer

    Lying down at night ( sleeping) actually triggers my pain sometimes. I have a knee pillow to allign my hips/spine but it hasn’t helped. I just figured out I have “piriformis syndrome.” I hate that it shoots down to my left ankle 😢

    ok. 50% wrong. I had the most nightmarish sciatic imaginable. Stretching it or anything pissed it off. The one thing I was able to do for brief periods of time was simply air squats. it’s caused by a muscle imbalance which creates either a tight or a loose piriformis. In my case, it was a loose piriformis. you wouldn’t think so considering there was so many knots. The second I’d get one worked out another one would start and spasm. try this. Use a resistance band around your knees and try to force your knees OUT against the band as you do squats and try to get in as many as you can. If it hurts when you squat with good posture. Ignore everything I said. if you notice it hurts a little less roll with it and keep on it. Your glutes need to build strength and the loose piriformis needs to be worked to regain its proper tension.

    There are workouts and stretches you can do for the inflamed piriformis. I've done physical therapy twice for my back and it wasn't until I took matters into my own hands and did research that my pain got better. Check out a couple of UA-cam stretches they worked wonders for me. Also you should probably strengthen your psoas and and ql muscles I take 40 minutes out of my morning for PT and it's been effective. Hope this helps someone Ive been in agony for 2 years and my life changed for the better after applying a couple of exercises to my life.

    Mine is not that, I dont think. I have nerve entrapment/adhesion in the TFL area on the side of my hip, very deep, pressing into the nerve and that can cause sciatica going down my leg and I even have felt it go up. My piriformis is actually, probably lengthened from very tight hip flexors and weak glutes. I can do the piriformis stretch with out any resistance on both sides. For me, it spasms when it's asked to shoulder more work, I believe.

      It sounds like you might benefit from addressing your TFL tightness. Try targeted stretching and foam rolling for the TFL, and strengthen your glutes to help balance hip mechanics. 😊

      How did you find that out?! I've had every test done and still no answers. I had sciatica pain but ALSO numbness in my butt and it actually caused me to not be able to even pee correctly for two weeks. Ended up in the ER with no answers

      @@AG-nn8lp how did I find this all out? Well, to start, I have si joint dysfunction due to injury on my rightside. The joint is hypermobile. I believe the joint is fine and I'm comforted to know it's less likely for people that the joint itself is the problem. No. For me, it's the ligaments which are the culprit.
      I also have psoas illac dysfunction on that right side and i have FAI hip impingement on that right side and i believe that could be related to my injury, which suggests a possible hip labrum tear that I never seriously considered up until recently, as I've been dealing with si for a few years on and off now and my attention has been all si.
      I also have L4-L5 minimal disc bulge and a partially sacalized L5-S1. The disc and the joint are in this seemingly viscous cycle. However, I have confirmed several times now between EMG during diagnostic ultrasound and through shockwave treatment that the nerve entrapment going into my sciatica notch is not coming from the disc. It's coming from my si joint area.
      So knowing where to focus ones efforts is important to understand. What tests have you had done?
      I have to tell you MRIs in my experience have been utterly useless for muscularskeletal issues. they dont pick up on ligamentopathy and tendinopathy, despite what doctors may say. Or nerve entrapments scar tissue or adhesions. For multidisciplinary issues, you need to find people who dont give you the Cookie cutter treatment and that often involves finding people who dont take insurance or maybe out of network at best.
      I dont know where you reside obv but I've seen providers in New York. I went to one guy who started putting me on the very beginning of the right path. He performed diagnostic ultrasound which includes EMG and found all kinds of issues with my whole body and, amongst other things, wanted me to get shockwave.
      So for you. I'd recommend finding someone who will perform diagnostic ultrasound to find all the possible points of ligamentopathy and or tendinopathy and possible nerve entrapments.
      Then I would look for a good shockwave guy. It's also called pressure wave and stem wave. Don't go to a standard PT with a puny shockwave machine that barely tickles. You need the heavy duty kind that will torture you during the treatment. Look for an adhesion provider. The idea is you likely have scar tissue and adhesions causing nerve entrapments and shockwave is one of the best tools to remove it. You need to find someone who understands the body and knows precisely where to place it.
      My guy knows the body like the back of his hand. He places it precisely on particular nerves, ligaments, you name it. Graston technique and manually scraping off adhesions is also helpful. He does that, too. There's another tech similar to shockwave. I think EMTT. They recommended me that, too, but it's more for inflammation and,to my understanding, doesnt penetrate as deep as shockwave and break up adhesions. Shockwave is a regen modality. it rebuilds tissue. EMTT and red laser therapy do this, too, but are not as strong and their efforts are better served for inflammation. With shockwave,you want a response similar to getting a shot. It's not a smoking gun but it has helped me quite a bit.
      Apart of diagnosti ultrasound and shockwave, I'd recommend you look up a MAT/MET specialist and a PRI specialist. Both can serve equally well or compliment one or the other. They can also help isolate the sources of your sciatica. You could give a myofascial release specialist a call, too.
      You need to see people who are experts in their craft and treat the whole body. You can sometimes get lucky finding someone who takes insurance. More often than not, for me, it's been seeking out people who dont take insurance or are out of network to get rolling in the right direction.
      I've gotten better. I'm still not out of the woods. but I know now what are the problems. I can always try and get a PRP or stem cell shot maybe at some point and that should do the trick but I want to fix this permanently and not use a shot to mask my symptoms, as this is my 2nd si joint flare up.
      For the piriformis, I can tell you that, for me, my kryptonite is external rotation followed by extension. That is what aggravates my si joint. Think clamshells off your back with a band. When you're in IR, your piriformis lengthens. In ER, it contracts. And that is where you can get numbness from piriformis/gluteal syndrome. For you, you probably have an issue in your hip that is putting you in ER, or your ER is weak, and or you have scar tissue and adhesions trapping nerves and they need to be released and shockwave is very good for that. Look up an adhesion provider in your area. MAT/MET specialists also may use shockwave. It could also be coming from your back if you're in extension.

    As someone in the medical field people greatly underestimate physical therapy!!! They’ll go a couple times and get frustrated, they won’t do their homework, they don’t want to be active because it hurts, but working through a certain amount of pain which the physical therapist typically should specify to what extent. (FINDING GOOD PT IS ALSO LIKE FINDING A GOOD DOCTOR TOO) Do your hw, be your own advocate, just don’t rely on medicine fully. Your quality of life is not going to just be improved by medication alone. Easy way out and a lot of people take it unfortunately.

      Put a tennis ball on the floor or against the wall, push it Into your glutes trying to get in behind and see if it feels sore, that would be your piriformes muscle. If you squeeze the ball against it, it may hurt a touch but hurt in a good way. If it feels better after this you KNOW it's you're piriformis

    I experience pain when standing, which eases when lying down. Every morning, I wake up feeling fine, but upon standing, pain starts in my lower left leg and discomfort develops in my left hip, making walking difficult. I've noticed that I walk with a stiff gait, similar to an older person. When sitting and lifting my left leg straight, I find it extremely challenging.
    I'm unsure of the cause, will it be gut or something else. The issues began with a sudden locking sensation in my lower back. After taking a week's rest and attending physiotherapy sessions, the pain shifted to my left leg. Its beens months from that.

    Mine gets worse when I lay down. In any position. It’s also pretty bad just from sitting. All I can do is walk around cause standing will hurt after a minute as well. But walking all day long is tiring. I prolly do. A million miles a day. Constantly pacing around the living room too. Can’t sleep for more than thirty mins before I wake up in pain and even then I have to sleep in a bent over position on my knees bent over the ottoman but with my back straight just in the knees position. Hard to explain. But yea this sucks. I have a herniated disk.

    I have solved this myself with an electric pulse device. Doctors thought it was hernia and advised surgery, which I rejected.

    Yeah, usually on the left because that's the side where back side of the pelvis (posterior outlet) typically gets compressed because that side is usually perpetually stuck in swing phase. Stretching it won't really do anything most of the time, Good luck getting any change to stick with a stretch, and could just piss it off. You need to learn how to shift back into that left hip with a left heel, left hamstring, Left ischiocondylar adductor, and contralateral right inferior glute max to push you over to that side so you know how to get into it.

      @@AG-nn8lp oof that's awful!! You probably need to learn how to bring your center of gravity back towards the base of support. You're likely in swing on both sides which will compress the back side of your pelvis where those nerves pass through. Look up a "Wall Supported Reach". That's a great activity to have in your repertoire just in case that sciatica strikes again. Hope you're feeling better!

      Stretch your hip flexors, do back bends ... Sometimes you gotta stretch the quadricep muscles alot sometimes you get adhesion in the muscle which adds pull adds to the tightness

      @@GodsKing69 only if you stretch the wrong muscles and yes if the nerve impingement is bad of the adhesions are bad it can require alot of stretching and sometimes it can feel worse before it feels better... Most people give up if it doesn't feel better immediately, but if you let it go for along time don't expect to do one little stretch and expect it to feel better...

      Typically for the majority of people: stretch your front muscles and strengthen your back muscles; that's why I said do back bends- stretch all front muscles at once.

    My right knee buckles with no pain, no accidents, it started 8 years ago, doctors cant find what it is, the last test was the nerve test and doctor claims is not the nerves. All this time, I still think is my sciatica. 😢

      I recommend that you not sleep on your stomach. It's the worst position for your lumbar back. Try sleeping on your side with a pillow between your legs, works great 👍🏼

    No it's not from overuse it's from the quadriceps and hip flexors pulling too much on the pelvis causing the piriformis to become overstretched generally but hypertonically overstretched because it's trying to stabilize the pelvis due to getting pulled too much by the quadriceps and hip flexors because people sot too damn much and it can be overuse of quad muscles

    Piriformis syndrome is only accountable for 0.4%-6% of all sciatica. So definitely not the most common cause 🤣

    1. Proper Stretching and i lift heavy...
    2. Strengthning back and glutes
    3. Proper nutrition

    Laying down with heat on it seems to soothe it but the moment i move or go to get up its excruciating and i cant stand or walk for long it's hell!!! I'm going through it at the moment its been 3 days now and i don't know how to get rid of it??? I also have S curve scoliosis with the lumber being at 65 degrees so that just makes things worse!!!😩

    Thank you for your video and also all of the constructive comments. I have been suffering from this terribly for 2 months now. In addition to having been diagnosed with Polymyalgia Rheumatica and being on prednisone for that, this additional pain has made life almost unbearable. I am a land Surveyor and need to walk for a living. Thank you again for the comments as I will try some of these suggestions provided.

    A chiropractor ran current through my glutes last night and it seemed to help. I just received a “TENS” machine from Amazon for $59.99 and will try it as soon as the battery charges!.🤞

    It Used To. I've Made It From 3 Herniated Disc. Sciatica and Piriformis. I Trimmed The Belly Got Back In Shape. Now At 42 I'm Feeling Better Than Most Losers.
