So basically, Phoenix invented the concept of extremely spoiled milk in Minecraft. How Phoenix is still not hired as a developer in Mojang is beyond me
Programmer here. When coding a system with lots of combinations of possibilities, it makes sense to handle the individual variables separately and create a system that "just works" when they come together. In this case when generating the death message I assume Minecraft does things like add a ".item" on the end if an item is involved in the death, to get the correct death message that involves an item for that death. So when translating, the translators (and I'm including English in this) will only create strings that make sense for the types of deaths that can normally occur. For example since Minecraft normally doesn't attribute explositions to items there was no need to write a translation for that. So you get the placeholder message instead indicating there was no translation string written for that particular case.

In defense of the Creeper climbing ladders without arms it's not like Steve uses his arms when climbing ladders either. He just kind of stands near a ladder and levitates.
I remember first seeing these death messages long ago with the musket from a old weapons mod, now we can cause this message to appear alot easier now, instead of using command blocks or mods.
Reason why the ladder thing is happening in the start The mobs are attempting to pathfind towards the only other block in the chamber theyre stuck in, and mobs are programmed to always try to climb ladders when within them, so when they reach the top, theyre pathfinding is still set to try and go towards that block in the pit, so they just fall back down
2:48 Mojang should add a warning label for this next April Fools Day similar to how some buckets have a warning not letting babies near the buckets because they can drown in them

And it just so happened that an ability in that sword can blow things up wherever you look, and when I used that ability looking downwards... the death message said that I was blown up by myself with the sword.

Creeps can climb because they climb sideways, so they have a huge grip on the ladder or vines with their feet. Creepers can go up scaffolding because they just lift a foot. 💀

The mobs are attempting to pathfind towards the only other block in the chamber theyre stuck in, and mobs are programmed to always try to climb ladders when within them, so when they reach the top, theyre pathfinding is still set to try and go towards that block in the pit, so they just fall back down
