I think I been in peri since around 35 ish . I'm now 41 and Doc said I'm in peri ,but changes started around 34-35 ish .. My periods still come every month but have gotten shorter ,but every so often they bounce. My cycle went from 28 day to 25 give or take a few.. Anxiety and insomnia suck as well as hot flushing and night sweats and a number of other things .. I'm praying this all goes away soon.. Doc mentioned doing an ablasion ,but I would have to have my tube's tied or whatever and I don't want that .. Plus getting ablasion won't stop all the peri symptoms so ..

For nearly two years, I've had a lot of spotting between periods. Last month, my period didn't "flow" at all and I had nothing but spotting for two weeks. That's the only sign I've had of perimenopause so far, so I'm probably at that early stage at 45.
I am in late perimenopause. I am 52, I started skipping periods for the first time in my life this past Christmas, I only seem to get them during a holiday. LOL I get hot sometimes when I sleep, right in the middle of my back and I wake up at 3am wide awake. Things that fixed that for me were intermittent fasting and low-carb diet. I still would wake up though. Then I saw Dr. Ben Bikman talking about this and he said to make sure you have 2-3 hours of digestion before you go to bed. So I started to eat dinner earlier and that fixed that for me. My main problems that are left are, weight gain around my belly and my hair is thinning on top. Low libido and skin feels dry. Fasting, took care of my brain fog too.
One thing I definitely learnt about perimenopause is that you can't cope in a toxic and an emotional abuse relationship or marriage anymore. I'm leaving my passive aggressive husband because he's making things worse

I’m definitely in late perimenopause! My periods have been irregular since 2020, over four years now. But they’ve gotten further apart in the past 1.5 years. I started on HRT 1.5 years ago also. Estrogen patch and progesterone. Just a month ago I started on testosterone after testing really low and increased my estrogen, because of some anxiety issues,’trouble with labido and metabolism. I am so happy that I am on HRT and I am following you few other fantastic doctors who well educated on perimenopause and menopause. I also exercise regularly, and I eat a very healthy food, high fiber and healthy fat diet. Knock on wood, I have very little hot flashesand I sleep fine
Did you go to your GYN for the medications or functional medicine? My dr just wants to put me on low dose BC pills. I’m almost 48 and in late perimenopause. I can go 6 months with no period. Lately my symptoms are getting worse even on BC pills. I feel so lost and unheard by the doctors. Went to my primary care they told me I basically have to deal until I actually hit menopause 🫤

At 48 I just started noticing more anxiety, less sex drive, hair loss, shorter more painful periods and dry V. But of course all my tests are normal ..till I found a MD who tested my testosterone, T3 and progesterone. Well well those tests came back low.
Judging by what you described, I'm in late peri. I'm 54. Hot flashes and anxiety are my main most bothersome symptoms. But they are lessened in intensity by eating low carb coupled with intermittent fasting. Also I upped my vitamin D3& K2, also a big help is I started taking a magnesium/ potassium supplement. As well as Flax seed oil. Other than just trying to minimize the symptoms, I'm just powering through it. I also keep moving. If I just sit around, my symptoms, especially aches and pains and anxiety get way worse. So I keep moving.
I think I’m in early peri-menopause. I’m 47 and still having regular periods. I have noticed the cramps and bleeding getting worse over last 10 years. My anxiety is increased and I’m hot all the time. Definitely noticing more mood swings as well. In a word: miserable.

And thank you so much for doing this videos, I saw one of your videos about two years ago, but didn’t think I was in the perimenopause stage. But now after these past couple of years, I’m realizing that this is hormonal and not what I’m eating or lack of exercise - because I’m doing both well. Thank you for recognizing hysterectomy. There are few experts that even take that in consideration. When you don't have a uterus and can't take periods in consideration, when in perimenopause or menopause you have to look at all other symptom which is a bigger challange and not as obvious as your periods are.

Think I'm late. I began menstruation at age 11 😒. Last year on my 50th I didn't get my period for aix months. This year beginning in February I got periods until September. It must be a birthday gift from my uterus for forty years of torture🤷🏿♀️. Waiting on my lab tests.

Wow! Never knew when people were referring to menopause, and all the symptoms that come with it, that they were actually describing peri-menopause! Today years old when I learned menopause is one day!
When I was 44 I thought I was going through perimenopause because I was having hot flashes. I skipped my 2020 yearly check up because of Covid and then when I went to my 2021 appointment I found out that I actually had hyperthyroidism. So make sure your doctors are checking your TSH levels, etc!
I have had heavy periods since I turned 40, increased anxiety, brain fog, vaginal dryness, insomnia, hair loss and breakage, a decline in sex drive, and hot flashes. My period has skipped months. The longest I have gone without my period is 4--5 months, then it came back. I have been using vitamin complex and smoothies with berries, vitamins E and D. I am trying the natural route, but it has not been easy. My insomnia is crazy sometimes.
I'm 47 and I haven't had a cycle since July. I had a weird 2 day spotting in August and nothing since. My cycle started changing by the length of days in between in late 30s and early 40s. Then they started being regular again but I was having 2 a month. Then they shortened to 4 day cycles for a few months...then to 2 day cycles. I have to say I'm glad to say goodbye to it. I'd much rather deal with menopause than with pms
We are the same age, and I haven't had a cycle since April.. a couple of years ago my periods started getting really close together and then they started drifting farther apart.. I'll be 48 in a few months. I don't have any other symptoms.
I started cycling HRT two years ago because of night sweats and insomnia. As I am cycling the hormons I get periods regularly. How I am supposed to know when menopause hits? Or would periods stop despite of HRT? I wished you could do a video on HRT and periods.
Very informative, thank you! Pretty sure I’m now in late perimenopause. I’ll be 50 next week. I thought I was actually entering menopause this fall…I hadn’t had a period at all for nearly 10 months, but got one just before thanksgiving. My periods started being irregular in 2022, going a few months without, and then having three or four within a very short space of time. I typically had very light periods for about 3 days, and surprisingly never became heavier. Now all of a sudden since thanksgiving I itch everywhere, I gained about ten pounds in three months, constant back pain, and I swear I have a uti every week. Knock on wood, no hot flashes or night sweats yet, and very slight vaginal dryness, but no decreased sex drive or anxiety/depression. It was a little confusing having all these things happen so quickly, but staying informed and learning what to expect and steps to take to help the transition is keeping me positive.
I've had hypothroidism for 26 years and I am 52 and finally had my first light period ever. So I am going to assume my body is changing. Symptoms of hypothroidism are very similar to the body changes your talking about. So I have no clue when anything actually started for me beside the light period. Maybe this will all be a cake walk for me😂
I will be 49 very soon! I’m transitioning into late perimenopause (nights -very sweaty Betty!, waking up during the night around 3 or 3:30 often, some bouts of insomnia, dry skin,hair and eyes, periods getting farther apart, hair thinning and loss, got a UTI first time since I was 4yrs old. My biggest complaints are the sleep changes, more fat around mid section and fatigue. The low libido doesn’t bother me.

You are exactly right! I started perimenopause around 38/39 and was officially menopausal at 46. I just recently got diagnosed with Hashimotos and believe that my subclinical thyroid issues caused my early menopause. I never really spotting between periods but to be honest, i noticed my periods changed when I was regularly drinking soy lattes, like for a decade. I finally gave it up around 31 but my periods never got normal after that. So long story short, soy is not good!
I have to be in early perimenopause! Sweating at night...waking up in the middle of the night even after taking for ever to fall asleep. Moods up and down, anxiety, etc.
I'm 41 and in early perimenopause. My cycles are getting shorter and shorter. My mood and anxiety levels have changed drsmatically, especially during PMS. Sometimes I don't even recognise myself. My doctor has just prescribed me progestin-only pill. I've been off the pill for almost ten years, but I'm happy to start it again. I can't stand all this suffering anymore. I hope it helps to conceal my symptoms.
What are the sign or symptoms you experienced cause I’m 40 now and I don’t know what is going on with my body. Having spotting b4 period, mood swings, tiredness, breast tenderness that is annoying and anxiety. I don’t want to be on any drugs.
@@sunshinelouis6391 first of all, my cicles became shorter (23-24 days, having two periods in a month) and my PMS symptoms became more intense. Then, it was like I was always in PMS. Symptoms, most psychological, lasted all month and tended to become much worse after the second week of my cicle. I was irritable, depressed, lacking energy (especially in the morning), felt a pressure on my forehead, having difficulty to concentrate and would cry about very nonsense things... I felt funny, as if I didn't recognise myself anymore. When my period was about to come all those symptoms reached such an intensity that I thought I was going to lose my mind. My doctor said I needed antidepressants, but I knew it wasn't the case. I found another doctor who is a specialist in perimenopause/menopause. I've been taking progeterone-only pill for a month to conceal perimenopause symptoms and I feel fantastic. My doctor said she will prescribe me estrogen as HRT as soon as I start having hot flushes.
@@sunshinelouis6391 At first I was very reluctant about starting the pill, as I'd always been against drugs. I've always been like that type of person with a 'super natural' lifestyle... I practise yoga, don't eat processed foods, don't drink alcohol, I'm against botox and fillers and don't even dye my hair. But I gave in and now I think the pill was a great decision and I certainly will go for HRT when the time comes.
I think I am transitioning into late perimenopause. For a number of years I’ve had shorter cycles. Then for a few years my periods have been extremely heavy. Like every hour or two requiring ultra tampons with a back up pad. I had 13 day cycle in June. In August I got my period and it lasted 16-18 days with those last days being light like spotting but it just wouldn’t end. Now here I am on day 42 and no period yet.
So how did you cope going out and daily living? Did you get blood clots? I am just getting my first heavy period lasting over 10 days so far and i am so exhausted worried and can’t go out. Any advice please?

I went 69 days with no periods now I’ve had it twice, back to the heavy craziness, the clots etc. I discovered a couple of years ago that Tampax makes an ultra tampon. I thought it just went up to super plus. Sadly even with that I can need to be in the bathroom every hour to two with a back up pad. So just yesterday I was in the ladies room every 1 to two hours at work using an ultra and wearing a pad for backup. I would say try the ultras if you have not already and a back up pad so you can at least live your life a bit. I will say that we went to a football game last night at my daughters school and I was able to go and get through but I wasn’t sure I was going to be able to with no bathroom access . Ugh. Due to this I also take an iron supplement during my period as I am anemic due to all of this. I too feel so worn down and exhausted. I think the iron supplementation helps with feeling so drained, I also try to make sleep a priority and give myself a break when it gets crazy and know that some self care and rest is necessary. Good luck, it’s such a pain.
I’m 51 and I think I’m in early peri-menopause. I still get regular periods but have noticed them coming slightly more frequently than my previous 28 days to 26 days and they are lasting longer. I’m actually going to get a mirena in to stop my periods, because it is annoying me so much. I since other symptoms like dry skin, acne (never had this before), brain fog, hair thinning. My mother went though late 50’s, so I’m probably like her.
I’m 47 and started with symptoms at 45. Anxiety hit me really hard and I had never experienced anxiety the way I am now. My period are irregular they short. They last only three days and at times I get them twice a month. So I’m not sure if I have high or low estrogen. I cry a lot because I get so stress over anything. 😢
I’m 48, and a year ago, I was inundated with symptoms of late perimenopause without significant changes in my period. However, now I have changes in my periods (bleeding & spotting) without noticeable symptoms! 😂 I went on vag ring birth control just to keep things more predictable, so now I have no period and no idea! 😂
I think I'm on perimenopause. I'm not 100% sure because I have an IUD so I'm not getting periods. The last two months I've been experiencing brain fog and difficulty sleeping. I think my Gynecologist has providers that are trained to consult with patients that are experiencing perimenopause and menopause. I messaged him to see if I can get tested. I just turned 44 in late August so I think I'm right on time. Ha ha my mom who is full Korean (I'm half Korean and half Caucasian) had an easy menopause shift but I don't have an aunt on my dad's side that I can ask about her menopause since my dad was an only child. So I'll just have to see how my lab tests turn out and see if I can get on a HRT. Well, since I have an IUD I would only need estrogen. If I can get an appt I'm going to ask which is better, a transdermal patch or topical estrogen. I think I'll Google that. Thanks for the video!

Thank you for this video! I’ve been trying to figure out where I am on the journey for a while. I’m 55 and think I’m in late menopause but my periods have stopped for over a year TWICE 🤷🏼♀️ I had abulation in 2008 which stopped my periods for about 8 years. They gradually began again but I have so much scarring that my gynaecologist couldn’t take a proper biopsy when I started having issues. My periods stopped for 18 months then I had perhaps 3 light periods. Then I had nothing for 2-3 years and suddenly the periods started again but they were getting heavier and more painful each time. My gp reduced my oestrogen dose and gradually the severe breast pain has gone away and so far I haven’t had any bleeding for around a month 🤞🏼🤞🏼🤞🏼
I wish the doctors I saw when I started a lot of these symptoms had listened to me. I did some research because I started having really bad anxiety, lots of painful cramping when I didn’t have any before etc. they kept telling me I wasn’t showing any signs of perimenopause. I also told them I think I have endometriosis and again they said I didn’t show signs. I ended up having to have a hysterectomy because I was physically getting very sick. I still have issues. 😢
In early Peri apparently but benefitting from using two Evamist sprays daily, otherwise so hot at night and numb arms, difficulty concentrating at work during the day. If I am cycling 100mg progesterone, Dr Hirsch, will it still be apparent when late peri starts?
Hi Dr Hirsch, is there any evidence that transdernal estrogen would help with teariness? I've had that symptom throughout perimenopause but seems to be more evident as time goes on. I've been on the pill but will likely be switching to Estrogen patch and progesterone. Thanks for your thoughts on this!
What does it mean if I only have 1 day of my period? First I had it irregularly, then it was quite normal, but it has always been greatly influenced by stress, then for many years I had one period in a year, and now I have 1 day every month. Does it all indicate perimenopause? My hormones in the luteal phase are: FSH=4.10mlU/ml LH=11.1mlU/ml estriadol=205pg/ml progesterone=20.10ng/ml
I am 45 with diagnosed PCOS since I was 13 years old. I used to have very spaced out periods. My first period was when I was 8, then I had another period at 12, then one more at 15, then one at 18. At 21 I started menstruating more frequently, but bleeding for three or four months consecutively. After that, I went through different treatments and had a more “regular” period, but it was about seven or eight years ago when I had a cysterectomy that my periods have become more regular with 30/35 day cycles. Then, when I turned forty I started to have 28 day cycles and for the first time experienced horrible mood swings. I kept having these regular periods until 2022, that my cycle got wierd and had a 13 day cycle, then went back to 30 day cycles. Then this year I have had two 55 ish day cycles, and this last one was a 19 day cycle. I am now on my period and have an appointment with my gynecologyst in two weeks. I have never had a normal period except from 2019 to 2022 so I don’t really know if I am on perimenopause or not… probably yes, but better be sure with the doctor
estrogen & insulin. 40 years old started hot flash during ovulation and week before period at 39. Severe brain fog (I thought I had early onset Alzheimer’s) starting at age 37. Estradiol gel sachet during ovulation and PMS week covered by California medicaid aka Medi-Cal.
I might be a bizarre case because I still get my periods even though they are closer and closer together, but I have the absolute worst night sweats, especially a few days before my period starts. So I can’t really tell whether I am earlier or late!😅

I think I'm in late peri. I don't have symptoms because I am on hormone therapy, but my fsh was extremely high without estradiol. But my periods never got heavier or closer together. I went right to skipping periods 6 months at a time. It was very quick and I didn't feel ready for it.
I'm 50 get migrain when starting period, get period every 2 1/2 to 3 weeks normal cycle not heavy not light, but every cycle I get so extreme fatigue I can't work, just sleep, feel weak ,lasts at least 2 days any thoughts, waiting for my blood work,

Is there any truth to perimenopause starting earlier for those who have never had a pregnancy? i'm 43, no kids, I think i've been in peri for at least a few years and recently began going into later peri due to the night sweats and dry skin. Got approved for hrt patches while my Mirena is still in use. Not sure what I'll do when it's time to take out the IUD. Can i keep using them just for logcal progesterone?

I turned 47 in June 2024 and that same month and July I had irregular periods, started late. This August, I never even started my period at all and I am still waiting for my September period. I have never wanted a period so bad. 🤷🏻♀️ Also in August the back of my head hair started falling out in chunks, all of my beautiful waist long hairs so now I have 3 sparse areas in the back of my low back hair, it looks horrible. In Early 2024, the back of my head hair texture changed to curly too, it’s the weirdest thing. In September 2024 I got my first night sweats and hot flashes. WoWser! No one can prepare you for that. I feel like this all happened over night, like my body is betraying me. Like someone pulled the rug from under my feet and I fell and hit my head. It’s very sad. From hearing this information I “thought” I was early perimenopause in 06/2024 but once I got the night sweats and hot flashes in September means late perimenopause. Sigh….
I went through that since 35 years old, it felt horrible and I thought I'm going crazy, because all lab results were ok. I started taking care of myself better, rest more, but hormonal therapy helped the most

I love your videos. I am having some issues with pressure and discomfort of the abdominal sides. I was check by and Euro gynecologist and everything was fine. I had ultrasound and the saw a Urologist and everything was fine. I started using estradiol for a while and some abdominal exercise and I was fine for about a year. Now I am having the same issue, and I saw my gynecololist and everything is fine but she wants me to have another ultrasound, and mct scan and see a GI. I am so tired of this discomfort and going constantly to urinate all the time. Help....MINDY My gynecologist says it could be a hernia.....but with the ultrasound it shows all good
I am Early PM I think. I’m 50 1/3 yrs old and my period comes every month without skipping. I get very mild hot flashes during my period every other day & goes away fast. I don’t have any other symptoms but I wish my periods were over and done because i’m iron deficiency anemic and is not fun how weak I feel at the end of the period. Now I need iron infusions to correct my low ferritin and iron absorption !
I believe that I am at the end of peri-menopause and was told at my last annual GP well visit that I was pre-diabetic based on my A1-C numbers. But after watching this video I'm wondering if that could that be a function of the low estrogen / insulin relationship. I would love to learn more about the relationship between estrogen and insulin because I do not want to be treated for diabetes if what I really need is HRT.
I’m 49 and have been on continuous oral birth control for about 18 months to manage PMDD and perimenopause symptoms. Aside from breakthrough bleeding, I don’t get a period. Since I’m on oral contraceptives, it seems like getting my hormones lab tested wouldn’t be an accurate way to determine how close I may be to menopause. Other than tracking the symptoms your mentioned, what would be the best way to figure out when I’ve hit menopause?

I’m not a doc (I’ve just read a lot) but I think you have a window of time after menopause to be eligible for HRT if you wait too long they won’t give it to you, (except local vaginal oestrogen which you can take no matter how long ago you started menopause)
Can you be in perimenopause when you are breastfeeding? Also my sister was saying you don’t go into menopause unless you have released all your eggs???? I’ve had 7 kids my youngest 6 months old, oldest 20 years old. Hi Dr Hirsch, I’m 55 years old in late perimenopause now but my period skipped only this month despite being on estradiol, progesterone and testosterone. I had to lower my estradiol bc it was too high and now my period skipped for the first time.
I’ve always been every 21 days since the day I got my period at 13. But I always had my period for 7 days until I had my daughter at 21 and then it changed to 5 days every 21 days. I am 46 now going on 47 in Jan2025. I am just starting to feel, this month my period is longer and heavier and I don’t like it. I’ve never spotted between period until this month. I started spotting before my period came and I’ve been bleeding for 9-10 days and clotting. 😒😒😒
I swear I am in backwards perimenopause: cycle became suddenly regular but short bleed but HEAVY at 37. Then tinnitus, vertigo, frequent UTI, no libido. Last four years: so heavy needed IUD, palpitations, hair loss, muscle loss, rage, irritability, apathy, shell of a person. Then in Jan had endometriosis flare up, needed surgery, lost both tubes and one ovary. Since then, periods every 13 days, months with panicking anxiety attacks in middle of the night, severe PMDD, feeling like i cant do this anymore, constant bloated, massive weight gain (40 lbs) daily menstrual cramps. Been on a mirena for 1.5 years ans 1 pump estrogel for over 1 year. Doc refuses testosterone. Urgh I am wondering if i need to add oral micronized progesterone and take the iud OUT!
I think I'm in early because my periods are 2 wks on 2 - w wks off. I was given micronized progesterone which helped a little in terms of not as heavy but I gained weight so not sure if the trade off was worth it.

Not really, since you continue to make your body "think " to be pregnant as long as you are on the pill. After quitting it you will be likely to experience various symptons like my mum did at the age of 52.

I think I been in peri since around 35 ish . I'm now 41 and Doc said I'm in peri ,but changes started around 34-35 ish ..
My periods still come every month but have gotten shorter ,but every so often they bounce. My cycle went from 28 day to 25 give or take a few..
Anxiety and insomnia suck as well as hot flushing and night sweats and a number of other things ..
I'm praying this all goes away soon..
Doc mentioned doing an ablasion ,but I would have to have my tube's tied or whatever and I don't want that .. Plus getting ablasion won't stop all the peri symptoms so ..

For nearly two years, I've had a lot of spotting between periods. Last month, my period didn't "flow" at all and I had nothing but spotting for two weeks. That's the only sign I've had of perimenopause so far, so I'm probably at that early stage at 45.

I am in late perimenopause. I am 52, I started skipping periods for the first time in my life this past Christmas, I only seem to get them during a holiday. LOL I get hot sometimes when I sleep, right in the middle of my back and I wake up at 3am wide awake. Things that fixed that for me were intermittent fasting and low-carb diet. I still would wake up though. Then I saw Dr. Ben Bikman talking about this and he said to make sure you have 2-3 hours of digestion before you go to bed. So I started to eat dinner earlier and that fixed that for me. My main problems that are left are, weight gain around my belly and my hair is thinning on top. Low libido and skin feels dry. Fasting, took care of my brain fog too.
One thing I definitely learnt about perimenopause is that you can't cope in a toxic and an emotional abuse relationship or marriage anymore. I'm leaving my passive aggressive husband because he's making things worse

I’m definitely in late perimenopause! My periods have been irregular since 2020, over four years now. But they’ve gotten further apart in the past 1.5 years. I started on HRT 1.5 years ago also. Estrogen patch and progesterone. Just a month ago I started on testosterone after testing really low and increased my estrogen, because of some anxiety issues,’trouble with labido and metabolism. I am so happy that I am on HRT and I am following you few other fantastic doctors who well educated on perimenopause and menopause. I also exercise regularly, and I eat a very healthy food, high fiber and healthy fat diet. Knock on wood, I have very little hot flashesand I sleep fine
Did you go to your GYN for the medications or functional medicine? My dr just wants to put me on low dose BC pills. I’m almost 48 and in late perimenopause. I can go 6 months with no period. Lately my symptoms are getting worse even on BC pills. I feel so lost and unheard by the doctors. Went to my primary care they told me I basically have to deal until I actually hit menopause 🫤

At 48 I just started noticing more anxiety, less sex drive, hair loss, shorter more painful periods and dry V. But of course all my tests are normal ..till I found a MD who tested my testosterone, T3 and progesterone. Well well those tests came back low.
Judging by what you described, I'm in late peri. I'm 54. Hot flashes and anxiety are my main most bothersome symptoms. But they are lessened in intensity by eating low carb coupled with intermittent fasting. Also I upped my vitamin D3& K2, also a big help is I started taking a magnesium/ potassium supplement. As well as Flax seed oil. Other than just trying to minimize the symptoms, I'm just powering through it. I also keep moving. If I just sit around, my symptoms, especially aches and pains and anxiety get way worse. So I keep moving.

Thank you for recognizing hysterectomy. There are few experts that even take that in consideration. When you don't have a uterus and can't take periods in consideration, when in perimenopause or menopause you have to look at all other symptom which is a bigger challange and not as obvious as your periods are.

When I was 44 I thought I was going through perimenopause because I was having hot flashes. I skipped my 2020 yearly check up because of Covid and then when I went to my 2021 appointment I found out that I actually had hyperthyroidism.
So make sure your doctors are checking your TSH levels, etc!
I have had heavy periods since I turned 40, increased anxiety, brain fog, vaginal dryness, insomnia, hair loss and breakage, a decline in sex drive, and hot flashes. My period has skipped months. The longest I have gone without my period is 4--5 months, then it came back. I have been using vitamin complex and smoothies with berries, vitamins E and D. I am trying the natural route, but it has not been easy. My insomnia is crazy sometimes.

I'm 47 and I haven't had a cycle since July. I had a weird 2 day spotting in August and nothing since.
My cycle started changing by the length of days in between in late 30s and early 40s. Then they started being regular again but I was having 2 a month. Then they shortened to 4 day cycles for a few months...then to 2 day cycles.
I have to say I'm glad to say goodbye to it. I'd much rather deal with menopause than with pms

I started cycling HRT two years ago because of night sweats and insomnia. As I am cycling the hormons I get periods regularly. How I am supposed to know when menopause hits? Or would periods stop despite of HRT? I wished you could do a video on HRT and periods.
Very informative, thank you! Pretty sure I’m now in late perimenopause. I’ll be 50 next week. I thought I was actually entering menopause this fall…I hadn’t had a period at all for nearly 10 months, but got one just before thanksgiving. My periods started being irregular in 2022, going a few months without, and then having three or four within a very short space of time. I typically had very light periods for about 3 days, and surprisingly never became heavier. Now all of a sudden since thanksgiving I itch everywhere, I gained about ten pounds in three months, constant back pain, and I swear I have a uti every week. Knock on wood, no hot flashes or night sweats yet, and very slight vaginal dryness, but no decreased sex drive or anxiety/depression. It was a little confusing having all these things happen so quickly, but staying informed and learning what to expect and steps to take to help the transition is keeping me positive.
I've had hypothroidism for 26 years and I am 52 and finally had my first light period ever. So I am going to assume my body is changing. Symptoms of hypothroidism are very similar to the body changes your talking about. So I have no clue when anything actually started for me beside the light period.
Maybe this will all be a cake walk for me😂
I will be 49 very soon! I’m transitioning into late perimenopause (nights -very sweaty Betty!, waking up during the night around 3 or 3:30 often, some bouts of insomnia, dry skin,hair and eyes, periods getting farther apart, hair thinning and loss, got a UTI first time since I was 4yrs old. My biggest complaints are the sleep changes, more fat around mid section and fatigue. The low libido doesn’t bother me.

You are exactly right! I started perimenopause around 38/39 and was officially menopausal at 46. I just recently got diagnosed with Hashimotos and believe that my subclinical thyroid issues caused my early menopause. I never really spotting between periods but to be honest, i noticed my periods changed when I was regularly drinking soy lattes, like for a decade. I finally gave it up around 31 but my periods never got normal after that. So long story short, soy is not good!

I'm 48, partial hysterectomy, and early perimenopause because I just started having the occasional night sweat this past year.
I'm 41 and in early perimenopause. My cycles are getting shorter and shorter. My mood and anxiety levels have changed drsmatically, especially during PMS. Sometimes I don't even recognise myself. My doctor has just prescribed me progestin-only pill. I've been off the pill for almost ten years, but I'm happy to start it again. I can't stand all this suffering anymore. I hope it helps to conceal my symptoms.
Progesterone pill is a game changer I felt so angry and irritable before taking it and now I'm alot calmer

@@sunshinelouis6391 first of all, my cicles became shorter (23-24 days, having two periods in a month) and my PMS symptoms became more intense. Then, it was like I was always in PMS. Symptoms, most psychological, lasted all month and tended to become much worse after the second week of my cicle. I was irritable, depressed, lacking energy (especially in the morning), felt a pressure on my forehead, having difficulty to concentrate and would cry about very nonsense things... I felt funny, as if I didn't recognise myself anymore. When my period was about to come all those symptoms reached such an intensity that I thought I was going to lose my mind. My doctor said I needed antidepressants, but I knew it wasn't the case. I found another doctor who is a specialist in perimenopause/menopause. I've been taking progeterone-only pill for a month to conceal perimenopause symptoms and I feel fantastic. My doctor said she will prescribe me estrogen as HRT as soon as I start having hot flushes.
@@sunshinelouis6391 At first I was very reluctant about starting the pill, as I'd always been against drugs. I've always been like that type of person with a 'super natural' lifestyle... I practise yoga, don't eat processed foods, don't drink alcohol, I'm against botox and fillers and don't even dye my hair. But I gave in and now I think the pill was a great decision and I certainly will go for HRT when the time comes.
I think I am transitioning into late perimenopause. For a number of years I’ve had shorter cycles. Then for a few years my periods have been extremely heavy. Like every hour or two requiring ultra tampons with a back up pad. I had 13 day cycle in June. In August I got my period and it lasted 16-18 days with those last days being light like spotting but it just wouldn’t end. Now here I am on day 42 and no period yet.
So how did you cope going out and daily living? Did you get blood clots? I am just getting my first heavy period lasting over 10 days so far and i am so exhausted worried and can’t go out. Any advice please?

I went 69 days with no periods now I’ve had it twice, back to the heavy craziness, the clots etc. I discovered a couple of years ago that Tampax makes an ultra tampon. I thought it just went up to super plus. Sadly even with that I can need to be in the bathroom every hour to two with a back up pad. So just yesterday I was in the ladies room every 1 to two hours at work using an ultra and wearing a pad for backup. I would say try the ultras if you have not already and a back up pad so you can at least live your life a bit. I will say that we went to a football game last night at my daughters school and I was able to go and get through but I wasn’t sure I was going to be able to with no bathroom access . Ugh. Due to this I also take an iron supplement during my period as I am anemic due to all of this. I too feel so worn down and exhausted. I think the iron supplementation helps with feeling so drained, I also try to make sleep a priority and give myself a break when it gets crazy and know that some self care and rest is necessary. Good luck, it’s such a pain.

I’m 51 and I think I’m in early peri-menopause. I still get regular periods but have noticed them coming slightly more frequently than my previous 28 days to 26 days and they are lasting longer. I’m actually going to get a mirena in to stop my periods, because it is annoying me so much.
I since other symptoms like dry skin, acne (never had this before), brain fog, hair thinning.
My mother went though late 50’s, so I’m probably like her.
I’m 47 and started with symptoms at 45. Anxiety hit me really hard and I had never experienced anxiety the way I am now. My period are irregular they short. They last only three days and at times I get them twice a month. So I’m not sure if I have high or low estrogen. I cry a lot because I get so stress over anything. 😢

I think I'm on perimenopause. I'm not 100% sure because I have an IUD so I'm not getting periods. The last two months I've been experiencing brain fog and difficulty sleeping. I think my Gynecologist has providers that are trained to consult with patients that are experiencing perimenopause and menopause. I messaged him to see if I can get tested. I just turned 44 in late August so I think I'm right on time. Ha ha my mom who is full Korean (I'm half Korean and half Caucasian) had an easy menopause shift but I don't have an aunt on my dad's side that I can ask about her menopause since my dad was an only child. So I'll just have to see how my lab tests turn out and see if I can get on a HRT. Well, since I have an IUD I would only need estrogen. If I can get an appt I'm going to ask which is better, a transdermal patch or topical estrogen. I think I'll Google that. Thanks for the video!

Thank you for this video! I’ve been trying to figure out where I am on the journey for a while. I’m 55 and think I’m in late menopause but my periods have stopped for over a year TWICE 🤷🏼♀️ I had abulation in 2008 which stopped my periods for about 8 years. They gradually began again but I have so much scarring that my gynaecologist couldn’t take a proper biopsy when I started having issues. My periods stopped for 18 months then I had perhaps 3 light periods. Then I had nothing for 2-3 years and suddenly the periods started again but they were getting heavier and more painful each time. My gp reduced my oestrogen dose and gradually the severe breast pain has gone away and so far I haven’t had any bleeding for around a month 🤞🏼🤞🏼🤞🏼

I wish the doctors I saw when I started a lot of these symptoms had listened to me. I did some research because I started having really bad anxiety, lots of painful cramping when I didn’t have any before etc. they kept telling me I wasn’t showing any signs of perimenopause. I also told them I think I have endometriosis and again they said I didn’t show signs. I ended up having to have a hysterectomy because I was physically getting very sick. I still have issues. 😢
In early Peri apparently but benefitting from using two Evamist sprays daily, otherwise so hot at night and numb arms, difficulty concentrating at work during the day. If I am cycling 100mg progesterone, Dr Hirsch, will it still be apparent when late peri starts?
Hi Dr Hirsch, is there any evidence that transdernal estrogen would help with teariness? I've had that symptom throughout perimenopause but seems to be more evident as time goes on. What does it mean if I only have 1 day of my period? First I had it irregularly, then it was quite normal, but it has always been greatly influenced by stress, then for many years I had one period in a year, and now I have 1 day every month. Does it all indicate perimenopause?

I am 45 with diagnosed PCOS since I was 13 years old. I used to have very spaced out periods. My first period was when I was 8, then I had another period at 12, then one more at 15, then one at 18. At 21 I started menstruating more frequently, but bleeding for three or four months consecutively. After that, I went through different treatments and had a more “regular” period, but it was about seven or eight years ago when I had a cysterectomy that my periods have become more regular with 30/35 day cycles. Then, when I turned forty I started to have 28 day cycles and for the first time experienced horrible mood swings. I kept having these regular periods until 2022, that my cycle got wierd and had a 13 day cycle, then went back to 30 day cycles. Then this year I have had two 55 ish day cycles, and this last one was a 19 day cycle. I am now on my period and have an appointment with my gynecologyst in two weeks. I have never had a normal period except from 2019 to 2022 so I don’t really know if I am on perimenopause or not… probably yes, but better be sure with the doctor
estrogen & insulin. 40 years old started hot flash during ovulation and week before period at 39. Severe brain fog (I thought I had early onset Alzheimer’s) starting at age 37. Estradiol gel sachet during ovulation and PMS week covered by California medicaid aka Medi-Cal.
I might be a bizarre case because I still get my periods even though they are closer and closer together, but I have the absolute worst night sweats, especially a few days before my period starts. So I can’t really tell whether I am earlier or late!😅

I think I'm in late peri. I don't have symptoms because I am on hormone therapy, but my fsh was extremely high without estradiol. But my periods never got heavier or closer together. I went right to skipping periods 6 months at a time. It was very quick and I didn't feel ready for it.
I'm 50 get migrain when starting period, get period every 2 1/2 to 3 weeks normal cycle not heavy not light, but every cycle I get so extreme fatigue I can't work, just sleep, feel weak ,lasts at least 2 days any thoughts, waiting for my blood work,

Im 44.5 and I think Im in early perimenopause. Poor sleep, anxiety. More period irregularity with 2 heavy periods this month plus spotting. Considering going on birth control but not sure.
You are the first doc I’ve heard mention an ablation. So I’ve wondered how the heck will I know when I am in it.
Is there any truth to perimenopause starting earlier for those who have never had a pregnancy? i'm 43, no kids, I think i've been in peri for at least a few years and recently began going into later peri due to the night sweats and dry skin. Got approved for hrt patches while my Mirena is still in use. Not sure what I'll do when it's time to take out the IUD. Can i keep using them just for logcal progesterone?
At 58 I haven't had a cycle since I had the lining burned off at 40. Did I go into early menopause? I don't have any symptoms my friends my age are experiencing. In loving kindness from Canada's Capital ❤🙏🥰
I turned 47 in June 2024 and that same month and July I had irregular periods, started late.
This August, I never even started my period at all and I am still waiting for my September period. I have never wanted a period so bad. 🤷🏻♀️
Also in August the back of my head hair started falling out in chunks, all of my beautiful waist long hairs so now I have 3 sparse areas in the back of my low back hair, it looks horrible.
In Early 2024, the back of my head hair texture changed to curly too, it’s the weirdest thing.
In September 2024 I got my first night sweats and hot flashes. WoWser! No one can prepare you for that. I feel like this all happened over night, like my body is betraying me. Like someone pulled the rug from under my feet and I fell and hit my head. It’s very sad.
From hearing this information I “thought” I was early perimenopause in 06/2024 but once I got the night sweats and hot flashes in September means late perimenopause. Sigh….
I went through that since 35 years old, it felt horrible and I thought I'm going crazy, because all lab results were ok. I started taking care of myself better, rest more, but hormonal therapy helped the most

I love your videos. I am having some issues with pressure and discomfort of the abdominal sides. I was check by and Euro gynecologist and everything was fine. I had ultrasound and the saw a Urologist and everything was fine. I started using estradiol for a while and some abdominal exercise and I was fine for about a year. Now I am having the same issue, and I saw my gynecololist and everything is fine but she wants me to have another ultrasound, and mct scan and see a GI. I am so tired of this discomfort and going constantly to urinate all the time. Help....MINDY
My gynecologist says it could be a hernia.....but with the ultrasound it shows all good

I am Early PM I think. I’m 50 1/3 yrs old and my period comes every month without skipping. I get very mild hot flashes during my period every other day & goes away fast. I don’t have any other symptoms but I wish my periods were over and done because i’m iron deficiency anemic and is not fun how weak I feel at the end of the period. Now I need iron infusions to correct my low ferritin and iron absorption !

I believe that I am at the end of peri-menopause and was told at my last annual GP well visit that I was pre-diabetic based on my A1-C numbers. But after watching this video I'm wondering if that could that be a function of the low estrogen / insulin relationship. I would love to learn more about the relationship between estrogen and insulin because I do not want to be treated for diabetes if what I really need is HRT.
I’m 49 and have been on continuous oral birth control for about 18 months to manage PMDD and perimenopause symptoms. Aside from breakthrough bleeding, I don’t get a period. Since I’m on oral contraceptives, it seems like getting my hormones lab tested wouldn’t be an accurate way to determine how close I may be to menopause. Other than tracking the symptoms your mentioned, what would be the best way to figure out when I’ve hit menopause?
I’m 49 had my 2nd Child at 41 all natural don’t know if this has got to do with it but this year my periods have been very light and skipping not regular anymore heart palpitations are the worst was in hospital as it was once over 200pb I’ve cut back on coffee drink lots of water walk take magnesium everyday I feel like I have an extra heart beat what else can I do as it’s making my life stressful & tiring?
I'm 45, and I started having hot flashes and sweating at night last year. Fast forward to today, this is the first period I've actually missed for 2 weeks. I also have endometriosis but still having ghost period pain. Not really sure what's happening.

I am nearly 45 and almost certain I'm early perimenopausal. I take the pill but when I go on a break to have my period, it never comes and hasn't come in over a year!! I'm extremely worried because I KNOW from much reading up on the subject that I want to go on HRT but am really confused about how to go about getting it here in Southern Spain. I'm from freetown sierra leone, I stopped to see my mestral period from may2024 up to now, age 42 with children, when I'm not ,I think also pregnant ,I did HCG test is negative ,I really doubt, Question Is possible to take medication to see mestral period again as for I want to pregnant

I am 60… never taking HRT…. I am having shortness of breath and heart arrhythmia and anxiety and someone suggested I might need HRT…. So confused… could this help?
I’m not a doc (I’ve just read a lot) but I think you have a window of time after menopause to be eligible for HRT if you wait too long they won’t give it to you, (except local vaginal oestrogen which you can take no matter how long ago you started menopause)

Can you be in perimenopause when you are breastfeeding? Also my sister was saying you don’t go into menopause unless you have released all your eggs???? I’ve had 7 kids my youngest 6 months old, oldest 20 years old. Hi Dr Hirsch, I’m 55 years old in late perimenopause now but my period skipped only this month despite being on estradiol, progesterone and testosterone. I had to lower my estradiol bc it was too high and now my period skipped for the first time.
I’ve always been every 21 days since the day I got my period at 13. But I always had my period for 7 days until I had my daughter at 21 and then it changed to 5 days every 21 days. I am 46 now going on 47 in Jan2025. I am just starting to feel, this month my period is longer and heavier and I don’t like it. I’ve never spotted between period until this month. I started spotting before my period came and I’ve been bleeding for 9-10 days and clotting. 😒😒😒
I swear I am in backwards perimenopause: cycle became suddenly regular but short bleed but HEAVY at 37. Then tinnitus, vertigo, frequent UTI, no libido. Last four years: so heavy needed IUD, palpitations, hair loss, muscle loss, rage, irritability, apathy, shell of a person. Then in Jan had endometriosis flare up, needed surgery, lost both tubes and one ovary. Since then, periods every 13 days, months with panicking anxiety attacks in middle of the night, severe PMDD, feeling like i cant do this anymore, constant bloated, massive weight gain (40 lbs) daily menstrual cramps. Been on a mirena for 1.5 years ans 1 pump estrogel for over 1 year. Doc refuses testosterone. Urgh I am wondering if i need to add oral micronized progesterone and take the iud OUT!
I think I'm in early because my periods are 2 wks on 2 - w wks off. I was given micronized progesterone which helped a little in terms of not as heavy but I gained weight so not sure if the trade off was worth it.
I am 54 had lots of heavy periods even seen period twice in a month ,now spotting now always tied insomnia at the high now

I’m 43 also on the pill no periods because of the mini pill. I guess in my case I can tell because I get hot flashes now .. but before that I never knew
Can you have symptoms while on birth control pills?
Not really, since you continue to make your body "think " to be pregnant as long as you are on the pill. After quitting it you will be likely to experience various symptons like my mum did at the age of 52.
