Vitamin B12 Deficiency Symptoms (ex. Depression), Why symptoms happen, Schilling’s test, Treatment

  • Lesson on Vitamin B12 Deficiency Symptoms including Depression, Fatigue and Irreversible neurological issues (ex. proprioception, paresthesia) along with why these symptoms happen (pathogenesis), how to diagnose Vitamin B12 deficiency (ex. Schilling’s test), and how to treat this condition. Vitamin B12 (cobalamin) is obtained from our diet and is required for production and health of red blood cells and health and function of the central nervous system. Vitamin B12 deficiency can be due to reduced dietary intake, and a variety of processes and conditions that compromise absorption. Vitamin B12 deficiency leads to particular reversible and irreversible neurological symptoms along with symptoms of anemia (due to megaloblastic, macrocytic anemia). The diagnosis of B12 involves laboratory investigations along with the Schilling’s Test. Treatment depends on whether B12 deficiency is due to nutritional or issues with absorption.

    Early recognition and treatment of cobalamin deficiency are crucial, because in the absence of adequate repletion, affected individuals can develop severe, life-altering complications, such as bone marrow failure and irreversible neurologic impairment. High-dose oral treatment is generally as effective as parenteral administration, even in patients with malabsorption, although in patients with severe neurologic manifestations, initial parenteral repletion is prudent.

      @@rabeehbabu3613 probably not as I took a medicine that robbed me of b12 and I nearly died. Breathing problems, falling, etc. I had to figure it out myself thanks to the Internet but I’m not OK.

    Question..... can anyone explain or give clarity on the following ... does any change to typical Purine & Pyrimidine, folate, B6 or B12 pathways during pre or early post natal periods result in heritable genetics that could be passed from that baby should they procreate in later life?

      Find a sublingual. I read they work as well as shots. Since they’re large potency (1000mcg) you probably only have to take once a week.

    I was very bad. I'm 6'1 and was 130 pounds basically skin and bones. I have memory problems was weak etc. Now the memory issues have gotten a bit better but I can't form imagines in my head and find it nearly impossible to remember days, weeks, months etc. Any tips?
    PS I'm now 150ish so my weight did get better and the numbness in my foot went away I forgot to mention this, I'm also worried I may have wet brain but this might be anxiety.

    I was tested for B12. My levers were 96. Severe. I have had these symptoms for years and it's lead to further problems.

    I have been diagnosed with severe vitamine B12 decency. It gets a little better then bad again. Sometime I am totally drained, memory problems, head pressure, oily nose, long winded, itching, cold and anxiety and depression. I eat healthy, but it's just that my body can't absorb enough of it. I was told by a doctor that I should be getting the B12 injections. I am going back to doc this week cause I am barely able to get out of bed and food is not working.

      Question have you had bloodwork that has come back normal. Mines is normal but I feel like I need a second opinion because my symptoms mimic vitamin b12 deficiency and everything else was ruled out

      Every family physician can make a blood test to check your vitamin B12 levels. If you believe you have a deficiency you should just go check it.

    So for about 3 months I thought I had vertigo and I needed to see a ear doctor then he told me I needed to see a neurologist, now we found out I have low b12 I have all these symptoms headaches, loss of balance, pins and needles on feet and hands, stuff is not fun, after the shot I started feeling a bit better but I hope whoever has this has some relief

      I had similar a few years ago and had to have a private blood test to diagnose low B12. GP refused to entertain the idea. Get injections now which have sorted it. Good to hear the docs sorted you out and tested for B12.

      @@karthikcinematime7879 mine was 73pmol/L total B12 which was classed as severely deficient. Lower limit is 145pmol/L. My active B12 was 9pmol/L. The low limit is 37.5pmol/L. Again, severely deficient.

    Hey guys, for the past few months Ive had a lot of these symptoms; and they usually fluctuate on a daily basis. At first I thought it was multiple-sclerosis, but then I had all neurological exams done and all of the tests were clear (MRI, eye tests, EMG). The symptoms havent stopped, but I started drinking a vitamin b mixture, and now I hope to see some improvements.

    This is pretty good but it must be quite old because schillings test is no longer available and also you would want to look at homocystine and MMA (urine test) because with low folate you will have low B12 in turn because they work together in unison as methylation .
    Not all symptoms appear at same time so maybe just have few or a lot like headaches that feel crushing or head swollen,sore mouth and geographic tongue ,glossitis ,gait balance ,mood ,depression,Brian fog , dizziness ,fatigue agressive behavior, Schizophrenia ,bipolar,drop foot ,buzzing ,shock ,numbness, tingling,sleep issues,bladder and bowel issues ,tinnitus and plenty more .

      @@glenncooper4379 get tested first B12 ,folate ,ferritin ,vitamin D ,homocysteine and MMA as taking B vitamins will skew results and need this done first .
      If have high level still may have a functional B12 deficiency.
      We need to be in optimal range /high to feel well (this is without supplements as means nothing once taken as maybe raised quite high but still not absorbing and you just raised serum level Which is sadly not a standard gold standard test.

    Not quite sure I get the point on the Schilling's test. If I am not absorbing it, why is it not coming out in the urine? Is it circulating in the blood and if so, for how long? Is this why your blood levels may look ok if you are taking supplements and the cells are still not getting access to it? Then what happens to the MMA? Will it not be elevated due to the B12 in the blood..? Thank you for a good video!

      So the way i understand it (if you still want a answer) is that, first they saturate the B12 in your body with the injection and then gives you a oral B12, then what they will check is if your body absorbed the oral B12 or not. If your body absorbs the B12 you'll find B12 in your urine, if your body for some reason did not absorb the B12 there'll be no B12 in the urine. So this way the doctor Will know if your body is properly absorbing it or not

    I have B12 deficiency horribly , very weak, no energy , I went to see my doctor and I was told to get B12 shots weekly. I did often then I stopped because it can be pricey during COVID. What would you think is the best way to get B12 , would it be the shot or would it be the vitamins? Thanks!

      I’m so sorry you have this. I also am prone to b12 deficiency and the best treatment for me has been Methylcobalamin sublingual tablets.

      Was your B12 test an active B12 or just a B12? A regular B12 test shows how much you have in your body, however an active B12 test shows how much of it is actually going into your bloodstream to your organs. Get the active B12 test instead. Or it could be your thyroid or vit D.

      I wish doctors knew more about vitamin B12. They really don't know much at all. At least that was my experience with over 30 doctors. It was a nightmare. Are you having any symptoms?

    B12 deficient, but have too much iron... the opposite of anemia. Ongoing gastro issues. Started b12 shots this week. Dizzy, blurred vision, foot numbness/ burning.

      @@MarcWilliams So happy to hear you’re having improved health. I too experienced a big improvement. Dizzy spells are gone. My blurred vision is still ongoing. My joint pain has it’s good days and less good days, but the less than good are more bearable. Going for more tests on gastro next week.

    Very interesting subject
    In 2020 I was one of the unlucky ones to get Covid 19
    And have had a 3 year battle get back to my regular self along this journey l got a point
    when I slowly became little better but there seems like a lot of similar symptoms such tired legs slow walking hopefully I’ll look into this subject a little more regarding the absorption of b12

    Even low vitamin D is bad too. I never thought. I’m going through it. My spine and hips have extreme pain + muscles cramps and aches. I’m losing clumps of hair. My bones aren’t supporting me which makes my organs slouch together and muscle weakness doesn’t help with supporting them either. I’m trying hard to get sun and drink milk. My doctor had to give me Rx because of how bad. If I don’t get this straightened out My bones will permanently deteriorate and break easily. I’m only in my 20s. I thought it was only old ppl problems.

    Is it normal to feel better within minutes of taking a B12 dissolvable film? I had horrible brain fog and confusion for weeks but sharpened up right after taking it. Or is this a placebo?

      That's not unheard of. Vitamin B12 affects me quickly too. Usually within an hour or two. How bad was your deficiency?

      In the European Union a value under 500 pg/ml is considered deficient. Its quite possible to experience anxiety from the deficiency as it affects your nervous system.

      European guidelines for doctors recommend 450-500 as minimal level for the health of nervous system. But more than 920 is thought too high

    I noticed I had some of the symptoms.
    Vibrating feeling, weakness, poor eye sight, tremors. I took 2 tablets of multivitamins with b12. Im feeling better within 2 hrs.

    My vitamin b12 is at 383 and it’s suppose to be at 800 , and I’m very scared I have tingles all thru my body and I just feel terrible n I’m so scared

      Usually, it is a symptom of Peripheral neuropathy - Try B-Complex - usually B1 and B12 deficiency. Avoid B12 Cyanocobalamin (synthetic) - Methylcobalamin is good. Understand that B1 deficiency can imitate a host of other illnesses. It's that dangerous. B-complex try not to exceed 25 mg of each. A high dosage of B6 also could trigger peripheral neuropathy. So stay within that 25mg limit for all B's except B12 (500-1000 mcg). In addition, take R-Alpha Lipoic Acid - 100mg, which seems to delay or reverse peripheral diabetic neuropathy; (keep in mind, R-alpha - more bio-available); and Zinc Picolinate 50 mg/day; Magnesium Glycinate -200-400 mg/day.

You should check with your doctor (functional medicine) and diagnose. It could be a sign of a nutrient-deficient diet. You might be calcium deficient (hypocalcemia) Vitamin B5 (pantothenic acid) deficiency / Magnesium deficiency (hypomagnesemia)/ Vitamin B12 deficiency, which can lead to ANEMIA, a cause of paresthesia / Vitamin E deficiency / Unlike Magnesium and B12, you must get your calcium from the diet like dairy foods, cheese, milk, spinach, fortified flour, fish where you eat the bones - such as sardines and pilchards. And Vitamin E from (nuts and seeds, wheat germ, which is found in cereals and cereal products, sunflower oil, olive oil, etc..).
      Any vitamin E your body does not need immediately is stored for future use, so you do not need it in your diet every day.
      Magnesium Glycinate 200 to 400 mg/day / Vitamin B-complex / Extra Thiamin (B-1) - best form is Benfotiamine / Vitamin B12 - 250 to 1000 mcg/day -I said, mcg and not Mg

      @@BobsClique i feel dizziness since last year’s december. It’s not spinning sensation but more like feeling unstable. Sometimes i have loud tinnitus and a little hyperacusis which lead me to feel anxious and nervous. I was later diagnosed with otitis media by ent specialist in december, not too severe. Given 2 round of antibiotics, but only three of the symptoms(ear drainage, pulsatile tinnitus and ear pain) are gone. Am still feeling dizzy till now after a month. It got worse when I lay down to sleep or sit and gone temporarily when i’m on a moving car. Betahistine isn’t helping at all. Specialist suggested me to consult with physiotherapist. Am so worried cause it affects my study a lot.

    I had a b12 deficiency low folate left untreated was told I had chronic fatigue, 15 years sick I went deaf tinnitus brain fog lost the ability to walk properly, then in 2018 had a cerebral stroke due to the b12 deficiency left I had high homocysteine levels .
    I have 12 injections due to malabsorption issues now , I take folic acid magnesium D3 k2 and I inject every other day on prescription to repair nerve to my legs -
    I joined the wake up b12 group on Facebook they have helped me as my doctor didn’t understand the b12 danger left and they do not do the full tests to diagnose Malabsorption issues .

      I was misdiagnosed in 2009. Due to neurological symptoms as MS, but i was severely deficient in D3 and B12. Of course, i was young and didn't know to read the blood tests, and doctors neglected all that and treated me just with the corticosteroids, so now i can bearly walk 5m without help. I am afraid i will fall into a wheelchair.

    Just found out my vit b12 results this morning. 227 pg/mL, very low. My chart says 211 - 911 is standard, but of course my result is on the lower end.
    For about a month now, I’ve been having episodes of sudden confusion, disorientation, limb weakness/heaviness/numbness. Stroke runs in my family, so I don’t mess around. Neurologist ordered MRI, carotid doppler ultrasound, etc. Everything is normal. b12 and d are low (vit d is just 8! 8!!!).
    During a follow up appt. today I received my first ever b12 shot. Doc said we will do a series to get where I need to be. This whole thing made me realize I need to pay better attention to my health. I’m too young (32) to be having this kind of trouble.
    Been having distorted vision as well, it waxes and wanes in intensity, which has also been checked out. Normal. So something is messing with me. I’ll try to check in again later to update. :) I appreciate all of you sharing your stories, thought I would share mine too ❤️

      That's unheard of. But it can certainly cause constipation, diarrhea, gas etc.
      These often mimic the symptoms of ibs but if none of the treatment has helped. B12 could be the reason.

    Depression is not a symptom of vitamin B12 deficiency. It slows suffers down. That slow down is biological and not mental. The confusion is due to a decrease in the levels of oxygen in the blood. It's not depression or mental illness. It is a medical condition. For years suffers were actually misdiagnosed with depression before discovery of the biological cause. This error was acceptable because women are a large portion of suffers. Also those from non western heritage suffer more from anemia. Being tired is not a mental illness. Actions, of lack there of due to being tired is not depression. It took a physician from North Africa to even diagnose my pernicious anemia (B12 deficiency). Western physicians disputed whether I have asthma or not. I have both. Both render a shortness of breath. He told me I was born with it. He gave me specific symptoms that have been life long. I was shocked especially regarding my left hip. Not once did he mention depression.
    The true issue is the once a month B12 shot. It's only good for two to three weeks.

    great, but I cannot eat any animal products, as I am totally aware of how human beings treat animals. Since i have been a vegan for decades (and healthy), can it be that the Intrinsic Factor synthesis is reduced in me (in all vegans) just because we do not trigger its synthesis?
    I sometimes use patches. Sometimes. And is it possible that we produce our B12 thanks to our intestinal bacteria, like cows do?

      I have the same thing in left foot as well, I took chlorella and it worked. Chlorella has like 600% of drv of b12 ... but eventually I started having gastro intestinal problems from the chlorella and had to stop, so maybe it works for you or someone else.

    Update. I found out that I was iron deficient. I am still taking supplements to treat the deficiency. Here were my list of symptoms and what I was experiencing
    Itchy face
    Strange sensation on nose
    Nose producing a lot of oil on the outer skin
    Severe breathlessness
    Feverish type feeling in the inside of body but no high temperature
    Fast and slow heart rate
    Pressure headaches and pressure type feeling inside of body
    Shaking uncontrollably
    Jaw chattering uncontrollably
    All symptoms have subsided since taking supplements

      @pardeep singh Yes no problem. I am still taking Galfer 305 mg capsules. I tend to just break the capsule open and take the iron that is inside the capsule. The reason I do this is so that I can take it with a high vitamin C drink to absorb the iron straight away. Very important to take vitamin C with the Iron. I take it on an empty stomach in the morning so that I can absorb it properly and then wait a few hours before I eat anything. I also take oxford vitality 34 mg tablets. I take one of these on top of one 305 mg open capsule just for extra iron. I started off taking Spatone liquid form iron. You should be able to get these at your chemist. If not then get them on amazon. I could only get the Galfer capsules at the chemist and not amazon and these were the ones that helped the most due to the high strength. The doctor can prescribe them or just ask for them behind the counter. Yes I was confused as well. It is very confusing. It took me a long time to figure out what was wrong with me. It can take about 6 months for your iron to be back to normal but as soon as you start taking the supplements you will notice the difference. Maybe start off taking a low dosage of iron and work your way up

      @pardeep singh 18 mg might not be enough for your iron supplements. Maybe get a higher dosage. For b 12 maybe get some high strength from the chemist. Maybe try 1000mcg of methyl cobalamin tablets. Hope this helps!

      @pardeep singh Okay. Update. Be careful not to exceed your iron levels and be careful with the dosage. Iron is toxic and can cause issues with your stomach. Probably best not to crush up the tablets also. I have learnt from experience. Slow releases is better I think. Probably best to bring your iron levels up gradually and slowly with the right foods rather than supplements.

      Animal products have b12 mostly because we supplement them before eating them. Cut the middleman, get a b12 supplement yourself, theyre not expensive.
      Animal foods are, in general, bad for your health when compared with whole plant-based foods. Also, really bad for the environment (and full of cruelty in their production, but a lot of people just dont respond well to this argument, and i can see why)

    I was good at my work before but started forgetting stuff I didn't took my blood test for a long time then when I decide to take it the results were pretty good overall but I found out that I had a 67 level of vitamin B12 doctor is now giving me injection every week. My required level is 400

    3 months ago my b12 level is 184. After i taking b12 supplement and now my b12 level is high 1200. So high b12 level can affect to my body ? I feel sometimes my body burn.

      @sanju b bro let me tell you something, we dont need b12 injections ! We already get b12 by eating meat etc. Injections are good if u really need this BUT side effecs still coming wich sucks. And it makes me also anxiety even after 3 days. Sry for my bad english. I hope you doing well.

      @sanju b b12 abhi or 1 month tak le sakte ho or d3 60k iu ho to vo sirf 8,9 weeks tak hi lena phir kidney stones ho sakti hai. Or ha pani jyada pite rehna so toxicity nai hoga or body k liye bhi achha rahega or b12 1,2 months tak bhi le sakte ho mene 3 months tak liya tha or abhi kuch time le leta hun per week 1 supplement

      @sanju b mera b12 level 184 tha or 3 months tak 1500mcg ka supplement liya tha mere ko leg me jalan hoti thi weakness rehti thi or kuch time vision ka bhi problem hota tha or headache or tongue ulcer bar bar hote rehte the ab tongue ulcer nai ho rahe but kuch time weakness or headache ho jata hai is liye me per week 1500 mcg supplement le raha hun 1 month k liye or d3 11.4 tha tab bone pain ho raha tha pure sarir pain karta tha ab 38 hua hai but abhi bhi kuch time joint pain se kat kat aavaj aati hai or mera weight bhi loss hua hai 4kg,5kg or ab badh nai raha or mere ko depression bhi rehta hai kuch time is liye comedy movie ,videos dekh leta hun . Ab 80% improve hua hai.

      @sanju b but aapka 228 hai so deficiency to nai hai but thoda low hai because 211 to 911 tak rehta hai level so aap apne dr. K bataye hui dose hi lena supplement.

      because that's not the only factor. most doctors recommended eating animals for b12 but nowadays farmed animals get their b12 from supplements

      It can be your ability to absorb B12 into your system. I am no expert so please research it and seek advice from your doctor. I believe that it may have something to do with the stomach, acid reflux, amongst other things.

    From what i understand,the good bacteria in your gut produces b vitamins, you also need to have strong stomach acid to absorb b12. Iv been sick for years with so many symptoms liw iron,b12,vitamin D found out h pylori lowers the stomach acid cant absorb nutrient its a nightmare

      @@batanguenalivinginamerica7322 just drink rich in probiotics and alkaline. Do not eat foods that has high preservatives. All we need is detox and cleansing, so drink 1 glass of water every hour. Vitamin B12 takes years to heal and it was not totally healed because this kind of illness is triggered by stress and we cannot avoid stress just learn to ignore your illness and continue to do the right thing, like you eat 3 times a day on time, sleep at least 8 to 9 hours, exercise everyday. Eat healthy foods, avoid drinks that rich in caffeine because it causes heart palpitations. Above all, pray earnestly because God is our great healer. I am in my 13th year of being deficient in B12. All glory and praises to Him for He has been always good to me even I am not.

    I'm raw vegan so I'm totally void of any b 12. My health is fine and nails don't break now that I'm vegan. When we fast and pray we get wisdom from Jesus Christ. Many people get better health when they go vegan so we should strive to avoid b 12 as much as we can
