    hi doctor,will conventional prp suffice for the caudal epidural?and how soon can we detect the improvement in back paon symptoms?since its not immediate relief compared to steroid

      There are studies using regular PRP injection in the epidural space for back pain, sciatica and radiculopathy. We use lysate in our epidural injections. It may take 3-6 weeks to notice changes after a PRP epidural injection. Check out our spine page:

    I have disc bulge in L4 L5 S1 touching nerve root. My symptoms are lower back pain radiating to leg and standing unstability. I feel unstable while standing though no pain.. What can suit me also how much it will cost

      PRP lysate epidural can help with sciatica and pain from bulging discs by calming down the inflammation around the nerves and discs. It also stimulates a healing response.       There are multiple papers published in the medical literature showing that PRP lysate epidural is a wonderful treatment for back pain, sciatica, radiculopathy and disc herniation and it is a great alternative to steroid injections. Here are a few links, hope this helps:

      PRP lysate epidural can be effective in treating nerve pain from inflamed and "pinched" nerves such as sciatica, radiculopathy and spinal stenosis. It will not eliminate bone spurs, but bone spurs are often not the major contributors to back pain and are often a part of a larger spinal issue such as spinal arthritis, disc degeneration, spinal segmental instability and weakness.     Thank you, Dr...I have L5 S1 for 9 months and I suffer from pain and difficulty moving. I do not have problems with footfall or urination. I use Tildene. My question is, is it possible to recover permanently through injections, or is injection a temporary treatment? Will injections spare me from surgery? I will be happy to answer my question.

      @@sebastianklisiewiczintegra9805ctor I have the same thing herniated disc L5, S1 but I also have sciatic pain and numbness down my left leg and foot. I have drop foot as well. I had about 4 epidural injections, & back surgery about 17 months ago and it didn’t change anything. I’m considering this procedure very soon. What are you thoughts on it helping the numbness go away so I can get back to my normal activities?

      Caudal epidural injection is typically a safe and effective for sciatica and "pinched nerves" in the back. It is also effective low back pain treatment. If the diagnosis is not right, an epidural may not work as you may need to treat the joints, ligaments and muscles of the spine. See this: ua-cam.com/video/pvDHCaS_9wA/v-deo.html
      Also, it may not work if the injection does not adequately spread towards the nerve root, and one may need a transforaminal epidural injection. . It may also take several injections for best results.

      PRP lysate epidural for sciatica (radiculopathy and back pain is safe as only the growth factors are injected into the epidural space. The plateles are broken up to release the growth and anti-inflammatory factors these are injected through a filter. This does not promote thrombosis (platelet clotting), thus making it safe and effective alternative for epidural injections.

    Dr Klisiewicz, how do you chose the type of agent PRP or PL? I have bulging L4L5 and L5S1 I have spine instability, load intolerance, I can't sit and L5S1 is compressing nerve 1mm. It's been 10mo of this.
    Do you go by MRI, Dematomes or physical examination to select the type of treatment ?

      For best treatment of spinal instability and bulging discs, you need to treat the supporting ligaments, joints, muscles and fascia in the lumbar spine (L3-4-5-S1) and the SI/pelvic ligaments. This is done with PRP and prolotherapy. To treat the bulging discs inside and the spinal nerves, you use platelet lysate via an epidural. The treatment is based on physical exam, symptoms and MRI findings all put together. You may need 2-3 treatments for best results.
