- Welcome to another episode of, "Ask Dr. Schulman!" Today's question is, Can I use my own fat instead of getting implants? Watch this video to see what Board Certified Plastic Surgeon, Dr. Matthew Schulman, says about using your own fat to make your breasts larger. In some cases, this can be a good alternative to implants, but everyone is different and this may not be the best option for you.
As you know, we commonly take fat from areas you don't want it and put in area you do, like your butt or your hips. So why can't we use it in your breasts? There are some very specific situations where that will work and give you the results that you want. But I'll talk about that in this video, so watch to see a complete explanation on where and when this procedure might make sense for you.

The biologic survival of autologous fat tissue depends upon the correct handling of the fat graft, of its careful washing (refinement) to remove extraneous blood cells, and of the controlled, blunt-cannula injection (emplacement) of the refined fat-tissue grafts to an adequately vascularized recipient site. Because the body resorbs some of the injected fat grafts (volume loss), compensative over-filling aids in obtaining a satisfactory breast outcome for the patient; thus the transplantation of large-volume fat grafts greater than required, because only 25-50 percent of the fat graft survives at 1-year post-transplantation.

You didn't mention that its fine for someone who doesn't like the implant look , and already has natural breast roundness, but just wants them to be a cup size fuller. That's what I need.

This really helped thank you! I lost 120 pounds and need to have my breasts lifted and most plastic surgeons made me feel like i needed an implant to be happy with my results. I think theyre just trying the sell me on the easier option. I have an autoimmune disorder that can also negatively impact my experience with an implant. Now that i know this is an option, i am going to pursue this kind of augmentation after my breast life 😱 thank you again!
i think when the breast is big and it went saggy they can just remove some skin under to make it round and full. Rather than put more maybe it's too big, medium is better.

Hi Dr. Schulman. Have you performed Fat Transfer to the breast procedures? If so, have you had patients come back for complications such as hard lumps in the breasts?
Would getting fat transfer for asymmetry still be optimal when done in addition to a breast lift?

I read about your technique using Sculptra to camouflage ripples. Are you still doing it and would you do it to camouflage cc? I'm not going to have another surgery but I would love to camouflage it. The contour irregularities only show in the cleavage, the rest is fine.

I want to do the fat grafting since my breast skin is so thin, I don’t think my skin can handle the weight of implants. I nursed 4 babies so my boobs have been through the ringer! I’m a very deflated small B cup. If I get a lift, can you add the fat instead of implants and still look natural?

I'm curious if you can use lab-grown silk (commonly known as artificial spider silk) in this method, and whether or not it would be temporary or more permanent? Cuz I know the proteins can Breakdown over time but depending on the medium, how its synthesized, and maybe some other bio compatible material would make it be a strong long-lasting option. When you think of silk its fibrous, but lab-grown silk separates all that and normally it's a liquid until it hits oxygen and then starts to gel/cure if it's put through a strainer it becomes thread but if it's just left in a big tub it becomes this gelatinous mass; and it actually reminds me of what your natural breast look like, how they're not completely rounded pieces of fat. I know it's going to sound weird but it kind of reminds me of cauliflower the way it branches off and has this thick bushel up clumps.
Hi doc so I have 34dd but its had loose skin due to the volume I had of being a g cup when I was pregnant I just want to make them Fuller so the loose skin will go away... Will the fat transfer be sufficient?

Would it ever be considered to have fat transfer and subsequent breast lift? I have had 6 babies and was always small to medium B cup but with breast feeding grew to DD cup. I am a healthy weight, somewhat thin but now quite deflated in the breasts. I don't want implants and if I just had a breast lift I don't think there would be enough breast fat to give me any more than maybe an A cup size. I just wonder about the fat transfer prior to a breast lift? I would imagine that it could not be done at the same time and probably a few months would need to be between the procedures.
L Your story is very similar to mine. The only difference is I have had seven children. I struggle with the temptation of getting my breast done all the time but honestly after a ton of research I come to the conclusion that it’s not worth it. bigger breast don’t mean more attractive or more feminine. Sometimes it’s hard but I encourage you to Focus on the attractive parts of your body and be content with the look of your breasts.
So basically what if you had big breast an the surgeon cut you to small an you just wanted to be a full D cup would a fat transfer be a good idea with a breast lift?

So I am a transgender who had their breasts done about 10 years ago. Before, I had my testicular sac distended and expanded to prep for saline implants in 2004 and had 2 600cc implants added to my testicular sac after I had both testicles removed. Shortly after I had silicone breast implantsdone. Since both are due for an update, I'm thinking of going silicone all the way, but it's an issue for the sac implants. I've heard of intraanal testicular implantation. An incision in made in the anus that leads directly unto the sac, and can be stretched( and my anus is already very large) to allow silicone implants. I'd like yo to get to 900cc if possible. Is this something your office offers?

i know im hellaaaa late to the party but my question is that if i were to only want my breasts evened out (currently the left is about a D and the right is half to 3 quarters of the size) would the graph be right for me? i just want a natural look just more even you have no clue what it is like to put on a shirt and be able to tell it is so embarrassing

I had breast cancer whereas they removed both breast we tried to do the implant but we didn’t make it that far. The doctor started with the expanders and I ended up having an allergic reaction to where as my skin tissue started to die. And they had to go back in and remove the expanders also remove all of the dead skin which caused my breast to cave-in. So do you think the fat grafting would be a good thing for me so that I may have breasts?
omg.am so sorry that u had to go through such horrible ideal; thank God u survived it tho.i think using the fat wld be a gd idea but after a while as u get older ur breast will shag; but don't take my word for it seek out ur options !hope it all works out for u.nd always make Dr visits just to be safe.ik that u may already knw this.but sometimes pple don't check on there status after surviving cancer...nd ends up having it come bck nd shit gets worse.i will keep u in my preyers .ALL the best
If you put fats around the edging of the silicone implant to hide the rough edges, would not the fat cells die since there is no blood supply? Thanks.
no, that fat wouldn't die because it isn't really put on the implant. It is still put in the breast tissue along the edge of the implant.But that is a good question.

Are you able to breast feed normally after getting a fat transfer? And does this ever interfere with breast cancer detection on mammograms or does it make it harder to find even benign or malignant masses?
The good thing about using your fat is that your body wont reject it. It wouldnt make sense for it to interfere with breastfeeding especially since your using a natural substance from your own body.
Neither getting implants or fat transfer will interfere with breast feeding. And you should be getting implants under the muscle so neither that nor fat transfer will interfere with mammograms

Will fat transfer work for asymmetry with the larger side being unevenly distributed (tuberous)? The doctor I previously consulted with wanted to do an implant in one side and a lift to shape the larger side. I would prefer to avoid an implant.

fat transfer alone is not great for tuberous breasts. I recommend an implant reconstruction and then sometimes we add a small amount of fat at the same time to improve the results.

If you lose weight following the surgery, can the breast return to its original size? And if the person gains weight again, will they gain in the breast too?
Yep, whatever fat survives will act as a normal fat cell, and it’ll grow and shrink with you

Fat transfers will technically last forever....but changes in weight can change the results. If you gain weight, it may get bigger, and if you loose weight, it may get smaller. It changes with your body

Great question. It's because fat likes to be around other fat. This is why it does well in the butt and not the breast. The breast is made up of breast tissue, which is not fat!

Thank you for your comments. This is an informational video about using fat versus implants for breast augmentation, and not designed to be an "advertisement". We appreciate your concern. The FDA put out an advisory regarding a rare risk of a specific type of cancer with very specific types and brands of implants, none of which are shown in this video.
He’s a plastic surgeon. It’s his job. Worry about the people that are getting illegal injections even though they know it’s fatal.
Only one rarely used subset of implants is associated with increased cancer risk. Smooth rounds, by far the most common type of implants are not associated with this risk.

I really want to get this because even the most natural breast implants feel fake to me (not dogging anyone who gets implants, I just want a dif look) but I'm like 125lb with a all muffin top so i don't even know if I'd have enough fat to transfer 😅

Here's a thought, maybe some women want to restore some fullness after children or middle age or weight loss. People aren't always looking for fake looking breasts. Most regular people just want completely naturally full looking breasts that are age appropriate. Duh my guy.
