Thank you Dr. Guts Sehmi for providing all the hard to find answers to anxiety producing questions! You have provided great mental relief to us who experience trauma in our contemplation of unanswered questions. Your openness, clarity and compassion are deeply respected and appreciated. I spent an hour in consultation and read a book published by an expert in implant surgery and was left with many questions, all of which you have addressed completely and elegantly in your brief video. Thank you a thousand times.
Before watching your videos, I would have requested the All-on-4 without considering that there might be a better option. I even joined an All-on-4 facebook group because I actually thought it was the best option. But your videos have opened my eyes and I thank you for that. This was an excellent video. I was one if the original followers who asked how do you properly clean an implant/all on four. Thank you for your expertise. I can imagine the technique/application of the dentist makes all the difference.
Oh my goodness…….you just answered several questions I had about cleaning your implants!! One question I have that no one has been able to answer is : “ When they put my dentures they made me, and and in about 12 weeks hook them to my 8 implants and they said I will have to wear them for 4-6 weeks, how will I clean those acrylic dentures without them cracking or breaking till I get my permanent ones made. Also, they put caps on my 8 implants at the time of surgery and in order for them to hook anything to my implants, they said they will have to do another small surgery and cut a slit on each side where the implants are, so they can take the caps off so they can hook the hybrid piece that holds the teeth and screw it all into place.” Have you seen this done before or did something similar to it. I would be so Grateful for a reply back. Take Care……5 Star plus! Take Care, Blessings to you my friend…. Debbie USA.
I have all on 4 made in Europe and the doctors took my bridge entirely off once half a year for cleaning . I can't find anyone in USA to want to do it. And I myself kind of afraid to damage the gum. And I saw what was under the bridge and don't think flossing can do such a good job as the doctor. Thank you for your input.
The problem is the doctor should of made the fake gum SERFICE CONVEX ( need to fill in) ,inbetwen the inplants they just leave it the way they get it food get trapped in causes infection I just seen a video sorry but that's just not a wery good work .
Thank you for an extremely thorough guide of how to floss around implants, I had been worried about really getting around the neck of the abutment but I see now it is necessary.
Terrific video and information. One common question or concern is how to find a reputable dentist to do the All on 4 denture procedure. How would you recommend people decide on a provider?
Dr Gurs. You are so informative! Wish I knew about you before I started my process of all on 4s I’m on my first temp teeth. I’m scared. I’m entering into the unknown. Not good with technology. Dr Sehmi do you perhaps have pictures/videos/illustrations of facial differences with full mouth dentures vs full arch all on 4 implants of patients with severe bone loss. Thx.
That would be SUPER helpful. I'd really like to know. Plus the consequences of doing nothing to a jawline where upper and lower molars are all missing....
Hugely helpful videos thank you Dr Gurs. So many pros and cons. Biggest thing I've learnt generally. Since I stopped eating sugar a decade ago apart from minimal and occasional lost no more complete teeth. As Dr Greger says, do gorillas have tooth brushes? When I was growing up sugar on everything and the NHS was an extraction service. If I'd gone on going to them I'd have no teeth left. They pulled out teeth to make more money.
Thank you for all the wonderful information you gave us on your video. I’m in the process of getting dental implants very soon, and I had no idea about all the information you covered. I will look forward to more of your video’s as they come out. Have a Blessed Day. Mrs. Debbie Feighner/Fort Wayne, Indiana,USA.
Ordinary dentures can get stained with drinks like red wine, but the removable denture can be soaked and cleared of stains, I suppose that would be quite difficult to do on the all on 4 fixed bridge.
I used a regular pick floss for years on my single implant, never had any pain/bleeding ect. when my crown got loose and needed replacement after 3 years of basic pick floss cleaning my implant was completely clean,no issues. I think if it had any debri like in your picture it would smell,i never had any bad smell. I also from time to time use a water pick. But now my hygienist told me that the pick floss can damage the gum under the crown and lead to infection/loss of the implant and told me to use interdental brushes,I bought the smallest ones but it hurts like hell,i cant even get it under the gumline on one side under the crown like I can do easily with a regular floss. My implant itself is a bit crooked so they shaped the crown to look even, so on one side theres no space while on other side the interdental bristle goes in normally. Dentists are so confusing,it's impossible to clean with the smaller bristles, it take me 10 seconds with a floss pick to get under and remove the debri very softly, now my hygienists is telling me i should use those small bristles that causes pain and doesnt go under the crown. Can you even damage the gum under the crown of implant if you clean with a pick floss?Im not using a regular floss,im using good quality pick floss that's gentle on my gums so I cant even wrap it around the implant and cause actual damage, all im doing is pushing that pick down to the end of the crown, bringing it close to the crown and slowly raising it up to raise up any debri.Isn't that's the whole point? Not those stupid ''tepe'' bristles that go from side to side like it's cutting into my gums while trying to push it to clean under the crown. With a regular floss im not even touching the gums to begin with. And my hygienist wrote down 0.5mm bristles and im using 0.4mm and it's still to big. Also I used implant floss but it's not for me with natural teeth and only 1 implant, the floss itself was to thick and after a few uses i noticed that it caught around the implant and got stuck under my gums, those work only for people with full implantation with no natural gum,no for people like me where the pocket is deep. Everytime I go to a hygienist I get random stuff recommended that simply doesn't work. Also it's impossible to find any info on people like me who have ALL natural teeth except for 1,all the info how to clean is shown on people with fake gumline with full on 4 implants where you can access the abutments to clean them easily.
Dr. Sehmi. Very informative video. I've had my all on 7 in for a few weeks. I've tried getting the super floss underneath it but no luck. I've been using a waterpik since day one and I think I'm doing a really good job. Is there any way to tell how good of a job I am doing. After all I've been through, I would hate to have any of the implants go south.. Thanks for your help and have a great day.
To be honest, you can only tell once the bridge is removed. Your dentist will be able to do this for you. Some dentists like to make a right seal with the gums, so you can't get super floss in, and this is fine, as long as when the bridge is removed, the gums look healthy.
@@drgurssehmi Thank you for getting back to me. He did say that it was design for a tight fit to keep food from lodging underneath it.. I use my Waterpik religiously and it seems like my mouth is clean. Maybe a once a year removal for a visual check. Thanks again.
Except for annual or 6 monthly (what is your recommendation? ) Dentist cleanings?? When the Dentist will disconnect the parts for a cleaning? Is this the regime?
Great video, thank you for making these! I got my all on 4, for me 5, and I'm currently in my temporary - I'm about 8 weeks in and I finally started using a waterpick, and the amount of white slimy mucous stuff that came out was disgusting! It actually looks a bit like that poorly designed, hard to clean bridge you showed. I too thought I was doing a fantastic job cleaning. 3-5x per day, brushing, flossing, and rinsing. What is that white stuff? Thanks
Hello Doc, thanks so much for your many videos. I have been enlightened beyond words on the pros and cons of dental implants. I’m considering doing an all-on-four for my upper jaw which will be a major smile restoration for me. I’ve got a single implant in front right. What are the problems you think may occur if I go ahead… btw, the 2 adjacent teeth to the implant have moved and that’s why I’d like them extracted and replaced.
Hi Doctor, thank you for the video. Your illustration with schematic and real video is very helpful. I did not realize that the "contact" spot between gum and abutment was much lower compared to natural teeth. I got another question, Is there a difference in the flossing for implants on molar teeth? as there is usually a space under the crown and above the natural gumline.
fantasticly informative - may I query - did you state they you yourself have all on four implants? of so I find this oddly reasuuring as to their suitability of purpose
Yes. Mine are removed once a year for cleaning, and they do x-rays and check everything. My lower prosthesis was removed a week ago. My 4 implants are looking good, and so are my gums. When they took my lowers out, they didn't have to clean anything. The dentist told me to keep doing what I'm doing because of how clean they were. The picture he showed in this video was disgusting. That is what will cause you to lose your implants.
I had a cleaning question. My dentist is recommending coming in every 3 months for removal, clean, reattach of all on 4. However, I have seen others that don't recommend unscrewing it unless needed. I am worried that the every 3 month clean is money based and would love advice. Thanks
It’s not easy cleaning underneath the bridge. I love the all on sixes I’m just hoping that I don’t get that black stuff on top. I wonder if gargling vinegar would help keep that black stuff away. I wish I didn’t see that black stuff now I might be forced to get a professional cleaning ever 3 months
Thank you for all your information. I am considering 4 in 1 and had no idea what it will take to cleanse my teeth. I have 6 fairly healthy front teeth and horrible molars. Is it wise to have the entire uppers replaced with 4in 1? Love your video

Can u plz share and help me learning so... I'm a prosthodontist in india... It will definitely add to my ractice... Thank you..
Brilliant. But I wouldn't use Corsodyl or any other strong disinfectant due to destruction of oral biome which as stated leads to gum shrinkage and bone loss due to local cells being killed off...
Is there a toothpaste you would recommend for brushing the all on four as I have heard you need to use a low abrasive toothpaste and if so what brands would you recommend?
One thing no one prepared me for after my All on Six was canker sores! A lot of them, especially on and under my tongue. They apparently are not uncommon, due to the trauma of the surgery, and start a few days to a week after surgery. So my cleaning routine is rinsing my mouth out with StellaLife after every meal. And they last 7-10 days(?!). I talk on the phone in my job. Gonna lose another week just with these things! Easier than being on crutches, at least.
Hi, I have had a full mouth restoration, I have fixed in denture bridges which is like plastic with fake pink gum in between. I have a build up of brown staining between my teeth on the pink parts and no amount of brushing will get rid of these stains. I have tried so many things but it won't come off. Can you tell me what I can use to remove these stains. Thanks
Within the last 12 months I have had Hybridge implants upper and lower. Where I have most issues is food getting jammed under them. Almost immediately I have chunks of whatever II happen to be eating which is uncomfortable and a bit gross. Especially when food works its way out at any given time. My only recourse is to floss or use my water pick which many times is not possible. For one example, if I’m out for dinner with friends. It’s a given that I’ll end up with food underneath both the top and bottom. I take every moment that I’m alone to force air between both upper and lower to remove all that I can which can be a disgusting thing. Or I force whatever I happen to be drinking between them. I do this if I’m with my wife only and she says it grosses her out. And I don’t blame her. Then during the day things can work their way out and I either have to spit it out or swallow it which is definitely gross. I never expected for things to be like this. I’m sorry if it’s hard to think about but the more I can tell you the better you can understand. Thank you Ray
How can you clean a bridge implant tooth 24, 25 & 26? 24 & 26 have screws but not 25. How do you use floss around 25 with super floss since 25 has no screw? Please advise? Thanks Rosa
From my experience water pick on its own is not great. Using a tepe brush is actually my current favourite! Water picks can be a bit strong and can damage the gums around the implants.
I have currently and recently received all on 4 or well , 6 actually and the dentist never explained how to clean them or what i should do from this point on. I am in America by the way, and was looking for a better way to clean my teeth as i was given a water floss and never told how or when to use it. It currently died and i was looking for another one to purchase. After seeing this video , what should I do now? it took more money (28k usd) for just the upper arch) than i have to get these very pretty teeth but they do wrap around my gums. What now????????? Please and thank you for your video!!!
My dentist just told me before the procedure that I just need to waterless twice a day ,nothing about floss or brushing,,maybe he's waiting till I get my permanent ones ,,I am a month into iit with healing bridges
My temporary All-in-four implant causes me to not pronounce word’s correctly. Will the permanent inhibit me from pronouncing words because I am a teacher which is very embarrassing when my students make fun of me or can not understand me?
Can you please tell me your thoughts on having snap on dentures for the lower arc and having implants on top arc . I simply can’t afford to have both fixed implants too and bottom.
@@mary-lou7746 Not really. I found a two spots where I can insert the floss under but since it is a ALLon4 I cant reach every part uder.
I wanna make it clear that should we need super floss or just normal floss for a single molar implant?
How much does dental implants coast
Thank you
Ray
How can you clean a bridge implant tooth 24, 25 & 26? 24 & 26 have screws but not 25. How do you use floss around 25 with super floss since 25 has no screw? Please advise? Thanks Rosa
could one clean the all on 4 with a water pick and a safe disinfectant?
From my experience water pick on its own is not great. Using a tepe brush is actually my current favourite! Water picks can be a bit strong and can damage the gums around the implants.
@@drgurssehmi and this tiny string don't? are you really serious about this whole video? Because I'm sorry I cannot take you seriously.
Can a water pik flosser be used effectively to clean underneath the denture
I have currently and recently received all on 4 or well , 6 actually and the dentist never explained how to clean them or what i should do from this point on. I am in America by the way, and was looking for a better way to clean my teeth as i was given a water floss and never told how or when to use it. It currently died and i was looking for another one to purchase. After seeing this video , what should I do now? it took more money (28k usd) for just the upper arch) than i have to get these very pretty teeth but they do wrap around my gums. What now????????? Please and thank you for your video!!!
Would the latest water jet cleaning device be suitable for cleaning all on 4/6
My dentist just told me before the procedure that I just need to waterless twice a day ,nothing about floss or brushing,,maybe he's waiting till I get my permanent ones ,,I am a month into iit with healing bridges
My temporary All-in-four implant causes me to not pronounce word’s correctly. Will the permanent inhibit me from pronouncing words because I am a teacher which is very embarrassing when my students make fun of me or can not understand me?
thank you, just got my implant crown put on today, this is helpful info!
Dr Semhi can you recommend someone in the states, I’m in southern California! Thank you I’d would go see you if I could !
I have seen some videos by Dr. Ramsey Amin and I am impressed by his work.
Can you please tell me your thoughts on having snap on dentures for the lower arc and having implants on top arc . I simply can’t afford to have both fixed implants too and bottom.
It took me about a month of flossing DAILY to get the hang of it. I went from 20 minutes to about 10 minutes.
Dr Gurs

I would like to know the changes that occur to the gum during healing with temporary over 6 months to the finals that are placed in.
Just one problem with floss and All on 4. It is easy to do it on other people mouth but alost impossible on your own.
Hi have you got the hang of the superfloss I've just had 2 implant bridges fitted and I can't get the hang of the floss 😒
@@mary-lou7746 Not really. I found a two spots where I can insert the floss under but since it is a ALLon4 I cant reach every part uder. I'm going again to dental technitian to try to figure it out and I will also try some hook like "EEZ-Thru Floss Threaders" if I manage to fing or make something like that. But mostly iti is just to tight to push simple superfloss end under it.
@Admin Hr thank you for your reply I can thread the floss under but can't move them along my bridges 😒dentist has givin me interdental brushes to use they work but I'm getting pain & bleeding I'm back to My dentist on Thursday fingers crossed nothings wrong tho I'm in alot pain with my gums 😒
Can I use a water pick
Am sure you can but on the lowest setting but am not sure when you can I.e 1 month later
Thank you for all the info!
thank you
Thank you Dr.
I am in USA, just in the process of all in one top teeth. I was told water pick on high setting after every meal. This is the first time I have heard about this floss. Am I damaging my gums? What do you tell people about developing a lisp after getting Temps. My temp has broken 2 times now.
wonderful!
Thanks Dr your video is really great many thanks from the Netherlands
Aap india ke doctor ho
Thanks! This is great! 8:25
Thank you so much for watching to 8:25 :)
What about a waterpik?
They can be good, but they are quite aggressive, so it's best to use them on lowest power, and maybe mix the reservoir 50/50 with water and chlorhexidine mouthwash (corsodly)

I used to have all my teeth I had gum Disease
Don’t feel bad I lost mine to gum disease and advanced tooth decay. They were pulled seven months ago, I don’t have dentures yet.
Meeting drohanon has been a blessing to me ever since I started using his natural remedies I was able to live without bad breath and now I and my family live happily:???
I UA-cam dental floss for my braces
That is good I had braces when I was child. Flossing is really good idea. After I got them off my teeth and gums were healthy.
I don’t understand why you can’t have an actual model and take a real dental floss and actually do the damn thing. Jesus!
Waterpic
Really speaking to show once would be more useful than talking for 16 minutes with no sufficient imagery.
All I hear is Blah blah blah. Get a model and demonstrate the damn thing.
Just wish you would not gulp saliva so obviously! Not nice. Perhaps your microphone needs repositioning?
If the sound of gulped saliva is a bothersome issue for you, I wonder if my assumptions are correct with assuming your the annoyingly pretentious one in your friend group!!
