    If your kidney function GFR has low numbers putting you in stage 3 or 4 kidney disease, many have gone on 5 day fast, started eating clean and were able to elevate their GFR numbers back to normal.

    I've always been suspicious of the TMAO argument and from Science news 11/6/2018 " "it seems that a fish-rich and vegetarian diet, which is beneficial or at least neutral for cardiovascular risk, is associated with a significantly higher plasma TMAO than red meat- and egg-rich diets, which are considered to increase the cardiovascular risk," researchers from the Medical University of Warsaw in Poland and the Polish Academy of Sciences wrote."

      Not to mention a study of older women who got levels up 10x for 24 weeks, no inflammation.

    I am confused by the choline TMAO issues. I have a liver issue and am now concerned about eating eggs at all. Does TMAO go away if you eat clean after eating something high in choline?

    Thanks Dr. Brewer, I just wanted to say that Deuterium isn't radioactive, so it isn't for radioactive labelling but still useful in organic chemistry because of its differences (compared with Protium) in spectroscopy techniques. Thanks for the information!

    Great topic that the media in general blurted out danger warning headlines/articles without the nuances adequately being discussed. I have never had a doctor say anything to me about kidney function from blood test results for yearly physicals, and I wonder if you look at eGFR and/or the results of an MACR or SDMA test to help quantify whether a patient is experiencing lower kidney function than might be expected due to age, sex, etc. People with lower kidney function are prone to hypertension and heart disease. It seems like IR/diabetes, lower kidney function and hypertension are all bound together in a bad set of negative feedback loops.

    Dr. Brewer,
    Valter Longo is recommending eating a plant-based diet most of the time with some occasional fish added for people who need extra protein to maintain muscle mass. It seems to me that using the longevity diet protocol our microbiome would be tilted toward a population of gut flora that does not favor the production of TMAO. Combined with good kidneys clearing TMAO from the bloodstream it would seem that the whole food plant based diet with occasional fish would be a pretty good idea. While a keto leaning diet rich in TMAO might be metabolically preferable, overall due to the atherosclerotic risk it might have some issues that have not been addressed. As far as I know, there are no long-lived populations that have a keto type diet. What are your thoughts on these comparative risks? Does fish produce less TMAO?

      Of all the foods you can eat, Fish has the highest level of pure TMAO, plus mercury.

      @@PianoPatterns123 However, it appears that Fresh Water fish don't seem to have the same problem. Specifically Trout and Walleye but I don't know about other Fresh Water fish. Cleveland Clinic study

    I'm guessing it comes down to gut dysbiosis as the culprit.
    There's substantial evidence on both sides of the fence. When you have that, I tend to look at the quality of the study, whether there is other evidence to support the findings, then the variables.
    Improper digestion, leaky gut, inflammatory issues are all value of that have to be considered. I have yet to read the study, but when the pendulum swings to extremes, this is what I suspect.

      I use the common and descriptive terms of "good" and "bad" concerning heart disease all the time. "Good" is being able to exercise, work and enjoy life without heart related pain or disability. "Bad" is being on a hospital gurney kissing your spouse goodbye as you head towards the double doors of the coronary surgical unit. ;-)))

      @@akbarshoed Well....that's my question also but right now, until I hear more that's definitive.....I don't think they really know. According to the National Institutes of Health...... "It requires more research." Arrggh!!!!!!

      Bottom line - a lot of people worry about TMAO. But not me. If your kidney function is good - TMAO doesn’t matter. If your kidney function is bad, you’ve got bigger problems. So, in the end, it’s kidney function- not TMAO.

    I did not understand how this is connected to the title, I presume TMAO affects coronary risk and you are saying the vegans may have a point. Or are you saying that if you have impaired kidney function you may be more sensitive to specific diets. P.S. Vegans are never right, they are mostly left(joke)

      Good joke. I don’t think TMAO is really an issue, like vegans may think, unless there is a major kidney problem.

      I can’t say I’ve had 6 eggs & bacon for breakfast. These days, I’m not eating any breakfast. But yes, I’m not concerned about TMAO. I’m concerned about kidney function.
