Definitely need to be safe
That milk spill reminded me of one of the worst incidents I've seen -- a forklift operator stuck his tines straight into the bottom layer of a milk pallet he was pulling down. It was a waterfall, an absolute nightmare.
On another note, my own worst forklift incident was a collision between the top of my mast and a dock door that wasn't all the way up and secured; that was back at my first warehouse job. Always check doors and plates before entering trailers. As shown by most of these examples, a good rule of thumb on a PIT is to never make assumptions about where you're driving or what you're handling.

Outrigger forsure
In my old warehouse we only had about 5 guys managing inventory in a 250,000sq/ft warehouse so they were full speed on the lifts. One guy ran a stack of 72" pallets full speed into a steel beam, shot pallet shit everywhere and nearly flipped the forklift. Things are alot more dangerous then these new guys in warehouses know.
