However, while passing through an altitude of 17,800 feet (just below its planned cruising altitude of 19,000 feet), the flight encountered icing conditions. These persisted, with ice accumulating on the plane's wings and cockpit windows.

Because of the extra expenses of De-Icing some pilots will take a chance, and lose. There are also two types of de-icing, one is expensive the other not so much. But, finally, after so many de-icing accidents the authorities finally wised up and move the de-icing machines from the gate closer to the runway. De-icing is only good for about 25 minutes then you have to de-ice again. Too expensive for some pilots so they will roll the dice.
