    21 yr old first officer, that's no age to die. This is a heartbreak, the guy just started to live his dream and because of one man's error his life has been taken away from him, that's sad.

      @@LimpoDragxon Oh I forgot to mention. The odds of dying because of heart disease is about 1 in 6 and cancer is about 1 in 7. It’s more common than you think. In fact you’re more likely to die because of heart attack or cancer than in a plane crash.

    The plane that flew too high ,I remember the crew hadn't been paid for months and were under a lot of stress too. Sometimes we blame the Pilots but issues start with the company they work for too.

    The ignored warning crash, that’s literally like when your mom is spilling there drink on the ground and they don’t notice but you just don’t correct it and tell her because your scared she’s going to ground you.

      Haha I love this. Well said, it’s shockingly shameful that the cap and f/o completely ignore the bank angle warning. Unfortunately a sink rate warning might have saved them but I don’t think they fell quick enough to initiate it

      @@jackg2630 Think it was late even for a sink rate warning, a big bank in a jet is hard to correct in high attitude not even talking about low attitude, once they've gone past 45° with negative v/s it was over

      only the captain that ignores the warning, the first officer know what is wrong with the plane but telling their captain (that is a lot older, which makes it worse for Asian people to try to console them even if there's a visible error ahead) that his AH isn't working properly while the officer's instrument tells the otherwise is viewed to be rude at some reason, especially on Korean culture

    My uncle flew for an airline for thirty years. Once, he did, as a copilot, "take" the plane from a captain when my uncle believed the captain's judgement was faulty.

    Korean Air Cargo Flight 8509 is a favorite of mine because it was the first crash I learned about where the country’s culture was a factor in the accident. Normally, culture isn’t a bad thing, but it can be lethal in circumstances like this one if it isn’t properly kept in check. In aviation, safety is the highest priority.

    If I sat in a plane, and I saw the first officer seated in front of me instead of in the cockpit. I would definitely question the situation and decide to jump out of the plane.

      The switch on the throttle quadrant had corrosion on it and the button slipped, causing it to fail from time to time

      At some point in time some pilots would pull the circuit breakers because the alarm was annoying. They never said if the breakers were ever checked.

      In some new devices guides you read the first step is to plug it in... Why would they put it first?
      Humans can be dumb in various situations.. specially when flying and they by default follow the book.. but boom! No put masks step so they knocked out

    Northwest Flight 255.
    I will never ever forget. I have ties to this terrible day, that I wished that I didn’t. It still brings tears to my eyes almost 35 years later. At the time, this was something like the 2nd worst airplane accident in US history. I was stationed in Tucson, Arizona at Davis-Monthan Air Force Base and was a Senior Airmen. I served with the Englerts, and Karen was a Staff Sargent and was the NCO (non-commission officer) in charge of my shop. Charles was in our shop but went into the reserves and attended college. They where also good friends. My wife did daycare for Charles Jr. (14 months old) and my wife and I house sat for them while they visited their parents back east. We were supposed to pick them up at the Tucson Arizona airport that night. As we waited for the time to go to the airport, Karen’s father called us and he was concerned that they were on that airplane. Of course, we went to the airport anyway. While waiting, we called the airline to get any information that we could. They asked if we were relatives and I lied. I said that my wife was Karen’s sister. The airline said that they were listed as being on that flight. We still waited at the Airport for the connection flight to Tucson hoping for a miracle, but they didn’t get off the airplane. As we privately hugged and cried, a news station was filming. I’m so glad that they did not approach us. I had a copy of the local news story on VHS that a friend recorded for us, but it was destroyed in a house fire several years later. We went back to their home, but didn’t get much sleep that night. In the days that followed, I was ordered to turn over the house keys to the Air Force. I was given the solemn duty to accompany an Air Force Officer to the airport to pickup their parents so they could pack up their belongings. The Air Force Base held a memorial service. This happened 3 months to the day that my Air Force service was ending and I spent my remaining time in a mental fog. I’m currently writing my autobiography and this will be a chapter in my book titled or sub-titled, “Our Living Hell”.
    I have made all the reservations and I’m planning to attend this year’s memorial.

    I always find the Md-80 shaped almost begging for stalls with the wings so far behind the middle centre. There’s so much fuselage before you encounter a wing it seems almost impossible that it can lift without any form of problems.
    And that Learjet... I keep being astonished they didn’t routinely put on the oxygenmask despite any checklist. Apparently it’s been literally trained to death to follow the list.

    If you can't stop watching these it's because you get satisfaction from having the final answer to the tragedy. Most people who've lost someone to tragedy needs answers to why it happened. And you probably have daydreams of jumping into the Captains seat and landing an out of control plane.

    I recently had a trip where I took 4 flights and the Smithsonian voice was playing in my head over and over. Terrible feeling but addictive episodes

      I once watched a 9/11 doco on a flight, didn't even register how poor taste it was until my husband leaned over and saw what I was watching and was like wtf why are you watching this??

      @@William_Clinton_Muguai i dont blame him for being so intimidated by being paired with a captain that could be old enough to be his dad. And the fact he was only 21. He was a kid. A comparitive kid paired with a bloke old enough to be his dad is not going to work with or without CRM. West Carribean 708 crashed bc of the incompetent captain. How do you mishandle a stall at cruising height. You just point the nose down and gain enough speed and then return to your assigned cruising altitude. I know this bc i was in flight school for Aer Lingus until i had to drop out due to suffering a DKA from as then undiagnosed Type 1 Diabetes. Since ive been diagnosed as a Type 1 Diabetic i am unble to fulfill my dream of being a pilot for my country's flag carrier.

      @@William_Clinton_Muguai to say it was an illness is putting it mildly. That DKA was so bad i was in a coma and my blood pressure was only 60/28 on arrival. I had no pulse. I was clinically dead on arrival. Yet 2 weeks later i walked out of hospital. Very shaky but alive and under my own power. Here in the UK where i live hospital beds are at a premium so the NHS discharges you ASAP to free up a bed. So as soon as you can survive at home with support they discharge you. Ideally i should have been in for a month. And i had a massive pressure blister on one heel which was so bad i had to wear a foot support for 2 months after i was discharged. I can still get a private pilots license but im never going to fly commerically unfortunately. I have a mind which works very quickly (ADHD) so maybe i could be a controller as long as i keep focussed on the job which when i am focussed on said job at hand i will do it to the best of my ability and more.

    About the first case, the cultural aspect is exactly what influenced the tragedy of the ferry Sewol which occured in April 2014

    I've always wondered with the first one, when you're a passenger in a plane you can feel the plane turning and banking right? You know when it's entering a turn or a bank even if you close your eyes so why aren't the pilots able to feel it?

      it is because they are specifically trained to never trust their feelings, because more often than not there are optical illusions that can lead to pilots thinking they are in a stall, a nosedive etc., and believe that it is their instruments that are wrong, and "correct" the error they believe to exist which causes an *actual* crash.
      there are a few fatal accidents that have happened because of this, and so in flight school it is drilled into pilots that they must never trust what their body is telling them and to trust their instruments. In this case, the instrument was wrong and the alarm went off to tell him it was wrong, but he was so stubborn and arrogant that he insisted that he trust the broken instrument instead of just looking over to cross check the other ones, and it caused him to crash.
      Very stupid and incompetent captain, he killed so many people out of his sole arrogance. Reminds me of the KLM pilot from the Tenerife accident, the worst accident to ever happen in history, which was, you guessed it, caused by pilot arrogance.

    Sully sulenberg for example sees the warning just b4 disaster strikes and he saves everyone on board

    Even as a passenger whenever an aircraft banks either sides I can feel it, how did the pilot not feel it and only trusted is faulty instrument. The body is pushed in the other direction.

      they tell you in training to never trust your gut for orientation, always the instruments. The stubborn captain should have cross checked with the other two horizons.

    I think as passengers, we should all get to know in advance, who out pilots are, what their experience is, how much sleep they got before the flight, how long they've flown that particular aircraft, and ratings. I mean Uber gives us ratings & driver details, why not pilots before the flight?

    What happend on the korean one is that the captain and the fo was in the military and the fo was a lower rank and the captain was an higher rank and the fo wouldnt talk saying that the captains adi wasnt working and the flight engineer did say something about it but the captain trusted his ADI which was mailfuntioning.

And then it gets even more confusing because Mayday is the name of the show in Canada, but in the United States, it’s called Air Disasters. I’ve found some success in finding Air Disasters on Paramount Plus - which is CBS’s new streaming service. Or you can watch it if you have the Smithsonian channel on your cable subscription package. But other than that, the episodes are hard to find.

    Fun fact: #7 and #6 are investigated by agencies named JIAAC along with the NTSB. However, the two JIAACs are different ones in two different countries (Venezuela and Argentina), just have similar names in Spanish and same abbreviations.

    Once trained and after a few flights pilots feel as though it's fairly easy to fly a plane. Simply because their only controlling it during take off and landing. While in air it's on autopilot for hours. So for something to go wrong I personally believe it takes someone who flies as a hobby to manual maneuver a plane.

    In Number 8 i know why: On 22 December, 1999 just after take-off from London Stansted Airport, Korean Air Cargo Flight 8509, headed to Milano-Malpensa Airport, crashed due to pilot error. The aircraft was a Boeing 747-2B5F, registered HL7451 and crashed into Hatfield Forest in the neighborhood of the Great Hallingbury village.
