I would dome the header so you can hammer right down to the edge of the head without hitting the header. and I'd saw it off at the length of rivet you want, and avoid all the fuss with the pilot hole and undersize punch.
IDEA!!!! Put a counterbore in the die the size of the rivet head. Then when you form the head all you'll have to do is smack it with the big side of the hammer and you'll have a completed part without having to hit it so many times!!
just read through the comments and didn't see anyone mention that you need a bigger hammer. let the weight of the hammer do the work rather than your arm.

regarding punching out the finished rivets go to a black powder or muzzler loader supply catalog and look at a tool called a short starter or a bullet starter I am thinking that if you had a couple more inches of that hex stock use that to replace the wooden ball just the weight of dropping that in the rivet hole would knock it out get Back to Me if you more help

What anvil did you make the rivets on?
+tellket I use a London pattern anvil for most of my work.
