  • Preparing for your consult
    1. List of Concerns - prioritized
    2. List of Questions - prioritized
    3. List of Medical Conditions and Surgeries
    4. Medications - name, dose, frequency, reason for taking
    5. Allergies - drug name, type and severity of reaction
    6. Bloodwork/Labs - in the past year, whether directly related to visit or not
    7. Imaging Studies - images/pictures from CT/MRI/etc...not just report
    8. Pathology Report - full report, not just diagnosis
    9. Other Medical Records - better to bring more than less
    10. Summary/Outline of your history - especially if you have a lot of records
    11. Make sure your records were received in the office prior to day of visit
    12. Ask for more time if needed when making the appointment - if case is complex
    13. Consider filling out forms in advance - saves time, less likely to forget something
    14. Make sure everyone is at the appointment…
    15. …or at least available by phone

    Hello sir, I'm from Bangladesh. My baby boy Who is now stand on 11 years has been fractured left side nose bone. He has an enlarged adenoid for that he cannot take proper breath and do snoring. He cannot sleep properly.
