Can you Have SURGERY for GERD (Anti-Reflux Surgery)?

  • Before having anti-reflux surgery, it is important to be sure that you actually have GERD. The symptoms of GERD are very common and can be caused by many other problems. So how do GI doctors decide whether you have GERD? We can run a number of tests to determine if you have GERD and if surgery will help your symptoms.
    An upper endoscopy (EGD) looks for evidence of severe reflux. A wireless endoscopic device or a nasal endoscopic catheter both measure the acid level in the esophagus. The catheter also allow us to conduct an additional test called Impedance Testing, which can detect non-acidic reflux. Esophageal Manometry evaluates the function of the esophagus and the lower esophageal sphincter (LES). An Esophagram looks at the length of the esophagus, how far reflux travels up the esophagus, and may note unexpected findings, which is all helpful for surgical planning. Finally, a Gastric Emptying Study measures how long the stomach takes to empty.
    I also discuss how the results of these tests tells doctors which patients are good candidates for the anti-reflux surgery and which patients shouldn't get the surgery and the reasons why.

    Here in Australia we can't get any of these tests or treatments, but at least the GP visits are free. He's a nice guy. Doesn't know how to fix anything, and just laughs if I ask if there's anyone I can be referred to, but he's a nice guy.

    Hi!
    My body has gone through a lot!!
    I had always been very dedicated to my body. I've never been overweight until recently. In my 30s, I was around 130 pounds now in my 50, I'm 150 pounds. First got diagestive issues when I moved to the USA. After living 3 years here, I contracted H. pilory. Then, ten years later, I started having issues with heartburn, and food would go to my mouth. The doctor found a hiatal hernia that measured 2 cm. I didn't know better, so I went under the knife. Then, 6 years later, the doctor found one gallstone that measured 2 cm, and they removed my gallbladder.😢
    The reason they also got gallbladder out is because Bile was moving to my stomach. I was having bile and acid reflex. Then, symptoms went away, but I remained feeling bloated after meals.
    Since then, I developed IBS symptoms, and just a few weeks ago, I started having heartburn again. 😫
    I have avoided sugar for many years. I've tried keto, but the diet makes me have loose stools. I have done fasting, but symptoms don't change.
    I saw a gastroenterologist last week, and he prescribed an acid blocker, but symptoms are not changing. The fact is, I told him that my right side was hurting, and he didn't say anything. Instead, he wanted me to get a colonoscopy!! Why?????
    I just recently had a colonguard test, and it came negative.
    If you tell a doctor about your symptoms and you get an upper respiratory infection because of the acid, how the acid blockers are going to resolve the problem? Why want to do a colonoscopy when right now my problem is at the other end?!!!!😵‍💫
    I have tried to eat more meat than vegetables, and all I get is diarrhea, with lots of bile, and sometimes I get constipated too. It is so hard to follow a diet, especially when your liver is not working properly and there's not a gallbladder to regulate bile.
    I sincerely regret both surgeries, but I didn't know better, I just wanted to get rid of the symptoms.....and I did, but not for very long.
    Today, I'm very discouraged, and I don't know who to trust. I don't even know if I can trust everything I watched on UA-cam or the diets......I have tried everything. Did I mention that I'm 51 years old and that I have menopause symptoms as well? Yeap!!!
    So. I get hot flashes, insomnia, and heart palpitations.
    I don't know which direction to go. I'm lost and frustrated.

      Hi! I'm sorry to hear that you are feeling sick, too!
      I just learned that the reason I'm having acid reflux and gastritis, it's because I have a bacteria called H. Pylori. So, even though I got my hiatal hernia fixed, my symptoms won't go away until I get the bacteria eradicated.
      The surgery really works, but it can't do anything good if I have the infection.

    I need your opininion please. I have GERD, constant heartburn even if I eat very alcaline food. Now I am on PPIs, but I know that when I stop taking PPIs, the symptoms back and I believe you know that PPIs are not the solution. Taking PPIs for long term causes a lot of damage. So for me I only see anti-reflux surgery as only solution. I dont have hiatal hernia, just cardia LES insuficiency. And if dont take the medication for GERD, acid will burn my food pipe and can lead to more serious complications. I am 31 now. Would you recommend a surgery to someone like me?

    • @MarkCooperMD
      @MarkCooperMD  7 місяців тому

      The best I know from study after study and meta analysis of those studies is that there is no convincing substantial harm that is typical of long term use of PPIs. From experience I see people do well with them, which seems to be your experience when taking them. Also from experience I see people have complications of surgery. If I got good results from a PPI I would continue them. I’ll defer your decision to you and your doctor but I hope that helps some.

    • @netkotreci3135
      @netkotreci3135 7 місяців тому +1

      @@MarkCooperMD thanks for your answer. I respect your opinion, but considering the PPIs I disagree. Now I take them for 2 months and soon I am off of them. In this period by taking PPIs (Ares 20 mg and month later 10 mg one tablet per day) I don't feel heartburn, but I often burp and I feel non acidic reflux. The more important thing is cause of the PPIs my stool is undigested, so minrelas and vitamins and other nutrients don't come in blood. GERD is a disease from hell, but stomach isn't "constructed" to be alcaline, and our guts isn't there to have undigested food.

    • @MarkCooperMD
      @MarkCooperMD  7 місяців тому

      Stomach Acid isn’t the sole mechanism by which the body digests and absorbs food. That’d be a bad design to have a single fail point. Fiber is supposed to be undigested-that’s the whole point. PPIs don’t cause our stomach to be alkaline but less acidic.

    • @netkotreci3135
      @netkotreci3135 7 місяців тому

      Ok. But why is there risk of hip fracture or lumbal spine fracture after taking PPIs after one year. In my blood test I made 3 days ago I have iron, calcium,...and other nutrients in not so good level. They are not in insuficiency bit it would be soon. I know that after stoping taking PPIs heartburn will return.@@MarkCooperMD
