Why is surgery seen as more important then counselling, when it comes to gender dysphoria? Surely counselling should be the most important starting point to resolve the issue.

It isn't, not by the trans community and not by the medical community. But it *is* seen as more important by society at large, which has very narrow, binary perspectives on sex and gender. This informs a lot of the decisions trans people make.
The biggest problem is waiting lists. You are not only waiting *at least* a year (now it can be upto two years) to even be *seen* by a nurse at a GIC (and they basically take your details and ask some questions and that is it until your next appointment, which could be another six months to a year away) and in that time there is nothing. No help, no counseling, no dedicated mental health care.
Trans people should be able to talk to someone at least once a month for that long, first years wait.
It might actually be better if they did provide blockers and/or hormones as soon as possible after a year of waiting to be seen, especially if there had been comprehensive counseling in the year leading upto that, but there is no counselling and little access to the GIC either, the services just aren't there.
However, that does not mean we need more gatekeeping, but we do need more comprehensive access to care.
Most trans people, however, have actually done most of the internal work by the time they come to apply to the GICs for care, they *know* who they are and what they need. However for people like Ritchie, who clearly needed a lot more care, in-depth counseling isn't there. Unfortunately 'one size fits all' does not work for Gender Care. Many people really do not need more delays, gatekeeping and disbelief (the old model) but some people *do* and every trans person would be grateful for at least being *offered* comprehensive counselling sessions. *However* it is vital that that offer isn't made *conditional* for receiving medical treatment. Rather, it should be available in addition to the medical route, and should remain available at all stages of transition, pre, during and post any medical intervention the patient in question requests. Essentially, I advocate for parallel care systems for trans people.
There should also be comprehensive mental health care available for *everyone* regardless of gender identity, but again, that is something that had been progressively stripped from the NHS by successive Conservative governments.

We need to recognise that people like Ritchie are being misdiagnosed. They are seeking help for mental health and instead of getting to the route of the issue, the doctors are following affirmative care and just putting people like Ritchie onto a conveyor belt. The doctors involved need to be held to account or we will see more and more people like Ritchie.

Clear BS. He was seeking this surgery himself.

Misdiagnosed is putting it mildly. People are not 'trans' traditionally they suffered from dysphoria and decided to transition because the dyphoria was so bad. But these would be people that may have suffered sexual abuse or like this young man gay and had an internalised homophobia. It's shocking how there is no gate keeping for these people now.

Way back, in the 1980s, a tv series documented the progress of George becoming Julia. He wanted to become she in a hurry, rather than wait two years. However, as sometimes happens, the surgery was not a success, and Julia went on to have a number of health problems. Latterly, she counselled people to look very carefully into their options instead of just rushing into sex reassignment. Unfortunately there is more heat than light regarding this issue, to everyone's detriment. We need more calm, considered reflection, not more trans activists waving flags in the street and trying to shut debate down.

As a mental health professional, I saw alot of people with the same issues that is why I am amazed that parents do not see the dangers of addressing gender issues in children. Their is alot of danger . Thankfully, in my day when practising, there was a while lot more scrutiny before ever prescribing hormones and surgery and I can remember a few cases where giving the rights supports, we prevented some bad outcomes and the individual are thriving as of today.
My advice to parents and persons : be extremely careful with this and wait....well into adulthood when making decisions with your body. Many people are making life changing mistakes and with many there is no way back.

Mr. Herron’s predicament is a tragic consequence of how little most general psychologists/psychiatrists know about Pure-Obsession (pure O) OCD and all of its various forms (harm OCD, gender/sexuality OCD, relationship OCD, etc.).
The problem is, the Pure O OCD pattern is entirely mental obsessions and not visible compulsions like repetitive hand-washing, so it’s not obvious that the patient’s distressing thoughts are due to OCD, as opposed to a deeply held belief. Most psychologists fail to diagnose pure OCD and don’t have training in its most effective treatment, Exposure and Response Prevention (ERP).
Someone with severe OCD is also more likely to turn their most important decision-making over to a trusted authority figure, because OCD-fueled uncertainty is very powerful and prevents the sufferer from getting in touch with their own deepest truest feelings and beliefs, which makes it nearly impossible to be decisive.

The main problem is the intrusive thoughts and placing huge emphasis on identification on other things like if I looked at a six pack, thoughts would come whether I was attracted to it. And if I saw a dress- though would come do I want to wear it. And the thoughts just don't stop.
The doctor gave me a really low dose of SSRI, and I took it for a month. And I didnt require it after that. So please if you have any kind of OCD, meditation, spirituality and books like I AM THAT will help immensely.

Thank you Ritchie for your candidness and courage. More an more often we are hearing about the devastating medical connotations with transitioning and the long term consequences. The high potential for infections years after post surgery and the constant threat to life and health afterwards always gets buried by transgender lobbyist who have never even undergone the surgeries themselves. The NHS has forced a lot of clinical psychologists to appease rather than to challenge or pursue self reflective therapy to determine the underlining causes of dissonance that create gender dysphoria. Many route causes seem to be either down to parents reflecting a desire that their child was born the opposite sex, lack of behavioural role models, or denying their child may be homosexual, and by endorsing they change their sex or appearance to typical gender norms, it makes such sexual proclivities regarding homosexuality more palatable. The amount of homosexual people being misdiagnosed as transgender is appalling, and evidence is showing such misdiagnoses is more common with female homosexuals.

The idea to affirm her mental perception as reality has NEVER been entertained by anyone, expert or otherwise, because that would be insane and would worsen her condition. Instead she is loved and supported for what she is going through and for what she endures in her mind. Her condition was brought on by trauma and that is dealt with as well. This case should be investigated. However, the story, as related, has me wondering if this person didn’t pressure the medical team, repeatedly affirming the need for and a right to “needed” gender reassignment surgery.
Contemporary western cultures put such weight on the claims and rights of various gender identities. Perhaps, this person and some family/friends expected too much from professionals who kept being told, “Yes, I want/need the surgery.”
It seems the NHS should tighten its policy on providing irreversible surgery to anyone who has serious mental health problems, regardless of how hard such patients push the need for surgery or a right to it.
This person’s diction changed toward the end of the story. Clearly, legal advice had been provided. There are legalistic references to “long-term studies”, “statistical”, etc.,and describing “pressuring”, despite the individual having admitted to being repeatedly asked about wanting the surgery.
Interesting story, involving some big questions about both sides involved.

I know that many people believe they have been helped by "therapy" but at one time I too was trying to convince myself that these seemingly kind, well educated adults with degrees from the fancy universities that I wanted to attend, couldn't possibly be harming me. But I knew they were putting the nails in my coffin and I was right. My need for help was real and urgent and all they did was gaslight me with really off-base fundamental assumptions and failing to listen to me while showing these academic degrees which supposedly were degrees in empathy and good listening. Left wing wanted their ideology to be affirmed. Nobody is allowed to go against it. No Dr is allowed to suggest therapy only allowed to affirm. It's from the govt to appease the left ideology. Otherwise it's seen as conversion. If we don't affirm we are bigots. Dr hands are tied.

Great interview. It's surely possible to acknowledge that some folks are sure of conversion & have no regrets and others suffer more from dissociation & ocd. Professionals pushing for any sex assignment surgeries should never occur! I've known 3 people who have gone through obsessive phases regarding gender, these issues aren't rare and need to be understood as relatively common features of some personality & other disorders. No, not always easy to sort out but it is critical to do so.
