  • This is controversial so please keep an open mind and do not persecute me for sharing my opinion. Accutane microdose could be an option for those suffering from persistant acne and oily skin. This medication does have side effects and you should talk to your doctor before microdosing accutane. Microdose accutane should be done under supervision. If you are going to microdose isotretinoin then make sure you are on the correct low dose isotretinoin.

    I am currently taking 5-10 mg per day. Absolutely love it, the only thing that clears my skin. I have 0 side effects, not even dry skin and mood is better than before, because acne can be very stressful.

      @@indigo_dreamz No, not me, especially not with the 5 mg per day. I will try to stay on it for at least another year or so, maybe longer. Depends what my doctor says.

    This is brilliant. I've been struggling with very mild acne and rosacea as a 38 year old, and it's made me consider this. This video was helpful. Nothing other than accutane has ever worked for me.

      @@DrJayFeldman I just tried various topicals - retinoids, clinique, practiv. None of them ever worked. It's terrible as an adult to suffer from mild acne when you feel like you already paid the price for acne in highschool and should be free from it now. Your idea of low dose accutane is definitely something I'm going
      to copy if I can find a doctor who will prescribe (which is a whole other challenge.... )

    I was on 80mg/day back in 1991. I’m now 50 and just started on 20mg/day as, like you, my oily skin came back. One month in and I see noticeable results. No side effects other than dryer skin/hair and chapped lips. But the transformation is nothing short of amazing.

    THANK YOU thank you thank you for this video. It saved me. After watching it I went on a very low 20mg/week dose (which I take once on a Monday for six months). After a battle with acne for two decade and an array of topical treatments this has been the only thing that has cured my skin. Mininmal side effects, just dryer lips but so worth it. Forever grateful for this advice 🙏🏽❤️

      ​@@MrSam00007 Hi Sam. I started seeing results after about two weeks. My skin cleared up after about a month. But everyone is different, I had milder acne. I recently completed my 6-month course and have only gotten a few pimples since (from an old suntan lotion). I'm monitoring my skin at the moment, should my previous acne come back, I will probably consider go on a second microdose.

    I had cystic acne starting around the age of 19 through my 30’s. Accutane cured me. Years later however I have a lot of sebaceous hyperplasia. I’ve taken a 2nd course of Accutane which helped, but when I stop it comes back. I’m considering taking 20-40 mg once per week forever.

      Google “ Beta Sitosterol dose & DHT & acne” . It cleared my acne and overactive sebaceous glands. Discuss with your physician. Not medical advice but sharing my own experience.

    I take 20mg of claravis once a week to control acne and oily skin. Even at that dose, I still have chapped lips everyday and have lost a significant amount of hair, especially along the frontal hair line. My hair sheds, but hairs are usually short 1cm-7cm with a bulb. I assume it's chronic TE from being on claravis for 2 years (even though it's ultra micro dosed). It's really helping my skin though. Thoughts on this? Is it likely retinoid TE or something else? Would taking 20mg every other week help minimize shedding, but still control oil and acne? I don't want to go bald. I can't even wear my hair up anymore due to how thin and sparse my frontal hair line is, esp the temporal region.

    Great video!
    I take 10 mg a week (!!) and I still have good results and I feel great. I am 39 and had 4 rounds of accutane between 22-36 years old. With a high doses for 6-8 months but it always came back.
    So now sinds 1.5 years i am low dosing. Ultra low dosing. Started with 3 x 20 mg a week for 5 months and then lowering 2x 20 mg and now 1x 20 mg a week. For 5 months now.
    I had some cystic acne around my mouth for a long long time and that made me sad for years ( insecure)
    Now, super happy. Feeling great and clear skin😀

      @@raymondregus9208 exactly, so hard to get. And lots of docs refuse to prescribe it especially more than once. I've suggested low dose to mine and he just shut me down. :(

      @@ML-df3jq interesting and it's legit yeh. Tbh think gonna supplement natural form of Vit A for a bit. As isotrenoin is synthetic deriative version of it plus they put other compounds in which not fan of. I've seen a good study of the comparison & less side effects with natural Vit A then you monitor it to more a lower dose. It's a myth of the toxicity, you'd have to have tons for that to happen & isotrenoin can be toxic too. Was pretty clear the stigma started when they tried to sell isotrenoin. Classic big pharma tactics.

    I have had Telogen Effluvium during my full course (8 months, 120mg/kg). It started after four months and now has stopped. However, I am still on a maintenance regimen of 10mg twice a week indefinitely. Do you think it is possible that my hairs will come back while taking such a low dosage?

    Great video! Incredibly grateful for isotretinoin, taking it for 5 months at 30mg (0.5
    mg/kg) in 2021 saved my skin. Micro-dose with 10mg every other day sounds even safer

    Hey doc! What exactly is the science behind cumulative dose of isotretinoin? Can this cumulative dose be taken during a period of couple of years (as in the case of microdosing) or is it necessarily have to be taken within a span of few months (at high dosages) to cure acne completely?

      Glad you asked! The science behind cumulative dose of isotretinoin is that it is the total amount of medicine you have taken over a period of time. It can be taken over a few months at high doses or over a period of several years at a lower dose, depending on the individual's medical needs. Have you discussed these options with your doctor?

    thanks for ur video! U mentioned, that on roacutan u also need to do thyroid test. Its first time I hear about it from doctor. Can roa affect thyroid. is it common knowledge? I have low thl after course, dont know is it linked or not

    I've been taking up to 40 mg everyday, over the course of 8 (!) Years. It worked wonders, and this was the best period of my life. But Oily skin came back after 'treatment'. Now i am going to go back to microdosing this time. It is the only treatment that has ever worked for me. And all in all - the cheapest, considering its miraculous efficacy.

    Can I ask a question doctor, I really pray I get a response! Going to see a dermatologist tmw, thinking of micro dosing now as I'm terrified of purging, baring in mind I have very mild acne, mild as in one or two here and there every couple of months maybe, but I hate them so much and nothing works, I've been on accutane before around 17 years ago and can't remember purging, bit still terrified I will now, can I ask please doctor, do you think I will purge if I only have a couple here and there and can you still go in the sun if uou micro dose, I love traveling and I like being in the sun, makes me feel better! Any advice please.

    Thank you for this video. I have Sabaceous Hyperplaxia. Would microdosing, 10mg every other day or 3 times a week help? I have tried electrocautery which didn’t work and also left my SH spots red for 8+ months.

      @@DrJayFeldman Thanks for the reply. I actually have not spoken to a Dermatologist about a treatment plan. For my case, I am pretty sure it would not be approved due to the severe side effects. But was researching other ways of eliminating or controlling SH and isotretinoin was an option. Just wanted to know if it helped SH and would be worthwhile to pursue.

I was wondering how 20mg three times a week would influence getting tattoos or laser hair removal treatments, things that are strictly forbidden on regular dose accutane. Would I still have to wait for the drug to be out of my system completely or is it safe bc of the drastically lower dose? Thanks a lot in advance!

      I’ve been on low dose accutane for many years. I’ve gotten laser hair removal and also gotten RF microneedling for acne scars. My derm said that laser hair removal is safe because it targets the hair. The place I had RF microneedling done said it was also safe on my low dose. I take one 40mg pill a week, which is about 5mg per day

    I've been using isotretinoin for about 3 months now. The doc prescribed me the 10mg dosage per day. My skin was super oily, I had huge breakouts and cystic acne, thankfully most of my cystic and general acne is gone, still oily skin but manageable with consistent skin-care routine. I do have post inflammatory erythema (red spots) which is kind of annoying. I'll continue using 10mg per day for a few more months and hopefully I'll have a clear skin by then. Also thank you so much for this video Doc. It really helps and encourages us to keep going and stay strong completing our journey.

    10 years taking low dose accutane here! 👋I started taking accutane since when I was 18 years old. My derm prescribed me 40mg per day. It took me 6 mos but I still had acne but not so severe. Then my derm prescribed me on low dose. 40mg per week. Just like the guy you mentioned, when i stop the accutane for a month or two, small bumps and comedones start to appear on my face then my derm would prescribe me again on low dose. My liver and kidney functions are all good. Ive been on different clinics and derms but thats all what they advised. Now i am 28 years old, i just stopped the accutane for 3 months now and i have so many closed comedones again. Im gonna be back on low dose isotretinoin next week. 💀

    hey jay yesterday i took my first modafinil but it didn't work on me and i had taken 50mg. Today i had taken 100mg but results are not that good. i guess i can focus only 2hrs and then its gone well, im not feeling sleepy but yah as you mention in your videos that it will energies you and you will feel motivated whole day it didn't. well im 18 and dont take any other medicine or drugs or alcohol. can you tell what should i do cause i have taken it without prescription for my studies

    Hey I've been suffering from closed comedones they're very mild but whenever they purge they scar and give a stubborn mark also they're persistent and over my T zone due to overactive sebaceous glands nd my skin is so oily. Ive used evey topical but they reappear again nd again ive also took deoxycylin but when i stopped they reappear again. Can i consider this microdosing?
    Also I've mild acne issue

    I wish i had known about this 15 years ago. It would've saved me so much stress and pain over the years! I am 38 and currently i take 20mg of Accutane once every 3 days. It has totally cured my acne that I've had since i was a teenager. Just incredible.

      @@MaximilianMlsk hey, thanks for asking! I'm a skinny guy, only 150 lbs. I had one breakout after the 1 month mark on Accutane but I think that was caused by using moisturizer on my face. I haven't had any bumps since I started washing my face with a basic bar of soap and not using any moisturizer. Twice a week I wash using an exfoliating cream, just to get rid of dead skin cells. I use adapalene gel at night before bed. That's all.

    I have been om Isotretinoin for 2 times. (Both 6 mounth- 40mg a day/ My Wright is 65kg). had Telogen Effluvium during my last course. It started after four months, and therefore I stoped the treatment. (October 19).
    Now the acne is back.
    Do you Think that a Low dose, Will start the hairloose again? - if it do, i would not start isotretinion again.

    I just got off of accutane after being on it for 6 months due to my extremely excessive oilynes. I am already seeing my excessive oil come back. I mentioned a long-term microdose of accutane, and she said she can’t do that. Is it possible to get prescribed a micro dose of accutane ?

    I agree with Dr J. I'm asian in early 40s w cystic acne on chin before . I'm taking 10mg everyday and my son even commented I look younger than before. He even commented some laugh lines is not there anymore. No cystic acne also for months

    Hi Dr Jay. Question with taking Accutane in low dose every other day how much more sensitive is ones skin to sunburn at this low dose? With all things being equal. My skin is very sun hardened due to my occupation and lots of sun.Thanks and thanks for the insightful video!!

      Hello there! Thank you for your question. Yes, even on a low dose, you may still be at risk of sunburn. I recommend wearing a broad-spectrum sunscreen and protective clothing when you are exposed to the sun. Do you have any other questions about sun protection?

    Are you going to take this dosage of 10mg every other day for lifetime?
    I have completed my course of 30 mg/day of 9 months, and my dermatologist said I can take 10 mg a day to maintain results for another 6-7 months, because my skin is extremely thick and very oily so I’m really enjoying the accutane and I got no side effects at all.
    Please tell me if you are planning to take this for lifetime?

    Hey! I have acne, malessezia folliculitis, and seb derm. My derm has prescribed me accutane, but I am thinking a life long micro dose would be better than the dose she has more me. I haven’t started taking it yet. Do you or anyone in this comments have any good research articles I could bring to my derm?

      I have this too. I done a full course of Isotretenoin and I had zero issues with my scalp. Since then I have had it return however it’s not been as bad. Nizoral and prescription medications don’t really help and just make my head more irritated. Considering a super low dose for maintenance
      My quality of life was so good. It gets me down so much.

    I have had persistent oily skin and acne for 18years. I finally took 10mg a day for 2 months. Perfectly cleaned up my face and pores! Then decreased to every 3 days for 2 months. Still no acne. I am now on 10mg a week one pill a week! No side effects! My skin is clear and healthy looking and not dry either. Just the perfect balance!

      @@Ali7wk I think it has! I now have no more pigmentation from acne and the scars i have a mild rolling scars which is now improving from topical trentinoin as well! I could do more to perfect my skin like micro needling but its expensive and im not too bothered by it.

    Hello doctor. I have been microdosing 40 mg (with occasional decreases and stops). Does that low dosage can have an impact on skin/mucose scarring? And can it contribute to develope stomach issues in general?

    I’ve been microdosing in the same way for about 10 years. I have very beautiful skin but now my back problems are pretty awful- extremely stiff lower back that constantly needs cracking to relève tightness and pain. Is it possible this is caused by accutane? How long can I microdose in this way without causing more damage?

    It is possible to get vbeam laser while taking low dose accutane? Generally it is recommended to get any laser treatment at least 6 months after last accutane pill but what if a patient is taking so low dose as 5mg per day?

    I am doing 20mg every other day and pretty much came to the same conclusion as you. Did a ton of research and no other side effects. Huge improvement in life quality, because I'm mainly using it to just control my oily skin. I have a very good diet, exercise regularly, but nothing helped (except for extreme high dosages of vitamin b5, but that's not bearable and actually had some unwanted side effects as well).

      I am also doing 20mg per week and I am really concerned about it. I was used to taking it daily for about an year, but after another year my acne seems to come back slowly so I started taking it to prevent acne and as I said I am concerned of not being a good thing to take it so rarely. Do you have any advice/ suggestions for me?

      @@MaximilianMlsk I'm a short sporty man, I weigh probably about 70kg. Everything is good so far. Never had problems with breakouts. I plan to take the low dose the rest of my life. I don't really see a problem with it, as long as blood tests are all right.

    I currently take 10mg 2 times a week. A can’t take full dose because I have chronic exfoliative cheilitis, normal dose would just kill my lips, so I try to microdose.
    Doesn’t your dose affects your lips at all? Can I try to increase dosage to 10mg every other day without it affecting my lips?
    And can I hope for long term remission after like 13-14 months on this dose, considering that I have mild-moderate acne and my weight is 56-57kg?

      That's great that you've found a dosage that works for you and your chronic exfoliative cheilitis! It sounds like you're managing your condition very well. Have you noticed any changes in your lips from taking a lower dosage? It might be worth trying increasing your dose to 10mg every other day and seeing if that has any effect on your lips. As for long-term remission, you may have to wait a bit longer than 13-14 months to see the full benefits, since it can take a while for the full effects to be seen. How have you seen your acne improve since starting this dosage?

      I haven’t noticed much difference in my lips so far. Which is good, that I can tolerate at least this dose. I’ve noticed that my skin is slightly less oily and more flaky now.
      As for my acne, I think I started to get more painful pustules, and for the last week I got way more big pimples that I’ve ever had at a time. I guess it might be a purge. I even starting to regret going on accutane.
      The reason I want to be on it for 13-14 month is that I’d like to undergo laser procedures for my scars, and as I understand,
      I have to stop accutane 6 months prior.
      Don’t you know if it safe to do co2 laser on microdose?

    How does microdosing (10mg / week or less) affect the skin to sun exposure? Still have to be extra careful or it’s OK to enjoy it normally, with proper sunscreen of course.

      Thanks for your great question! Microdosing does not affect the skin's sensitivity to sun exposure, so you should still take the same precautions to protect your skin (like using sunscreen) even if you are microdosing. Have you had any personal experience with microdosing?

      @@DrJayFeldman This is good news. During my research, it was suggested that while on this medicine any and all sun exposure was to avoided if possible. That would really be discouraging since summer is coming :). I have not started microdosing. Hopefully it’s an option which can help.

      I’ve read some reviews. Most people say it does have results but not that miracle and they still have acne after 😖 I’ve done so many things like lasers, LED, mesotherapy, chemical peel + expensive routine and nothing works as good as accutane.
      I’m currently still using some in-office procedures (mostly mesotherapy) while on low dose accutane, I think combining them if you can tolerate is the better solution. But nothing smooth my skin out like accutane

    Ive just been to my Dermatologist in the UK and weve been trying to figure out an effective dose.
    Ive been on 20mg a day for about 2 weeks to get it back into my system,now on 20mg 3 times a week as i dropped to 20mg a week previously and my acne came back a huge cyst came next to my mouth and in my hair.
    2 weeks in things are settling its the only treatment that works for me

    After 5 months of finishing my accurane cycle ( 40 mg first 3 months , 60 mg next 2 months , 80mg last month ) I started to be oily again and start getting acne , then I started taking a microdose , 10mg day on day off ( for 2 years ) I have a baby skin after having the worst skin ever .
    I also use 1% bha ( 3 times per week ) , sunscreen always , and a moisturizer

    It’s my third time going back to accutane and everytime I’m on accutane my left eyelid becomes so saggy that I get triple eyelids just like what usually happens when I’m sleep deprived. However on accutane it looks even worse and it happens everyday even when I have a full night of sleep. When I took a six months break on accutane it slowly went back to normal but now that I’m on accutane again, it’s back. Why does this keep happening and how do I prevent this? Please help I look so horrible with it that it makes me want to cry everytime I see my face in the mirror 😢

    Thanks for the video Doc! My derm put me on 5mg per day cause I’ve been struggling with closed comedones for years! It’s a very mild case but was on tretinoin for a while and didn’t completely clear them. I live in Australia so fortunately it’s pretty easy to have access to dermatologists and I get my prescription for AU$ 23. I must admit I’ve been TERRIFIED of the side effects but videos like this really help. As a side note, down here many derms adopt the European approach to isotretinoin with the low dosage and whatnot, which never heard of before. We only hear about the horror stories so thanks again for the video!

I know in some countries like the USA doctors have to discharge patients within a certain timeframe and that’s why they tend to follow a weight based approach so finding a doctor who’s willing to prescribe low dose Accutane is definitely more challenging.

    My skin isn’t oily, it’s kinda dry in some areas (used to be oily but I’ve done so many things so it’s not anymore now. I also had 2 rounds of accutane before) but I still get many acne now. I start taking accutane again this week: 20mg/day for the first month and then 5-10mg/day for years. But as my skin is already dry, idk why acne’s still there

    I'm 43 and have had two full treatments with tretinoine. Unfortunately, after a year or two of clear skin, acne has always come back to my life😢 I very reluctantly agreed to microdosing Accutane. I have been on and off it several times, trying to find more natural ways, special creams or cosmetic procedures and... nothing works better than these pills. However, if I take 10mg every other day, I still get very dry lips. How low, from your experience, can I get so that it can be still effective? My doc tells me to experiment with the dose but I wanted to get to know your opinion. I'm desperate, I hate my acne more than my wrinkles! Thanks in advance 🙏🙏🙏

      Your the same age as me and my 🇬🇧 Dermatologist has recommended 3x 20mg a week as with 1 x20mg my acne came back a couple of huge cysts and smaller ones that wouldn't heal on there own.
      I've just had a 2 week booster at 20mg a day then to 20mg 3x a week.
      I have 3 monthly check ups.
      Getting big spots at our age is really difficult.
      I had sure a huge under the skin one I cancelled everything for a week and felt so depressed hiding every opportunity

    Any advice on how to get prescribed this? I’m 0-2 in visiting dermatologists after long wait periods.

      Yeah, this has been incredibly frustrating. The responses I've gotten have been, "you're insane to even consider this but we'll gladly prescribe you 100 gallons of clindamycin even though it's never helped you before."

    I did this from July 2020 until about September 2022. I was taking 10mg every other day, so essentially 5mg a day. The only minor side effect i had was dry lips but my lips are already dry as is so that wasnt that bad. I been off it for all this time and i still get a few breakouts here and there so been thinking about jumping back into it maybe for 3 to 6 months again and then just use Retinal

    I just did two full course treatments, currently on 10mg every three days for oil that came back till death do us part (off label)

    28yo male just started taking 10mg Accutane every day for a month. I just know that it works I can feel it. It however did purge a bit for the first 2 weeks though.

      How is the progress now? Im starting today for the first time in my life. Im also taking 10mg a day . Is your skin still clear, any side effects on a small dose of Accutane?

      @@Jan-lv1gv it’s great! My acne is not completely gone but rarely breakout. Got a bit of dry skin, body and face, but nothing moisturizer can’t fix. I still believe small dose Accutane is the best way to go. Just need to check bloodwork every now and then but it should be no problems.

    You said that you used accutane when you were a teenager, what dose did you use then? 40-50mg per day? You say on Instagram that these doses are dangerous

      Yeah I took 40mg 1x per day for 4 months at the age of 12. I had to quit because I was getting almost every side effect. No wonder why

    Hi doc, I am on Isotretinoin treatment but my derm advised me to do pulse dosing, to take 30mg isotretinoin for a week, 3 weeks off Isotretinoin and then the next dose is next month. This is for a year. Do you think this will be effective? I’m kinda having doubts because my acne is still so painful and my derm didn’t gave me some topicals to help with the acne for the 3 weeks off Isotretinoin. I wanna know your thoughts on here, thank you 🙂

I had a full course of accutane when I was about 16 with great results, however I'm 21 now, and deal with excessive sebum production and pimples, 10 mg isotretinoin is available over the counter where I live and was interested in trying it out. What do you think ?
