She's so right. For my work, I document injuries and causes of death for the insurance and legal industries. I can tell you that most people ultimately die from bedsores. Whether it's cancer or diabetes or Alzheimers, it is a breakdown in the integrity of the skin that will get you in the end. People stop moving when they become ill. Those who have reasonably healthy skin can survive a period of bed rest without it leading to skin breakdown. But once your skin breaks down it's only a matter of time before infection sets in and becomes systemic. You develop sepsis, septic shock and multi-system organ failure.

PhD biochemist here: I remember reading a peer reviewed study which said that melatonin levels don't actually decrease with age. What happens is that the melatonin receptor sites become much less sensitive. My 100 year old mother-in-law took melatonin for decades as a sleep aid, but after age 90, it was no longer working. So she was given a synthetic melatonin analog which was designed to be better accepted by the receptor sites. Sorry, I can't remember the name of the analog melatonin, but it has worked like a charm. BTW, that's a great discussion you prepared here. Nice!

My primary care doctor advised not using melatonin for sleep long term because it can lead to memory loss. I think it’s important to emphasize the distinction between TOPICAL melatonin and ingesting it. It’s really interesting to hear about the skin benefits that I didn’t know before. But just a word of caution to those who take it orally to please consult your doctor. Melatonin supplements are not regulated and often the dosage is way higher than what the body requires. Long term use could lead to changes to the brain and dependency.

I think she worded it weird. Melatonin does not directly increase the risk of or cause of dementia and Alzheimer’s. Infact, the only relationship that they have is that Melatonin is significantly lower in those who are diagnosed with neurodegenerative diseases as such. And Too high of Melatonin dosage is infact the catalyst to further put those with such diagnoses at risk, simply because of the natural side effect which is the commonly known and anticipated onset drowsiness that lingers after waking, thus making day to day functionality a higher risk. It is the temporary impairment to essential decision making and delay in alertness that further puts those who suffer from neurodegenerative conditions at risk. For comparison, this is no different from refraining from driving while on meds. There are compelling arguments and some sound data that point towards the benefits of Melatonin.. but perhaps it is safer to take in significantly lower doses than what is normalized on the market.

I take a low dose (3 mg.) of melatonin for sleep, recommended by my doctor. I've had insomnia my entire adult life. My doctor has me take my melatonin 1 hour before bed and Trazodone, an antidepressant, that acts like a sleeping pill, at bedtime. The melatonin helps me go to sleep and the sleeping pill keeps me asleep. It works for me. I used to take Ambien, but I would get up in the night and do things like bake cookies, clean my bathroom or rearrange everything in my kitchen cabinets. It was a sleeping pill that didn't let me sleep a lot of nights, but I never did leave my apartment. Some people have.
I had no idea there were topical melatonin products. I have a receding hairline, arrg. I started out with a high forehead anyway, so this is adding insult to injury. I'm going to try a melatonin topical for the scalp. Wish me luck. I'm thinking of adding a night cream with melatonin, too. Both are pricey for my meager budget, but if I can swing it, I'll give it a try.

I've been taking melatonin for insomnia on and off for many years. It's best to start with the lowest strength and gradually increase only if necessary. And always discuss with your physician why you want to take it. I only take it at bedtime as it does make me drowsy, which in my case is the point. If it also offers skin benefits, that's a bonus I didn't know about. 😊 Thank you for covering this Dr. Dray, awesome info! 🤩

Be careful not to take it long term or your body will become dependent on it and will stop naturally producing it on its own. I learned the hard way. I took 10 mg of melatonin for several months and when I stopped taking it, I could never fall asleep. It took many months for my body to go back to normal and it was agonizing being sleep deprived for many months.
Also taking the supplement orally doesn’t guarantee that it will always get to the skin.

Hi Dr Dray, I'm a layman and not a derm, but I'm curious as someone who loves learning about the body, skin etc, if Melatonin is lipophilic and so good at penetrating the dermis, is it used to help other ingredients penetrate the skin so deeply? Or is it not unique in its abilities to penetrate the skin deeply?

Years ago I read in the book "Melatonin Miracle" that in rat studies, they lived 1/3 longer on melatonin, so I decided to try it on our new Golden Retriever. The breed has an average lifespan of 10-12 years. Our girl is now 15 and going strong in spite of miraculously surviving a very serious accident a couple of years ago. All of her contemporary goldens that we knew passed away years ago. So the rat data is consistent with her results. Naturally my wife and I are on it too.
