Southwest Airlines Flight 1380 was a scheduled passenger flight from New York LaGuardia to Dallas Love Field on April 17, 2018, that experienced a catastrophic engine failure. The left engine suffered an uncontained failure, causing debris to strike the fuselage and a window, leading to rapid cabin depressurization.
A passenger, Jennifer Riordan, was partially ejected through the broken window and later died from her injuries despite efforts by other passengers and crew. The pilots, Captain Tammie Jo Shults and First Officer Darren Ellisor, managed to safely land the damaged Boeing 737-700 in Philadelphia, demonstrating exceptional skill under pressure.
The National Transportation Safety Board (NTSB) investigation attributed the incident to metal fatigue in a fan blade, prompting industry-wide inspections of similar engines. This marked the first fatality on a U.S. airline flight since 2009 and highlighted the importance of engine safety and maintenance protocols.

ALWAYS keep your seatbelt on. I’m just a private pilot, but the biggest risk is probably clear air turbulence. You could be bounced off the ceiling and break your neck. There’s no reason to keep your seatbelt unfastened. If you need to you can keep it a little bit loose, but keep it buckled.

I feel naked without a seat belt, whether on an aircraft or a car. I put it on even if I'm just moving spots in the same parking lot.

is the radio quality still so darn poor in the US? How can air safety be maintained this way?

That pilot stayed calm and professional. Her plane has only one working engine, part of the plane is missing and while in the air she responds "good day"!!

"Calm", she practically gave no sense of urgency what-so-ever, to the point of almost being giggly about it in her tone.

I don't know when the pilots were informed that someone went outside of the plane. The woman talking was calm and then when she informed the tower that someone went out she sounded a little shaky. Imagine being in a [lane and someone get sucked out0 everyone worst nightmare.

The pressure inside the cabin is significantly higher than the pressure outside at cruising altitude. Therefore, any opening in the fuselage would result in air rushing out of the aircraft rather than passengers being pulled out.Apr 25, 2024

I recently read some info about an aviation parts company that got caught selling used / bogus parts which included the 737 engines. I wonder if the FAA found out any info if this may have contributed to the damage.

Jennifer riordan’s family hopefully received a big settlement from this. Such a sad story. No doubt that was a violent death. Likely killed as soon as she was sucked out of an 8x12 window.

Things like this are practiced at every recurrent training . First thing the crew should have done is switch to guard freq and declared an emergency on 21.5 single frequency through landing, which reduces the work load of changing frequencies and possibly missing the correct one which definitely adds work load. Also, and maybe they did, we just aren’t sure, was to call on the lead FA to get them a better status of the cabin and pax. How many were injured or missing and how bad is the structural damage. Many recurrent classes will analyze this incident, and mechanics will study that engine.

Okay, maybe it's just me, but I got the feeling that the APPROACH ATC seemed forced to talk to the female pilot like she was slow. It was interesting when ATC hesitated after hearing that "someone went out." I'm sure hearing that changed the entire dynamic of the incident.
