    I’m 66 and I just had a stress test Everything looked good. My cardiologist weaned me off of verapamil 240 and put me on Monoximil. Blood pressure dropped significantly. He gave me two choices To put me on additional medication or diet and exercise. Losing the belly fat. I’m choosing the diet and exercise. Intermittent fasting I like how you spoke about intense interval training to keep the legs strong. Thank you so much as I want to be around for the grandchildren 🙏

    After my father had passed away 5 years ago I decided to go get my heart checked out and get it fully scanned to make sure it wasn't going to be inherited(Genetic) from him to me in my later life. At about 19:3 0 - 19:42, Dr. Brewer makes an invaluable insight about the difference between fitness and health. About 20 years ago, I had a friend in his 40's who died of a heart attack due to clogged arteries. Except for 1-2 cigars/month, he had every appearance of fitness: an active runner and strong-as-an-ox iron worker with a BMI in the 20-25% range. While "fit" he was not healthy.

      I'm in my early thirties. Stress from family is quite high. Stress from work is very high. For an outsider, I look healthy. I go for walk. Make sure I hit minimum 8000 steps a day. I always skip breakfast. But when the stress is high I eat non stop. I gulp down food like I have not seen food for years. I drink a lot when I'm stressed. When I have high stress, especially from my family, I have chest pain. I can't sleep at night. I look 45 even though I am just 34. I know I'm going down early. There's no way anybody survives like this. Fit and healthy are two different things for sure.

    As usual, a truly EXCELLENT video by Dr Ford Brewer !!!
    Sadly for people living outside the US, none of the tests mentioned is available in Europe ...
    Cleerly Ai analysis (added to a CT Angiogram) will hopefully come someday to Europe ...
    Similarly HeartFlow and Elucid, which have also received FDA approval (like Cleerly) are only available in the US.
    When it comes to CIMT, the very same rule does apply : CardioRisk, Carotid Care and Vasolabs have not expanded their operations outside of the United States.
    Very recently, I came across CaRi-Heart designed by Cardiologists at the University of Oxford.
    Caristo is based in the UK and focuses primarily on Inflammation.
    The Fat Attenuation Index (FAI score) provides an indicator to help stratify the risk, and they do propose also a Plaque Analysis.
    Hope this info will help future readers
    Take Care😀

    In my 60s they will recommend a stent. Especially Kaiser. Well I don't want one. Stents can also get blocked then you need brachytherapy. I started taking a low dose ASA almost daily, walking, good diet. Unfortunately I am going to have to get this pain In my leg when I sleep., checked out thanks doc! My mother and father did not have heart disease. Thats a bonis for me.

    • @eshea3621
      @eshea3621 2 місяці тому +1

      Also have Kaiser. They deliberately have chosen interventional cardiology. When I pointed out that 20 years of trials showed that " medical" management was just as effective and sometimes more effective than the Stent, statin, blood thinner route the chief of cardiology acknowledged that was true but then said Kaiser has chosen this as their treatment protocol and if you will not accede there is no other option. Basically refused to treat me which of course is blatantly medical malpractice but since you can not sue the big K ( arbitration agreement required 2b patient) they know t by ey don't have to worry.
      Btw the reason K chooses interventional cardiology- Medicare pays them 5- 10 times and sometimes more for it than for medical management. Follow the money. Plus I would not be at all surprised to find they have an arrangement w big pharma that rewards their over prescription of statins and the newer still on patent blood thinners.

      @@PrevMedHealth I am finally sorting out a few mental issues leading to the drink. That was my foray into industrialised drinks, otherwise I eat and drink ok, I follow Dr Sean Omara a lot and PrevMed etc. So I'm just saying I had essentially a addiction like others have for soft drinks and cereal, juice, list goes on, it's in your face every day which is worse, most advertising is for processed foods and all the really flash and popular food shops are processed foods. Addiction is where I think these days a lot. Hopefully rfk Jr has success at a macro level. But like after the SATTLERS were gone the opioid addiction remains, humanity has a global addiction to processed foods, it's tasty, often cheap but even when it's not, it's ADDICTIVE! Dr Brewer, I don't know if you can answer this, but I will ask anyway. I'm a 63 year old male who's been doing some form of low carb/keto/carnivore for about 6 years. I had my first calcium score CT scan done 2 1/2 years ago at age 61, the results being a CAC score of 70 with vessel level scores of LM: 43 LAD: 27 CX: 0 RCA: 0 with no calcification of the aortic valve, visualized thoracic aorta nor the mitral valve annulus. Also diagnosed with a fusiform 4 cm ascending thoracic aortic aneurysm. I just had another heart CT scan last week, 11/13/24, with the new results being a CAC score of 102 with vessel level scores of LM: 0 LAD: 89 LCX: 0 RCA/PDA: 13 and now mild calcification of the aortic valve and thoracic aorta, but still none on the mitral valve. Plus the ascending aorta is still at 4 cm, but now no longer any mention of an aneurysm, so perhaps that's just my normal size aorta (I was a part time bicycle racer for over 25 years, age 18-47). Does this degree calcification and increased CAC score in 2 1/2 years fit in with what you'd expect, per your explanation of what might occur once on a low carb keto style diet? Also of note, I've taken 300 mcg of MK7 K2 and 5000-8000 IU vit D3 daily for at least 2 years, among other supplements, and my (hs)C-RP scores have always been less than 0.5 mg/L, and my triglycerides always lower than my HDL. Thank you for any comments.

      This happened to me. I'm currently 64 years old and I began low carb with 16/8 intermittent fasting in July 2021. I lost 60 pounds and threw away my blood pressure medication. In April of 2022 I had a CAC test and it was 32. I also quit taking statins in 2022. In November of 2024 I had another CAC and it was 79. I had the CAC test at a different location and had heard that results may vary by machine. Maybe the explanation is that of soft plaque turning to calcified plaque may be the cause.

He is due to see his cardio next month. What is bothering him at this time is, that he gets these shooting pains up his chest ( from around the nipple to the left shoulder, always on the same side. He says they feel like a shock, sharp and of varying intensity) Our GP sent him for a stress test and an ultrasound on the area. Nothing showed up, stress test was very good. Actually, the correlation between plaque build-up in the carotid artery and the likelihood of heart attack is not great. That's why we obtain our CIMT measurements in the common carotid artery which tends to be free of plaque. It's the inflammation of the artery wall that correlates with heart attack (small vessel disease), not the development of plaque in the large arteries. The typical diabetic patient, for example, will have squeaky clean large vessels throughout their entire body, but severely calcified small vessels (medial calcinosis), leading to heart attack, limb loss, kidney failure, blindness, etc.

    As usual, your videos are very knowledgeable and very helpful. One question if one has calcium score 771 by CT Angio, he took very good lifestyle measures. Three years later, CIMT SHOWS nothing, and it is 0.71 and 0.58. Does this makee sense. Does it mean he has no plaques any more. Thank you.

    • @jrkob1156
      @jrkob1156 2 місяці тому

      Of course not. A CIMT will show you plaque in your carotid arteries only. There is "some" correlation with blockage in coronary arteries, but it is not definitive.
      In April 2023 I did a CIMT that turns out clean. 2 months later in June 2023 I did a CTA and it showed a 70% blockage in my LAD coronary artery.
      The predictive value of a CIMT as far as coronary arteries is concerned has limits.
      DrBrewer has been advocating the CIMT as "very reliable" for coronary arteries (96% predictive value) for years, but unfortunately you won't know if in your particular case this predictive value worked for you until you have... actually done a CTA to verify. At the moment, there's no way around this.
      (an ECG will flag blockages in coronary arteries >50% so better than nothing and without radiation, but 50% is obviously a high threshold, with a CAC score of 771 you probably want to know if you have blockages before reaching this level of blockage).

    I am in my 60s. My friends are in that group and older. Even if they go to the doctor for an ingrown toe nail, they ALWAYS come out with one or more stents. Cha-ching! These things are put in when they are not needed because they are PURE profit for the corrupt corporate hospitals and the industry in general.
    Plus, installing stents is not risk free. It is EXTREMELY dangerous. These installations kill a small but significant number of the recipients. Or worse, destroy what health they had left. The stent can break away plaque and cause embolisms and strokes, puncture the artery walls requiring immediate chest splitting (my absolute worst nightmare!) and other complications.

But when I came back to my smaller town, all the cardiologist could tell me was that I had 50 percent blockage in one of my arteries and that I should now get a coronary angiography. How do I get the information about the soft plaque vs hard plaque? Do I now need to ask for a coronary angiogram with AI analysis?

    Stents can be life saving if you have unstable angina or having a heart attack.
    You can also get heart attacks even with life style changes . It is a multifactorial complex disorders or even with unmeasurable high sensitivity CRP. It is not pus, it is denatured and oxidized fat and Lp(a). Pus is bacteria and white cells.
    Twenty to 30 % have elevated Lp(a). Calcification is a consequence of elevated LP (a).,

    I am a retired Pathologists' Assistant. Plaque doesn't look like your photo. It is tan-grey with a rubbery to fibrous consistency. It is not at all yellow and fatty looking. Plaque is made of layers of aged blood clots. Sometimes there are darker red layers. The most recent layer is dark red from the red blood cells in the clot that haven't broken down yet. There is a small amount of cholesterol crystals (less than 2%) from the free cholesterol in the red blood cell membranes. There is also some fatty areas from the scavenger cells ( macrophages or foam cells) that have cleaned up the LDL which is damaged by trying to clean up the mess.
    What happens in the heart also happens throughout the major arteries where the pressure is highest: the aorta and arterial branches where the blood leaves the aorta to feed the organs. Damage to the artery lining causes blood clots to form which are stabilized by LP(a) which is a LDL look-alike. Over time the clot decolorizes and gets rubbery with the oldest layers getting calcified.

People need to concern themselves with their mixed macro diets of carbs and fat that cause glycation effects that WILL be causal for arterial plague. Similarly high blood pressure is a causal factor for arterial issues.

I would think that the US will already have this technology quite widely available?.
