    Excellent explanation However
    Can someone or Dr. Brewer explain to me what is the way to check if I have insulin resistance if I am on a keto diet?... glucose tolerance test then probably makes no sense and is unreliable. I don't want to come off keto because then it's hard to come back. .... but my glucose is almost 100 ....isuline 5 to 7

      If you plan to remain keto, you might not want to test. But the only reliable way to test is a glucose challenge. Get a fasting glucose. Then take 75 or 100 gm glucose. Test again at 60 & 120 minutes. The Kraft Insulin Surevy protocol uses 100 gm glucose. It tests at fasting, 30,60,120, & 180 minutes. It also includes insulin at each glucose test. It is definitive. You miss about 20% of insulin resistance if you don’t test for insulin.

      Research what an HOMA-IR score is. It is meant to determine how insulin resistant you are.

    Dr Brewer, you really went 'all-out" with this video. It has to be the most detailed, yet easily understood, explanation of insulin resistance, pre-diabetes and cholesterol physiology I have seen packed into just shy of 42 minutes. I will recommend this to my physician colleagues. It is a perfect 'primer' toward understanding metabolic disease.

    I woke up this morning and did 300 situps, 40 pushups, walked around the block, ate 4 grass fed eggs, two strips of bacon and a fresh tomato from the garden. I learned about Carnivore almost a year ago and find it very easy to stick with. The thought of eating cereal or bread doesn't even cross my mind. I'll be 60 YO in December, 2024!

      ​@@PrevMedHealthpeople get upset when someone has like a tomato, or some coffee, or blueberries but my cats are obligate carnivores they eat grass, and what happens to the organ meats when a wolf eats a bunny? They get undigested plant matter in theirs.
      I wish more people that talked about carnivore or ketovore had these ideas. 20 years ago what I called carnivore/ketovore, was a "Nomad Diet" you eat meat and if you come across some veggies or an occasional fruit you ate some and when stuff wasn't in season you didn't.
      Wish I had eaten like that back then rather than follow the standard advice and got from 220 to 370lbs and now back to 220 after keto made the lbs slip away and I can do push-ups for the first time in my life despite going to the gym as a 220lb 20 year old. Now I'm a 220 lb 40 year old that can do pushups.

    This is one of your BEST videos to date. I would opine that it puts most cardiologists and lipidologists to shame when it comes to educating patients.
    About a year ago, I showed my cardiologist a picture of a box of "Heart Healthy" Cheerios. He just smiled at me and said he really did not have an opinion about that "Heart Healthy" labeling!

    I'm 71, haven't been healthier in a couple of decades, am probably slightly underweight (for the first time), have a reasonable blood pressure, and my overall cholesterol is ... 300(!) I've been on statins for about five days, but...I'm going to go OFF of them, thanks to MANY doctors (including YOU) letting me know that the Lipid Panel used today seems...obsolete. I WILL likely get an apoB evaluation, or a more detailed study of my LDLs. I use ghee, cold-pressed sesame oil (NO other seed oils), MCTs in my coffee (1or2 per day) and a high grade olive oil. Very LITTLE sugar! I really learned a lot from this presentation. THANKS!

    Well Doc. Ford, your Geek Talk is easy to understand. Others not so much. Few days ago I watched an University Prof. of the sciences being interviewed by Rogan. Rogan asked a simple question and this Prof. went on and on and on going around in circles and he ever did answer the question. I posted a comment that this Prof. did not know what he was talking about, plus I answered the question in short order and my comment got highlighted with a thumbs up and perhaps 3 replies agreeing with me, so Geek away anytime

    The carbohydrates specially Fructose produce a serie of conditions: Obesity non alcoholic fat liver disease , insulin resistance , type 2 diabetes, hypertension dyslipidemia and some cancers. Cholesterol is part of cellular structures and hormones. So you are right doctor.

    Thank you for taking your time to really help people Doc. Five years ago I woke up at 3am with PMR. Six months later I started a keto life style. Today I played tennis for 2+ hours in hot and humid Florida, then went to a diner and had 3 scrambled eggs with 5 slices of bacon just like I eat every day. Sometimes 6-7 slices. I'm 78 and feel better than I did when I was 50. My PCP sends me to everyone he can think of for further testing. My numbers are pretty dang good-- better than his, I bet.

    Good information, you don’t have to starve yourself to be healthy, you just have to eat food with nutrition!!! Carbohydrates don’t have Nutrition !!! Carbohydrates are just glucose, fuel to your body but not nutrition!!! Eggs, protein, healthy fats, vitamins and minerals are nutrition to your body! Food with nutrition are simple whole food that you cannot overeat!! How many eggs can you eat? How much meant can you at one serving ?

    Thanks for sharing, Dr. Brewer. You're a wealth of information. I'm postmenopausal; recently turned 57. Two years ago, my functional med doc ordered a non-fasting lipid panel. My LDL-C was 191. My HDL-C A 92, and my Triglycerides A 46. Since the latter 2 are "good", my doc did no treatment. Fast forward about 3 months ago, I go to a cardiologist just to check in on things (and preventative measures). He ordered a calcium score test and said if it looks good, he won't prescribe a statin. Well, my score is 176, so a big red flag for them to prescribe a statin. I asked for a current blood panel. No orders to fast or not to fast; if it was a complete panel, or just ldl/hdl/trigylcerides, so I waited to talk to the APNP, last week, before taking the statin. Scary for me to just take 20mg of a statin. What statin? I had a lot of questions. I knew from watching a video of yours that you took Crestor (and with my own research), I felt more comfortable taking it, but still didn't want to. Well, from the fear of the plaque, plus her telling me that I have coronary artery disease and that I had to get my cholesterol down, asap, or I could have a heart attack, scared me into taking the statin (2-days in). There is paternal heart disease in my family. My dad passed at 56 from it. He was also a heavy smoker. I am healthy overall. I am 5'9" tall; weigh 107; my weight is too low at this point (went on a plant-based diet). I don't smoke. I'm fairly active, but could be more as time & my energy returns. I don't want to be on a statin for the rest of my life. I am a big believer in naturalpathic medicine; life style and wholistic healing. What do you think about Curcumin? Curamed brand?

      Have you tried taking Niacin within a meal? I had bothersome flushing taking it even after a meal. I found that if I take in the middle of the meal, I don't generally get flushing at all, and if I do, it is much less intense. Hopefully that doesn't mean it is being negated by the food. (I don't think so... it appears to have raised my HDL by 10% and lowered my LDL by about the same amount.) That was taking 1000 mg per day of the Nicotinic Acid variety. Also, I have seen in multiple places that if you take an aspirin just before the Niacin, the flushing is eliminated or reduced, but I haven't tried this.

    Interestingly, Dave Feldman's latest preview of their Cleerly CCTA results show that LMHRs and the MI Heart matched cohort not using LDL meds both have about the same soft plaque. For some reason LMHRs' plaque isn't calcified as much as those in the MI Heart cohort. This raises even more questions about the traditional Lipid Hypothesis. But it also begs the question about how plaque progresses without getting calcified.

    I'm SUPER happy to have found Dr. Brewers channel in the midst of my struggle. And while I'm slightly different than the average due to heavy metal poisoning and hypersensitivity/allergies yet despite my documented proof, my primary consistently gaslights me and focuses on ldl & cholesterol.
    I've lost 43 lbs on carnivore but that's been my only benefit. I'm feeling well, in a LOT of joint & muscle pain and a crp that's now through the roof.
    I just wish I could get a doctor who does carnivore or keto and takes insurance

    Thank you for this info. Please give more information about genetics. Most people think because their parents or siblings had cardiovascular disease they will also have cardiovascular disease and it is because of genetics. Most often it is because they have unhealthy diets and life style and are obese, insulin resistant, and metabolically unhealthy . I am a nurse and see that happening all the time. Many people immediately take a statin drug because they say they have high cholesterol and heart disease because of genetic. Most doctors do not discuss diet and lifestyle. Some may be due to genetics but not all. Once a doctor tells a patient it is because of genetics, they feel hopeless and can’t due anything about it except take the drug. Also,more info on APO-a and its role in genetics.

    I got my annual wellness checkup Thursday, Lab test results Friday. According to my lipid panel I probably won't make it another week, but I feel pretty darn good. I'm expecting the doctor's office to call Monday and tell me to get on down to the pharmacy right now for you know what...... Follow the money? Nooooo follow the blood work.

      At this time we do not have the technology to measure the cholesterol in the blood, chemically or physically by using light, color or transparency. Based on agreed knowledge of the last few centuries, there are NO minerals, crystals (like sugar and salt), fat bobles/balls (like cholesterol), oxygen (as gas) or CO2 (as a gas) in blood plasma.
      There are NO Clots, it is only cartoons and photoshops and criminal fake doctors. Fake doctors in Providence Saint John told me that I had a clot in my RCA and they placed a metallic stent which dissolved and disappeared in my blood, later I learned that it was all fake and was about charging thousands of dollars.

    What statin is best for cardiovascular Inflammation? My husbands doctor prescribed atorvastatin 20mg - he is in secondary prevention has he suffered a heart attack 1.5 years ago and was recently diagnosed with CVD. Also think heart attack was brought in my the use of chantix (varenicline) thank you for your videos they are very helpful in understanding what the common doctor do not know or care to disclose

      Thank you. Atorvastatin is the very last statin. I use (less than half a dozen times over the past 5 years) because it doesn’t work as well for females or prediabetics. Try Rosuvastatin or Pitavastatin.

    I recently went on 250 miles of backpacking trips and got off LCHF diet for about a month during and between the two trips. Total C went way down but Trigs/HDL ratio suffered a little (stayed under 2 though); hba1c went up. For me, diet clearly beats exercise as far as metabolic health goes. Or maybe better to say the foundation of metabolic health starts with diet. Next year, I will go through the effort to stay on LCHF out on the trail.

      @mystatinfreelife, I find I can hike backpack much easier staying low carb fat adapted. No energy bonks! Just steady energy through the day and can hike like a mountain goat, love it.

    This is easy to digest and makes so much sense. At age 28, my LDL was 160-155, and in my 30s, it's 181-190. Since earlier it was around 160, I don't have FH, right, Doc? By the way, my HDL is always around 58-65. Thanks, Doc, for the lipid energy clarification.

      People that hit an LDL of 180 or greater have an 80% chance of having FH or LMHR.

    Excellent.
    By far the best most practical explanation I have seen that ties together multiple aspects of metabolic disease and various test results. Including great graphics.
    I have been sloppy keto for several years. Many of my markers are improving. (-30 lbs. Trig/HDL = 1.5)
    I will continue to inch toward carnivore to combat my insulin resistance. (my 2hr insulin = 76 µIU/mL)
    I would love for you to do a version of this video focusing on Lp(a). (my Lp(a) 360, is 99 percentile)

    Great explanation Dr Brewer. You’re 100% right about insulin resistance. Will take your advice. About like style changes 71 yrs old. LDL 125 , HBA1c 5.6. , fasting glucose 100. 66% calcification on CAC score.

    One other thing. There are rolled oats and steel cut oats. Instant oats are rolled oats but not all rolled oats are instant oats.

      I looked it up. I see conflicting information. Some says rolled oats cook quickly. Another says it doesn't. That is critical to me. If it cooks quickly, it seems the body could get the sugar out quickly. What are your thoughts??

    My hypothesis:
    "ldl cholesterol concentration" is positively linked with "systemic microbial load", and serves as an "antibiotic function". Accordingly, reduction of "ldl cholesterol concentration" would reduce systemic antimicrobial capacity, thereby accelerating rate of biological aging and progression of aging associated diseases (like sarcopenia and dementia).

      Good point. However, for those immune to "placebo effects", statins may reduce their cognitive and physical healthspan. There is already a growing body of published academic research to indicate the possibility of such a phenomenon. For example, convincing research published in the "journal of nuclear medicine" by Dr. Silverman's team at UCLA reveal that statin use is associated with more than double the risk of converting to dementia over eight years of follow-up compared with statin non-use, and with highly significant decline in metabolism of posterior cingulate cortex -- the region of the brain known to decline the most significantly in the earliest stages of Alzheimer’s disease.

    Excellent video, please do a video on blood markers for heart disease e.g. Lp(a) and measuring blood glucose - especially for those that cannot afford a CGM 🙂 - thanks! PS The book "the Clot Thickens" by Dr Malcolm Kendrick completely blows the LDL-heart hypothesis out of the water.

    Like others, I also found this vides one of the best of yours, many useful details.
    However, you mentioned here and in a few of your previous videos, the use of low dose statins for those who already have plaque. I suggest that you have a separate more detailed video on this and cover pros/cons of this approach, suggested dose, etc. For example as I learned from other sources, the main side effects of statins are: impact on brain, possibility of future dementia, raised blood glucose (which would contradict to the goal of treating insulin resistance).
    Again, thank you very much for this video, I did learn a lot.

    How is heart protected from high LDL-c due to fasting as well as lchf diet? Cholesterol,hdl-c and LDL-C rises and triglycerides decreases with lchf diet. It does not effect heart. What are the reasons? Are there researches in that respect?

    I went to steel cut organic oats. I limit my sugar and carb intake and avoid highly processed foods. I think grass fed animal protein is healthier and more nutrient dense. same with pasture raised eggs and chicken.

    Low carb, pasture raised meat , eggs and butter raw cheeses some organic and fermented veggies sardines, salmon, nuts, olives and high quality olive oil. No prossessed foods and feeling great!

      ​​@@johnsiekrause4109 from a olive oil store by me, the owner imports his oil from growers in Italy Spain and Portugal that he visits each year and works with the growers. Very high quality olive oil with some of the highest antioxidant levels I've seen and very fresh within a month or two of pressing. It's expensive, 25-30 dollars for a 500 mil bottle. Wine bottle size are about 35-70 depending on the verity quantity and antioxidant level. They sell out fairly fast on the high antioxidant level olive oils. The tast is unbelievable compared to store bought. All his oils are third party tested by one of the best labs and then results are shown above each selection !

    I agree!!! I exercise and eat very clean. My TRI is 83 and HDL is 74. My LDL is 148. I’m NOT worried about it!! I’m 57 and feel 25!!!

    I did a magnet test on Cheerios and it was full of metal! Is that normal to find metal in food?
    Crushed put in glass of hot water, ran a magnet on the outside and the metal followed the magnet!

    Brilliant. DOC B is a brilliant Dr. Dr Atkins was talking this in 1972.
    Also, if you are a carnivore or truly keto; don't, do the craft test unless you consumed carbs prior to the test for at least 72 hrs. If not, your pancreas will be so sluggish, it will not know what to do with 75 mgs of carbs, and you will look like a type 2 or 1 diabetic.

    Regarding A1c screening missing diabetes, the opposite can also be true. A1c can be elevated due to causes other than average glucose. This isn’t discussed much. I’d love to hear a deep dive on this.
